(Related application)
This application is related to US Patent Application No. 60 / 175,658, “Glaucoma Pressure Relief Valve and Drug Delivery Device,” filed Jan. 12, 2000, the contents of which are incorporated herein by reference. Insist on its rights.
[0002]
(Technical field to which the invention belongs)
The present invention generally relates to systems and methods for reducing intraocular pressure. In one embodiment, the present invention relates to an implantable device for draining aqueous humor to release the high intraocular pressure characteristics of glaucoma.
[0003]
(Conventional technology)
The eyeball has a substantially spherical structure whose shape and condition is maintained by an endogenous fluid that fills the outer hollow collagenous sphere. The interior of the eyeball is divided into two chambers, an anterior chamber and a posterior chamber. Between these tufts, the lens and the tissue supporting it and the associated tissue hang. The posterior chamber is filled with a glue-like material called vitreous humor that is not thought to contribute significantly to the intraocular pressure level called intraocular pressure (IOP). In contrast, the anterior chamber is filled with a watery fluid called aqueous humor that is constantly produced and reabsorbed. This fluid exerts pressure on the overlying cornea and all surrounding structures. If the amount of aqueous humor produced is excessive, the pressure inside the anterior chamber and inside the eyeball will increase. Normal IOP results from a healthy equilibrium between aqueous humor production and reabsorption.
[0004]
Aqueous humor is produced behind the base of the iris and flows into the anterior chamber. Reabsorption occurs through the trabecular meshwork, from where this fluid passes into the capsular blood vessel and is taken up into the bloodstream. A specific range of pressure in the anterior chamber is considered normal and is generally between 10 and 21 milliHg. The pressure in the anterior chamber is determined by how quickly aqueous humor is produced and how quickly it is drained through the trabecular meshwork. Impairment to the drainage system can result in elevated intraocular pressure. Elevated IOP persistence can cause a condition called glaucoma that can eventually lead to blindness if not treated properly, as elevated IOP can damage the optic nerve and affect vision.
[0005]
Various treatments are available for glaucoma. Clinical practice is continually striving to improve fluid spillage or reduce fluid production to reduce IOP. Available therapies may include topical eye drops or systematic dosing. However, treatment with pharmaceuticals may fail due to patient failure to follow, high costs, or any one of many well-recognized complications and side effects. If medical treatment is unsuccessful, it is more invasive either to alter the normal anatomy or to introduce an implantable drainage device to release the excess of aqueous humor Treatment can be provided to the patient. For example, laser surgery can be recommended to alter the trabecular meshwork anatomy and facilitate drainage of the anterior chamber. In addition, for the treatment of glaucoma, ophthalmologic processing via other lasers can be used. Despite medical treatment and laser intervention, glaucomatous eyes that continue to have elevated intraocular pressure may require definitive surgical treatment.
[0006]
As one example, conventional types of surgical intervention aim to create a fistula or other drainage channel outside the anterior chamber of the eye. This directs aqueous humor to flow into a surgically created subconjunctival or capsular cavity often referred to as a “blister” from which the fluid is reabsorbed. Can do. This surgery reduces intraocular pressure by allowing excess fluid to flow out of the anterior chamber. However, such processing is associated with some known limitations. First, normal healing of the wound tends to interfere with fistula clarity and drainage cavity dimensions, so these operations may fail at an unacceptable rate. To increase the success rate of this type of surgery, physicians can recommend adjuvant therapy with drugs that modulate the normal healing of wounds. Such treatments increase the incidence of a second type of problem associated with these treatments, namely, excessive or too rapid outflow of aqueous humor. Too quick removal of aqueous humor can cause intraocular pressure to drop rapidly to dangerously low levels, a condition called hypotonia, and is a potential cause of many complications that threaten vision. It is well known. To prevent this problem, the surgical site must heal well enough to create a controlled aqueous humor drainage. For this to happen, normal healing of the wound is essential. Thus, treatments that prevent wound healing increase the risks associated with excessive aqueous humor drainage. A third type of problem associated with this type of conventional wastewater treatment is an increased risk of infection. The drainage of aqueous humor into capsular or subconjunctival blisters poses a risk of infection by providing a fluid environment in which microorganisms can enter. In addition, if infection is established in a fluid-filled cavity, microorganisms can reverse through the drainage channel and enter the anterior chamber of the eye, which infects as well. This is a more serious situation.
[0007]
In order to address some of the problems associated with conventional surgery, many implantable devices have been proposed that make efforts to drain excess fluid from the anterior chamber. The problems described above that affect soft tissue surgery, however, also affect implant surgery. Even if the surgery involves implantation of an intraocular implant, the wound healing mechanism remains intact. Indeed, artificial materials can over-stimulate the local healing of wounds, resulting in the formation of excess scar tissue. Furthermore, even if artificial devices are included in the process, control of the outflow rate of the aqueous humor remains essential. In addition, the risk of infection remains. Any mechanism with a mechanical conduit that can be used to deliver microorganisms from the outside of the eye to the inside is desirable to reduce retrograde infection. Finally, the eye, like most tissues in the human body, has limited resistance to the long-term presence of artificial materials. Locally placed implants may abnormally stimulate surrounding tissue. Of course, the eyes are particularly sensitive. Devices implanted on the surface of the eye may be perceived by patients as chronic, stubborn, and annoying foreign bodies. Finally, because the tissue of the eye is so delicate, the implant must be designed and placed so as not to damage the vulnerable, adjacent, underlying or overlying tissue. However, even if properly placed initially, the implant may be moved by the movement of the tissue at that location or may be ejected by the contractile healing process of the wound.
[0008]
Various devices of the prior art are intended to provide solutions to some or all of these problems. For example, some prior art devices divert aqueous humor to a water reservoir or drainage area implanted in the capsule or under the conjunctiva. However, as already mentioned, these devices face the problems of regulating aqueous humor outflow, preventing infection, and abnormal stimulation and trauma to local tissues. The primary problem of regulating aqueous humor outflow is that the drainage rate of this fluid substantially affects the mechanical properties of the implant until the wound has healed sufficiently to biologically limit fluid outflow. It happens to depend. Effectively balancing the biological and mechanical resistance to the outflow of aqueous humor remains a problem for implant-based wastewater treatment. Prior art devices utilize a variety of mechanisms to limit aqueous humor outflow. However, each of these mechanisms can be burdensome if wound healing is established. Limiting factors within the implant, when combined with limitations enabled by wound healing, can unduly reduce the rate of aqueous humor outflow, possibly to non-therapeutic levels. The second problem of possible intraocular infection arises because the presence of the implant provides a conduit through which bacteria can enter the interior of the anterior chamber. Some prior art drainage devices have introduced filters or valves or other conduit systems to prevent retrograde delivery of infection into the anterior chamber. However, these mechanisms have limitations. That is, they have a hydraulic effect on the outflow of fluid that, when effective in preventing the passage of microorganisms, can also prevent effective drainage. Finally, local tissue tolerance issues are due to the fact that these foreign bodies can stimulate tissue responses and maximize local swelling or protrusion, which can be perceivable and unpleasant for the patient. This occurs in some prior art devices. These responses to the presence of the implant can make the use of the implant clinically inappropriate.
[0009]
Devices installed through the transparent cornea to enable aqueous humor drainage are intended to avoid certain limitations associated with implantation in the capsule or subconjunctiva. Certain devices, such as U.S. Pat. No. 3,788,327 and U.S. Pat. No. 5,807,302, and U.S. Pat. No. 5,743,868, provide an anterior chamber on the surface of the cornea for mixing with tears. A transcorneal conduit for draining the fluid. The devices taught in the above-mentioned patents include features directed to efflux regulation, microbial restriction, local tissue affinity, and position stability issues. As already discussed, these problems affect as well as the cross-corneal device. Thus, there remains a need for an anterior chamber drainage device that is compatible with the body that allows for a well-controlled outflow of aqueous humor, despite the wound healing wound. There remains a further need for a drainage device that can limit the entry of microorganisms, thereby protecting the interior of the eye from infection. In addition, there remains a need for ophthalmic drainage devices that are well tolerated and comfortable for the patient. Finally, the problem of position stability has not been solved satisfactorily. There is a need in the art for a drainage device that can be safely and reliably installed in an appropriate location without the risk of draining, moving, or protruding.
[0010]
In addition to the aforementioned need for permanent or durable drainage of the anterior chamber in conditions such as glaucoma, there is an additional need for temporary anterior chamber drainage or decompression. For example, an increase in IOP over a short interval (1 hour to 2 weeks) may be present following many ophthalmic treatments, including cataract extraction and retinal detachment repair. In addition, it is advantageous for physicians to use shunting to temporarily control IOP in glaucoma before embarking on other surgical procedures for glaucoma that do not employ long-term shunting. You may find it. There is a need for a device to meet the need for short-term anterior drainage in these and similar situations.
[0011]
There is a further need to provide a delivery system that is particularly adapted for intact insertion of a transcorneal drainage device. Advantageously, such a delivery system can hold the drainage device firmly so that the drainage device can be installed by the surgeon. Such a delivery system further allows immediate release of the drainage device when it is inserted through the cornea. It is further desirable that the delivery system be constructed to avoid introducing any additional damage to the corneal epithelial tissue and the interstitial delicate tissue.

In one embodiment, aqueous humor can flow through an opening in the foot, enters a channel in the body, passes through it, exits through a slit in the head, and the surface of the cornea Flowing over. In one embodiment, the head and foot are formed integrally with the body. In other embodiments, the head, foot, or body can be made of a dewaterable polymer. In certain embodiments, the external surface of the head or foot can be configured to minimize cell adhesion or adhesion. In certain embodiments, the exterior surface of the body can be configured to promote tissue adhesion or fixation, or to have affinity for tissue. The foot can be specifically shaped to facilitate the introduction of the diversion channel through the cornea. In certain embodiments, the body has a smaller outer circumference than the head or foot. The elongate filter can be retained within the body channel by packing or any other suitable mechanism. The elongate filter can be placed at the proximal end of the body or any other location inside.
[0013]
In other embodiments, the system of the present invention can include an implant that can be placed across the cornea to drain the anterior chamber of the eye. The implant has a head, foot, a tubular conduit between the foot and the head with an internal channel in fluid communication with the anterior chamber, and to regulate the outflow of aqueous humor and to limit or prevent microbial ingress A filter can be included that can be stuffed inside the anterior chamber to minimize ingress or prevent passage of microorganisms.
[0014]
In still other embodiments, the system of the present invention can include a transcorneal branch, and can further include a delivery device for implanting the branch in the transcorneal location. In certain embodiments, a transcorneal flow channel that is implanted using a delivery device includes a head, a foot, a substantially cylindrical body between the head and the foot having a channel therethrough, and the channel. There may be a filter installed in the channel to regulate the flow rate of the aqueous humor and further limit the ingress of microorganisms. In certain embodiments, the delivery device can include a tip that is dimensioned to hold the shunt and to install the shunt for insertion through the outer surface of the cornea; A plunger that is slidable from a proximal position to a distal position, wherein sliding the plunger pushes away the shunt and further promotes the shunt through the outer surface of the cornea to the transcorneal position Can be included.
[0015]
It is a further object of the present invention to provide a method for reducing anterior chamber fluid pressure, thereby treating glaucoma and other abnormalities characterized by elevated anterior chamber pressure. These methods include the steps of providing a transcorneal shunt, providing a delivery device for installing the shunt in a transcorneal position, and applying an outer surface of the cornea to allow insertion of the shunt. A step of cutting through the guide hole, and a step of employing a delivery device for inserting the diversion channel into the corneal crossing position. In one embodiment of the invention, the provided diversion channel can have a substantially cylindrical body, head, foot, and filter. Yet another object of the present invention is to provide a method for temporary drainage of anterior chamber fluid, thereby reducing intraocular pressure. Temporary drainage can be made removable at the end of the temporary drainage cycle, or use a device that may be capable of in vivo degradation to be reabsorbed at the end of the temporary drainage cycle. For example, it is understood to be performed over a short period of time from one hour to several weeks. Such devices may serve for implantation following those treatments that may be followed by an increase in IOP, or may serve as a temporary correction of an abnormality characterized by increased IOP.
[0016]
The shunt flow according to the present invention is intended to solve the specific problems mentioned above that have persisted in ophthalmological techniques for the treatment of elevated IOP. First, the diversion channel, its delivery device, and how to use them are adapted for the installation of drainage systems across the transparent cornea, thereby avoiding the difficulties associated with subconjunctival or subcapsular drainage It is. Secondly, the outflow of aqueous humor is constantly regulated by the filter system without involving the wound healing mechanism, so on the one hand to avoid the risk of low tension and on the other hand to avoid inappropriate drainage, Predictable runoff rate can be calculated. Third, the filter provides a restrictive path to prevent bacterial entry. In addition, the slit that opens in the head is shaped and sized to prevent entry of bacteria. Furthermore, the head itself is assembled from materials that prevent cell adhesion, including microbial adhesions. Fourth, the device is made of a material that the cornea is well tolerated. The head and foot prevent cell adhesion and reduce scarring over the device, while the body is made of a material that promotes cell adhesion, thereby firmly anchoring the device in a transcorneal location. These and other objects, features and advantages of the present invention will become more apparent from the following discussion and the accompanying drawings in which like numerals represent like components.

The method for producing a shunt according to the invention includes by machining a tissue integration layer applied in a disposable mold or as a curable composition. For example, corneal implants or shunts can be used to form HEMA, methacrylic acid, dimethacrylate crosslinks, and a single partial silicon mold with a cavity formed by imprinting using a pre-shaped die. It can be cast from a mixture of free radical initiators. As an alternative, the corneal implant or shunt channel can be machined and subsequently a tissue integration layer can be applied to the external surface of the shunt channel. The tissue integration layer is a curable composition comprising a copolymer of HEMA and alkyl methacrylate, monomeric HEMA, dimethacrylate cross-linked product, free radical initiator, and volatile solvent. Other methods for manufacturing corneal implants or shunts according to these systems and methods are readily discernable by those skilled in the relevant art.
[0030]
The system and method of the present invention includes a shunt or drainage device that retains a shunt or other drainage device, installs the shunt or drainage device at a predetermined location adjacent to the cornea, and occupies a cross-corneal location A delivery device adapted to insert the tube across the corneal surface can be advantageously employed. In certain embodiments, the delivery device can include an insertion tip adapted to releasably hold the shunt and to install the shunt for insertion through the outer surface of the cornea. A slidable inserter from a proximal position to a distal position, wherein sliding the inserter from the proximal position to the distal position pushes the shunt away from the insertion tip and through the outer surface of the cornea An inserter can be further included to facilitate the shunt flow in the transverse position. Advantageously, guide holes or other small reaching wounds may be inserted prior to inserting a shunt or drainage device to reduce resistance when the delivery system is used to deliver the device to a predetermined cross-corneal location. Can be made in the corneal surface or can be extended in or through the corneal stroma. The delivery system according to the present invention, in certain embodiments, can be adapted to indicate to the surgeon that the shunt has been properly installed.

By referring to the drawings described above, a particular method for reducing anterior chamber fluid pressure in accordance with the present invention can be understood. In one implementation of the invention, a diversion channel is provided for draining the aqueous humor and a delivery device is suitably provided for inserting the diversion channel. The shunt channel can be adapted to drain the aqueous humor at a predetermined rate and to further prevent invasion of microorganisms. After appropriate anesthesia, the site is selected for insertion of the drainage channel. A guide hole can be created that extends across the outer surface of the cornea, can extend through the corneal stroma, and can extend into the anterior chamber. The dimensions of the guide hole are determined by the individual operator based on surgical judgment and the individual patient's anatomy. Any of a needle, trocar, scalpel, or a number of instruments familiar to ophthalmologists can be used to form a guide hole or similar insertion site. The shunt channel can be inserted into the delivery device by the operator, or the shunt channel can be pre-inserted into the delivery device during manufacture of the delivery device. While specific exemplary dimensions for shunt size are disclosed herein, it is understood that a range of shunt sizes can be utilized to match individual anatomical changes. . Various sizes of delivery devices can be provided to engage different sized shunts, or a single size delivery device is suitable for implanting all different sized shunts It will be further understood that In the case of a diversion channel fixed within the insertion tip of the delivery device, the surgeon advances the delivery device toward the outer surface of the cornea. When the delivery device reaches a predetermined position on the cornea, the diversion channel is promoted to the transcorneal position using the delivery device mechanism to advance and push the diversion channel. When the shunt channel is properly installed to extend through the cornea, aqueous humor can be drained onto the cornea surface. Proper placement of the shunt channel can be verified by the presence of a visible droplet of aqueous humor on the head of the implanted device.
[0039]
It is understood that such a device can be useful for implantation following a process that can be followed by an increase in IOP, or can serve as a temporary correction for an abnormality characterized by increased IOP. The For temporary correction following retinal surgery, cataract extraction, or other invasive ophthalmic surgery, the device is implanted for 2 hours up to a month or until the IOP is stable. In contrast, permanent or long-term implants using the device of the present invention are used in the treatment of glaucoma in diabetic patients.
[0040]
It will be understood that the specifications provided above are merely exemplary of the invention and specific exemplary embodiments, together with its drawings and description. It is further understood that changes and modifications can be made to the various components and structures and methods of the shunt and its delivery system without departing from the scope of the present invention. Rather, it is understood that the invention is defined by the appended claims.
