本発明は医療用内視鏡システム等の複数の被制御装置を制御するシステム制御装置に関する。

【従来の技術】
複数の被制御装置を制御するシステムとして、医療用内視鏡システムが挙げられる。
一般的な医療用内視鏡システムでは、内視鏡のほかTVカメラ装置、光源装置、モニタなど、また外科手術で使用される気腹装置、電気メスなどが用いられる。

従来これらは同時に使用され、各装置が個々に操作、制御されて煩雑となっていた。このため、特開平7−303654号公報において開示されるような、複数の装置を容易に操作、制御し、被制御装置の機能を表示する手段と、被制御装置を操作する手段を設けたシステム制御装置が使用されている。

この内視鏡システムの被制御装置の1つに気腹装置が含まれている。従来、気腹装置は腹腔内に送気を行って腹腔を膨らませるとともに、腹腔内圧力を監視し設定圧力値に基づいて制御するようになっている。

【発明が解決しようとする課題】
ところが上記気腹装置、およびそれを含むシステムでは、気腹開始時に炭酸ガスを送るための腹腔内挿入器具(気腹針等)の先端が腹腔内に到達したかどうか分からず、腹壁に刺さったままの状態で送気してしまい、皮下気腫等になる危険性があった。

また特開平7−327920号公報では、上記問題発生時に高い送気圧力が直に腹壁に加わるのを避けるため、送気圧力より低い基準値を設定し、基準値以上になった場合にバルブを閉めて送気口の圧力を送気圧力より低くなるようにした気腹装置を開示している。この場合、高い送気圧力が腹壁に加わるのは防げるが、気腹針が腹腔に到達していない状態での送気は行われ、またその状態の解除、報知等については特に述べられていなかった。

(発明の目的)
本発明は上記問題点に鑑み、気腹開始時の腹腔圧力値等から、送気が腹腔内で行われているかどうかを判別し、異常な状態が発生した場合にはその状態を検出して報知し、速やかにその異常な状態を解消できるようにするシステム制御装置を提供することを目的とする。

【課題を解決するための手段】
本発明は、複数の被制御装置を制御するシステム制御装置において、送気機能を有する被制御装置との通信を行うことにより、該被制御装置において測定された、送気開始時の腹腔圧力値および積算流量値を受信する通信手段と、閾値を示す関数としての、腹腔内圧の値と送気流量の値との相関を示す判別用データが予め保持されたデータ保持手段と、前記腹腔圧力値と前記積算流量値との相関を示す実測値データを算出し、該実測値データ及び前記判別用データに基づき、前記被制御装置の送気状態が正常であるか否かを判別する判別手段と、前記判別手段の判別結果に基づき、前記被制御装置の送気状態が異常な状態である場合に、該異常な状態を報知するための制御を行う制御手段と、を有することを特徴とする。

この構成によれば、被制御装置を用いての気腹動作の際、腹腔内挿入器具の先端が腹壁に刺さった状態での送気動作の状態を判別でき、異常な状態ではその報知によりユーザが速やかに解消することができ、正常な状態での送気動作を確保できる。

【発明の実施の形態】
以下、図面を参照して本発明の実施の形態を説明する。
(第1の実施の形態)
図1ないし図6は本発明の第1の実施の形態に係り、図1は第1の実施の形態を備えた医療用内視鏡システムの全体構成を示し、図2は図1における主要部の構成を示し、図3は気腹装置の構成の詳細を示し、図4は気腹装置操作画面の具体例を示し、図5は正常な状態と異常な状態で気腹した場合の圧力と流量の概略の関係を示し、図6は異常な気腹状態と判断した場合に報知するメッセージの具体例を示す。

本実施の形態では、複数の被制御装置を有するシステムの具体例として、医療用内視鏡を備えた医療用内視鏡システムで構成及び動作を説明する。
図1に示す医療用内視鏡システム1は手術ベッド2に横たわる患者3の両側に配置された第1のカート4a及び第2のカート4bに複数の被制御装置としての内視鏡周辺装置が搭載されている。

本実施形態では、第1のカート4aには内視鏡用カメラ装置(或いはカメラコントロールユニット)5a,5b、光源装置6a,6b、画像処理装置7、VTR8、モニタ9a、システムコントローラ11、集中表示パネル12及びこれらの装置における複数の装置(例えば内視鏡用カメラ装置5a,5b、光源装置6a,6b、画像処理装置7、VTR8、システムコントローラ11、集中表示パネル12)がそれぞれシリアルインタフェースで接続され、双方向の通信を行う第1の中継ユニット13が搭載されている。
また、内視鏡用カメラ装置5a,5b及び光源装置6a,6bは内視鏡14a、14bにそれぞれ接続される。

また、第2のカート4bには高周波電源装置(本実施の形態では電気メス装置と略記)17、気腹装置(気腹器ともいう)18、モニタ9b、集中操作パネル19及びこれらの装置における複数の装置(例えば電気メス装置17、気腹装置18、集中操作パネル19)がそれぞれシリアルインタフェースで接続され、双方向の通信を行う第2の中継ユニット20が搭載されている。

各カート4a、4bには、電源と各内視鏡周辺装置の間を電気的に絶縁する絶縁トランス21,22がそれぞれ設けられ、カート4a、4bに搭載された各内視鏡関周辺装置に電源を供給している。但し、前記光源装置6a,6b及び電気メス装置17は消費電力が大きいため、この絶縁トランス21,22を介さずに直接手術室の壁のコンセントから商用電源を供給する構成になっている。

そして、前記第1のカート4aの中継ユニット13と、第2のカート4bの中継ユニット20とは、中継ケーブルユニット23により接続されて2つのカート4a、4b間で双方向の通信を行う通信手段が形成されている。

また、各内視鏡周辺装置の動作状態を集中表示する集中表示パネル12と、また遠隔的に集中操作する集中操作パネル19とは中継ユニット13等を介してシステム全体の制御を行うと共に、通信手段を兼ねるシステムコントローラ11に双方向の通信を行うことができるように接続され、このシステムコントローラ11で各内視鏡周辺装置の動作を集中制御していると共に、集中操作パネル19による操作に対応した制御も行うことができるようにしている。

上記集中操作パネル19は、液晶ディスプレイ等の表示部と、この表示部の上に一体的に設けたタッチセンサとにより構成され、各装置の状態表示や操作スイッチなどを設定画面として表示する表示機能と共に、タッチセンサの所定領域を触れることで操作スイッチによる操作を行う操作機能を有し、この遠隔的な操作でシステムコントローラ11を介して各内視鏡周辺装置の操作を直接行うのと同等の操作を行うことができるようにしている。

このように、複数の内視鏡周辺装置を備えた内視鏡システム1によって、内視鏡14a等により被処置部位の観察を行いながら、例えば電気メス装置17によって切除処置などの各種処置が行えるようになっている。そして、複数の装置を同時に使用する本内視鏡システム1では、手術を行う執刀医、看護婦などそれぞれの操作者に適した操作環境が得られるように、図1に示すように複数の装置を集中的に操作及び制御等ができるように構成している。

なお、カート4a,4bの上段には例えばCRTなどからなる画像表示装置としてのモニタ9a,9bをそれぞれ固定するための台座24a,24bが設けられており、カート4a,4b本体に対して回動自在に配置されている。

また、モニタ9aが載置される台座24aの側端部に集中表示パネル12が支柱25を介して固定されている。この集中表示パネル12を支持する支柱25は、図1では集中表示パネル12の裏側に位置し、図示しない蝶番などにより向きを自在に変更可能なアングル機構を有しており、モニタ9aと独立して集中表示パネル12の表示面の向きを変えられるようになっている。

さらに、前記カート4bの側面部には、集中操作パネル19を固定するための一対のスライドレール26が上下方向に配設されており、この集中操作パネル19の両端部がスライドレール26に係合して保持されている。
これにより、集中操作パネル19はスライドレール26上を任意の位置に上下方向に移動できるようになっている。

図1では手術ベッド2に取り付けた内視鏡支持アーム27に一方の内視鏡14bを把持している。また、この手術ベッド2には、各内視鏡周辺装置の機能のうち所定の機能のみを操作するための第2操作パネル28が設けられ、ケーブルを介してシステムコントローラ11に直接接続されている。

また、患者3には図1の主要部をより詳細に示す図2で示すように腹腔29内に手術器具を導入するためのガイド管(以下、トラカールと称する)31が刺し込まれ、内視鏡14aの硬性の挿入部が挿入される(図2では一方の内視鏡14aのみで示すが、他方の内視鏡14bもトラカール31を介して刺し込まれる)。

この内視鏡14aは硬性の挿入部内に照明光を伝送するライトガイドと、挿入部の先端に設けた対物レンズによる像を挿入部の後端側に設けた接眼部32側に伝送するリレーレンズ系とが挿通されている。

また、この接眼部32にはカメラアダプタ33を介してCCD(電荷結合素子)等の固体撮像素子を内蔵したカメラヘッド34が装着され、この固体撮像素子と接続された信号線を挿通したカメラコード35がカメラヘッド34から延出され、このカメラコード35は固体撮像素子を駆動する駆動系と映像信号を生成する映像信号処理系とを内蔵した内視鏡用カメラ装置5aに接続される。

また、内視鏡14aはライトガイドが挿通されたライトガイドケーブル36を介して光源装置6aに接続され、光源装置6a内のランプで発生した照明光をライトガイドケーブル36内のライトガイドを介して内視鏡14a内のライトガイドに供給し、このライトガイドを介してその先端面から伝送した照明光を出射し、腹腔29内の臓器等の検査部位を照明する。

そして、対物レンズにより照明された検査部位の像を結び、その像をリレーレンズ系で後方に伝送し、カメラヘッド34内の固体撮像素子に結像し、この固体撮像素子で光電変換した画像信号をカメラコード35を介して内視鏡用カメラ装置5aに送る。そして、映像信号に変換し、モニタ9a等で表示できるようにしている。

また、上記トラカール31とは別のトラカール37の挿通孔38には高周波処置具39が挿通されている。このトラカール37には気腹を行うために挿通孔38の深部側に通じる気腹通路40が設けてあり、この気腹通路40の外部に開口する口金には気腹チューブ41の一端が接続され、この気腹チューブ41の他端は気腹装置18に接続される。

なお、図1及び図2では気腹装置18に接続されたトラカール37には高周波処置具39が挿通されて高周波切除、凝固処置(電気メスによる処置)がされる状態を示しているが、この高周波処置具39が挿通される前に、トラカール内針と共に刺入されて気腹されたり、図3に示すように、気腹針53が刺入されて気腹される。

上記高周波処置具39はアクティブコード42を介して電気メス装置17のアクティブ電極43に接続される。また、患者3に広い面積で接触した対極板44は電気メス装置17の患者電極45に接続される。

この電気メス装置17は高周波を発生する高周波発生回路(HF発生回路)46と、この高周波を電力増幅するHF出力アンプ47とを内蔵し、このHF出力アンプ47の2つの出力端がアクティブ電極43と患者電極45に接続されている。

また、上記気腹装置18はバルブユニット50を内蔵し、このバルブユニット50の供給側口金はガスチューブ51を介して二酸化炭素等のガスを圧縮して充満したガスボンベ52にに接続されている。

ガスボンベ52内の二酸化炭素ガスは、バルブユニット50を介してトラカール37に送られ、さらに気腹通路40を介して腹腔29内に供給することで腹腔29内を膨らませて、腹腔29内の手術のための作業空間、内視鏡14aの視野及び高周波処置具39の視野を確保する。この時、腹腔29内への二酸化炭素ガスの供給量はバルブユニット50によって制御される。

図3は内視鏡システム1で制御される気腹装置18の構成を詳細に示すものである。
気腹装置18のバルブユニット50は一端がガスチューブ51を介してガスボンベ52に接続され、他端には気腹チューブ41を介して気腹針53に接続される。ガスボンベ52はガスチューブ51、管路55を介して減圧器56、圧力センサ57に接続される。減圧器56の出口は(管路55を介して)減圧器58に接続され、同様にこの減圧器58はバルブ59に接続される。

バルブ59は3つに分岐した管路に設けたバルブ60、61、62にそれぞれ接続される。バルブ60、61、62は内部の径がそれぞれ異なり、バルブを介することで異なる流量に制限する。バルブ62は圧力センサ63、64に接続され、またバルブ65に接続される。バルブ60、61、65の出口の管路は1つの管路に合流し、更に2つに分岐されたバルブ66と口金67に接続される。

また、本実施の形態では気腹装置18と双方向の通信を行い、この気腹装置18を制御するシステムコントローラ11は気腹装置18に接続された腹腔内挿入器具としての気腹針53等を刺し込み、腹腔29内に送気して気腹する気腹開始時(或いは送気開始時)に、気腹装置18の制御部68に対し、腹腔圧力値及び流量値(或いは積算流量値)の送信の指示信号を送り、制御部68を介して送られる腹腔圧力値及び流量値(或いは積算流量値)を受信して、気腹開始時における腹腔圧力値及び流量値(或いは積算流量値)の変化の状態をモニタし、気腹が正常に行われている正常な状態か、異常な状態を判別するチェック機能を設けている。

そして、異常な状態と判別した場合にはその異常な状態であることをユーザに報知する手段を形成している。

このため、例えば図4に示す気腹装置操作画面71には気腹装置18には本来設けてない「送気開始チェック」スイッチ73を設けている。そして、気腹針53を刺し込み、気腹装置18により送気を開始する時、この気腹装置操作画面71で「送気開始チェック」スイッチ73をONにしておくことにより、気腹装置18で検出された圧力値等のデータがシステムコントローラ11に送られる。

システムコントローラ11は、気腹針53の先端が正常に腹腔29内に到達して送気を行った場合に想定される腹腔内圧と送気流量の正常な増加率のデータと、腹腔29に達しない状態等で想定される腹腔内圧と送気流量の異常な増加率のデータとを判別する閾値の増加関数データを判別用データとして予め保持し、この判別用の増加関数データと実際に送られてくるデータとを比較する等して正常な状態か、異常な状態を判別する。そして、異常な状態と判別した場合にはその異常な状態であることを、例えば気腹装置操作画面71上で表示してユーザに報知するようにしている。

なお、図4の気腹装置操作画面71の下端には複数の操作タグ74が表示されるようになっている。これら操作タグ74は、この内視鏡システム内の各装置の操作画面を呼び出すためのスイッチであり、「観察装置1」スイッチ74a、「気腹器」スイッチ74b、「電気メス」スイッチ74cなどのタグスイッチを押すことにより、各スイッチに対応する装置の操作画面が表示されて、詳細な設定を行うことができるようになっている。また、「メニュー」と書かれている「メニュー」スイッチ75を選択することによってメニュー画面にすることができる。

次に本実施の形態の作用を説明する。
まず、医療用内視鏡システム1を構成する気腹装置18において、ガスボンベ52から二酸化炭素ガスが送られる。ガスボンベ52の出力は満タン状態で、5、6MPaの高圧であるため、減圧器56、58により減圧する。また圧力センサ57からガスボンベ52の圧力を検出し、制御部68でガスボンベ52の内容量を判断し、表示するようになっている。

バルブ60、61、62はそれぞれ径が異なり、気腹装置18のフロントパネル69(詳細は、図示せず)に設けられた腹腔圧設定スイッチ、流量設定スイッチで設定した値に応じて、制御部68によってこれらを切り替えることで、腹腔29内が設定された圧力になるように送気する。

腹腔29内の圧力(つまり腹腔内圧)の測定は圧力センサ63、64で行う。腹腔内圧が設定圧力より高い状態が続くと、保護回路が働き、開状態のバルブ60(或いは61,65)を閉じて送気を停止する。

実際に患者3の腹部に気腹装置18に接続された穿刺針53を刺し込み、送気して気腹を開始する場合には、図4の気腹装置操作画面71に設けた「送気開始チェック」スイッチ73をONにして、送気スイッチ72bを操作して送気開始を行う。

このようにして、ユーザは気腹針53が腹腔29内に到達し正常に送気されている状態かどうか、容易に確認することができる。また異常な状態を確認することにより、正常に送気できる状態へ簡単に直すことができ、正常な状態で送気を続行して、設定値に設定できる。
このため、正常な送気状態で送気を確実に行うことができる。

また、本実施の形態によれば、気腹装置18自体には気腹開始時の送気状態をチェックする手段を備えていない場合にも、システムコントローラ11側にその機能を設けることにより、気腹装置18にその手段を設けたのと同様に遠隔的に操作及び制御できる。

なお、正常な状態で設定値の腹腔内圧に設定した場合には、気腹装置18による気腹動作を終了して送気を一旦停止し、その後は腹腔内圧を設定値に保持する動作を行う。そして、腹腔内圧を設定値に保持して視野が確保された状態での内視鏡検査とか、高周波処置具39での処置を行うことができる。
この場合、腹腔内圧が設定値から下がると、気腹装置18を動作させて送気を行い、設定値に保つようにする。

上述の説明では、気腹針53により穿刺した状態で送気して気腹する場合を主に説明したが、これに限らず、図2のようにトラカール37自体に気腹通路40を設けたものの場合にはこのトラカール37の挿通孔38にトラカール内針を挿通して送気したり、先端が尖った高周波処置具39を挿通して送気したりする場合にも適用できる。

なお、本実施の形態では送気開始時に正常に送気或いは気腹が行われているか否かの判断をシステムコントローラ11で行うようにしているが、図7に示す変形例の気腹装置18′のように、気腹装置18′自体に送気開始のチェック機能を持たせても良い。
