本発明は、薬、特に液体若しくは流動形状の粉末の薬の、測定された投与量を患者に分配する装置及び方法に関する。

噴霧器及び吸入器が、薬をガス状にして患者に分配するために、開発されてきた。
【0003】
吸入器は、概略、2つのカテゴリに分けられる。その2つは、与圧計量投与量吸入器(pMDI)と乾燥粉末吸入器(DPI)であり、そのどちらも患者に吸入するためのマウスピースを備える。しかしながら、多数の患者、とりわけ小児科の患者に対し、吸入器を効果的に利用するのは困難である。

従来の、即ち非呼吸作動式の、与圧計量投与量吸入器に関して言うと、吸入器の効果的な操作に際して、薬を肺奥深く引き入れるために吸入の最初に患者が吸入器を作動させる必要があるために、上記の困難は発生する。この調整を行なうのは、小児科の患者にとっては特に困難なことである。典型的には、与圧計量投与量吸入器が吸入の開始前に作動すると、薬の大部分がのどの奥を打ちつけるし、与圧計量投与量吸入器が吸入の開始後に作動すると、薬の大部分がのど即ち気管支に残る。そこに残っても効果は無い。

呼吸作動式の与圧計量投与量吸入器及び乾燥粉末吸入器は、作動と吸入に関するそのような調整は必要ないものの、小児科の患者に対し利用することは困難である。というのは、特に乾燥粉末吸入器では30リットル/分である、所定の流量速度を実現する十分な強さで吸入することを、これらの吸入器は患者に要求するからである。呼吸作動式の与圧計量投与量吸入器でのこの十分な強さによりエアゾルキャニスタを起動するのであり、乾燥粉末吸入器でのこの十分な強さにより吸入器を介して空気を引き入れるのである。特に小児科の患者は、そのような流量速度を実現するのに必要な一回換気量を生成させることができない。小児科の患者にとっては、一回換気量は、典型的には、約3から約15リットル/分の範囲にすぎない流量速度を生じさせる10から150mlの範囲である。

この変動は、小児科の患者にはフェースマスクの利用が要求されることに、本質的に起因する。そもそも小児科の患者はマウスピースを効果的に掴むことができない。しかしながら、フェースマスクの利用は、噴霧器若しくはスペーサと、患者との間の、デッドスペースを増加させてしまう。このことは大人の患者においては通常、問題ではない。大人は、概して噴霧器若しくはスペーサの下流にあるデッドスペースよりはるかに多い一回換気量を有するからであり、ガス中の薬の濃度により増加される吸入体積と同様に、患者に受け入れられる投与量は相当程度の正確値にまで近づき得る。
