【発明の属する技術分野】
この発明は、生体組織を把持し、凝固及び切開する1対のジョーを備えた手術器械に関する。

【従来の技術】
一般に、ジョーに発熱体を設け、生体組織を把持した状態で前記発熱体を発熱させて生体組織を凝固及び凝固部位を切開する手術器械として熱凝固切開鉗子が知られている。この手術器械は、通常、生体組織に含まれる血管の止血、生体組織の表層の病変部、出血点の焼灼、避妊を目的とした卵管の閉塞などの多種症例に用いられる。そして、手術器械が血管の止血や、卵管の閉塞を目的として用いられ、患者の処置対象の生体組織を凝固したり、凝固した生体組織を切開したりすることができるようになっている。

上述のような手術器械においては、ケーブルが挿入部やジョーなどの内部に挿通され、このケーブルを介して、このケーブルに接続された発熱体に通電するようになっている。そして、使用時に発熱体から発生する熱が、熱の伝達されてはならない部材、例えばジョーに伝わらないように、通常、発熱体とこれら部品との間に断熱部材が設けられている。従来、これら断熱部材は、一般的に、切削加工や成型加工で製造され、単一部材として形成されている。

前記断熱部材は薄肉に設計されており、ジョーの小型化が図られるとともに、ジョーを前記発熱体の熱的要因による劣化から保護し、手術器械の耐久性を高めるようになっている。

【発明が解決しようとする課題】
しかし、単一部材からなる断熱部材を採用した場合、特にジョーを小型化するために断熱部材の肉厚を薄くするのには限界がある。また、断熱部材が単一部材であると、製造上、必要とする部分に配置される断熱部材の厚さにすべての部分の厚さを合わせなければならないので、断熱させる必要のない部分にも断熱部材が配置されることが考えられる。したがって、断熱部材を単一部材から製造することは、ジョーの小型化に関して適当ではない。上述のような手術器械は生体内へ挿入されるため、ジョーの小型化は最も重要な課題の1つである。

また、従来の手術器械では、ジョーを開閉する操作の際、前記挿入部とジョーとの間で前記ケーブルが撓み、これによってこのケーブルに接続された発熱体に力が加わることがあり、これら発熱体を損傷する可能性がある。しかも、前記ジョーが小型化すると、ケーブルの曲がる動きに対して十分なスペースを確保するのが困難になるので、前記力の発熱体に与える影響が大きくなる。

本発明の目的は、発熱手段を設けた可動ジョーと挿入部との間での通電手段を可撓性チューブに封入して機械的耐久性を有する手術器械を提供することにある。

【課題を解決するための手段】
本発明は、先端側が生体内へ挿入される挿入部と、前記挿入部の基端側に設けられた操作部と、前記挿入部の先端側に設けられ前記操作部によって開閉される可動ジョーと、前記可動ジョーの少なくとも一つのものに設けられ通電により発熱する発熱手段と、前記発熱手段から前記挿入部にわたり配設され前記発熱手段に通電を行う通電手段としてのケーブルと、前記発熱手段を設けた可動ジョーと前記挿入部との間に配設され、前記可動ジョーと前記挿入部との間にわたり位置する前記ケーブルを封入する可撓性チューブとを具備したことを特徴とする手術器械である。
また、前記可撓性チューブの先端を、前記発熱手段を設けた可動ジョーの部材に固定したことを特徴とする。

また、前記発熱手段と前記ジョー並びに/もしくは前記通電手段との間に複数の位置に分割して配置された複数の断熱部材を具備したことを特徴とする手術器械である。

このような構成とすることにより、断熱部材が、断熱される部品ごとに分割して配置されているので、それぞれの断熱部材の形状を単純にすることが可能である。したがって、これら断熱部材の肉厚を薄くすることができ、ジョーの小型化を達成することができる。

また、通電手段を前記挿入部とジョーとの間で固定する固定手段を有することによって、ジョーの開閉時に挿入部内の通電手段に力が加わらず、これに連結されている発熱手段に影響を及ぼさない。したがって、発熱手段の損傷が少ないので、手術器械の耐久性を向上させることができる。

【発明の実施の形態】
図1ないし図4を参照して、以下に本発明の第1の実施の形態に係る手術器械のうち熱凝固切開鉗子1について説明する。図1(a)は、熱凝固切開鉗子の側面図、図1(b)は、鉗子ユニットの側面図である。図2(a)は、鉗子ユニットが取り外された状態の操作部を詳細に示す部分断面側面図である。図2(b)は、着脱部が閉じた状態を示す図2(a)の2B−2B断面図である。図2(c)は、着脱部が開いた状態を示す図2(a)の2B−2B断面図である。

図1(a)に示されているように、熱凝固切開鉗子1は、操作部2と、この操作部2に着脱自在に設けられた鉗子ユニット10とから構成されている。前記操作部2は、本体部5と、この本体部5と一体的に形成された固定ハンドル6と、この固定ハンドル6に対して開閉できるように、前記本体部5に枢支軸7を支点として回動自在に連結された可動ハンドル8とを有する。

前記鉗子ユニット10の先端側には、図1(b)に示されているように、生体に先端側が挿入される円筒状の挿入部3が設けられ、この挿入部3の先端部には、対をなす第1のジョー4aと第2のジョー4bとが設けられている。また、前記挿入部3の基端部には、前記本体部5に対して、この挿入部3を、これの軸心を中心に回動させるための環状に突出した回動ノブ9と、この回動ノブ9より基端側に位置して形成された円周溝9aとを有する着脱部202が装着されている。

前記鉗子ユニット10の基端部には、細長い駆動軸11が延出している。この駆動軸11の先端部は、前記挿入部3に同軸に挿通され、この駆動軸11の基端部は、チューブ201を介してコネクタ200に接続されている。このコネクタ200は、ケーブルを介して電源装置(図示されていない)に接続される。前記駆動軸11と挿入部3との連結部分には、前記回動ノブ9から軸方向に所定の距離離間した着脱ストッパ204が装着されている。また、前記駆動軸11には、環状に切欠した切欠部203が形成されている。この切欠部203は、前記鉗子ユニット10が前記本体部5に接続されるときに、前記可動ハンドル8に係合される。この係合については後述する。

図2(a)(ここで鉗子ユニット10は、2点鎖線で示されている)に示されているように、前記固定ハンドル6と前記可動ハンドル8とは、前記駆動軸11に関して同じ側(図2(a)においては下側)に位置されている。この固定ハンドル6のほぼ中央後端面には、棒状の開閉ストッパ5aが前記可動ハンドル8に向かって突設されている。この開閉ストッパ5aは、これらハンドル6,8が完全に閉じた状態にあるとき、前記可動ハンドル8のほぼ中央先端面に当接するように位置している。

これらハンドル6,8を連結している前記枢支軸7の周りには、両端がL字状に曲げられたタイプのねじりコイルばね(第1のねじりコイルばね)305が設けられている。この第1のねじりコイルばね305の一端部は前記可動ハンドル8のほぼ中央部分に嵌合し、中間部は前記枢支軸7と同軸的に巻回し、また、他端部は、この枢支軸7と平行になるように曲がって可動ハンドル8の基端部に係合している。この第1のねじりコイルばね305は、前記1対のハンドル6,8が互いに関して閉じるように、前記可動ハンドル8を固定ハンドル6の方へ付勢している。

前記可動ハンドル8の基端部には、前記鉗子ユニット10が装着されて駆動軸11の前記切欠部203と係合する係合部306が設けられている。また、前記固定ハンドル6の基端部には、前記着脱部202の外周面とほぼ相補的な形状を有する保持孔302が形成されている。この保持孔302の内周面には、着脱部202の前記円周溝9aと係合可能な、環状の突条部5bが形成されている。前記保持孔302は、前記固定ハンドル6の基端部に位置した上部本体300と下部本体301との対向面によって規定されている。この下部本体301は、下部で別のピン402によって前記本体部5に固定されている。また、前記上部本体300と下部本体301とは、図2(b)に示されているように、前記保持孔302を矢印C方向に開閉自在なように、一端で枢支ピン401により枢支されている。

前記上部本体300の他端には、両端がストレートタイプのねじりコイルばね(第2のねじりコイルばね)304が取着されている。この第2のねじりコイルばね304には、先端に爪形状部分303aが形成された爪部材303が、前記上部本体300に対して回動自在に取着されている。そして、この爪部材303は、前記第2のねじりコイルばね304によって、時計回りの方向へ付勢されている。

前記1対のジョー4a,4bの先端部は、これらジョー4a,4bが回動する面と直交する方向に湾曲しており、生体組織を剥離しやすいように形成されている。また、前記第1のジョー4aの把持面、すなわち生体組織との接触面側には、熱伝導性の優れた材質からなる薄板状の発熱板40が設けられ、前記第2のジョー4bの把持面には、柔軟部材41が設けられている。この発熱板40は、前記第1のジョー4aの長手方向に沿って配置された長方形状の金属板であり、この発熱板40の先端部は、第1のジョー4aの前記先端部の湾曲形状に合わせて湾曲している。この発熱板40の下部は、先端に向かって肉薄となるようにテーパー状になり、かつ長手方向に沿って形成された刃部42になっている(例えば、図4(d)参照)。

この刃部42と前記柔軟部材41とは、前記2つのジョー4a,4bが閉じた状態にあるときに互いに当接する。また、前記第1のジョー4a、及び第2のジョー4bのそれぞれの先端部は、前記刃部42及び前記柔軟部材41より長手方向に延出して、互いに対向する面に把持面を形成している。この把持面は、これらジョー4a,4bの先端部の間の間隔が狭くなるように、互いに所定の距離離間して対向する平面形状になっている。このような構成により、生体組織の剥離操作性が向上する。

前記発熱板40の上部には、互いに長手方向に所定の距離離間した複数の突出片43a,43b(図面上では2つ)が上方に向かって形成されている。これら突出片43a,43bの後端面には、それぞれすり割が形成されており、このすり割には、前記発熱板40と共に発熱手段を構成する薄膜加熱抵抗素子などの発熱体47a,47bが挿入されている。これら発熱体47a,47bは、前記すり割内で、前記発熱板40に接触しており、これらの間に、熱結合部44a,44bが規定されている。前記発熱体47a,47bは、温度分布が均一となるようにそれぞれ等しい間隔で配置され、それぞれ独立して通電手段としての同軸ケーブル50a,50bに接続されている。

前記発熱板40は、銅、銀、タングステンなどの熱伝導性の良い材料で形成されるのが好ましく、前記発熱体47a,47bは、薄膜抵抗素子、セラミックヒーター、カートリッジヒーター、PCTヒーターなどで構成され得る。また、前記柔軟部材41の材質として、ゴム(シリコーンゴム、フッ素ゴム、エチレンプロピルゴム、ブチルゴムなど)、ゲル(シリコーンをベースとしたαゲルなど)、テフロン(登録商標)などが使用されるが、これらに限定されない。

以下に、図1ないし4を参照して、上記構成による本発明の一実施の形態に係る手術器械の動作について説明する。手術器械は、生体組織(図示されていない)を第1及び第2のジョー4a,4bで把持し、この状態で、前記第1のジョー4aに設けられた発熱体47a,47bを発熱させることによって、生体組織を凝固・切開する。

この手術器械は、前記操作部2の本体部5に前記鉗子ユニット10を装着することができる。この装着のときには、前記本体部5に設けられた上部本体300を開き、前記回動ノブ9と着脱ストッパ204との間の前記着脱部202を下部本体301側の内周面、すなわち保持穴302の下面に載せる。そして、前記切欠部203と係合部306、及び前記円周溝9aと突条部5bとをそれぞれ係合させる。この状態で、前記上部本体300を閉じ、前記爪部材303と前記凸部5cとを係合することによって、前記鉗子ユニット10を前記本体部5に対して、回動自在な状態で長手方向にロックすることができる。

前記可動ハンドル8を回動させて前記ジョー4a,4bを開閉させるとき、可動ハンドル8によって前記駆動軸11が進退されると、この駆動軸11のチャンネル内に接着された通電手段である同軸ケーブル50a,50bも動かされる。このとき、これら同軸ケーブル50a,50bは、固定部材である樹脂チューブ105によって封入されており、この中に1つに束ねられて摺動することができるので、前記駆動軸11が進退することによって加わる力の影響をほとんど受けない。したがって、前記同軸ケーブル50a,50bに接続されている発熱体47a,47bに過度の力が加わることが無いので、これら発熱体47a,47bの破損を防止する。

また、前記ジョー4a,4bが小型化して前記挿通空間37aが狭くなっている場合、前記同軸ケーブル50a,50bは、この挿通空間37aにおいて動くことができる空間が制限されることが考えられる(例えば、前記駆動軸11の進退動作に対して、つかえてしまう)。しかし、これら同軸ケーブル50a,50bが前記樹脂チューブ105に封入されているので、比較的狭い空間内であっても前記ジョー4a,4bを適切に開閉させることができる。

上述のように、生体組織を把持した状態で、図示されていない電源装置から、前記コネクタ200と、このコネクタ200に接続された前記同軸ケーブル50a,50bとを介して、前記発熱体47a,47bにそれぞれ電流が通電される。通電された2つの発熱体47a,47bは、これらの電気抵抗により発熱し、この熱が前記熱結合部44a,44bを介して前記発熱板40に伝達される。そして、この発熱板40表面に接触している血管などの被処置部の生体組織が凝固・切開される。

前記2つの発熱体47a,47bには、それぞれ独立的に前記同軸ケーブル50a,50bを介して前記電源装置に接続されている。したがって、前記発熱体47a,47bをそれぞれ独立に設定された発熱温度に制御することができる。

すなわち、前記発熱板40の生体組織に接触している部分、例えばこの発熱板40の先端側の発熱量が低下し、温度が低下した場合には、この部分に対応して位置された先端側の発熱体47aの抵抗値を検出してこの発熱体47aの出力を増す制御を行い、前記発熱板40の生体組織に接触していない部分、例えばこの発熱板40の基端側の発熱量が増加し、温度が上昇した場合には、この部分に対応して位置された基端側の発熱体47bの抵抗値を検出してこの発熱体47bの出力を減らす制御を行うことができ、前記発熱板40の温度ムラを防止して、凝固・切開を迅速かつ確実に行うことができる。

本発明に係る手術器械は、前記発熱板40の両側に加えて、上側、並びに同軸ケーブル50aとの間に、複数の断熱部材100,101,102,103が分割して設けられている。したがって、上述のような発熱板40の制御において、前記ジョー4a、4bや同軸ケーブル50a,50bに熱的損傷が生じるのを防止することができる。

また、前記断熱部材100,101,102,103が複数設けられているので、高度に断熱が必要な場所には、肉厚の断熱部材、省スペースを必要とされる場所には、薄肉の断熱部材をそれぞれ配置するなど、断熱部材に関する設計時の自由度が高い。したがって、前記ジョー4a,4bの小型化を容易に達成することができる。

BACKGROUND OF THE INVENTION

In general, a thermocoagulation incision forceps is known as a surgical instrument for providing a heating element on a jaw and causing the heating element to generate heat while holding the living tissue to coagulate the living tissue and incise the coagulation site. This surgical instrument is usually used for various cases such as hemostasis of blood vessels contained in living tissues, lesions on the surface of living tissues, cauterization of bleeding points, occlusion of fallopian tubes for the purpose of contraception.
