本発明は、血液浄化器に関し、さらに詳しくは血液透析療法等に利用され、血液浄化器内の中空糸膜有効長を増加させる血液浄化器に関する。

【従来の技術】
医療分野では、血液透析療法、血液濾過療法、血液透析濾過療法、血液吸着療法、および血漿交換療法等の血液浄化療法等に血液浄化器が使用されている。

ここで、血液浄化器は、血液の透析等を行うための中空糸膜の束と、この中空糸膜の束を収納する筒状のケースとを備えて構成されているものが一般的である。また、この血液浄化器において、血液と透析液との流れは、平行かつ同方向で流す並流操作よりも、平行かつ反対方向に流す向流操作のほうが、高い拡散除去効率を有することから、向流操作のほうが一般的となってきている。

一方、血液浄化器内において、血液や透析液は、流れ方向に圧力を損失するため、血液と透析液との流れが向流の場合、血液―透析液間の膜間圧力差は、血液浄化器長さ方向で大きく変化し、内部濾過(血液側から透析液側への正濾過、および透析液側から血液側への逆濾過)が生じる。

この内部濾過を意識的に促進させ、溶質除去効率の良い血液透析濾過(hemo dia filtration;HDF)による溶質除去をすることが血液浄化器に求められてきている。この内部濾過を促進するためには、血液や透析液等の流体の圧力損失を大きくすることが必要である。この圧力損失を大きくして、内部濾過を促進させる因子として、例えば、以下の因子が挙げられる。

血液を提供する患者を主因とする患者因子としては、ヘマトクリット(血液全体に占める赤血球の割合(%)、HCTと略す。)、総蛋白質濃度(Cpと略す。)等が挙げられる。また、流量因子として、血液流量(QBと略す。)、透析液流量(QDと略す。)等が挙げられる。さらに、中空糸膜の形状に起因する形状因子として、中空糸内径(Dと略す。)、中空糸膜有効長(Leffと略す。)、中空糸膜充填率(FDRと略す。)等が挙げられる。

以上に述べた因子のうち、圧力損失を大きくするために、最も有効であるのが、中空糸膜有効長(Leffと略す。)の増加である。この中空糸膜有効長(Leffと略す。)を増加させるため、例えば、筒状のケースをできる限り長くする長尺型の血液浄化器が提案されている(特許文献1参照)。

本発明は、このような従来の問題点を解消し、中空糸膜の束を収納するケースの大きさを大幅に変更することなく、中空糸膜有効長を増加させる血液浄化器を提供することをその課題とする。

この発明の前記課題を解決するための手段は、
筒状ケース内に装填され、前記筒状ケース内で一方向に血液を流通させる順方向流通中空糸の束および筒状ケース内で反対方向に血液を流通させる逆方向流通中空糸の束と、前記筒状ケースの長さ方向に直交する断面を閉塞するとともに、前記順方向流通中空糸の束および逆方向流通中空糸の束を別々に固定する血流方向変換部と、前記血流方向変換部および前記筒状ケースの端部内壁とで仕切られる空間と、前記順方向流通中空糸の束および前記逆方向流通中空糸の束に接触しつつ血液が流通する方向に対して透析液が向流する透析液流路とを備えて成り、前記順方向流通中空糸の束および逆方向流通中空糸の束の端面は、前記血流方向変換部の外側端面に露出し、前記筒状ケースに設けられた血液流入口から血液が流入する前記順方向流通中空糸の束は、前記筒状ケースに設けられた血液流出口に前記血液が流出する順方向流通中空糸の束又は前記逆方向流通中空糸の束を構成する本数よりも多い本数で構成されていることを特徴とする血液浄化器である。

【発明の実施の形態】
[第1実施形態]
本発明の第1実施形態に係る血液浄化器を図1により説明する。第1実施形態に係る血液浄化器1は、筒状ケース11内に装填され、筒状ケース11内で一方向に血液を流通させる順方向流通中空糸の束12、および筒状ケース11内で反対方向に血液を流通させる逆方向流通中空糸の束13と、順方向流通中空糸内を流通する血液を逆方向流通中空糸に導入する血流方向変換部14と、順方向流通中空糸の束12および逆方向流通中空糸の束13に接触しつつ血液が流通する方向に対して透析液が向流する透析液流路15とを備える。

筒状ケース11は、筒形状であればよく、その断面形状は特に制限されず、例えば、円形、楕円形、または三角形、四角形、五角形等の多角形状であってもよい。この筒状ケース11の両端部には、それぞれ血液流入口111および血液流出口112が設けられている。また、この筒状ケース11の側面部には、血液流出口112側に透析液流入口113が、血液流入口111側に透析液流出口114が設けられている。

順方向流通中空糸の束12および逆方向流通中空糸の束13を構成する中空糸は、血液を流通させることができる内部空間を有する中空円筒状の膜である。この中空糸には、無数の小さな孔が形成されており、その大きさにより、膜を通過する物質が選択される。一般的な孔の直径は、平均ポアサイズで30Å〜300Å(3nm〜30nm)程度である。

この中空糸は、厚み方向の構造が均一である均質膜でもよいし、厚み方向に孔径の異なる非対称構造(不均質)膜でもよい。非対称構造膜は、膜の平均厚みを厚くしても溶質透過性が低下しにくく、支持層により強度が高められるので均一膜より優れている。

この中空糸は、膜の平均厚みが厚くとも50μmである中空糸である。平均厚みを薄くすることにより膜の溶質透過性が高められる。平均厚みが50μmより薄ければ特に中空糸の内径および外径について制限はないが、中空糸膜の内径が100〜500μmの範囲にあることが好ましい。

この中空糸としては、セルロース膜に代表されるセルロース系膜、およびポリスルホン系膜およびポリエステル系膜に代表される合成高分子膜を選択することができる。中でも、ポリアリレート樹脂とポリスルホン樹脂とを含むポリエステル系ポリマーアロイ膜は、本来の性質として機械的強度、耐熱性、耐薬品性に優れるだけでなく、生体との適合性および物質の吸着能に優れるので好ましい。

本発明の第2実施形態に係る血液浄化器を図4により説明する。第2実施形態に係る血液浄化器2は、筒状ケース21内に装填され、筒状ケース21内で一方向に血液を流通させる順方向流通中空糸の束22、および筒状ケース21内で反対方向に血液を流通させる逆方向流通中空糸の束23と、順方向流通中空糸内を流通する血液を逆方向流通中空糸に導入する血流方向変換部24と、順方向流通中空糸の束22および逆方向流通中空糸の束23に接触しつつ血液が流通する方向に対して透析液が向流する透析液流路25とを備える。
