【発明の属する技術分野】
本願は、1998年7月14日に付与された米国特許第5,779,716号である1995年10月6日に出願された「体内の管、体腔、及び組織から固形物を除去するための装置」という標題の米国特許出願第08/539,875号の一部継続出願である1998年1月26日に出願された「逆にできるバスケットを含む、体内の管、体腔、及び組織から固形物を除去するための装置」という標題の米国特許出願第09/013,178号の一部継続出願である。

本発明は、凝血塊及び塞栓性材料を心血管系の血管から除去するための装置に関し、更に詳細には展開可能なサックがガイドワイヤの先端と隣接してガイドワイヤに取り付けられた装置に関する。

【従来の技術】
体内の管、特に心血管系の血管内で血栓塞栓を捕捉し、除去することが臨床的に非常に重要である。血栓は、自然に形成して静脈又は動脈の内壁に付着し、内腔を部分的に又は完全に遮断する、血液の血小板、フィブリン、凝血因子、及び細胞成分の凝集物である。

血栓形成は、急性又は慢性のいずれかである。慢性形成は、遺伝学的傾向又は透析患者の房室グラフト等の異物に対する生理学的応答に関する。慢性形成により血管内腔が徐々に減少し、これに伴って関連した組織への酸素送出量が減少する。治療には、バルーン血管形成術、ステント装着を用いるバルーン血管形成術、血栓溶解、又は血栓摘出術を用いる血栓溶解が含まれる。急性形成は、鈍力(blunt force)外傷又は塞栓時の応答である。急性形成は、血管の内腔を直ちに完全に閉塞する。末梢血管では、急性形成に応じた治療には、例えばウロキナーゼ、塞栓摘出術、又はこれらの両方を用いる溶解が含まれる。

冠状血管の狭窄又は閉塞は一般的である。治療は、冠状動脈バイパスグラフト(CABG)手術又は経皮的血管内腔拡張術(PTCA)のいずれかである。CABG手術は、別の血管を動脈に移植し、閉塞部の周囲で血液をシャントすることによって、冠状動脈内の狭窄部又は閉塞部を越えて心筋への通常の血流を回復するために広範に使用されている外科的処置である。グラフトは、別の場所、通常は患者の脚部から採取する。伏在静脈がグラフトとして最も頻繁に使用される。

BACKGROUND OF THE INVENTION

The present invention relates to a device for removing clots and embolic material from cardiovascular vessels, and more particularly to a device in which a deployable sac is attached to a guidewire adjacent to the guidewire tip.

It is clinically very important to capture and remove thromboembolism in the body's ducts, especially in the blood vessels of the cardiovascular system. A thrombus is an aggregate of blood platelets, fibrin, clotting factors, and cellular components that spontaneously form and adhere to the inner wall of a vein or artery and partially or completely block the lumen.

Thrombus formation is either acute or chronic. Chronic formation relates to genetic trends or physiological responses to foreign bodies such as atrioventricular grafts in dialysis patients. Chronic formation causes a gradual decrease in vascular lumen, with a concomitant decrease in oxygen delivery to the associated tissue. Treatment includes balloon angioplasty, balloon angioplasty using stenting, thrombolysis, or thrombolysis using thrombectomy. Acute formation is a response upon blunt force trauma or embolism. Acute formation immediately and completely occludes the lumen of the blood vessel. In peripheral blood vessels, treatment in response to acute formation includes lysis using, for example, urokinase, embolectomy, or both.

Emboli occur randomly during certain procedures to treat chronic conditions and during emergency clinical procedures. These emboli are caused by thrombi or plaques (cholesterol and cell debris) that move in the bloodstream during the procedure and move to other small blood vessels and impede blood flow. The resulting complications include stroke, myocardial infarction, renal failure, limb loss, or even death. These complications can be reduced by using a device to capture an obturator that occurs untimely during the procedure.

Embolization is a well-accepted procedure. A device-mediated procedure to mechanically remove clot formation from the blood vessel is performed across the lumen. A Fogarty balloon catheter is a commonly used device for performing embolectomy. Embolectomy is the treatment of choice for patients with acute vascular embolism. Immediate treatment may be required to save the limb. Causes of acute embolism include rheumatic heart disease, arteriosclerotic heart disease, atrial fibrillation, and blunt trauma.

Fogarty balloon catheter means an acute case. The blood vessel to which the obturator is attached and the position of the obturator in the blood vessel where the position is determined are confirmed. The vessel lumen is then accessed through either the incision or the access port. If an incision is used, the blood vessel is clamped on the side of the incision remote from the clot.
