【発明の属する技術分野】
本発明は、X線コンピュータ断層装置(以下、「X線CT装置」と称する。)に関し、血液等の流速測定、或いはリアル・プレップスキャン対応のX線CT装置に関する。

【従来の技術】
X線CT装置は、検体を透過したX線の強度に基づいて、検体についての情報を画像により提供するものであり、疾病の診断、治療や手術計画等を初めとする多くの医療行為において重要な役割を果たしている。このX線CT装置による断層画像の撮影形態は、一回転のスキャンで検体についての複数枚の投影データを収集可能なマルチスライススキャン、検体を螺旋状にスキャンして広範囲の投影データを得るヘリカルスキャン等種々のものが存在する。

近年のマルチスライスX線CT装置では、造影剤を使用する検査において最適なスキャンタイミングを得るために、リアル・プレップスキャンという機能が提案されている。これは、プリスキャンにより血管内のCT値を連続的に測定し、所定の設定値(閾値)を越えたタイミングで本スキャンを開始することで、造影剤効果が十分反映されたCT画像を取得するための手法である。また、このプリスキャンにおける血管内のCT値測定により、血液の流速を測定することも可能である。従来のX線C装置では、表示画像のCT値の変化に基づいて本スキャン等のタイミングが計られている。

しかしながら、一般に、血流の状態は、患者の個体差や疾病によって異なる。従って、表示画像のCT値の変化を測定対象とするのでは、最適なタイミングと取ることができない場合がある。この場合、患者の個体差や疾病等に応じてCT値或いは他の測定対象を選択でき、これらに基づいて高い時間分解能でタイミングをとることが望ましい。

また、より時間分解能を向上させることは、より精度の高いリアル・プレップスキャンを実現するための重要な要因の一つである。

【発明が解決しようとする課題】
本発明は、上記事情を鑑みてなされたもので、患者の個体差や疾病に関わらず、適切に血液等の流速を測定でき、また、より高い時間分解能にて血液の流速等を把握し、リアル・プレップスキャンにおいて、本スキャンを最適なタイミングで実行することができるX線CT装置を提供することを目的としている。

【課題を解決するための手段】
本発明は、上記目的を達成するため、次のような手段を講じている。

請求項1に記載の発明は、血液中に造影剤を注入された被検体に、X線を曝射するX線曝射手段と、被検体の体軸方向に少なくとも二列以上配列された検出素子列を有し、前記被検体を透過したX線を検出し投影データを発生するX線検出手段と、画面上のCT画像に所望する指定領域を入力するための入力手段と、前記二列以上の検出素子列のうち、一列以上の間隔を開けた異なる第1の検出素子列と第2の検出素子列とが発生した前記投影データにおいて、前記指定領域に対応する指定投影データを特定する特定手段と、前記指定投影データを用いて前記指定領域内への造影剤の流入を判定し、前記第1の検出素子列と前記第2の検出素子列との間における造影剤流入時刻の時間差を取得する計測手段と、前記時間差と前記二つの検出素子列間の距離とに基づいて、血流速度を演算する演算手段と、を具備することを特徴とするX線CT装置である。

このような構成によれば、患者の個体差や疾病に関わらず、適切に血液等の流速を測定でき、また、より高い時間分解能にて血液の流速等を把握し、リアル・プレップスキャンにおいて、本スキャンを最適なタイミングで実行することができるX線CT装置を実現することができる。

【発明の実施の形態】
以下、本発明の実施形態を図面に従って説明する。なお、以下の説明において、略同一の機能及び構成を有する構成要素については、同一符号を付し、重複説明は必要な場合にのみ行う。

まず、本実施形態に係るX線CT装置10の概略構成を、図1を参照しながら説明する。

なお、X線CTスキャナ装置には、X線管球と検出器システムとが1体として被検体の周囲を回転する回転/回転(ROTATE/ROTATE) タイプ、リング状に多数の検出素子がアレイされ、X線管球のみが被検体の周囲を回転する固定/回転(STATIONARY/ROTATE) タイプ、電子ビームを偏向させることで電子的にX線源の位置をターゲット上で移動するタイプ等様々なタイプがあり、いずれのタイプでも本発明を適用可能である。ここでは、現在、主流を占めている回転/回転タイプのX線CT装置を例として説明する。

また、1ボリュームのボクセルデータ(又は1枚の断層像)を再構成するには、被検体の周囲1周、約360度分の投影データが、またハーフスキャン法でも210度〜240度程度分の投影データが必要とされる。本発明の技術的思想はいずれの方式にも本発明を適用可能であるが、ここでは、一般的な前者の約360度分の投影データから1ボリュームのボクセルデータ(又は1枚の断層像)を再構成するものとして説明する。

図1は、X線CT装置10のブロック構成図を示している。また、図2は、図1のX線CT装置10による断層像撮影形態の斜視図を示している。図1に示すように、X線CT装置10は、装置本体100と画像処理装置200とからなる。それぞれは、次に述べる構成を有する。

(装置本体)
装置本体100は、X線管球101、回転リング102、二次元検出器システム103、データ収集回路(DAS)104、非接触データ伝送装置105、前処理装置106、架台駆動部107、スリップリング108、高電圧発生装置109、ホストコントローラ110、記憶装置111、補助記憶装置112、データ処理装置113、再構成装置114、入力装置115、CT監視部116、画像処理部118、表示装置119、データ/制御バス300を有している。また、データ/制御バス300を介して、当該装置本体100には外部の画像処理装置200が接続されている。

X線管球101は、X線を発生する真空管であり、回転リング102に設けられている。当該X線管球101には、X線の曝射に必要な電力が高電圧発生装置109からスリップリング108を介して供給される。X線管球101は、供給された高電圧により電子を加速させターゲットに衝突させることで、有効視野領域FOV内に載置された被検体Pに対してコーン状のX線を照射する。

回転リング102には、X線管球101及び検出器システム103が設けられている。回転リング102は、架台駆動部107により駆動され、X線管球101及び検出器システム103とともに1回転あたり1秒以下という高速で被検体の回りを回転する。

二次元検出器システム103は、被検体Pを透過したX線を検出する検出器システムであり、X線管球101に対向する向きで回転リング102に取り付けられている。当該二次元検出器システム103には、シンチレータとフォトダイオードとの組み合わせで構成される複数の検出素子が、被検体の体軸方向とそれに直交するチャンネル方向とに関して2次元状に配列されている。例えばチャンネル方向に関しては1,000個(1,000チャンネル)程度の検出素子が配列されている(以下、この1,000個の検出素子が配列された一の列を「検出素子列」と称する)。また、体軸方向に関しては、有効視野領域FOVを体軸方向に拡大するために、例えば250列程度の検出素子列が、30cmの範囲に高密度に配列されている。これにより、チャンネル方向と体軸方向との両方に高空間分解能が実現されている。当該構成を有する二次元検出器システム103によれば、後述するように一列以上の間隔を開けた異なる二列以上の検出素子列を使用して、精度の高い血流速度を計測することが可能である。

なお、二次元検出器システム103のスイッチ群の各スイッチ切り換え制御は、スキャン条件に基づいて、ホストコントローラ110によって実行される。すなわち、ホストコントローラ110は、二次元検出器システム103が有する各検出素子とデータ収集素子との接続状態を切り換え、各検出素子で検出されたX線透過データを所定の単位で束ねる。そして、スキャン条件に対応した複数スライスのX線透過データとして後段のデータ収集回路104に送り出し、所定の処理を実行する。

データ収集回路(DAS)104は、複数のDASチップを有し、2次元検出器システム103で検出されたM×Nの全チャンネルに関する膨大なデータ(1ビューあたりのM×Nチャンネル分のデータを以下“投影データ”という)を入力し、増幅処理、A/D変換処理等の後、一括して光通信を応用した非接触データ伝送装置105を介して固定側のデータ処理ユニットに伝送する。

非接触データ伝送装置105は、収集したX線透過データを光学的に次段装置に伝送する。当該非接触データ伝送装置105やデータ収集回路104等については、2次元検出器システム103において、膨大且つ高速に発生する2次元投影データを時間遅れなく伝送するための超高速処理化が図られている。

架台駆動部107は、診断用開口内に挿入された被検体の体軸方向に平行な中心軸のまわりに、X線管球101と二次元検出器システム103とを一体で回転させる等の駆動制御を行う。

高電圧発生装置109は、X線の曝射に必要な電力をスリップリング108を介してX線管球101に供給する。X線管球101に高電圧を供給する装置であり、高電圧変圧器、フィラメント加熱変換器、整流器、高電圧切替器等から成る。この高電圧発生装置109によるX線管球101への高電圧供給は、スリップリング108により行われる。

ホストコントローラ110は、撮影処理、データ処理、画像処理等の各種処理に関する統括的な制御を行う。例えば、撮影処理においては、ホストコントローラ110は、予め入力されたスライス厚等のスキャン条件を内部メモリに格納し、患者ID等によって自動的に選択されたスキャン条件(あるいは、マニュアルモードにおいて、入力装置115から直接設定されたスキャン条件)に基づいて、高電圧発生装置109、図示しない寝台駆動部、架台駆動部107、及び寝台の体軸方向への送り量、送り速度、X線管球101及び二次元検出器システム103の回転速度、回転ピッチ、及びX線の曝射タイミング等を制御し、被検体の所望の撮影領域に対して多方向からコーン状のX線ビームを照射してX線CT画像の撮影処理を行う。

また、ホストコントローラ110は、データ処理装置113からのX線強度上昇の時間情報、或いはCT値監視部116からのCT値上昇の時間情報と、二次元検出器システム103の検出素子列の幾何学的情報と、に基づいて、血液の流速を計算する。

さらに、ホストコントローラ110は、データ処理装置113が検知したX線強度上昇、或いはCT値監視部116が検知したCT値上昇をトリガとして、モニタスキャンから本スキャンへ移行し、X線管球101から診断用CT画像撮影のためのX線を曝射させる。

また、データ処理装置113は、少なくとも二以上の検出素子列に関する再構成前の二次元投影データの特定の領域において、検出X線の強度(カウント)を監視し、後述するような強度の変化を検出した場合には、その変化のタイミング(時間情報)をホストコントローラに送り出す。

再構成装置114は、データ処理装置113によりデータ処理されて得られた投影データを再構成処理して、所定のスライス分の再構成画像データを生成する。なお、当該再構成装置114は、一枚の断層像の再構成に必要な多方向の投影データを収集するのに要する時間より、短時間で多方向の投影データから断層像を再構成する、所謂リアルタイム再構成を行う。

(画像処理装置)
再構成、断面変換などのデータ処理及び表示オペレーションは、X線CT装置10内で行われるのが一般的である。しかし、外部の画像処理装置200において実行するようにしてもよい。外部の画像処理装置200を使用する場合、X線CT装置本体100から、画像処理装置200に送られるデータは、再構成前でも、再構成後でも、データ処理後の表示直前でも、いずれの状態でも本実施形態の効果を妨げるものではない。

画像処理装置200は、補助記憶装置201、データ処理装置202、再構成装置203、入力装置204、表示装置205、CPU206、CT値監視部116、画像処理部118を有している。各構成要素の機能は、X線CT装置本体に設けられたものと略同一である。

前処理装置106で感度補正やX線強度補正等を受けた360゜分の2次元投影データは、直接あるいは記憶装置111に一旦記憶された後、再構成装置114に送られて、二次元画像再構成、或いはFeldkamp法に代表される3次元画像再構成アルゴリズムによる再構成を受け、体軸方向を横切る複数断面毎のX線吸収系数の2次元的分布データ、或いは体軸方向に広い対象領域(ボリューム)内におけるX線吸収系数の3次元的分布データ(ボクセルによる三次元ボリュームデータの集まりであり、「ボクセルボリュームデータ」と称される。)に再構成される。

再構成されたデータは、直接あるいは記憶装置111に一旦記憶された後、画像処理装置118に送られて、オペレータの指示に基づき、任意断面の断層像、任意方向からの投影像、3次元表面画像等のいわゆる疑似3次元画像データ等、表示の為の画像処理が施され、表示装置119に表示される。

また、本X線CT装置10では、本スキャン及びプリスキャンによる撮影が可能である。本スキャンとは、関心部位についての診断用画像データを収集するためのスキャンであり、プリスキャンとは、本スキャン開始の適切なタイミングを計るために、本スキャン前に比較的低いX線強度で実行されるスキャンである。さらに、本X線CT装置10では、これらを組み合わせたリアルプレップスキャンによる撮影が可能である、これは、プリスキャン及びリアルタイム再構成により、リアルタイムで表示される関心領域の断層画像から造影剤流入タイミングを把握し、当該タイミング把握をトリガとして本スキャンを実行する手法である。本X線CT装置10によるリアルプレップスキャンについては、後で詳しく説明する。

次に、本X線CT装置10が有する各種血液等の流速測定機能について説明する。

(CT値の上昇タイミングに基づく流速測定)
図4は、本X線CT装置10により、図2に示した形態で、表示画像のCT値上昇に基づいて血液の流速測定を行う場合を説明するための概念図である。

図4左側に示すように、スライス方向に配置された血管において、造影効果のある血液が矢印で示す方向に流れている場合、CT値監視部116は、最初に第1の検出素子列103aにおいて造影効果によるCT値の上昇を判定し、その所定時間後、第4の検出素子列103dにおいて造影効果によるCT値の上昇を判定する。なお、一般には、各列のCT値の計測は随時実行され、当該計測の結果に基づいてCT値の上昇を判別する。

すなわち、所定の時間間隔Δtで画像を収集している場合、CT値監視部116においては、図4右側下段に示すように第1の検出素子列103aではn番目の表示画像データにおいてCT値上昇が判定され、一方、第4の検出素子列103dでは、n+N番目の表示画像データにてCT値上昇が判定される。当該CT値上昇の判定は、例えば所定のCT値が所定の閾値以上になった場合に、CT値上昇と判定する構成が考えられる。

なお、一般にX線CT装置においては、再構成されたCT画像データを全て画像処理し表示画像とする訳ではなく、例えば一定の周期ごと等の所定の条件を満たす画像データのみ表示画像とされ、表示される。

当該血液の流速測定は、検出器システム103の検出素子列が二列以上であれば可能である。しかしながら、より精度の高い流速測定を実現するためには、検出素子列の数が多く、且つ、少なくとも隣り合っていない二つの検出素子列におけるCT値の変化を測定することが好ましい。

(中間断層画像領域を使用した血液の流速測定)
上述した流速測定機能は、ユーザに提供する表示画像領域でのCT値の上昇に基づいて、血液の流速を測定するものであった。これに対し、本機能は、表示画像以外の画像領域におけるCT値の上昇に基づいて、血液の流速を測定するものである。

すなわち、上述の如く、X線CT装置においては、収集された断層画像は、全て表示画像として表示される訳ではない。本機能では、表示画像として使用しない中間的な断層画像領域(具体的には、例えば、画像処理部118に入力されていない、再構成後の断面毎のX線吸収系数の2次元的分布データからなる画像。以下、この画像を「中間画像」と称する。)におけるCT値上昇を判定し、血液等の流速の測定に使用することで時間測定の分解能を向上させ、高精度の流速測定を可能にするものである。

図5は、中間画像を使用した血液の流速測定機能を説明するための図である。一般に、収集した投影データから表示画像を生成するまでには、複数段階の処理に対応した中間状態の画像が存在する。同図5では、投影画像、中間画像、表示画像と3段階に分けた例を示している。

図5の第1段階において、プリスキャンにて各投影データ(再構成処理前のデータであり、「生データ」と呼ばれることもある。)が収集され、続いて、第2段階において、投影データを再構成して中間画像が生成される。当該中間画像は、例えば反転表示やウィンドウ変換前等の段階にあるものである。CT値監視部116は、これらの中間画像を入力し、指定された領域内のCT値の変化を監視し、CT値が上昇したと判定した場合には、当該上昇のタイミングをホストコントローラ110に出力する。一方、中間画像のうち、所定周期のもののみ第3段階において所定の画像処理を受け、表示画像としてリアルタイム表示される。
