医療機関は、かかった医療費について診療報酬明細書（以下、レセプトという）を作成し、患者本人が負担する自己負担金を除いた残りを保険機関に保険金として請求する。医療事務計算機は、レセプト発行に際して、実施した診療行為とともにその患者の病名もレセプトに記載する。また、電子カルテ装置も、従来の紙のカルテを電子情報に置き換えて管理するものであるので、患者の傷病名は必須項目である。そこで、これらの装置にあっては、医師が患者に実施した検査や処置、注射や投薬処方といった診療行為を入力して、患者の診療データとして保管するとともに、それらの診療行為の原因となった傷病名も入力し、管理している。

診療データの入力は受診のたびに発生し、しかも、窓口処理では患者を待たせないために、大量のデータを高速に入力する必要がある。そこで、診療データの入力に際しては、前述したように大量・高速に入力する必要があるので、コードで入力するのが普通である。そのために、個々の診療行為についてその名称や保険点数、及び入力用のコードとを登録した診療コードマスタを備えている。そして、病名についても長い病名をそのまま記憶するのではなく、診療データと同様にコードで記憶している。そこで、病名とそのコードを登録した病名コードマスタを備えている。そして、傷病名の入力はそれほど頻繁には発生しないので、診療データの入力と傷病名の入力とは別の入力画面で行われている。
