本出願は経皮カテーテルに関し、さらに詳しくは患者の体内の所定の部位に経皮カテーテルを固定するための固定器具に関する。

【従来の技術および発明が解決しようとする課題】
経皮カテーテルは、一般に、患者の体内に流体を分配したりあるいは体内から流体を除去する医療用途に使用される。患者の治療に静脈カテーテルを利用してある流体を患者の血液内に直接注入することは非常に一般的なことである。また臓器内の閉塞した通路にバイパスを付けるカテーテル法を利用することも一般的である。例えば、腎臓と膀胱の間の通路が閉塞すると、尿を排出するため腎臓のカテーテル法によって、認められている治療を施すことができる。

一般に、カテーテルは、軟質チューブ、あるいは開口またはカニューレを通して延びる内腔を有するカニューレからなっている。カテーテルの配置は通常、縫い針を被覆したその縫い針よりわずかに大きな外径を有するイントロデューサシース(introducer sheath)を患者の体内の所定の部位に挿入し、その後、その縫い針を抜き取る工程を有する。その後、カテーテルがそのイントロデューサを介して患者に挿入される。静脈カテーテルの場合、注入された流体は、カテーテルが挿入される静脈内にカテーテルを通して直接流れる。尿ドレナージカテーテル(urinary drainage catheter)の場合、尿は腎臓から直接医務室に排出される。

カテーテルには様々な用途があるため、カテーテルも様々な長さがある。例えば、静脈カテーテルの場合、例えば手の周縁に配置される短静脈カテーテル、静脈が太く血液希釈に良好で、血液流が達成される、例えば、手から上腕までの静脈内に約6ないし8インチ挿入される中線(ミッドライン)カテーテル、および周縁に、すなわち腕に配置され、例えば上大静脈に挿入されるかなりの長さの中心静脈カテーテルを含む数種類の長さのカテーテルがある。長さに関係なく、カテーテルが体内でその位置を維持するようにカテーテルを固定することが望まれている。患者の体内でのカテーテルの位置決めは体に注入される流体の濃度と毒性を測定する上で特に重要である。カテーテルはその位置の維持を確実にするには、カテーテルを患者の体の外部に固定しなければならず、カテーテルの位置が変わる機会が少なくなるように、理想的には、カテーテルの挿入部位あるいは接合部にできるだけ近くに固定する必要がある。

一般的な固定器具にはカテーテルカニューレに取り付けられるテープダウンまたはロックダウンウイングがある。固定位置カテーテルロックダウンウィングを接合部に固定するように、カテーテルカニューレの全長が患者体内に配置されれば、そのウイングはよく機能する。しかしながら、余分なカテーテル材料が挿入部位または接合部からロックダウンウイングを分離するように、患者体内のカテーテルの位置を短くする決定がなされると問題が起きる。そのウイングを挿入部位から十分離れた位置で患者に固定すれば、カテーテルカニューレの移動とピストン運動が発生することがある。この問題を克服する1つの方法は、カテーテルを体内に挿入する前、カテーテルカニューレをある長さに切ることである。殆どのカテーテルはその遠位端部にハブを有しているため、唯一の切り易い端部は挿入側端部である。しかしながら、カテーテルカニューレは通常、挿入がし易くなるように、すなわち、患者に傷を少なくするように、挿入端部がテーパーの付いた先端で構成される。そのテーパー付き先端が例えばカテーテルの一部をクリップを使用して抜く場合、そのような患者への配慮は無視される。さらに、短くした判断が不正確であることがわかると、医師あるいは看護婦はなお処理しなければならない余分なカテーテルカニューレ材料を有する。接合部から延びる余分なカニューレ材料を処理する他の方法は、カテーテルが短くなったか否かは別にして、ウイングを患者に固定する前に、固定ウイングの周囲に余分なカテーテルカニューレを巻き付けることである。この方法により固定器具が接合部に固定されるが、扱いにくい嵩張った器具となる。

一旦、カテーテルを配置すればカテーテル接合部に配置される調節可能な器具を挿入した。しかしながら、これらの器具は嵩張るか、接合部位に効率的に取り付けることが困難である。調節可能な固定器具の1つは、その底部に沿ってスプリットを備えたフレキシブルな第1部品が接合部に隣接するカニューレに適合し、カテーテル器具が動けないように第2剛性部品が第1部品に対してスナップ(snap)して、第1部品でカニューレを加圧する2個構成器具である。それで、全体の装置に縫合糸を取り付けるかあるいは皮膚にテープを付ける。このタイプの器具についての問題は、その器具にサイズ分けが必要で接合部でカテーテルカニューレに嵌合する2個構成器具であるということである。固定器具の配置は、患者に迷惑がかからず、個々の部品を損なわないように技巧的に行なわれる必要がある。さらに、第2剛性部品は器具を嵩張らせる。

他のタイプの調節可能な固定器具はアイレット(eyelet)2個構成絞り嵌め締結器具を加圧している。この締結器具は、カテーテル周囲とそのカテーテルカニューレに取り付けた2個構成締結器具内部に適合する少なくとも1つの円筒状アイレットを利用している。締結器具に力を加えると、アイレットは加圧され摩擦力をカテーテルにかける。この固定器具は容易に置き違えられる少なくとも3個の部品を含み、締結器具を接合部に操作し取り付けることは困難である。

【課題を解決するための手段】
患者の体内の所定位置のカテーテルを保持する調節可能な固定器具を開示する。この固定器具はチューブ状本体部分を備えた固定部材とそのチューブ状本体部分を通して延在する開口を有する。その開口とチューブ状本体部分は、カテーテルを摩擦係合(接触)することによってカテーテルを保持するように適合した第1寸法を有する。チューブ状本体部分は、固定部材がカテーテルの周りをスライド可能に移動できるように、第2寸法を備えた開口を形成するように拡張可能でもある。固定部材は、限定はしないが管カテーテル、神経カテーテルおよび尿ドレナージカテーテルを含む様々なタイプの全てのカテーテルを固定するようになっている。

【発明の実施の形態】
本発明は患者の体内の所定位置にカテーテルを保持するための調節可能な固定器具に関する。本発明を以下図面を参照して説明する。以下の説明では特定の材料、形態、およびカテーテルタイプのような多数の特定の詳細を示す。しかしながら、これらの特定の詳細は本発明を実施するのに使用する必要がないことは当業者に明らかであろう。また、本発明を不必要に曖昧にするのを避けるためよく知られた材料あるいは方法は詳細に説明していない。

As described above, according to the present invention, it is possible to obtain an adjustable fixing device that can be easily attached to a patient's joint and is not bulky and difficult to handle.
