Description
【発明の属する技術分野】
この発明は、身体通路ないし管路内に使用するための拡大可能な管腔内ステントに関するものであり、特に、病気により狭窄ないし閉塞した血管を修復するためにとりわけ役立つ、改良型の可撓性があって拡大可能な管腔内血管ステントと、拡大可能な管腔内ステントを移植するための装置に関するものである。

【従来の技術】
管腔内血管内部移植(ステンティング(stenting))は実験により証明され、従来の血管手術に対して可能性のある代替手段を示している。管腔内血管内部移植は、管状人工装具移植片を血管中に経皮挿入することや血管組織内部の所望の位置にカテーテルを介してこの移植片を送出することを必然的に含むものである。従来の血管手術を越えたこの方法の利点は、外科手術上、欠陥のある血管を露出したり、切り目を入れたり、除去したり、置換したり、バイパスしたりする必要性をなくすことができることを含む。

以前に管腔内血管ステントとして使用されていた構造物は、渦巻き状のステンレススチールばねと、拡大可能な感熱材料から製造され、らせん状に巻かれた渦巻きばねと、ジグザグパターンのステンレススチール線材から形成され、拡大するステンレススチールステントとを含むものである。一般に、前述の構造物は、共通してある重大な不都合点を有する。この構造物をつぶれた状態にある特定の定められた体内通路内の所望位置に送出せねばならない限りにおいて、この体内通路を通過させるための、各構造物の最終的な拡大した配置構造におよぶ効果的な制御方法はない。例えば、特別な渦巻きばねタイプの移植片即ち「ステント」の拡大性は、渦巻きばね構造を製造するために利用される特別な材料のばね定数と弾性係数によって決定される。これらの因子はジグザグパターンのステンレススチール線材から形成されているつぶれたステントの拡大量を予め決定することになる。加熱時に拡大する感熱材料から形成される管腔内移植片即ち人工装具の場合には、その拡大量は管腔内移植片の製造に使われる特殊合金の熱拡大特性によって同様に予め決定される。

【発明が解決しようとする課題】
このように、前述タイプの管腔内「ステント」が動脈ないし静脈内部のような体内通路内部の所望の位置で一旦拡大すれば、当該ステントの拡大規模を変更できない。もしも所望の体内通路の直径が誤って計算されたならば、小さめのステントは体内通路の内表面と接触して固着するには不十分にしか拡大しないかもしれない。それゆえ、体内通路の所望の位置から離れて移動していくおそれがある。

従来の血管外科手術に対する他の代替手段は、カテーテルを取り付けた血管形成バルーンの使用による弾性のある血管の狭窄症ないし閉塞症の経皮バルーン拡張法がある。この手法においては、当該血管の壁の構成部分を切り裂き破裂させて拡大した管腔を得るために、血管形成バルーンは狭窄血管ないし体内通路の内部で膨張される。動脈アテローム性硬化病変に対しては、相対的に圧搾できない局面は不変のままであるが、身体通路のより弾性のある中央および外膜層はその局面の周りでいっぱいに広がってしまう。この過程により、身体通路壁層の***、即ち分解と断裂を生み、これにより内膜即ち動脈や身体通路の内面は裂け目発生を被ってしまう。この***が管腔を通じて血液の流れを減少させたり、管腔を閉塞するおそれのある下部組織の「フラップ(flap)」を形成する。一般に、身体通路内部に広がる管内圧力は***層即ちフラップを適所に押さえ込むことができる。バルーン拡張手法により創り出された内膜フラップが拡張した内膜に対して適所に維持されないならば、その内膜フラップは管腔中へ折り畳まれて管腔の流れをせき止めたり、切り離されて身体通路に入り込むおそれがある。この内膜フラップが身体通路の流れをせき止める場合には、当面の外科手術にはこの問題を矯正する必要がある。

このバルーン拡張手法は一般に前述の問題が存在するので病院のカテーテル挿入ラボにて行われるが、内膜フラップが血管ないし体内通路を遮断するといけないので待機中の外科医がいつも必要となる。さらに、内膜フラップが血管から離れて管腔を遮断する可能性のために、バルーン拡張手法は、心臓に通ずるたとえば左方主要冠状動脈などの一定の重大な身体通路には行うことはできない。もしもバルーン拡張手法によって形成された内膜フラップが突然下がってきて、たとえば左方主要冠状動脈などの重大な身体通路の流れをせき止めたならば、患者はどんな外科手術手法であろうともそれが済むまえに死んでしまうであろう。

弾性のある血管狭窄症のバルーン拡張手法と関連したさらなる不都合点は、狭窄症病変の弾性反動が原因で多くは失敗してしまうことである。これは、その病変中の高度な線維コラーゲン性の内容物のせいで大抵発生し、時には拡張されべき領域のある機械的特性のせいで発生する。このように、身体通路は初めにバルーン拡張手法によってうまく広げることができるけれども、次に、先に拡張された身体通路の管腔の大きさを減少させる身体通路壁の反動が原因で初期の再狭窄症が起こることがある。例えば、拡大する力が腎性動脈それ自身に対してよりもむしろ大動脈壁に対して加わるので、腎性動脈のその入り口における狭窄症はバルーン拡張法には制御しがたいことが知られている。

たとえば腎臓透析用取り付け通路のフィステル(fistulas)においてみられるものなどの新内膜線維症によって生ずる血管狭窄は拡大することが困難であることが証明されたが、これは高い拡大圧力とより大きなバルーン直径が必要だからである。同様な困難さは、移植動脈吻合狭窄症および動脈内膜切除後(postendarterectomy)の回帰性狭窄症の血管形成においても認められる。タカヤス動脈炎および神経線維腫症動脈狭窄症の経皮血管形成では、これらの病変の線維上の性質のせいと思われる貧弱な初期反応と再発を示すおそれがある。

【課題を解決するための手段】
この発明は、人間および動物における身体管腔の開通性を維持するための管腔内ステント移植に関するものであり、特に、血管中に使用するための管腔内ステント移植に関するものである。本発明は一般に形状が円柱状でバルーン拡大可能ステントの改良からなるものであり、円筒状の本体の縦軸に沿ってこれに柔軟性を与えるように接合された複数の金属構成要素を備え、これにより当該ステントは曲がった身体管腔に整合できるものである。このようなステントの一つは、独立したループが互いに移動できる巻線構造の線材ループで作られた金属構成要素を有する。この構造をしたステントは体内管腔中に拡大される場合、その巻き具合は身体管腔の曲線にそったものとすることができる。本文中に参考文献で取り入れ、パルマツ(Palmaz)に付与された米国特許4,733,665号と同4,776,337号に開示されたステントは、ステント同士を接合して使用する関節接合の特色をよく示すものである。

他のこのようなステントは、ヴィクタ(Wiktor)タイプ(メドトロニック社(Medronic,Inc.))のステントがあり、可撓性部材によってつながれた独立ステント切片でできた金属構成要素を備え、このステント切片が曲がった身体管腔に適用できることによって改良されたものである。このようなステントはそのデザインに適した手段によって身体管腔中に配置することができる。例えば、ヴィクタタイプのステントの場合には、バルーンカテーテルの拡大可能な構成要素に対して適合させバルーンによって拡大しこのステントを身体管腔に押し付けて接触させることができる。あるいは、例えば、パルマツとパルパツ−シャッツステント(Palmaz and Palmaz−Schatz)(ジョンソンアンドジョンソンインターベンショナルシステム)の場合には、身体管腔を通して送出されるようにステントをしっかり把持しそれからステントを解放しこれが身体管腔と接触して拡大できるカテーテルにこれを取り付けることができる。この配置展開は、ステントが経皮的に差し込まれ、経管腔的に(transluminally)移送され、カテーテルによって所望の位置に配置することにより達成される。

これらのステントの重要な利用法は血管壁や狭窄症の局面の一部が血管内の血液の流れを遮断ないし閉塞する場所で見いだされる。血管の拡張では通常血管の閉塞を矯正することが受け持たれ、即ち、バルーンカテーテルはPTCA手法にて利用され血管の閉塞箇所を拡大するものである。しかしながら、この閉塞の拡張では、拡張された血管の再閉鎖のおそれがあるフラップ、裂溝、***を形成し、あるいは血管壁に穿孔さえも作ることがある。金属ステントの移植によりこのようなフラップと***に対する支持物を付与することができ、それによって血管の再閉鎖を防止したり、矯正外科手術が行われるまで穿孔した血管壁に対してつぎはぎ修理を与えることができる。その上、このような良好な縦方向の可撓性を有した金属ステントは曲面と凹凸を有する血管に対して容易に整合することができる。しかしながら、このようなステントは、金属構成要素間の間隔の取り方のせいで、穿孔した血管を効果的につぎ当てをする能力を制限している。同様に、このような金属ステントはたとえば抗狭窄症剤、抗凝固剤などの薬剤を運搬し送出する能力もまた制限を受けることになる。

したがって、上記に引用されたような現行ステントの開発に先だって、以下のような拡大可能な管腔内血管ステントと、身体通路の管腔を拡大するための方法および装置は存在しておらず、それは身体管腔における狭窄症の再発を防止し、たとえばある患者の心臓の左方主要冠状動脈などの重大な身体管腔に対して柔軟に利用でき、身体通路壁の反動を防止し、管腔内移植片を多様な大きさに拡張することができ、これにより移植片が所望の場所から離れて移動してしまうのを防止するとともに拡張した移植片によって身体通路が裂傷するのを防止することができるものである。それ故に、当該技術は、以下のような拡大可能な管腔内血管移植片と、身体通路の管腔を拡大するための方法および装置を追求し、それは身体通路における狭窄症の再発を防止し、たとえば心臓の左方主要冠状動脈などの重大な身体通路に役立てることができるものと思われ、身体通路の反動を防止し、身体通路の内部で多様な大きさに拡張し所望の場所から離れて移植片が移動するのを防止するとともに拡張された移植片による身体通路の裂傷を防止することができる。

この発明によれば、この拡大可能な管腔内血管ステントを介して上述の利点は達成された。本発明は、第1および第2の端部を有した管形状部材と第1および第2の端部の間に配置された壁表面とを含み、この壁表面は複数の交差した細長い部材によって形成され、少なくとも管形状部材の第1および第2の端部の中間にあって少なくとも細長い部材のいくつかは互いに交差しており、管形状部材は管腔を有する身体通路中へ管腔内送出するのを許す第1の直径を有しており、管形状部材は拡大した第2の直径を有しており、管形状部材の内部から半径方向、外方向に拡大する力を加えた時に、この第2の直径は可変であり管形状部材に加えられる力量しだいであることと、これにより管形状部材は拡大され身体通路の管腔を広げることができる。

これらのステントは、(1)らせん状のループ構造、あるいは、(2)身体管腔により大きな可撓性を与えるステントの縦軸に概して略垂直な半分離れた正弦曲線状のコネクタによって連結されており、いずれにせよ身体の関節接合を形成するために特に適用されるようなステントをもたらす。

この発明によれば、身体通路の管腔を拡大する本方法により前述の利点もまた達成される。本発明の方法は、カテーテルに配置された管腔内移植片を、身体通路内部の所望の場所に隣接して配置されるまで身体通路中に挿入する段階と、カテーテルの一部を拡大して、身体通路の所望の位置に身体通路の管腔が拡張されるまで、身体通路と接触して半径方向、外方向に広がる管腔内移植片をもたらし、これにより管腔内移植片は、身体通路が拡大した管腔の大きさをつぶし減少させるのを防止する。

この発明によれば、前述の利点はまた身体通路を管腔内的に増強する本装置を通じても達成される。本発明は、第1および第2の端部を有した拡大可能な管形状人工装具と、第1および第2の端部の間に配置された壁表面とを含み、壁表面は複数の交差した細長い部材によって形成され、カテーテルと組み合わせて用いることができ、これと関連した拡大可能で膨張可能な部分を有し、この拡大可能で膨張可能な部分に拡大可能な管形状の人工装具を取り付け保持する手段を含み、これにより、カテーテルの拡大可能で膨張可能な部分が膨張する際に、この人工装具は身体通路と接触して半径方向、外方向に形成される。丸みのある線材フレームループないしステントの縦軸に略垂直である半分離れた正弦曲線状のコネクタ形式の関節接合により同様なステント本体同士をつなげるものである。同様に、ステントの丸みのあるコネクタと短くしたセルはより大きな可撓性を考慮したものである。

本発明の拡大可能な管腔内血管ステント、身体通路の管腔を拡大する方法、および身体通路を管腔内的に補強する装置は、以前に提案された従来技術の管腔内ステントと比較すると、これを移植する方法およびバルーン拡張技術は以下の利点を有しており、狭窄症の再発を防止し、たとえば心臓の左方主要冠状動脈の重大な身体通路におけるステントの移植を可能にすると思われ、身体通路の反動を防止するとともに移植片の拡大を可能にし、これにより身体通路内部の状態に依存するとともにステントの関節接合の位置に依存して移植片を多様な大きさにするものである。

【発明の実施の形態】
図1〜6において、身体通路用の拡大可能な人工装具であるステントが例示されている。本願の方法、装置および構造は、血管即ち身体通路の部分的に閉塞した区域を広げるための拡大可能な管腔内血管移植に関連して利用できるだけでなく、多くの他のタイプの身体通路に対して拡大可能な人工装具として多くの他の目的に対してもまた利用できる限りにおいて、「ステント」と「人工装具」は本願記載ではある程度まで交換可能に使用されることが理解されよう。例えば、拡大可能な人工装具は、以下のような目的に対してもまた使用できる。
(1)経管腔内的(transluminal)再疎通によって開かれた遮断動脈内部への支持移植片の配置。
(2)手術ができない癌によって閉塞された縦隔および他の静脈を通してカテーテル通路を続ける類似の使用。
(3)門脈圧亢進症を患っている患者の門脈静脈と肝静脈の間に創り出された肝臓内連絡カテーテルの増強。
(4)食道、腸、尿管、尿道の狭い所の支持移植片の配置。
(5)再開および以前に閉塞された胆管の支持移植片の増強。
したがって、「人工装具」という用語の使用は、各種タイプの身体通路内部への前述の使用を網羅し、「管腔内移植」という用語の使用は、身体通路の管腔を拡大するための使用を網羅する。さらに、この点については、用語「身体通路」は、人間の血管組織内部のいかなる静脈、動脈、血管と同様に、たとえば前に記載されたものなどの人体内部の管路を網羅している。

図1,4について説明する。拡大可能な管腔内血管移植片(即ち人工装具)70は、一般に、第1および第2端部72,73を有する管形状部材71と、第1および第2端部72,73間に配置された壁表面74とから成ることが示されている。好ましくは、この壁表面74は、複数の交差した細長い部材75,76によって形成され、これらは、例えば交差点77に示されるように、管形状部材71の第1および第2端部72,73間に介在して、少なくともいくつかの細長い部材75,76は互いに交差している。以下丹念に述べると、管形状部材71は第1の直径dを有しており、これにより当業者において既に公知の管腔を有する身体通路70中への管形状部材71の管内送出を可能にする。管形状部材71の内部からの半径方向、外方向に広がっていく力を加えた場合には、以下丹念に述べると、管形状部材71は拡大した第2の直径d’を有し、この第2の直径d’は大きさが可変であり、管形状部材71に加えられる力量にしたがうものである。

管形状部材71の壁表面74を形成する細長い部材75,76は、血管移植片即ち人工装具70が接触する可能性のある人体および体液と共存できる適当な材料ならいかなるものでもよい。細長い部材75,76はまた、管形状部材71が図1に示される配置構造から拡大構造まで広げられることができ、さらに管形状部材71はその拡大構造が拡大直径d’で保持するのに必要な強度および弾性特性を備えた材料からできていなければならない。管形状部材71の製作に適した材料は、銀、タンタル、ステンレススチール、金、チタン、あるいは先に述べた必要な特性を備えた適当な塑性材料ならいかなるものをも含むものである。好ましくは、細長い部材75,76は316Lステンレススチールから製作される。好ましくは、この細長い部材75,76は、円筒状の断面形状を有する小径のステンレススチール線材である。もちろん、細長い部材75,76はそれぞれ、三角形、四角形、長方形、六角形等の他の断面形状を有することができることは理解されよう。さらに、複数の部材75,76は、たとえば交差点77などにおいて互いに交差する場合には、互いにしっかりと固定されるのが好ましい。細長い部材75,76は、いかなる従来方法においても互いにしっかりと固定されることができ、たとえば溶接、はんだづけ、適当なエポキシ樹脂を用いたようなのりづけなどによっても固定できる。しかしながら、交差点77は連続した部片であることが好ましい。細長い部材75,76を互いにしっかりと固定することにより、管状部材71は半径方向のつぶれに対して比較的高抵抗が与えられ、管形状部材71はその拡大径d’を保持する能力を備えている。好ましくは、管形状部材71は、十字模様の管状パターンで編まれた連続したステンレス線材でできていて、ワイヤメッシュ管として一般に表現することができるものを形成する。

管形状部材即ちワイヤメッシュ管71を製作する時に、初めに、図1に示される直径dをもつ配置構造に製作することができる。あるいはまた、初めの直径dよりも大きな直径で製作することができ、この製作後に管形状部材71は図1に示される直径dをもつように慎重につぶすことができる。管形状部材即ちワイヤメッシュ管71をつぶしている最中に、隣接した細長い部材75,76の重なりを避けされることを保証するように注意が払われなければならない。

ステント70の顕著な部分としてそのままでは曲線状のコネクタ77aは、戦略的に人工装具70の外周辺りに配置される。これらのコネクタ77aは「長方形」77,75,76,77の長さをを短くし、これによりステント70の可撓性を高めている。これが生ずるのは、ある開いた「長方形」77,75,76,77aにみられる2つのセル長(2c)と比較すると、スロットの長さに沿って1つのセル長(c)と同じくらいに閉じた「長方形」77,75,76,77は小さくできるからである。開いた「長方形」と閉じた「長方形」との比率は、所望の可撓性しだいであり、1:1よりも大きい程度に低く、5:1程度に高くすることができる。最も望ましくは、1つには約1.5から約3.0までの可撓性比率を使用することができることが分かっている。即ち、「開いた」セル長は、「閉じた」セル長の1.5倍から3倍の間のどれかである。スロット(c)それら自身は1.5cm以下で役立つことが分かっている。その上、この新規な「長方形」は開いた側面を有しており、いっそう大きな可撓性を創り出している。ステント全長は、冠状動脈ステントに対する約3cmから大動脈に対する約18cmまでに渡り、0.0025インチ(0.00635cm)と0.008インチ(0.0203cm)の壁の厚みを有している。

ステントに沿って半径方向ならびに縦方向に間隔を開けて配置された開いたセルを有することが役に立つことが分かっている。それにもかかわらず、これらのコネクタは対称的に間隔を開けて配置される必要はないことがさらに分かっている。事実上、ある決められた方向における曲がりをより良く誘導するためには、ステントの片側に集中された一定の開いた領域を有することは役に立つと思われている。これらの開いた領域がこの開いた領域の方向への曲がりを与える。その後、この曲がりはその決められた方向に十中八九に向くようになるであろう。

さて図2,5を説明する。拡大可能な管腔内血管移植片即ち、人工装具170,170’の他の実施の形態は類似して示されている。図2,5の管腔内血管移植片即ち人工装具170は、バー78,79が互いに交差する場合には好ましくは互いにしっかりと固着される複数の細いバー78,79によって形成された複数の細長い部材75,76を有する。これは図1,4のステントに類似している。さらに、ステントに沿ってずっと開口部182を形成するのを助ける複数の曲線状のコネクタ77aが含まれている。これらのバー78,79は好ましくは、細めの長方形の断面形状を有し、たとえば溶接、ブレイジング、はんだ付けなどのいかなる従来方法でも互いに結合することができ、あるいは、ポリマー、たとえばスチールないし記憶金属(例、ニチノール(nitinol))などの従来金属により互いに一体に形成することができるが、優先的には、ステント170はワンピースから形成されるものである。最も好ましくは、管形状部材171は初めは薄壁で囲われたステンレススチール管であり、交差しているバー間の開口部182は、たとえば電気機械ないしレーザエッチングなどの従来のエッチング工程によって形成され、これにより、出来上がった構造は、複数の交差した細長い部材75,76を備えた管形状部材171である。図2の拡大可能な管腔内血管移植片即ち人工装具170は、同様に、管形状部材171の内部から半径方向、外方向に拡張する力を加えた際に、先に図1と関連して述べられたような拡大された配置構成を仮定することができる。図2の管腔内血管移植片即ち人工装具170の実施の形態は、一般にワイヤメッシュ管としても表現できることがさらに理解されよう。

図2においては、開口部182の長さが開口部182’の3倍であることが分かる。図5においては、開口部182は開口部182’の長さに等しい。それにもかかわらず、可撓性コネクタ77aにより、図5のステント170’は図2のステント170に類似して、同様に可撓性がある。

第3の顕著な実施の形態(図3参照)のステント270は一続きのセルタイプのステント271,272の間にらせん状のコネクタ「S」を含んでいる。これらのらせん状のコネクタは、形状は線形か波状かのどちらかであるように形成することができる。このコネクタの最終的な形状は、らせん状のコネクタとしてステント間の関節接合の領域でより大きめかより小さめかの可撓性を持たせるユーザの要望に主として依存するものである。

本発明の方法および装置はより丹念に述べられるであろう。本発明の方法および装置が、たとえば人間の血管組織の動脈、静脈、血管などの身体通路の管腔を拡大するために役立つだけでなく、先に述べた他の身体通路ないし管路を管腔内的に補強するための前述の手法を成し遂げるために役立つことが理解されよう。図6に示されるように、図1,2または3と関連して先に述べられたタイプの拡大可能な管腔内血管移植片即ち人工装具は、カテーテル183に配置されるか取り付けられるかする。カテーテル183はこれと関連した拡大可能で膨張可能部分184を有している。カテーテル183は、カテーテル183の拡大可能で膨張可能部分184に拡大可能な管腔内血管移植片即ち人工装具170を取り付けて保持するための手段185を含む。

好ましくは、この取り付けて保持するための手段185はカテーテル183の拡大可能で膨張可能部分184に隣接してカテーテル183に配置されるリテーナリング部材186を備え、リテーナリング部材186はは拡大可能な管腔内血管移植片即ち人工装具170の各端部172,173に隣接して配置される。

図6に示されるように、リテーナリング部材はカテーテル183と一体に形成されるが、カテーテル183の前先端187に隣接したリテーナリング部材186は、以下により丹念に述べられるが、身体通路170の管腔181中に挿入されるときに移植片即ち人工装具170を保護し保持するためにカテーテル先端187から上方に向かって離れていくように傾斜をつけてある。図6に示されるような残りのリテーナリング部材186はカテーテル183の先端187から下方に向かって離れていくように傾斜をつけてあり、身体通路180からカテーテル183の容易な取り外しを確保する。拡大可能な管腔内血管移植片即ち人工装具170はカテーテル183に配置された後に、先に述べた方法で、この移植片即ち人工装具170とカテーテル183は通常の方法による身体通路180のカテーテル挿入法によって身体通路180内部に挿入される。

通常の方法では、カテーテル183および移植片即ち人工装具170は
身体通路180内部の所望の位置に送出されが、しかるに一方、管腔内移植片170を介して身体通路180の管腔181を拡大することが望まれたり、人工装具170を移植することが望まれる。X線蛍光透視法および/または他の従来の技術はカテーテル183と移植片即ち人工装具170は身体通路内部の所望の位置に送出されることを保証するために役立てることができる。移植片即ち人工装具170は、それからカテーテル183の拡大可能で膨張可能な部分184を広げることによって拡大され、これにより、移植片即ち人工装具170は、半径方向、外方向に身体通路160に押し付けられて接触される。その点については、カテーテル183の拡大可能で膨張可能部分は従来の血管形成バルーン188であってもよい。人工装具即ち移植片170の所望の拡張が達成された後には、血管形成バルーン188は身体通路180から通常の方法で除去してもよい。尚、必要に応じてカテーテル183は身体通路180から通常の方法で除去してもよい。尚、必要に応じて移植片即ち人工装具170を有するカテーテル183は初期に通常のテフロン(商標名)シース189のケースに入れられ、このシース189は人工装具即ち移植片170の拡大よりも前にこれから引き抜かれるようにしてもよい。

人工装具即ち移植片170は初期に、ワイヤメッシュ管即ち管形状部材171は、先に述べたように身体通路180中に挿入することができるように、図1と関連して述べたような、つぶれた所定の第1の直径dを有していることが気づかれよう。先に述べたような目的のために身体通路180内部に人工装具170を移植することが望ましい場合には、このワイヤメッシュ管即ち人工装具170は第2の直径d’まで拡大され、拡大された第2の直径d’は可変であり身体通路180の内径によって決定される。したがって、血管形成バルーン188が膨らむときに、この拡大した人工装具170は身体通路180内部の所望の位置から移動できなかったり、人工装具170の拡大により身体通路170の破裂を引き起こすことはない。

狭窄症の領域を有する身体通路180の管腔181を拡張するために拡大可能な管腔内移植片170を使用することが望まれる場合には、血管形成バルーン188による管腔内血管移植片170の拡大は、狭窄領域は制御した拡張が与えられると同時に血管移植片170の拡張も制御されたものとなり、これにより、血管移植片170は、身体通路180が図4に示されるような管腔内血管移植片170の拡張された直径d’の大きさをつぶし減少させることを防止し、これは可変であり身体通路180の所望の拡張された内径によって決定される。このようにして、拡大可能な管腔内移植片170は血管形成バルーン188のしぼみ時には、身体通路180内部に所望の位置から離れて移動したりはせず、管腔内移植片170の拡大により身体通路180の破裂を引き起こすことはない。

移植片170を拡張するために血管形成バルーン188を膨らますことは一度必要となるだけなので、経管腔内血管形成中の内皮浸食の程度がバルーン膨張時間に比例している限りにおいては、大部分の内皮ないし内膜の内側層または身体通路の内面は保護されるものと思われる。さらに、ステント170の細長い部材間のそのままの内皮つぎはきは、実験的研究によって示されるように、急激な多中心の内皮化パターンをもたらすことになると思われる。

この発明のステントは重合体ないし金属性の材料からできていることが可能であり、いずれにせよ弾性の限界を越えて拡張できるものである。さらにまた、これらはニチノールのような形状記憶材料でできており、従来技術にて開示したように、それらに分与されたある形状記憶を有するものでもよい。

図1,2および3と関連して先に述べられたタイプのステント70,170,270が示され、移植片即ち人工装具170の管形状部材71はこれの壁表面74に配置された生物学的に不活性なコーティングを有する。適当な生物学的に不活性なコーティングの例としては、多孔性ポリウレタン、テフロン(商標名)、あるいは他の従来の生物学的に不活性な可塑性材料がある。コーティングはステントの所望する拡大を妨害しないように薄くて高度に弾性を有するものであるべきである。

この発明は、変更例および等価物が当業者には明らかなもののような、明示され記載されたような構造、動作、正確な材料や実施の形態の正確で詳細な記述に限定されないことが理解されよう。例えば、人工装具即ち移植片を拡大するための手段は、カテーテルに配置された複数の水(油)圧式動作の固定部材でもできるし、複数の血管形成バルーンは人工装具即ち移植片の拡大により利用することができる。したがって、この発明は添付のクレームの範囲によってのみ制限されるものである。

この発明の具体的な実施態様は以下の通りである。
(1)複数の開いた接合はステントの片側に集中され、所定の方向に曲がりを助長する請求項4記載のステント。
(2)渦巻き線は波形である請求項5記載のステント。
(3)上記渦巻き線は形状が略線形であるステントのコネクタ。
(4)少なくとも1つのスロットは他のスロット長の約1.5倍から3倍である請求項1記載のステント。
(5)少なくとも1つのスロットは他のスロット長の約1.5倍から3倍である請求項2記載のステント。
(6)少なくとも1つのスロットは他のスロット長の約1.5倍から3倍である請求項3記載のステント。
(7)少なくとも1つのスロットは他のスロット長の約1.5倍から3倍である請求項4記載のステント。

【発明の効果】
以上説明したように、この発明のステントによれば、身体通路における狭窄症の再発を防止し、身体通路の反動を防止しその内部で多様な大きさに拡大することができるので、身体通路の所望の場所から離れてステントが移動していくのを防止するとともに拡張したステントによる身体通路の裂傷を防止することができる効果がある。

Claims (1)
- 弾性特性を有する複数の細長い接合部材が接合されて形成されており、内部から力が加えられたときに直径が拡大するステントであって、前記ステントは管形状を形成している複数の閉じたセルと、管の一部が開いている複数の開いたセルとから構成されており、前記開いたセルは接合部材によって第1の端部でつながっているとともに第2の端部ではつながっていない隣接した細長いコネクタによって形成され、前記閉じたセルは第2の細長い接合部材に隣接した細長い接合部材によって形成されており、さらに、前記複数の開いたセルはステントの片側に集中して配置されていることを特徴とするステント。
