本発明は、中耳腔の機能変動を指標とした生理的下顎位を定量的に表現するための下顎運動に伴う中耳動特性の表示方法およびその測定装置に関するもので、さらに詳しくは、いわゆる顎関節症とされる顎機能異常、低位咬合、無歯顎などの3次元的下顎位(咬合位)の判断、設定を定量的に行うための下顎運動に伴う中耳動特性の表示方法およびその測定装置に関するものである。

【従来の技術】
従来より、下顎位(咬合位)の判断には、下顎安静位による方法、発音による方法、嚥下位による方法、咬合力による方法、下顎運動による方法、筋電図による方法、X線写真による方法など多数の指標がある。

下顎安静位による方法とは、被検者が口を自然に閉じた状態をその被検者から直接問診する方法であり、発音による方法とは、被検者が上下の歯を噛んでカチカチという音を聞いて判断する方法であり、嚥下位による方法とは、被検者の下顎にセンサを取り付けて水を飲ませその時の音を聞いて判断する方法であり、咬合力による方法とは、被検者にフィルム状の薄いセンサを噛ませたときの咬合力により判断する方法であり、下顎運動による方法とは、被検者に下顎の運動をさせて問診する方法であり、筋電図による方法とは、被検者の下顎に筋電図センサを取りつけ咬合時の筋電図により判断する方法であり、X線写真による方法とは、X線写真を撮影し下顎の静的状態を観察する方法である。

【発明が解決しようとする課題】
以上のような方法は、いずれも医師などの検者の経験による独自の定性的な判断であって、客観性に乏しく、患者の個人差や検者の主観を排除しえず、定量的な判断ができないものであった。

本発明は、中耳腔の機能変動を指標とした生理的下顎位を定量的に表現して表示する方法と、顎関節症とされる顎機能異常、低位咬合、無歯顎などの3次元的下顎位(咬合位)の判断、設定を定量的に行うための測定装置を提供することを目的とする。

【課題を解決するための手段】
本発明は、耳腔内の動的コンプライアンスと上下顎の咬合位からの下顎運動に伴う距離とをそれぞれ2次元軸に設定し、前記動的コンプライアンスと前記距離とをパラメータとし、前記咬合位からの下顎運動に伴う距離の変化に伴う前記動的コンプライアンスの変化を2次元のグラフを構築して表示するようにしたことを特徴とする下顎運動に伴う中耳動特性の表示方法である。また、被検者10の外耳道11に装着され耳腔の動的コンプライアンスを検出するプローブ13と、このプローブ13を取付け被検者10の顔面に装着されるフェースボウ43、44と、被検者10の下顎に取り付けられるシーネ55と、このシーネ55と前記フェースボウ43、44との距離を測定する距離測定手段33とからなり、前記プローブ13は、その耳栓14に、音導孔15、16、エア孔17が外耳道11内に向けて設けられ、前記エア孔17には、検出圧力を電気信号に変換する圧力センサ18が接続され、前記音導孔15には、マイクロホン23とこのマイクロホン23のアナログ信号値をコンプライアンス信号値に変換するコンプライアンス回路26が接続され、前記音導孔16には、信号音を出力するイヤホン27とプローブトーンを発振する発振器30が接続されていることを特徴とする下顎運動に伴う中耳動特性の測定装置である。

距離の測定
距離測定手段33の振動子が発振して超音波が発信し、反射板34で反射してくる。これを同じ振動子で時間信号として受信して上下顎間の距離Lとして出力する。下顎を少しずつ開口すると、反射板34が下降し、変化する距離の変化は、時々刻々と検出されて制御回路32へ送られる。

被検者10の開口運動に随伴して中耳伝音系のインピーダンス値が変動する。この音響インピーダンスがマイクロホン23で検出され、コンプライアンス回路26では、動的コンプランアンス(音響容積値)が測定される。
下顎位の開口距離(動的コンプランアンスに同期して変動する距離)は、計測回路39で測定し、制御回路32に送られている。
そのため、制御回路32では、動的コンプランアンスをY軸とし、距離変動値をX軸とするグラフを構築し、表示器40にリアルタイムで表示し、かつプリンタ41で印刷する。このグラフは、両耳同時に測定される。

【発明の実施の形態】
(1)本発明の測定原理
被検者が口を開いたり閉じたりすると、この開口運動に随伴して、密閉した外耳内の中耳伝音系インピーダンス値が変動する。この音響法応用の音響インピーダンスから動的コンプランアンスが測定される。同時に下顎の開口距離(動的コンプランアンスに同期して変動する距離)を測定し、動的コンプランアンスをY軸とし、距離変動値をX軸とするグラフを構築する。
そして、このグラフから無歯顎者における特性を健常有歯顎者の特性と比較して患者の咬合状態や入れ歯の位置を調整すれば、顎関節症とされる顎機能異常、低位咬合、無歯顎などの3次元的下顎位(咬合位)の判断、設定を定量的に行うことができる。
本発明は、動的コンプランアンスにより生理的に安静な下顎位を数値的に測定して、この測定されたデータにより顎関節症とされる顎機能異常、低位咬合、無歯顎などを治療する指標を得ることのできる装置を提供することを目的とするものである。

前記音導孔16には、信号(226Hz)音を出力する小型のイヤホン27、イヤホン駆動用の第4のアンプ28、この第4のアンプ28とコンプライアンス回路26回路への信号処理をするプリアンプ29、プローブトーンとして226Hzの正弦波を発振する発振器30が順次接続されている。

前記距離測定手段33は、発振子と受信子兼用の超音波振動子からなり、この距離測定手段33の入力側には、距離測定手段33を駆動する駆動回路35と400KHzを発振する発振器36が接続され、出力側には、距離による反射信号の大小に対処するAGC内蔵の第5のアンプ37、デジタル波形処理回路からなる比較器38、距離測定手段33で検出したデータを上下顎間の距離として変換する計測回路39が順次接続されている。

被検者10は、図4の実線のように口を閉じておく。
発振器36から出力した400KHzの信号が駆動回路35に送られ、距離測定手段33の振動子が発振して超音波が発信し、反射板34で反射してくる。これを同じ振動子で時間信号として受信して第5のアンプ37で増幅し、比較器38で時間差を比較して、これを計測回路39で距離Lとして出力する。
下顎を少しずつ開口すると、鎖線のように、一体のシーネ55も開口して反射板34が下降する。このとき、反射板34は、ピン57で回動自在に支持されているので、ジャイロ機構により、常に水平になっている。このような距離の変化は、時々刻々と検出されて制御回路32へ送られる。

発振器30から226Hzの正弦波形の信号がプリアンプ29、第4のアンプ28を通してイヤホン27に送られ、このイヤホン27で一定音圧の音に変換され、音導孔16を通り外耳道11内に導びかれる。
この状態で被検者10が口を開いたり閉じたりすると、この開口運動に随伴して中耳伝音系のインピーダンス値が変動する。この音響法応用の音響インピーダンスがマイクロホン23で検出され、第3のアンプ24でS/N比を改善し、かつ増幅して、フィルタ25で226Hz帯域のみを通過させる。コンプライアンス回路26では、プリアンプ29の信号と、フィルタ25からの信号に基づいて動的コンプランアンス(音響容積値)が測定される。この動的コンプランアンスがA/D変換回路22を介して制御回路32へ送られる。

図1(a)(b)は、耳腔内コンプライアンスと上下顎の咬合位からの距離とをそれぞれ2次元軸に設定し、前記コンプライアンスと距離とをパラメータとし、咬合位からの距離の変化に伴うコンプライアンスの変化を2次元表示するようにした表示方法である。
すなわち、動的コンプランアンスをY軸とし、距離変動値をX軸とするグラフである。このグラフのうち、(a)に示す特性は、被検者10が健常有歯顎者の場合のパターンを示しており、(b)に示す特性は、被検者10が咬合支持喪失(無歯顎)者の場合のパターンを示している。計測に際しては、被検者10は、プローブ13を耳に差し込み、口を閉じたときの距離を0とし、次第に口を開いた(下顎が顎関節を支点として下方に開いた)ときに、動的コンプランアンスがどのように変化したかを示している。

図1(a)(b)および図2に示すパターンの詳細な説明の前に、図3について説明する。
図3は、いわゆるベッセルのバナナ特性図で、右側特性におけるCR−PP間の曲線は、下顎を上下運動したときの顎関節の軌跡であり、CR−NW間の曲線は、下顎を右へ捩じったときの左顎関節の軌跡であり、CR−W間の曲線は、下顎を左へ捩じったときの左顎関節の軌跡である。
また、左側特性におけるCO(咬頭嵌合位)−RP(後方限界位)間の曲線は、CRを支点とする下顎の軌跡であり、RP−MaxOP(最大開口位)間の曲線は、CRからPPへ顎関節が外れながらの下顎の軌跡であり、MaxOP−PP(前方限界位)間の曲線は、PPで顎関節が外れたままでの下顎の軌跡であり、PP−CO間の曲線は、正常位置へ戻るときの下顎の軌跡である。

以上のように、点CRは、顎関節が生理的安定な状態での位置を示し、下顎の軌跡のうち、点COは、下顎が生理的安定な状態で閉じている安静位点であり、CO−RP間の下顎の軌跡は、関節位置CRをを中心にした円弧をなし、口を通常の状態で開閉しているときを示している。変極点RP−MaxOP間の下顎の軌跡は、口を大きく開閉したとき、関節位置がCRからPP点に次第に外れてゆく過程を示している。
また、点線のCO1、PP1は、無歯顎者や低位咬合者の特性を示しており、図3の特性からだけでは、無歯顎者などの咬合異常者の安静位点COが特定できないことを示している。

しかるに、本発明の下顎位を特定する表示方法によれば、図1(a)(b)に示す特性図から、咬合異常者の安静位点COが特定できることが判明した。

まず、図3と前記図1(a)(b)のグラフとの関連について説明する。
図1(a)に示す健常有歯顎者のパターンでは、下顎が生理的安定な状態で閉じている安静位点COでは、下顎の咬合高径(X軸の距離)が略0(図中のD0)であり、この安静位点COに対応する動的コンプランアンス(Y軸)を極値点D0として、口を通常の状態で開いてゆくと、動的コンプランアンス(Y軸)は、次第に上昇して極値点D1に達し、このD1点を境として再び下降する。このときの極値点D1が図3における変極点RPに相当する。
このD1は、下顎頭の終末蝶番運動と前方運動との境界点の極値点であり、さらに口を大きく開くと、この極値点D1から一旦極値点D2まで動的コンプランアンス(Y軸)が減少する。この極値点D2がMaxOP(最大開口位)であり、さらに大きく開こうとすると、顎関節が外れて再び急激に上昇する。

図1(b)に示す特性線は、咬合支持喪失(無歯顎)者のパターンを示している。このパターンにおいて、安静位点COに対応する動的コンプランアンス(Y軸)の極値点がD01であるから、図3では判明できなかった安静位点が図1では明確に判明できる。すなわち、無歯顎者が口を閉じてゆくと、D01から極値点Dx1まで一旦上昇し、口を完全に閉じて上下顎の基礎床が互いに接した、距離Lが0のときは、再び下降する。この距離Lが0のとき、図3におけるCO1に相当する。このパターンから、無歯顎者におけるD01−Dx1間は、安静位空隙を示し、このD01−Dx1間におけるD01に近い点が安静点であることを示している。
したがって、このときのX軸におけるD01までの距離Lを測定し、この距離Lに対応した状態で固定ねじ65、66を固定すれば、その距離データが患者の咬合状態や入れ歯の位置を調整するのに最適な位置であることを表している。

外耳道11がプローブ13で密閉された状態で被検者10が口を開いたり閉じたりすると、この開口運動に随伴して、外耳道11内の圧力が変化する。この圧力の変化は、圧力センサ18で電気信号として検出され、第1のアンプ19でS/N比を改善し、かつ増幅して、第2のアンプ20でドリフト調整を行い、A/D変換回路22を介して制御回路32へ送る。
下顎位の開口距離は、前述のように距離測定手段33で検出され、計測回路39で測定し、制御回路32に送られている。
そのため、制御回路32では、圧力をY軸とし、距離変動値をX軸とするグラフを構築し、表示器40にリアルタイムで表示し、かつプリンタ41で印刷する。このグラフは、両耳同時に測定される。

本発明は、上述のように構成したので、中耳腔の機能変動を指標とした生理的下顎位を定量的に表現できるとともに、この測定データから顎関節症とされる顎機能異常、低位咬合、無歯顎などの3次元的下顎位(咬合位)の判断、設定を定量的に行うことができる。
