【発明の属する技術分野】

本発明は、歯科用診療器具によって代表される、水(清浄水)を噴射、放出などすることによって診療を行う診療器具を備えた診療装置で用いられ、給水源から供給された水を診療器具に導出させるための給水管路を洗浄する管路洗浄方法及び装置、この管路洗浄装置を備えた診療装置に関する。
【従来の技術】

歯科用診療器具によって代表される、清浄水を噴射、放出などさせて治療をおこなう診療器具としては、たとえば水と空気を混合させたものを噴出させて口腔内を掃除する歯科用シリンジなどが知られているが、このような診療器具では、その給水管路は、その治療にもちいる水の長期残留による菌の繁殖や、治療中の患者から感染菌が混入するサックバックなどの問題がある。このため、給水管路は定期的に洗浄、消毒滅菌をする必要がある。

そこで、従来より、給水管路を洗浄する種々の方法が提案されているが、本出願人は給水管路の消毒滅菌が簡易な方法で完全にでき、また、消毒滅菌後は、給水管路を清浄水で十分にすすぎ洗浄できる管路洗浄方法として、特開2000−5200号公報に記載されたものを提案している。

図17は、この管路洗浄方法を実現する装置の管路系の概略構成を示す系統図である。ここでは、管路洗浄装置の例として、歯科用の場合を示し、以下の他例についても同様である。

この管路洗浄装置50の管路系は、この図17中の17で示すように、給水源Wから、それぞれの診療器具HPに給水する給水管路12と、消毒滅菌液タンク13などと、吸引源Vから吸引力を得て、排水などを吸引してバキュームタンクVTに排出する吸引管路vと、それぞれの排水などを回収して排水先Dに排出する排水管路dから構成され、給水管路12の消毒滅菌洗浄機能を備えていることを特徴とする。

給水管路12は、給水源Wから供給された水を、各々に開閉弁SWを設けた診療器具HPに導出するための給水枝管路部14を有し、この給水枝管路部14の上流には、給水源Wを切替え接続する切替制御弁16、この切換制御弁16の一方出口に接続され開閉弁51を設けた消毒滅菌液すすぎ管路15と、この消毒滅菌液すすぎ管路15の開閉弁51の出口側に合流し、消毒滅菌液タンク13に接続された消毒滅菌液注入管路31とを備えた基本構造となっている。

ここで、手動給水弁V0は、長時間、管路洗浄装置50を使用しない場合に、誤動作で給水されることがないように、給水を手動で開閉するものである。逆流防止弁V1は、給水の逆流を防止するものである。減圧弁V2は、給水管路12dの水圧をフィードバックしており、水圧を一定値に保つものである。給水元弁SV1は、電磁弁で構成されており、制御回路により、自動的に操作されるようになっている。給水フィルタF1は、診療器具HPに供給される水を濾過するためのもので、水道水などに含まれる不純物などを取り除く。

切替制御弁16は、自動作動し得る2方向切替弁で、その出力ポートの一方には、給水枝管路14が接続され、他方には、消毒滅菌液すすぎ管路15が接続されている。

切替制御弁16に接続された給水枝管路部14aはT字状の分岐46を有しており、その一方の分岐には、消毒滅菌液タンク13に通じている消毒滅菌液注入管路31が接続されている。給水枝管路部14bは、分岐46の他の一方から延び、給水ウォーマWAに接続されている。この給水ウォーマWAは、歯科治療に用いる場合、給水を適温に加熱するためのものである。

給水ウォーマWAからは、給水枝管路部14bがさらに延びており、ここで歯科用に必要な診療器具HP(1〜5)の分だけの給水枝管路部14c(1〜5)に枝分かれし、給水枝管路部14c(1〜4)においては、それぞれ、途中に給水開閉弁SW(1〜4)を有して、それぞれの診療器具HP(1〜4)に接続されている。給水枝管路部14c(5)は、開閉弁なしに、あるいは開閉弁を介して、診療器具HP(5)に接続されている。

ここで、例としてあげた歯科用管路洗浄装置においては、診療器具HP(1)はコップ給水であり、患者の必要に応じて、口中洗浄のためのうがい水をコップに給水するためのものである。診療器具HP(2)はエアータービンハンドピースであり、高速で歯面を研削するものであるが、研削中に、研削処置部を適宜冷却し、また、研削屑などが飛散しないように給水の必要のあるものである。診療器具HP(3)はマイクロモータハンドピースであり、低速で歯面を研削するものであるが、同様に給水の必要のあるものである。診療器具HP(4)はスケーラであり、歯垢などを除去するためのものあるが、この場合も、処置部の冷却などのために給水が必要とされるものである。診療器具HP(5)はスリーウエイシリンジであり、別途供給される加圧空気とともに給水を噴射して、処置部を適宜、冷却あるいは清掃するものである。

また、ここでは、給水枝管路部14a、14b、14c(1〜5)を総称して、給水枝管路部14と呼んでおり、給水管路12a〜12fと給水枝管路部14を総称して、給水管路12と呼んでいる。

この管路系を示す図は、消毒滅菌洗浄時のものであり、これらの診療器具HP(1〜5)はそれぞれ、通常の待機状態あるいは使用状態ではなく、消毒滅菌洗浄のために洗浄タンクWBにセットされ、この洗浄タンクWBは、歯科用管路洗浄装置のスピットンSPの上に載せられ、コップ給水である診療器具HP(1)からの排水も受けるようになっている。ここにスピットンSPとは、歯科治療時に、主に、患者が口腔内をうがい洗浄したのちの排水を吐出させるための吐出皿のことをいい、通常、スピットンSPの上部には、コップ受けと、そのコップに給水するコップ給水が設けられている。

消毒滅菌液すすぎ管路15は、切替制御弁16の出力ポートに消毒滅菌液すすぎ管路15aを介して接続されたすすぎ開閉弁51、すすぎ開閉弁51から管路洗浄装置50に備えられた消毒滅菌液タンク13に通じる消毒滅菌液すすぎ管路15bから構成されており、すすぎ開閉弁51によって、給水源Wから給水枝管路部14への給水を開閉する。

消毒滅菌液注入管路31は、給水枝配管部の分岐46に接続された消毒滅菌液注入管路31aと、その消毒滅菌液注入管路31aの他端に接続された逆流防止弁33と、逆流防止弁33に消毒滅菌液注入管路31bを介して接続されたポンプ32と、そのポンプ32と消毒滅菌液タンク13を接続し、タンク13内の消毒滅菌液Qをポンプ32で吸い上げるようにした消毒滅菌液注入管路31cから構成されている。このポンプ32は、電気モータまたはエアーモータなどで構成されている。

なお、上記で説明した、消毒滅菌液タンク13、消毒滅菌液注入管路31b、31c、ポンプ32、逆流防止弁33、すすぎ開閉弁51、消毒滅菌液すすぎ管路15aなどからなる部分で、図に点線で囲った部分を消毒滅菌液ユニットPUと呼び、この消毒滅菌液ユニットPUは、管路洗浄装置50の本体内に内蔵されていることを原則とするが、別体とすることもできる。

ここで、バキュームタンクVTは、トラップ手段(不図示)を有し、吸引管路v3によって吸引されてきた排水などを、分離回収するもので、吸引源からは、吸引管路v2によって吸引力を供給されている。この分離回収された排水は、ドレン部VTaに貯留される。また、バキュームシリンジVSは、通常は、口腔内に供給された診療器具HP(2〜5)などによる水や切削屑などを吸引回収するためのものであるが、消毒滅菌吸引時には、洗浄タンクWB内の貯留排水を吸引するために用いられる。

排水管路dは、スピットンSPの排水口と排水先Dを接続しており、通常は、スピットンSPに排出された、患者のうがい後の排水などを排水する。また、その途中には、バキュームタンクVTからの吸引ドレン管路v4が接続されており、バキュームタンクVTのドレン排水も排水する。

消毒滅菌洗浄時には、図示するように、スピットンSPの上に載せられた洗浄タンクWBに貯留される消毒滅菌液Qとすすぎ洗浄の排水は、主として、バキュームシリンジVSで吸引排出され、その排水だけが、バキュームタンクVTで分離回収されて、吸引ドレン管路v4を介して、排水管路dから、排水先Dに排水される。しかし、バキュームシリンジVSの吸引能力が不足した場合などには、洗浄タンクWBには、オーバーフロー孔(不図示)が設けられており、そのオーバーフロー穴から、タンクWBに貯留した排水があふれ出て、スピットンSPの排水口から、排水管路dを介して、排水先Dに排水される。

この管路洗浄装置50における基本動作について説明する。

この管路洗浄装置50では、通常使用時において、切替制御弁16は、給水枝管路部14側にセットされており、給水源Wからの水は、給水枝管路部14から、給水開閉弁SWを有した診療器具HPに供給され、治療や処置が行われる。

消毒滅菌時には、まず、切替制御弁16を消毒滅菌液すすぎ管路15側に切替え、すすぎ開閉弁51を閉じる。この状態では、給水枝管路部14への給水源Wからの給水は停止される。そして、消毒滅菌液注入管路31に設けられたポンプ32を作動し、消毒滅菌液タンク13に貯留された消毒滅菌液Qを消毒滅菌液注入管路31を通じて、分岐46から給水枝管路部14bに注入し、給水開閉弁SWを開き、それぞれの診療器具HPから消毒滅菌液Qが吐出するのを確認してポンプ32を停止し、給水枝管路部4と各診療器具HPに消毒滅菌液Qが充填された状態で所定時間放置する。

ついで、切替制御弁16を消毒滅菌液すすぎ管路15側に切替保持したままで、すすぎ開閉弁51を開き、更に給水開閉弁SWを開いて、給水源Wからの水を消毒滅菌液すすぎ管路15に供給する。

この状態で、給水は、すすぎ開閉弁51を介して、消毒滅菌液注入管路31を通って給水枝管路部4へと供給されるので、消毒滅菌液注入管路31もすすぎ洗浄され、さらに、給水枝管路部14、それぞれの給水開閉弁SWと診療器具HPもすすぎ洗浄され、消毒滅菌液Qは、このすすぎ洗浄の水とともに洗浄タンクWBに貯留され、バキュームシリンジVS、吸引管路v3、バキュムタンクVTを介して、あるいはスピットンSPから直接、排水管路dに排水される。

こうして、すすぎ洗浄終了後は、すすぎ開閉弁51を閉じてから、切替制御弁16を給水枝管路14側に切替え、給水開閉弁SWを必要に応じて開閉して、通常の治療や、処置が行われる。

このように、この管路洗浄装置によれば、診療器具と給水枝管路部の部分の消毒滅菌洗浄を完全に行うことができるとともに、消毒滅菌後は、その給水枝管路部などを消毒滅菌するために消毒滅菌液を注入する消毒滅菌液注入管路を含めて、すすぎ洗浄することができた。

したがって、消毒滅菌洗浄後、切替弁16を給水枝管路14側に切り換えることにより、管路には、消毒滅菌液は一切残っておらず、通常の使用時に、誤動作などで、消毒滅菌液が給水に混入することがなかった。また、消毒滅菌液は、消毒滅菌に必要な時に、必要な時間だけ、管路系に注入、滞留するだけなので、管路系への消毒滅菌液の悪影響を最小限に抑えることができた。
【発明が解決しようとする課題】

しかしながら、上記従来の洗浄方法でも、まだ、給水管路の消毒滅菌、洗浄に比較的長時間を要しており、また、洗浄に用いる洗浄水のさらなる節約の要請や、より効率的な消毒滅菌、洗浄の要請にも応える必要があった。さらに、この給水管路の洗浄が通常の診療の支障にならないようにする必要もあった。

本発明は、このような問題を解決しようとするもので、給水管路の消毒滅菌、洗浄の時間を更に短縮でき、洗浄水を節約して、より効率的な消毒滅菌、洗浄ができ、さらに、給水管路の洗浄が通常の診療の支障にならないようにすることができる管路洗浄方法及び装置、この管路洗浄装置を備えた診療装置を提供することを目的としている。
【課題を解決するための手段】

請求項1から6においては管路洗浄方法を、請求項7から12においては管路洗浄装置を、請求項13においては、管路洗浄装置を備えた診療装置を提案している。

ここで、洗浄とは、有機物や汚れを物理的に除去することをいい、消毒とは、人に対して病原性を有する微生物を殺滅することをいう。一方、滅菌という場合は、物質中の全ての微生物を殺滅あるいは除去することをいうが、この滅菌に対して、消毒は、洗浄から滅菌に至る中間的なものを含む幅広い概念である。さらに、消毒滅菌という場合は、消毒だけを行う場合と、滅菌だけを行う場合と、消毒、滅菌の双方を行う場合とを含むこととする。

請求項1に記載の管路洗浄方法は、診療のために水を用いる診療装置で用いられ、給水源から供給された水を診療器具に導出させるための給水管路を洗浄する管路洗浄方法であって、前記給水管路から排出される洗浄用済水を一時的に受けるための洗浄用済水受け容器と、この洗浄用済水受け容器に貯留される水を吸引して前記診療装置外に排出するバキュームタンクとを備え、バキュームタンクに設けられた満水センサの検知信号と、前記検知信号が所定時間内に検知されたかどうかを判断する制御装置により、このバキュームタンクの作動を監視して、前記洗浄用済水受け容器に貯留される洗浄用済水が前記洗浄用済水受け容器からオーバーフローしないように監視するオーバーフロー監視手段を設けたことを特徴とする。

このタイムチャートは、横軸に時間(T)をとり、この時間経過に沿って、上述の給水元弁SVw、開閉弁SVa〜SVd、送気弁SVeがどのように開閉制御されるかを示したもので、斜線部分が弁が開かれている状態、直線部分が弁が閉じられている状態を表している。また、最下部に示した「ON」、「OFF」は、図1s(b)の洗浄スイッチSによる洗浄工程の開始と終了を示しているが、この洗浄工程は、洗浄スイッチSのONによって開始され、一連の工程が終了した際には、制御装置6によって自動的にOFFされ、終了するものである。

この洗浄工程では、洗浄スイッチSをONする前は、給水管路は待機状態、つまり、残留保持状態となっており、給水元弁SVwは開かれているが、それぞれの診療器具HPへの開閉弁SVa〜SVd、送気手段1の送気弁SVeは閉じられた状態となっている。

ここで、洗浄スイッチSをONすると、送気弁SVeが開かれ、同時に開閉弁SVa〜SVdも開かれて、加圧源PAからの加圧空気が押出し空気として、一斉にそれぞれの給水枝管路1fa〜1fdに供給され、この押出し空気によって、それぞれの給水枝管路1fa〜1fd、開閉弁SVa〜SVd、給水枝管路Ta〜Td、また、診療器具HP(図5参照。ここでは省略。)が接続されている場合には、これらの診療器具HPに残留した残留水が排出される。

この工程を、送気押出し工程といい、この場合は、送気押出しの対象となる管路1fa〜1fd全てに一斉に押出し空気を送気する一斉送気押出しを行っている。

この送気押出し工程が一定時間保持されると、送気弁SVeが閉じられ、代わりに給水元弁SVwが開かれ、新しい水道水である洗浄水が供給可能となり、これに応じて、それぞれの開閉弁SVa〜SVdが順次個別に開閉され、それぞれの給水枝管路1fa〜1fdなどに一定時間ずつ洗浄水が供給され、全ての管路への洗浄水の供給が終了すると、洗浄工程を終了する。

こうして、洗浄工程に送気押出し工程を含むようにすると、前に残留していた残留水などをいったん排出してから、本来の洗浄用の洗浄水を供給するので、供給された洗浄水が残留水と混じるようなことがなく、少ない注入量でその効果を発揮することができ、結果、洗浄水などを節約することができ、時間の短縮にもなる。

管路洗浄の際には、図示するように、バキュームタンク4のバキュームシリンジVSを洗浄用済水受け容器3に差し込み、診療器具HPa〜HPdから排出され、貯留されている洗浄用済水をバキュームタンク4の方へ吸引するが、この場合も、満水センサ4bの満水信号によって、溜まった水を一気に吸引ドレン管路4dから排水管路dに排出するという動作を繰り返して、吸引されてくる洗浄用済水でバキュームタンク4が溢れないようにすると共に、洗浄用済水受け容器3へ貯留される洗浄用済水がこの容器3から溢れないようにしている。

ここで、バキュームタンク4の満水センサ4bの検知信号が、図5(b)に示すように、制御装置6に送られており、制御装置6は、所定の時間内に満水信号がない場合には、吸引ポンプ4cの異常と判断し、アラームを鳴動させると共に、給水元弁SVw、あるいは、開閉弁SVa〜SVdに閉信号を送り、これらの弁を閉じさせるようにしている。

つまり、吸引ポンプ4cが正常に動作している間は、診療器具HPa〜HPdから排出される洗浄用済水は、洗浄用済水受け容器3からバキュームタンク4へ吸引され、タンク4が満水になりしだい排水管路dに排出されるので、洗浄用済水受け容器3に溜まった洗浄用済水がオーバーフローすることはないが、吸引ポンプ4cが正常に動作していない場合は、洗浄用済水は、洗浄用済水受け容器3からバキュームタンク4へ吸引されず、オーバーフローすることなる。

このため、タンク4が満水にならない時間を検知することで、吸引ポンプ4cの異常を検知し、オーバーフローを防止しているのである。こうすることによって、従来より用いている満水センサ4bの検知信号だけで、吸引ポンプ4cの異常、ひいては、洗浄用済水受け容器3のオーバフローを検知することができ、オーバフロー検知のために余分なセンサを設ける必要がない。

こうして、順調にバキュームタンク4が作動している間は、洗浄用済水受け容器3から洗浄用済水が溢れ出さないようになっており、万一、バキュームタンク4が作動不良で、満水センサ4bが所定の時間内に満水信号を発生させない場合には、洗浄水の供給を止めるようにして、洗浄用済水受け容器3から洗浄用済水が溢れないように洗浄用済水受け容器3のオーバーフローを監視している。

一方、こうして順次洗浄する場合に、図9(b)に示すように、管路抵抗の少ない給水管路の順に順次洗浄する抵抗順順次洗浄とすることもできる。そのようにすると、管路抵抗の少ない、つまり、時間あたりの通水量の大きい給水管路を介して、複数の給水管路へ枝分かれする前の部分にある残留水(この部分の残留水が装置全体の残留水の9割を占める。)をより早く洗浄水に入れ換えることができ、その後、管路抵抗の大きい給水管路を洗浄する場合は、枝分かれ後の部分の残留水(残り1割)の排出、洗浄だけを考慮すればよくなるので、全体の洗浄時間が短くなる。
