【発明の属する技術分野】
本発明は、探触子の針誘導路内に挿入して使用されるバルーン付き穿刺針および穿刺針組立体に関する。

【従来の技術】
従来より、超音波診断装置を使用し、生体の卵巣を撮像し、その映像を見ながら卵子を採取することが行われている。このような場合、超音波診断装置に接続される超音波プローブ(探触子)に、針ガイドを装着したものを生体内に挿入する。そして、針ガイド内に採卵針を挿入して、卵子を採取する。
また、卵巣嚢腫の施術においても、上記の探触子を生体内に挿入した後、針ガイド内に、穿刺針を挿通し、卵巣嚢腫部位に穿刺針を穿刺し、エタノール等の処置液を注入し病変部位を変性させた後、吸引するという方法が用いられている。

【発明が解決しようとする課題】
しかし、従来の穿刺針を用いた治療方法では、エタノール等の処置液の病変部位への注入時に、処置液が病変部付近に流れ、患者に苦痛を与えることがあった。
本件出願人は、現時点において、上記の問題点を解消するものを開示する文献を知らない。

そこで、本発明は、処置液の病変部の周辺への流出を抑制することができる探触子の針誘導路内に挿入して使用されるバルーン付き穿刺針およびバルーン付き穿刺針組立体を提供するものである。

【課題を解決するための手段】
本発明は、探触子に装着された針挿入ガイド内に形成された針誘導路もしくは探触子に設けられた針誘導路内に移動可能に挿入されるバルーン付き穿刺針であって、該穿刺針は、先端に刃面を有する穿刺用中空針と、該穿刺用中空針内に収納され先端が前記中空針の先端部に位置し内部にルーメンを有する内管と、前記中空針と前記内管間により形成されるバルーン用ルーメンと、前記中空針の先端部においてバルーン用ルーメンを閉塞する閉塞部と、前記中空針の先端部の外面であって前記閉塞部より基端側に形成された第1の側孔と、該第1の側孔より基端側に形成された第2の側孔と、前記第1の側孔を被包する第1のバルーン部と前記第2の側孔を被包する第2のバルーン部に区分されるとともに内部が前記バルーン用ルーメンと連通するように該中空針の先端部の外面に取り付けられた膨張可能な筒状バルーンと、前記バルーン用ルーメンと連通するバルーン用ポートと、前記内管ルーメンと連通する内管用ポートと、前記中空針の外面を移動可能に設けられ、前記探触子もしくは前記針挿入ガイドに当接可能なストッパー部材とを備え、前記穿刺針は、前記探触子の前記針誘導路内への挿入時に前記筒状バルーンより所定長さ基端側から前記刃面までを前記針誘導路の先端より突出可能であり、さらに、前記ストッパー部材は、先端部が小径部となるものであり、後端側がエッジのある端面であり、かつ、前記探触子の前記針誘導路の後端内に侵入し係合するものであり、該ストッパー部材の前記針誘導路の後端との係合によって、前記穿刺針の前記探触子の前記針誘導路の先端側および後方への移動の防止が可能となっていることを特徴とする探触子の針誘導路内挿入用バルーン付き穿刺針である。
前記第1のバルーン部は、前記第2のバルーン部よりも大きいものである上記の探触子の針誘導路内挿入用バルーン付き穿刺針。

【発明の実施の形態】
本発明の探触子の針誘導路内挿入用バルーン付き穿刺針を図面に示す実施例を用いて説明する。
図1は、採卵針挿入ガイドを装着した探触子(超音波診断装置用プローブ)に本発明の探触子の針誘導路内挿入用バルーン付き穿刺針を挿入した状態の外観図である。図2は、本発明の実施例の探触子の針誘導路内挿入用バルーン付き穿刺針の部分省略外観図である。図3は、図2に示した探触子の針誘導路内挿入用バルーン付き穿刺針の先端部の拡大断面図である。図4は、図2に示した探触子の針誘導路内挿入用バルーン付き穿刺針の基端部の拡大断面図である。図5および図6は、本発明の他の実施例の探触子の針誘導路内挿入用バルーン付き穿刺針のバルーン部分を説明するための説明図である。

本発明の探触子の針誘導路内挿入用バルーン付き穿刺針1は、探触子10に装着された針挿入ガイド11内に形成された針誘導路12もしくは探触子10に設けられた針誘導路12内に移動可能に挿入されるバルーン付き穿刺針である。
穿刺針1は、先端に刃面2aを有する穿刺用中空針2と、穿刺用中空針2内に収納され先端が中空針2の先端部に位置し内部にルーメン3aを有する内管3と、中空針2と内管3間により形成されるバルーン用ルーメン4と、中空針2の先端部においてバルーン用ルーメン4を閉塞する閉塞部4aと、中空針2の先端部の外面であって閉塞部4aより基端側に形成された側孔5と、側孔5を被包するとともに内部がバルーン用ルーメン4と連通するように中空針2の先端部の外面に取り付けられた膨張可能な筒状バルーン6と、バルーン用ルーメン4と連通するバルーン用ポート7と、内管ルーメン3aと連通する内管用ポート8とを備える。そして、穿刺針1は、探触子10の針誘導路12内への挿入時に筒状バルーン6より所定長さ基端側から刃面2aまでを針誘導路12の先端より突出可能なものとなっている。

この実施例の穿刺針1は、中空針2、内管3とバルーン用ポート7と、内管ルーメン3aと連通する内管用ポート8とを備えるハブ21とを備える。
中空針2は、先端に患部への穿刺用の刃面2aを有する中空体である。中空針としては、金属針が用いられる。金属としては、ステンレス鋼が好適である。また、中空針の外径としては、0.5〜5mm程度が好適である。長さは、120〜450mm程度が好適である。

内管3は、中空針2内に挿入されるとともに、先端が中空針の刃面より露出しないように収納されている。内管としては、中空針の内径より外径が小さいものが用いられる。両者間の径の差異は、0.2〜3mm程度であることが好ましい。内管としては、金属管、樹脂管が使用できる。金属管としては、ステンレス管が好適である。樹脂管としては、フッ素樹脂系の薄肉のものが好適である。具体的には、PTFE、ETFEなどが好適である。

そして、中空針2と内管3間により形成されるバルーン用ルーメン4が形成されている。さらに、中空針2の先端部においてバルーン用ルーメン4は閉塞部4aにより閉塞されている。この実施例では、中空針の先端部外面にシール用の側孔2bが穿たれており、このシール用側孔よりシール剤を注入することにより閉塞部4aが形成されている。シール剤としては、作業時には液状でありその後硬化するものであり、かつ使用する中空針および内管との接着性を有するものが用いられる。シール剤としては、エポキシ樹脂系接着剤、シリコーン樹脂系接着剤などが使用できる。

また、中空針2には、中空針2の先端部の外面であって閉塞部4aより基端側に側孔5が穿たれている。この側孔5は、バルーン膨張用のためのものである。そして、この側孔5を被包するとともに内部がバルーン用ルーメン4と連通するように中空針2の先端部の外面に膨張可能な筒状バルーン6が固定されている。具体的には、バルーン6の軸方向の中央付近に側孔5が位置するように配置されている。さらに、このバルーン6は、上述したシール用側孔2bを被覆していることが好ましい。バルーン6は、先端部および基端部が中空針に固定されるとともに、先端部と基端部間は固定されていない。このため側孔5より流入する液体により膨張する。バルーン6の先端部および基端部の中空針2への固定は、接着剤により行うことができる。接着剤としては、バルーン形成材料および中空針形成材料との接着性を有するものが用いられる。具体的には、上述したエポキシ樹脂系接着剤、シリコーン樹脂系接着剤などが使用できる。また、バルーン6の先端面には、図3に示すように接着剤により環状の傾斜面からなる段差解消部31を設けることが望ましい。また、バルーン6の後端面には、図3に示すように接着剤により環状の傾斜面からなる段差解消部32を設けることが望ましい。

また、バルーン6の固定方法としては、図5に示すように、バルーン6の両端部を糸33で巻き付けることにより固定したものであってもよい。さらに、この糸33と接着剤とを併用してもよい。接着剤を併用する場合には、上述したように段差解消部31を設けることがよい。

また、バルーンの固定方法としては、図6に示すように、バルーン6の先端部および基端部をより所定距離中央側にて糸33を巻き付けることにより固定し、さらに、先端部および基端部を折り返して、糸巻付部をバルーンにより被包したものであってもよい。この場合、糸巻付部となる部分より端部側のバルーン部分は、膨張する部分よりも肉薄なものとすることが好ましい。さらに、この折り返し部分に接着剤を塗布してもよい。また、この実施例において、バルーンを併用してもよい。また、この実施例においても、バルーン6の先端面には、環状の傾斜面からなる段差解消部31を設けることが望ましい。また、バルーン6の後端面には、環状の傾斜面からなる段差解消部32を設けることが望ましい。

バルーンの形成材料としては、弾性材料が使用される。特に、ゴムが好適であり、シリコーンゴム、ラテックスゴムなどが好ましい。バルーンの大きさとしては、膨張時の外径が8〜30mm程度であることが好ましい。

穿刺針1の基端部には、ハブ21が固定されている。ハブ21は、バルーン用ルーメン4と連通するバルーン用ポート7と、内管ルーメン3aと連通する内管用ポート8を備えている。そして、バルーン用ポート7は、バルーン用ルーメン内に流体を注入するための注入器具(例えば、シリンジ)が取付可能なものとなっている。さらに、バルーン用ポート7もしくはポート7と接続されるチューブ7bには、流路閉塞部材7aが設けられている。この実施例では、流路閉塞部材7aとして、ポート7にその開閉を行う活栓が設けられている。なお、流路閉塞部材としては、注入器具の装着時に開放状態となり、非装着時に閉塞状態となる逆止弁であってもよい。

ハブ21は、具体的には、中空針の基端に固定された中空針ハブ22、内管の基端に固定された内管ハブ23と中空針ハブ22間に配置された分岐部材24を備える。分岐部材24には、端部にバルーン用ポート7を備えるポートチューブが接続されている。内管3は、中空針ハブ22および分岐部材24を貫通し、内管ハブ23にその基端が固定されている。また、中空針2は、その基端が分岐部材の先端部に侵入し液密に固定されている先端部にて終端している。このため、分岐部材内に形成される通路は、バルーン用ルーメンとのみ連通する。また、内管ハブ23は、分岐部材の後端部に液密に固定されている。なお、ハブ21は、このようなタイプのものに限定されるものではない。

さらに、穿刺針1は、中空針2の外面を移動可能に設けられ、探触子10もしくは針挿入ガイドに当接可能なストッパー部材9を備えていることが好ましい。このようなストッパー部材9を設けることにより、穿刺針の予定外の先端側への移動を防止することができる。さらに、ストッパー部材9としては、その先端部が、探触子の針誘導路の後端部と係合し、穿刺針の後方への移動も規制するものであることが好ましい。具体的には、ストッパー部材9の先端部が小径部となっており、探触子の針誘導路の後端内に挿入し係合するものが考えられる。
そして、この穿刺針は、卵巣嚢腫治療用に特に有効である。

このストッパー部材9は、中空針の先端側に押すもしくは後端側に引くことにより、中空針2上を摺動する。そして、この実施例のストッパー部材のように、ストッパー部材9の後端側をエッジのある端面とすることにより、上記の移動作業が容易なものとなる。ストッパー部材9は、弾性材料により形成することが好ましい。形成材料としては、例えぱ、ウレタンゴム、シリコーンゴム、ブタジエンゴムなどの合成ゴム、ラテックスゴムなどの天然ゴム、軟質塩化ビニール、合成樹脂エラストマー等が使用できる。
この実施例の穿刺針が使用される探触子10(超音波診断装置用プローブ)は、経膣プローブであり、図1に示すように、先端が曲面状に形成されたプローブ本体51を備え、内部に超音波発信および受信機構が収納されている。そして、プローブ本体51の外面には、針挿入ガイド11が装着されている。針挿入ガイド11は、ガイド管61とガイド管固定具63a、63bからなり、プローブ本体51に対して着脱可能となっている。ガイド管61内に、針誘導路12が形成されている。ガイド管61は、先端が斜めにカットされ、刃先状先端面を有する管状体である。ガイド管は、内径1〜7mm、長さ100〜400mmとなっている。ガイド管61は、通常、ステンレス鋼、チタン、チタン合金などの金属管により形成されている。

そして、穿刺針1は、探触子10の針誘導路12内への挿入時に筒状バルーン6より所定長さ基端側から刃面2aまでを針誘導路12の先端より突出可能なものとなっている。
なお、本発明の穿刺針は、上記のような探触子に取り付けられた針挿入ガイドの針誘導路に挿入されるものに限定されるものではなく、探触子自身が、針誘導路を備えるものにも使用できる。
また、本発明の穿刺針は、卵巣嚢腫以外の治療にも用いることができる。

図7は、本発明の他の実施例の探触子の針誘導路内挿入用バルーン付き穿刺針のバルーン部分を説明するための説明図である。
この実施例と上述した実施例の穿刺針1との相違は、中空針2に穿たれた側孔の数と、バルーンの形態のみである。その他については、上述した穿刺針1と同じであり、上述の説明を参照する。
この実施例の穿刺針40では、中空針2の先端部の外面であって閉塞部4aより基端側に第1の側孔5aと第2の側孔5bが穿たれている。これら側孔5a,5bは、バルーン膨張用のためのものである。そして、この側孔5a、5bを被包するとともに内部がバルーン用ルーメン4と連通するように中空針2の先端部の外面に膨張可能な筒状バルーン6が固定されている。具体的には、バルーン6の軸方向の中央より先端側に第1の側孔5aが位置し、バルーン6の中央より基端側に第2の側孔5bが位置するように配置されている。さらに、このバルーン6は、上述したシール用側孔2bを被覆していることが好ましい。

バルーン6は、先端部および基端部が中空針2に固定されている。さらに、バルーン6は、中間固定部41により、側孔5aと側孔5b間にて穿刺針2に固定されている。このため、バルーン6は、第1の側孔5aと連通する第1のバルーン部6aと、第2の側孔5bと連通する第2のバルーン部6bに区分されている。なお、これら第1のバルーン部6aおよび第2のバルーン部6bは、ともに同じバルーン拡張用ルーメン4と連通している。この穿刺針40では、実施的にダブルバルーンとなっている。そして、第1のバルーン部を卵巣内に配置し、第2のバルーン部を卵巣外もしくは卵巣表面部付近に配置することにより、穿刺針の固定状態が安定となり、良好な治療を行うことができる。中間固定部41は、この実施例では、糸などにより固定さている。さらに、図7に示すように、糸の外面を接着材により固定補強することが好ましい。接着剤としては、どのようなものでもよいが、上述したエポキシ樹脂系接着剤、シリコーン樹脂系接着剤などが使用できる。また、第1のバルーン部6aは、第2のバルーン部6bより大きいことが好ましい。このようにすることにより、卵巣への固定状態がより安定するものと考える。

また、バルーンの形態としては、上述した穿刺針40のようなものに限定されるものではなく、図8に示すようなものであってもよい。
図8は、本発明の他の実施例の探触子の針誘導路内挿入用バルーン付き穿刺針のバルーン部分を説明するための説明図である。
この実施例と上述した実施例の穿刺針1との相違は、中空針2に穿たれた側孔の数と、バルーンの形態のみである。その他については、上述した穿刺針1と同じであり、上述の説明を参照する。
この実施例の穿刺針45では、中空針2の先端部の外面であって閉塞部4aより基端側に第1の側孔5aと第2の側孔5bが穿たれている。これら側孔5a,5bは、バルーン膨張用のためのものである。そして、この側孔5a、5bを被包するとともに内部がバルーン用ルーメン4と連通するように中空針2の先端部の外面に膨張可能な第1の筒状バルーン46aと第2の筒状バルーン46bが固定されている。具体的には、第1のバルーン46aは、その軸方向の中央付近に第1の側孔5aが位置するように中空針2に固定されている。また第2のバルーン6bは、その軸方向の中央付近に第2の側孔5bが位置するように中空針2に固定されている。さらに、第1のバルーン46aは、上述したシール用側孔2bを被覆していることが好ましい。

バルーン46a、46b、それぞれの先端部および基端部が中空針2に固定されている。特に、この実施例の穿刺針45では、第1のバルーン46aの基端と第2のバルーン46bの先端は、中間固定部47により穿刺針2に固定されている。この穿刺針45では、第1の側孔5aと連通する第1のバルーン46aと、第2の側孔5bと連通する第2のバルーン46bを備える。なお、これら第1のバルーン46aおよび第2のバルーン46bは、ともに同じバルーン拡張用ルーメン4と連通している。この穿刺針45は、いわゆるダブルバルーンタイプである。そして、第1のバルーンを卵巣内に配置し、第2のバルーンを卵巣外もしくは卵巣表面部付近に配置することにより、穿刺針の固定状態が安定となり、良好な治療を行うことができる。中間固定部47には、接着剤が使用される。接着剤としては、どのようなものでもよいが、上述したエポキシ樹脂系接着剤、シリコーン樹脂系接着剤などが使用できる。なお、中間固定部は、上述した穿刺針40と同様に、糸を用いて固定してもよい。
また、第1のバルーン46aは、第2のバルーン46bより大きいことが好ましい。このようにすることにより、卵巣への固定状態がより安定するものと考える。

本発明の探触子の針誘導路内挿入用バルーン付き穿刺針1の使用方法について、卵巣嚢腫治療に用いる場合を例にとり説明する。
最初に、針挿入ガイド53を装着した探触子10を準備する。なお、この状態では、穿刺針1は、針誘導路内に挿入されているものの、誘導路の先端より突出させない。この配置状態を維持するために、ストッパー部材を用いてもよい。そして、径膣的に探触子10を体内に挿入し、探触子の先端面を卵巣に接触させ、卵巣内の超音波映像を行う。そして、卵巣内の嚢腫部位を確認した後、穿刺針1を前進させて、その卵巣嚢腫部位まで刃面が到達するように穿刺する。なお、ストッパー部材により進行不能状態となっている場合には、ストッパー部材を後方に移動した後これを行う。そして、嚢腫部位への刃先の到達を超音波映像により確認した後、バルーン用ポートに接続されているシリンジ(図示せず)により、し、液体(例えば、滅菌水)を注入、卵巣内にてバルーン6を膨張させるとともに膨張状態が維持されるようにする。具体的には、流路閉塞部材7aのコックを操作し流路を閉塞状態とする。そして、若干、穿刺針を後方に引く。これにより、穿刺針の先端側に若干の空間を形成させることができる。続いて、ストッパー部材を先端側に移動させて、穿刺針1を探触子10(具体的には、ガイド11)に固定する。そして、内管用ポートに接続された処置液供給手段(図示せず)を作動させて処置液(例えば、エタノール)を卵巣嚢腫部位に注入する。続いて、注入された処置液により変性した卵巣嚢腫構成物を処置液とともに内管ポートより吸引し除去する。そして、バルーン6内に注入した液体を吸引し、バルーンを収縮させた後、穿刺針を針誘導路内に収容し、探触子10とともに生体より抜去する。

次に、本発明の探触子の針誘導路内挿入用バルーン付き穿刺針組立体を図面に示す実施例を用いて説明する。
図9は、採卵針挿入ガイドを装着した探触子(超音波診断装置用プローブ)に本発明の探触子の他の実施例の針誘導路内挿入用バルーン付き穿刺針組立体を挿入した状態の外観図である。図10は、本発明の他の実施例の探触子の針誘導路内挿入用バルーン付き穿刺針組立体の部分省略外観図である。図11は、図10に示した探触子の針誘導路内挿入用バルーン付き穿刺針組立体の先端部の拡大断面図である。図12は、図10に示した探触子の針誘導路内挿入用バルーン付き穿刺針組立体の基端部の拡大断面図である。
本発明の探触子の針誘導路内挿入用バルーン付き穿刺針組立体50は、探触子10に装着された針挿入ガイド11内に形成された針誘導路12もしくは探触子10に設けられた針誘導路12内に移動可能に挿入されるバルーン付き穿刺針組立体である。
穿刺針組立体50は、第1のチューブ71と、第1のチューブ71内に配置された第2のチューブ72と、第1のチューブ71と第2のチューブ72間に形成されたバルーン用ルーメン74と、第1のチューブ71の先端部においてバルーン用ルーメンを閉塞する閉塞部74aと、第1のチューブ71の先端部の外面であって閉塞部74aより基端側に形成された側孔75と、側孔75を被包するとともに内部がバルーン用ルーメン74と連通するように第1のチューブ71の先端部の外面に取り付けられた膨張可能な筒状バルーン76と、バルーン用ルーメン74と連通するバルーン用ポート77とを備えるバルーン付外管70と、バルーン付外管70内に移動可能に挿入され、かつ外管70の先端より突出する刃面90aを有する穿刺用内針90とを備える。そして、穿刺針組立体50は、探触子10の針誘導路12内への挿入時に筒状バルーン76より所定長さ基端側から刃面90aまでを針誘導路12の先端より突出可能なものとなっている。

この実施例の穿刺針組立体50は、バルーン付外管70と穿刺用内針90とを備える。
穿刺用内針90は、この実施例では、中空針となっており、先端に刃面90aと、内針ルーメン92を備え、後端に内針ハブ93を備えている。内針ハブ93は、ポート98を備えている。内針90としては、金属針が用いられる。金属としては、ステンレス鋼が好適である。また、内針の外径としては、0.2〜4mm程度が好適である。長さは、120〜450mm程度が好適である。なお、内針90は、上記のような中空針に限定されるものではなく、中実針であってもよい。また、内針90は、刃面90a部分がバルーン付外管70の先端より突出可能なものとなっている。
バルーン付外管70は、第1のチューブ71、第2のチューブ72と、バルーン用ポート77とを備えるハブ81とを備える。

第1のチューブ71の外径としては、0.5〜5mm程度が好適である。長さは、120〜450mm程度が好適である。第2のチューブ72は、第1のチューブ71内に挿入されるとともに、先端部において、後述する閉塞部により第1のチューブと固定されている。第2のチューブとしては、第1のチューブの内径より外径が小さいものが用いられる。両者間の径の差異は、0.2〜3mm程度であることが好ましい。第1のチューブおよび第2のチューブとしては、樹脂チューブが使用できる。樹脂チューブとしては、ポリエステル、ポリオレフィン、ポリアミド、熱可塑性ポリウレタン、ポリアセタール、ポリビニル、フッ素樹脂などの樹脂チューブが使用できる。ポリエステルとしては、ポリエチレンテレフタレートが好適である。ポリオレフィンとしては、ポリエチレン、ポリプロピレンなどが好適である。フッ素樹脂としては、PTFE、ETFEなどが好適である。

そして、第1のチューブ71と第2のチューブ72間によりバルーン用ルーメン74が形成されている。さらに、第1のチューブ71の先端部においてバルーン用ルーメン74は閉塞部74aにより閉塞されている。シール剤としては、作業時には液状でありその後硬化するものであり、かつ使用する中空針および内管との接着性を有するものが用いられる。シール剤としては、エポキシ樹脂系接着剤、シリコーン樹脂系接着剤などが使用できる。
また、第1のチューブ71には、第1のチューブ71の先端部の外面であって閉塞部74aより基端側に側孔75が穿たれている。この側孔75は、バルーン膨張用のためのものである。そして、この側孔75を被包するとともに内部がバルーン用ルーメン74と連通するように第1のチューブ71の先端部の外面に膨張可能な筒状バルーン76が固定されている。特に、側孔は、軸方向に複数設けられていることが好ましい。この例では、2つの側孔75a,75bが設けられている。具体的には、このバルーン76は、上述した側孔75a、75bを被覆している。バルーン76は、先端部および基端部が、第1のチューブ71に固定されるとともに、先端部と基端部間は固定されていない。このため側孔75より流入する液体により膨張する。バルーン76の先端部および基端部の第1のチューブ71への固定は、接着剤により行うことができる。接着剤としては、バルーン形成材料および第1のチューブ形成材料との接着性を有するものが用いられる。具体的には、上述したエポキシ樹脂系接着剤、シリコーン樹脂系接着剤などが使用できる。また、バルーン76の先端面には、図11に示すように接着剤により環状の傾斜面からなる段差解消部31を設けることが望ましい。また、バルーン76の後端面には、図11に示すように接着剤により環状の傾斜面からなる段差解消部32を設けることが望ましい。

また、バルーン76の固定方法としては、図5に示したものと同様に、バルーン76の両端部を糸33で巻き付けることにより固定したものであってもよい。さらに、この糸33と接着剤とを併用してもよい。接着剤を併用する場合には、上述したように段差解消部31を設けることがよい。
また、バルーンの固定方法としては、図6に示したものと同様に、バルーン76の先端部および基端部をより所定距離中央側にて糸33を巻き付けることにより固定し、さらに、先端部および基端部を折り返して、糸巻付部をバルーンにより被包したものであってもよい。この場合、糸巻付部となる部分より端部側のバルーン部分は、膨張する部分よりも肉薄なものとすることが好ましい。さらに、この折り返し部分に接着剤を塗布してもよい。また、この実施例において、バルーンを併用してもよい。また、この実施例においても、バルーン76の先端面には、環状の傾斜面からなる段差解消部31を設けることが望ましい。また、バルーン76の後端面には、環状の傾斜面からなる段差解消部32を設けることが望ましい。
バルーンの形成材料としては、弾性材料が使用される。特に、ゴムが好適であり、シリコーンゴム、ラテックスゴムなどが好ましい。バルーンの大きさとしては、膨張時の外径が8〜30mm程度であることが好ましい。

バルーン付外管70基端部には、ハブ81が固定されている。ハブ81は、バルーン用ルーメン74と連通するバルーン用ポート77とを備えている。そして、バルーン用ポート74は、バルーン用ルーメン内に流体を注入するための注入器具(例えば、シリンジ)が取付可能なものとなっている。さらに、バルーン用ポート77もしくはポート77と接続されるチューブ77bには、流路閉塞部材77aが設けられている。この実施例では、流路閉塞部材77aとして、ポート77に、その開閉を行う活栓が設けられている。なお、流路閉塞部材としては、注入器具の装着時に開放状態となり、非装着時に閉塞状態となる逆止弁であってもよい。
ハブ81は、具体的には、第1のチューブ71の基端に固定された第1ハブ82、第2のチューブ71の基端に固定された第2ハブ84を備える。第2ハブ84は、いわゆる分岐ハブであり、側面には、端部にバルーン用ポート77を備えるポートチューブ77bが接続されている。第2のチューブ72は、第1ハブ82を貫通し、その基端部が、第2ハブ84の基端部に固定されている。この例では、固着部材85により、第2のチューブ72は、第2ハブ84に固定されている。また、第1のチューブ71は、その基端が第1ハブの先端部に侵入し液密に固定されているとともに、終端している。このため、第2ハブ内に形成される通路は、バルーン用ルーメンとのみ連通する。さらに、この例では、第2ハブ84の後端には、操作部86が設けられている。操作部86は、第2ハブ84の基端部外面に形成された螺子部と螺合可能な螺子部を備える外筒部86aと、第2ハブ84内に侵入する内筒部86bを備えている。また、第2ハブ84内には、操作部86の内筒部86bにより押圧される環状弾性部材87が収納されている。この例では、環状弾性部材87は、固着部材85と内筒部86bにより押圧され、押圧状態において、穿刺用内針90を液密状態に固定する。そして、この操作部86を操作し、操作部を後方に移動させることにより、環状弾性部材87の押圧が解除され、穿刺用内針90の後方への移動操作が可能となる。また、穿刺用内針90の外面には、外管70内への内針の刃面の収納告知用マーカー88が設けられている。このため、操作部86を操作し、内針90をこのマーカー88が確認できる位置まで後方に引くことにより、内針90の刃面を外管内に収納することができる。そして、操作部86を操作することにより、内針90をハブ81に固定することができ、刃面を収納した状態を維持する。なお、ハブ81は、このようなタイプのものに限定されるものではない。

さらに、この実施例の穿刺針組組立体50においても、上述した穿刺針1と同様に、ストッパー部材を設けることが好ましい。組立体50は、第1のチューブ71の外面を移動可能に設けられ、探触子10もしくは針挿入ガイドに当接可能なストッパー部材79を備えていることが好ましい。このようなストッパー部材79を設けることにより、穿刺針組立体の予定外の先端側への移動を防止することができる。さらに、ストッパー部材79としては、その先端部が、探触子の針誘導路の後端部と係合し、穿刺針の後方への移動も規制するものであることが好ましい。具体的には、ストッパー部材79の先端部が小径部となっており、探触子の針誘導路の後端内に挿入し係合するものが考えられる。
そして、この穿刺針組立体は、卵巣嚢腫治療用に特に有効である。
このストッパー部材79としては、ストッパー部材9において説明したものが好適に使用できる。

この実施例の穿刺針組立体が使用される探触子10(超音波診断装置用プローブ)は、経膣プローブであり、図9に示すように、先端が曲面状に形成されたプローブ本体51を備え、内部に超音波発信および受信機構が収納されている。そして、プローブ本体51の外面には、針挿入ガイド11が装着されている。針挿入ガイド11は、ガイド管61とガイド管固定具63a、63bからなり、プローブ本体51に対して着脱可能となっている。ガイド管61内に、針誘導路12が形成されている。ガイド管61は、先端が斜めにカットされ、刃先状先端面を有する管状体である。ガイド管は、内径1〜7mm、長さ100〜400mmとなっている。ガイド管61は、通常、ステンレス鋼、チタン、チタン合金などの金属管により形成されている。
そして、穿刺針組立体50は、探触子10の針誘導路12内への挿入時に筒状バルーン76より所定長さ基端側から刃面90aまでを針誘導路12の先端より突出可能なものとなっている。
なお、本発明の穿刺針組立体は、上記のような探触子に取り付けられた針挿入ガイドの針誘導路に挿入されるものに限定されるものではなく、探触子自身が、針誘導路を備えるものにも使用できる。
また、本発明の穿刺針は、卵巣嚢腫以外の治療にも用いることができる。

本発明の探触子の針誘導路内挿入用バルーン付き穿刺針組立体50の使用方法について、卵巣嚢腫治療に用いる場合を例にとり説明する。
最初に、針挿入ガイド11を装着した探触子10を準備する。なお、この状態では、穿刺針組立体50は、針誘導路内に挿入されているものの、誘導路の先端より突出させない。この配置状態を維持するために、ストッパー部材を用いてもよい。そして、径膣的に探触子10を体内に挿入し、探触子の先端面を卵巣に接触させ、卵巣内の超音波映像を行う。そして、卵巣内の嚢腫部位を確認した後、穿刺針組立体50を前進させて、その卵巣嚢腫部位まで内針90の刃面90aが到達するように穿刺する。なお、ストッパー部材により進行不能状態となっている場合には、ストッパー部材を後方に移動した後これを行う。そして、嚢腫部位への刃先の到達を超音波映像により確認した後、バルーン用ポートに接続されているシリンジ(図示せず)により、液体(例えば、滅菌水)を注入、卵巣内にてバルーン76を膨張させるとともに膨張状態が維持されるようにする。具体的には、流路閉塞部材77aのコックを操作し流路を閉塞状態とする。そして、若干、穿刺針組立体を後方に引く。これにより、穿刺針の先端側に若干の空間を形成させることができる。続いて、ストッパー部材を先端側に移動させて、穿刺針組立体50を探触子10(具体的には、ガイド11)に固定する。そして、操作部86を回転させて、外管70への内針90の固定を解除し、内針90をマーカー88が視認できる位置まで引き、刃面90aを外管内に収納する。そして、操作部86を回転させて、内針90を再び外管70に固定する。そして、内針ポート98に接続された処置液供給手段(図示せず)を作動させて処置液(例えば、エタノール)を卵巣嚢腫部位に注入する。続いて、注入された処置液により変性した卵巣嚢腫構成物を処置液とともに内針ポートより吸引し除去する。そして、バルーン76内に注入した液体を吸引し、バルーンを収縮させた後、穿刺針を針誘導路内に収容し、探触子10とともに生体より抜去する。

【発明の効果】
本発明の探触子の針誘導路内挿入用バルーン付き穿刺針は、先端に刃面を有する穿刺用中空針と、該穿刺用中空針内に収納され先端が前記中空針の先端部に位置し内部にルーメンを有する内管と、前記中空針と前記内管間により形成されるバルーン用ルーメンと、前記中空針の先端部においてバルーン用ルーメンを閉塞する閉塞部と、前記中空針の先端部の外面であって前記閉塞部より基端側に形成された側孔と、該側孔を被包するとともに内部が前記バルーン用ルーメンと連通するように該中空針の先端部の外面に取り付けられた膨張可能な筒状バルーンと、前記バルーン用ルーメンと連通するバルーン用ポートと、前記内管ルーメンと連通する内管用ポートとを備え、かつ、該穿刺針は、前記探触子の前記針誘導路内への挿入時に前記筒状バルーンより所定長さ基端側から前記刃面までを前記針誘導路の先端より突出可能である。
このため、生体治療部位への穿刺後、バルーンを膨張させた後に病変部位に処置液を注入させることにより、処置液の病変部の周辺への流出を抑制することができ、処置中における処置液流出による患者への苦痛を軽減できる。
本発明の探触子の針誘導路内挿入用バルーン付き穿刺針組立体は、探触子に装着された針挿入ガイド内に形成された針誘導路もしくは探触子に設けられた針誘導路内に移動可能に挿入されるバルーン付き穿刺針組立体であって、該穿刺針組立体は、第1のチューブと、該第1のチューブ内に配置された第2のチューブと、前記第1のチューブと前記第2のチューブ間に形成されたバルーン用ルーメンと、前記第1のチューブの先端部においてバルーン用ルーメンを閉塞する閉塞部と、前記第1のチューブの先端部の外面であって前記閉塞部より基端側に形成された側孔と、該側孔を被包するとともに内部が前記バルーン用ルーメンと連通するように該第1のチューブの先端部の外面に取り付けられた膨張可能な筒状バルーンと、前記バルーン用ルーメンと連通するバルーン用ポートとを備えるバルーン付外管と、前記外管内に移動可能に挿入され、かつ前記外管の先端より突出する刃面を有する穿刺用内針とを備え、さらに、該穿刺針組立体は、前記探触子の前記針誘導路内への挿入時に、前記筒状バルーンより所定長さ基端側から前記刃面までを前記針誘導路の先端より突出可能である。
このため、生体治療部位への穿刺後、バルーンを膨張させた後に病変部位に処置液を注入させることにより、処置液の病変部の周辺への流出を抑制することができ、処置中における処置液流出による患者への苦痛を軽減できる。また、治療時に、内針を抜去もしくは内針の刃先を外管内に収納することできるため、治療作業が容易なものとなる。
