【発明の属する技術分野】
本発明は、2成分混合型プレフィルドシリンジに関する、さらに詳しくは、混合すると不安定な薬剤を予めバレル内で分離して保存しておき、用時混合して使用する2成分混合型プレフィルドシリンジに関する。

【従来の技術】
混合あるいは溶解した場合に、製剤的、薬効的に安定性を保つことの困難な薬剤は従来、使用直前までアンプルやバイアルなどに別々に保存しておき、用時にシリンジや移注針などを用いて調製し使用していた。しかし、この方法は操作が煩雑であるため調製に手間と時間を要する上に、空中浮遊細菌の侵入のおそれがあり、またアンプルにおけるガラス片や、バイアルにおけるゴム片などのような異物が混入するおそれが高かった。そこで近年、このような問題を解決するものとして、2種類の異なる薬剤を同一のシリンジ内に互いに分離して保存しておき、用時混合して用いる、いわゆる2成分混合型プレフィルドシリンジが実用化されている。

現在市販されている2成分混合型プレフィルドシリンジは、概ね図6に示すような構造(従来例1)をしている。すなわち、バレル13内を第1ガスケット14と第2ガスケット15で2つの空間に隔て、バレル13の中央部内壁にバイパス溝131を凹設し、プランジャー16を押し込んだとき、後方空間17の液剤18(例えば溶解液)がバイパス溝131を通って前方空間19に進入し、固形製剤20(例えば抗生物質)と混合されるようになっている。
また、バレル内をガスケットで分割しバイパス溝を設けないものとしては、特開2001−104482号(従来例2)や特開平10−337321号公報(従来例3)に記載されたものがある。
従来例2は、図7に示すように、プランジャー21を押すと、第1のガスケット22に設けられた中空針23が第2のガスケット24を貫通することにより、後方空間25に収納された液剤26(例えば溶解液)が前方空間27に進入し、固形製剤28(例えば抗生物質)と混合されるようになっている。
従来例3は、図8に示すように、第1のシリンジ29と第2のシリンジ30との二重構造からなり、第1のガスケット31内で第2のシリンジ30を軸回転させることで図8(b)に示すように第1のガスケット31に設けられた通液孔311と、第2のシリンジ30の通液孔301を重ね合わせ連通路32を開通させることにより、第2のシリンジ30に収容された液剤33(例えば溶解液)が第1のシリンジ29内に進入し、固形製剤34(例えば抗生物質)と混合されるようになっている。

【発明が解決しようとする課題】
しかるに、上記の従来例1、2、3では次のような問題があった。
従来例1では、薬剤の混合時にプランジャー16を誤って強く押しすぎて第2ガスケット15がバイパス溝131の部分を通り過ぎてそれよりも前方に急速に移動してしまうと、液剤18が前方空間19に完全に流入しないうちにバイパス溝131が閉鎖されてしまい、液剤18と固形製剤20との混合が完全に行われないというトラブルを発生し易く、使用時に注意を払う必要があり、取り扱いの容易性の点で問題がある。また、ガスケットに含まれている添加剤、例えば硫黄や加硫促進剤などが保存中に薬液中に溶出する虞がある。さらに、薬剤の混合が完了した後、さらに別の薬液(例えばビタミン溶液)を吸引するべく第2ガスケット15を引いた際には、第1ガスケット14もそれに伴い摺動し、さらにバイパス溝131が設けられているため、薬液は前方空間19と後方空間17の両方に吸引されてしまい、正確な量を吸引できないという問題がある。
従来例2では、バイパス溝は設けていないため、従来例1のような注意を使用時に払う必要はないが、ガスケットからの溶出の問題は残っており、また、薬剤の混合後さらに別の薬液を吸引する際に、前方空間27と後方空間25は、第1のガスケット22に設けられた中空針23が第2のガスケット24を貫通することによって連通されているため、プランジャー21を引いても、薬液は前方空間27と後方空間25それぞれに吸引されるため、従来例1と同様、定量的な吸引ができない。さらに、中空針23で第2のガスケット24を穿刺した場合に生じるコアリングが不可避であり、コアリングにより薬液中に異物が混入するという問題がある。
従来例3では、液剤33と固形製剤34を混合した後に第2のシリンジ30を再び軸回転させ、連通路32を閉じた後に第2のシリンジ30を引くことで、通常のシリンジの場合と同様、定量的な薬液の吸引が可能である。しかし、第1のガスケット31に設けられた通液孔311と、第2のシリンジ30の通液孔301の重ね合わせは容易ではなく、薬液を吸引するためには、第2のシリンジ30を再び回転させなくてはならないという独特の操作が加わるため、従来例1と同様に取り扱い性の点で問題がある。さらに、製造時に第1のガスケット31への第2のシリンジ30を挿入するのは容易ではなく、また、第1のガスケット31と第2のシリンジ30の嵌合面の液密性、ならびに第2のシリンジ30の回転性を同時に満たすことは困難である。

本発明は、如上の事情に鑑みてなされたもので、薬剤を混合する際の取り扱い性に優れ、コアリングが生じず、実質的にガスケットからの溶出の虞がなく、薬剤の混合後さらに別の薬剤を容易に吸引することができる2成分混合型プレフィルドシリンジを提供することを目的とする。

【課題を解決するための手段】
本発明者等は、上記課題を解決するために鋭意検討の結果、本発明に想到した。
すなわち本発明の2成分混合型プレフィルドシリンジは、閉塞されたノズルを先端に有する基端が開放されたバレルと、バレル内に液密かつ摺動自在に挿着されたガスケットと、ガスケットに液密に取り付けられた略円筒状の蛇腹容器とから構成され、ガスケットは、基端から先端に貫通する貫通孔を有しており、蛇腹容器には、先端に開封可能に封止された薬液通路が設けられるとともに、基端にプランジャー装着部が設けられている。また、このプランジャー装着部の先端側には係合手段が設けられており、ガスケットの貫通孔には前記薬液通路が液密に挿着され固定されており、蛇腹容器を収縮させたときに係合手段が、薬液通路を通過して薬液通路の先端と係合するようになっている。

前記薬液通路は、シール部材を剥離可能に弱溶着することにより封止されていることが好ましい。また、前記ガスケットの基端から貫通孔に、フランジを基端に有する先端および基端が開口した筒状部材が、液密に挿嵌あるいは螺合していてもよく、この場合、前記蛇腹容器はこの筒状部材の基端側より取り付けられる。なお、蛇腹容器を収縮させたときに、前記係合手段は薬液通路を通過して、筒状部材の先端と係合するようにしてもよい。さらに、前記筒状部材の先端は、シール部材を剥離可能に弱溶着することにより封止することもでき、前記蛇腹容器の先端または前記筒状部材の先端に弱溶着されるシール部材の一部分は、弱溶着部分と比べて剥離し難く強溶着されていてもよい。なお、バレルの先端とガスケットの間には固形製剤を充填し、前記蛇腹容器の内部には液剤を充填して用いることが可能である。

【発明の実施の形態】
次に、本発明の実施例を図面に基づいて説明する。
図1は、本発明の実施例を示す縦断面図であり、図2は、係合手段が筒状部材の先端に係合した状態を示す本発明の実施例の部分拡大斜視断面図であり、図3は、本発明の係合手段の他の実施例を示す図であり、図4および図5は、本発明のプレフィルドシリンジの使用状況を説明する図であり、図6は、従来例1を示す図であり、図7は、従来例2を示す図であり、図8は、従来例3を示す図である。
図1に示す2成分混合型プレフィルドシリンジは、キャップ1で閉塞されたノズル2を先端に有する基端が開放されたバレル3と、バレル3内に液密かつ摺動自在に挿着されたガスケット4と、ガスケット4に液密に取り付けられた筒状部材5と、筒状部材5に液密に取り付けられた蛇腹容器6を含む構成からなり、ガスケット4は基端から先端に貫通する貫通孔41を有し、この貫通孔41には基端にフランジ52を有する筒状部材5の先端が液密に挿嵌されている。筒状部材5の基端側からは、基端にプランジャー8を装着することのできるプランジャー装着部7を有する蛇腹容器6が取り付けられている。このプランジャー装着部7は、蛇腹容器6の内部に向けて突出する係合手段71を有しており、この係合手段71は、プランジャー装着部7を先端側に押し進めることで、先端に設けられたフック711が筒状部材5の先端と係合するようになっている。

バレル3は、一般に使用されるシリンジと同様の形状であって、先端にノズル2を有している。図1に示すノズル2は、図5の▲4▼に示すような注射針12のハブ121と嵌合するような形状であるが、注射針12のハブ121と螺合するロックネジを設けたものも採用できる。このノズル2は、合成樹脂、ブチルゴム、熱可塑性エラストマー等で形成されたキャップ1で液密に閉塞されるが、ノズル2が液密に閉塞されるなら、キャップ1の材料は特に限定はされない。なお、バレル3の材質は、ガラス、ポリプロピレン、ポリエチレン、環状ポリオレフィン等、内部に収容する薬剤を長期保存可能なものであれば、特に限定されない。また、バレル3内に充填した薬剤を凍結乾燥する場合には、ガスケット4の貫通孔41から水分を蒸発させてもよいし、バレル3基端部にバイパス溝を設けて、シール部材10で封止した筒状部材を挿着したガスケット4をバレル3に半打栓した状態で、このバイパス溝から水分を蒸発させた後、ガスケット4を真空打栓してもよい。

ガスケット4は通常、ブチルゴム、シリコンゴム、熱可塑性エラストマー等で形成され、バレル3内に液密かつ摺動自在に挿着されている。そして、ガスケット4には基端から先端に貫通孔41が形成されており、後述の蛇腹容器6の内部とガスケット前方の空間部9を連通できるようになっている。さらに、この貫通孔41には、筒状部材5が液密に挿嵌合されている。

蛇腹容器6は通常、ポリプロピレン、ポリエチレン、ポリエチレンテレフタレート、ポリエチレンテレナフタレート、ポリエステル、軟質ポリ塩化ビニル、熱可塑性エラストマー、ポリカーボネート等の弾性を有する各種高分子材料で形成されるが、内部に収容する薬剤を長期間保存可能な材料であれば特に問題ない。そして、この蛇腹容器6は、伸縮自在な蛇腹状胴部61と、先端がシール部材11で封止された薬液通路62と、基端に設けられたプランジャー装着部7と、プランジャー装着部7から蛇腹容器6の内部に向けて突出する係合手段71を有している。また、蛇腹容器6の内部には、図4に示す液剤Sが収容されている。この蛇腹容器6は、基端にプランジャー装着部7を液密に取り付け、薬液通路62から液剤Sを充填した後、薬液通路62をシール部材11で封止する等して製造される。薬液をこのような別容器に保存しておくことにより、ガスケットに含まれている添加剤、例えば硫黄や加硫促進剤等が薬液中に溶出するのを防ぐことができる。
薬液通路62は、前述の貫通孔41またはガスケット4にはめ込まれた筒状部材5に挿入され、図2に示すように、肩部63が、筒状部材5のフランジ52の裏面に接着または溶着されている。ここで、蛇腹容器6先端に本例のような円筒状の薬液通路62を設けない形状も採用できるが、薬液通路62を貫通孔41または筒状部材6に挿入する際の位置決め手段ともなりうるという効果を有することから、本例の形状が好ましい。
プランジャー装着部7は、通常合成樹脂で形成されており、基端側からプランジャー8を脱着自在に装着可能なように雌ねじ部72が形成されており、使用時には先端に雄ねじ部81を有するプランジャー8が装着される。
また、プランジャー装着部7の先端側には、蛇腹容器6内部に向けて突出する係合手段71が設けられている。この係合手段71は、先端側が内側に撓むように形成された一対のアーム712で構成されている。そして、アーム712の先端には外側に向けて突出するフック711が設けられており、このフック711は、図2に示すように、蛇腹容器6を収縮させた際に、筒状部材5の先端面53に係合するようになっている。
また、アーム712が撓んだ際、アーム712どうしが干渉し合うと薬液通路の通過に支障をきたす虞があるため、図2に示すようにアーム712の横断面形状を楔型としてもよいし、図3に示すようにアーム713の撓む方向をずらしてもよい。

シール部材10、11としては、筒状部材5先端の開口部または薬液通路62の開口形状にあわせて形成された樹脂フィルムが好ましく採用できる。そして、封止方法としては、この樹脂フィルムの周囲を剥離可能に弱溶着あるいは一部分を強溶着とし他の部分を弱溶着とする等の方法が挙げられるが、薬液中へ異物として混入するおそれのない後者のほうが好ましく採用できる。

また、本実施例では空間9に固形製剤充填し、蛇腹容器6内に液剤Sを充填しているが、空間9内に液剤を充填してもよく、蛇腹容器6内には、固形製剤Dを充填してもよい。

次に本発明のプレフィルドシリンジの使用について、図4および図5を用いて説明する。
先ず、蛇腹容器6内に液剤S(例えば溶解液)を充填し、ガスケット4前方の空間9に固形製剤D(例えば凍結乾燥した粉末状の抗生物質)を充填した本発明の好適な実施形態であるプレフィルドシリンジPを用意し、図4の▲1▼に示すように、プレフィルドシリンジPのガスケット4にプランジャー8を結合する。次に、図4の▲2▼に示すようにプランジャー8を矢印A方向に押し、プランジャー装着部7を前進させると、蛇腹容器6が収縮し薬液通路62の先端部を封止しているシール部材11が強溶着部分を残して剥離するとともに、蛇腹容器6が挿着されている筒状部材5の先端部を封止しているシール部材10も剥離し、蛇腹容器6内の液剤Sがガスケット4前方の空間9に流入する。
さらに、プランジャー8を押し進めると、図4の▲3▼に示すように蛇腹容器6は完全に収縮し、内部の液剤Sの殆ど全てが、空間9へと移動する。そして、プランジャー装着部7に設けられた係合手段71の先端のフック711が、ガスケット4の前方に突出し、筒状部材5の先端に係合する。ここで、フック711は、薬液通路62の先端に係合するようにしてもよい。
しかる後に、プレフィルドシリンジP全体を振盪し、固形製剤Dを液剤Sに完全に溶解させ、薬液LD1を調製する。最後に、図5の▲4▼に示すようにキャップ1を取り外し、プレフィルドシリンジPの先端のノズル2に注射針12を接続すれば、患者の静脈などに薬液LD1を注射することができる。
また、本発明に係るプレフィルドシリンジPでは、薬液調製後さらに別の薬液を配合する必要がある場合にも、図5の▲5▼に示すように、通常のシリンジと同様、プランジャー8を矢印B方向に引く操作のみでバイアルV等に収容された別の薬液LD2を吸引することが可能である。

【発明の効果】
以上述べたことから明らかなように、本発明によれば、薬剤を混合する際の取り扱い性に優れているため、簡便かつ短時間で薬剤の調製作業を行うことができ、コアリングが生じず、実質的にガスケットからの溶出の虞がなく、薬剤の混合後さらに別の薬剤を容易に吸引することができる。
