本発明は体外から超音波を照射して治療を行う超音波治療装置に関する。

【従来の技術】
近年、医療の分野では、患者の術後の生活の質(Quality of Life:QOL)の向上が重要視され、最少侵襲治療(Minimally Invasive Treatment:MIT)と呼ばれる治療法が注目を集めている。一例として、体外衝撃波結石破砕装置の実用化があげられる。本装置は、体外から強力な衝撃波を体内の結石に向けて照射し、外科的な手術をすること無しに結石を破砕治療する装置であり、泌尿器科系結石の治療法を大きく様変わりさせた。他方、癌治療の分野でもMITは一つのキーワードとなっている。特に癌の場合、その治療の多くを外科的手術に頼っている現状から、本来その臓器が持つ機能や外見上の形態を大きく損なう場合が多く、生命を長らえたとしても患者にとって大きな負担が残ることから、QOLを考慮した低侵襲治療(装置)の開発が強く望まれている。

このような流れの中で、癌細胞を加熱し壊死に導くハイパーサーミア療法が開発された。これは、腫瘍組織と正常組織の熱感受性の違いを利用して、患部を42.5℃以上に加温・維持することで癌細胞を選択的に死滅させる治療法である。特に、生体内深部の腫瘍に対しては、深達度の高い超音波エネルギを利用する方法が考えられている。(特開昭61−13955号公報参照)。また、上記加温治療法を更に進めて、ピエゾ素子より発生した超音波を患部に集束させて腫瘍部分を加熱し、熱変性壊死させる治療法も考えられている(米国特許第5150711号参照)。本治療法では、超音波のエネルギを集束させ、幅1〜3mm程度の限局した領域をおよそ1秒以下で80℃以上に加温することが可能である。これに対し、数mmにわたる広い領域を一度に加温させたいとのニーズもあり、特開平6−78930号に開示されているように、位相制御を用いて焦点領域サイズを電子的に拡大する手法が提案されている。本手法では、隣合う超音波発生素子群に交互に位相差を与えて駆動することにより焦点サイズの拡大を達成し、位相差を調整することにより焦点拡大率を変更する。また、登録特許第2,036,277号に開示されているように、超音波発生源の中心軸上における圧力をゼロに維持するような制御法もある。

上記の手法によれば、駆動位相量を制御することにより超音波エネルギの集まる作用領域のサイズを変更可能としているが、音場は固定であり、内部に強い圧力を受け続ける場所とほとんど圧力が加わらない場所が混在していた。このため熱的な効果は温度の平均化により焦域全体で作用するが、圧力による作用は均一とは言い難く、癌治療を目的とするような場合は治療効果にムラができ、再発の危険があった。また、これを避けるためには複数種類の音場を時間的に切り替えて与えれば良いが、そのためには複数の位相差を与えるための駆動回路が必要となり、構造が複雑且つ高価になるという問題があった。

【発明の実施の形態】
以下、図面を参照して、本発明による超音波治療装置を好ましい実施形態により具体的に説明する。図1は、本実施形態に係る超音波治療装置の構成を示している。治療用超音波アプリケータ1は、治療用超音波発生源2を有している。治療用超音波発生源2の略中央部分には孔が開けられており、ここに焦点付近の断層像(形態画像)を取得するための超音波プローブ(インナープローブとも称する)が挿入されている。
