【発明の属する技術分野】
本発明は、血管、胆管、気管、食道、尿道、その他の臓器などの生体内に形成された狭窄部及び瘤のような病変部の改善に使用される体腔内病変部治療用器具に関する。

【従来の技術】
従来より、血管、胆管、食道、気管、尿道、その他の臓器などの生体管腔または体腔の病変部(狭窄部及び瘤)にステントを留置して管腔または体腔空間を確保する、体腔内病変部治療用器具が提案されている。
上記治療用器具を構成するステントとしては、機能および留置方法によって、バルーンエキスパンダブルステントとセルフエキスパンダブルステントとがある。

バルーンエキスパンダブルステントは、ステント自身に拡張機能はなく、ステントを目的部位に留置するには、例えばステントを目的部位まで挿入した後、ステント内にバルーンを位置させてバルーンを拡張させ、バルーンの拡張力によりステントを拡張(塑性変形)させ目的部位の内面に密着させて固定する。
これに対して、セルフエキスパンダブルステントは、ステント自身が収縮および拡張機能を有している。このステントを目的部位に留置するためには、収縮させた状態にて目的部位に挿入した後、収縮状態の維持のために負荷した応力を除去する。例えば、目的部位の内径より小さい外径のシース内にステントを収縮させて収納し、このシースの先端を目的部位に到達させた後、ステントをシースより押し出す。押し出されたステントは、シースより解放されることにより応力負荷が解除され、収縮前の形状に復元し拡張する。これにより、目的部位の内面に密着し固定する。
このタイプのステントは、ステント自身が拡張力を有しているので、バルーンエキスパンダブルステントのような拡張作業は必要はない。

【発明が解決しようとする課題】
セルフエキスパンダブルステントとして、側面に膜が形成されたものがあり、側面に膜を備えることにより、血管などの体腔の狭窄部などの病変部の内壁との接触面積も大きくなり、病変部の改善効果は高い。このため、このような膜を備えるセルフエキスパンダブルステントを利用するものとし、さらに、この膜を備えるセルフエキスパンダブルステント(自己拡張型ステント)を目的留置部位まで容易に挿入でき、かつ目的留置部位での位置決め、外径拡張および固定に至るまでの留置操作が比較的容易であり、かつ正確に行える体腔内病変部治療用器具を研究した。しかし、研究過程において、ステントが膜を備えるが故に、ステント留置操作時に血流を一時的遮断するという問題点を知見した。

そこで、本発明の目的は、側面が膜により被覆された自己拡張型ステントを利用する体腔内病変部治療用器具であって、ステントの目的留置部位までの挿入、目的留置部位での位置決め、配置留置操作が比較的容易であり、かつ正確に行うことができ、更に留置操作時に血流を遮断することがほとんどない体腔内病変部治療用器具を提供するものである。

【課題を解決するための手段】
上記目的を達成するものは、略円筒形状に形成され、生体内挿入時には外径が圧縮され、生体内留置時には外径が拡張して元の形状に復元する自己拡張型体腔内留置用ステントと、該ステントの外径を圧縮させた状態で先端部内面に保持した内側シースと、該内側シースを収納する外側シースと、前記ステントを生体内にて吐出させる吐出機構とを備える体腔内病変部治療用器具であり、前記ステントは、側面が膜により被覆されており、前記外側シースは、先端より前記内側シースの先端部を吐出可能であり、さらに、前記内側シースの先端部は、該外側シースの先端より吐出後に、前記ステントにより拡張可能であるとともに、少なくとも先端より若干基端側となる位置に血液流通用開口を備え、かつ、該血液流通用開口は、前記内側シースの先端部であり、かつ前記内側シースのステント収納部の後端部より基端側に形成された側孔により構成されている体腔内病変部治療用器具である。

そして、前記外側シースより突出された前記内側シースの先端部の前記ステントによる拡張は、該ステントの最大外径未満であることが好ましい。また、前記血液流通用開口は、前記内側シースの先端部に形成された多数の側孔である。また、前記内側シースの先端部は、伸縮もしくは折り畳み可能となっていることが好ましい。さらに、前記外側シースの先端部は、前記ステントによる拡張不能となっていることが好ましい。また、前記ステントを生体内にて吐出させる吐出機構は、前記内側シース内に収納され、先端に前記ステントの後端部と当接し、該ステントを前記内側シースより吐出させるためのステント吐出用先端部と、該ステント吐出用先端部と連通し後端が該内側シースより突出した把持部を備えるステント吐出用部材であることが好ましい。そして、前記ステント吐出用先端部は、前記内側シースの拡張に追従して拡径するものであることが好ましい。また、前記体腔内病変部治療用器具は、例えば、体腔内の狭窄部及び瘤治療用器具である。

【発明の実施の形態】
本発明の体腔内病変部治療用器具(体腔内狭窄部治療用器具)を図面に示した実施例を用いて説明する。
図1は、本発明の体腔内病変部治療用器具の一実施例の断面図である。図2は、本発明の体腔内病変部治療用器具の部分破断外観図である。図3は、本発明の体腔内病変部治療用器具に使用される体腔内留置用ステントの一例の斜視図である。図4は、本発明の体腔内病変部治療用器具に使用されるステント吐出用部材の一例の斜視図である。図5ないし図8は、本発明の体腔内病変部治療用器具の使用方法の説明図である。

本発明の体腔内病変部治療用器具1は、略円筒形状に形成され、生体内挿入時には外径が圧縮され、生体内留置時には外径が拡張して元の形状に復元する自己拡張型体腔内留置用ステント2と、ステント2の外径を圧縮させた状態で先端部内面に保持した内側シース3と、内側シース3を収納する外側シース4と、ステント2を生体内にて吐出させる吐出機構5とを備えている。ステント2は、側面が膜22により被覆されており、外側シース4は、外側シース4の先端より内側シース3の先端部を吐出可能であり、さらに、内側シース3の先端部は、外側シース4の先端より吐出後に、ステント2により拡張可能であるとともに、少なくとも先端より若干基端側となる位置に血液流通用開口31を備えている。
なお、病変部としては、体腔(例えば、血管、胆管)の狭窄部、閉塞部、血管瘤(例えば、動脈瘤)形成部などである。

この実施例の体腔内病変部治療用器具1は、自己拡張型体腔内留置用ステント2と、ステント2の外径を圧縮させた状態で先端部内面に保持した内側シース3と、内側シース3を摺動可能に収納する外側シース4と、ステント2を生体内にて吐出させるステント吐出用部材5とを備えている。
体腔内病変部治療用器具1に使用されるステント2は、図3に示すように、略円筒形状に形成された縮径可能なステント本体21と、ステント本体21の側面を封鎖する膜である筒状カバー22とを備えている。
特に、図3に示すステント2は、略円筒形状に形成された縮径可能なステント本体21と、ステント本体21を被覆する熱可塑性樹脂層と、ステント本体21の側面を封鎖するとともに、熱可塑性樹脂層(図示せず)に固着された筒状カバー(膜)22とを備えている。

このように熱可塑性樹脂により被覆されたステント本体21の側壁(外周または内周もしくは外周面もしくは内周面)は、図3に示すように、筒状カバー22により被包(封鎖)されている。このため、ステント本体21の開口および切欠部などのステント側壁に形成された連通部分(孔)は、カバー22により封鎖されているので、外部より生体組織がステント2内に侵入することを防止する。

ステント2は、筒状体であり、両端に開口端を有している。ステント2は、外径が2.0〜50mm、好ましくは、2.5〜30mm、内径が1.0〜39mm、好ましくは1.5〜29mmのものであり、長さは、5〜300mm、より好ましくは10〜200mmである。

ステント本体21は、図3に示すように、円筒体の側面に形成された複数の切欠部25または複数の開口24を有しており、これにより応力負荷時に外径が縮径する方向への変形を補助する変形補助機能が形成されている。
ステント本体21は、具体的には円筒状フレーム体であり、フレーム26a,26bにより区画(囲撓)された開口(または孔)24およびフレーム26aにより区画された切欠部25を有している。ステント本体21の端部は、1つの円上にあり、連続しない複数の円弧の集合体により構成されており、それらはほぼ等角度離間している。さらに、フレーム(ステント本体21)の外面は、全体においてエッジがなく面取りされた状態となっている。

このステント本体21では、端部に切欠部を有するので、ステント2の端部の変形が容易となり、特に、端部の部分的変形が可能となり、留置される血管の変形時に対する応答が良好になる。また、ステント本体21の端部は、複数のフレーム26aの端部により形成されているため、つぶれにくく、十分な強度を有する。また、両端部間には、フレーム26a,26bにより囲まれた開口24が形成されており、この開口24は、フレーム26aの変形により容易に変形する。このため、ステント本体21は、その中央部(フレーム本体21体の中央部)での変形が容易である。

なお、この実施例では、開口24は、圧しつぶれた形状の六角形となっており、切欠部25は二等辺三角形となっている。切欠部25は、それぞれの端部に複数、具体的には6個形成されており、それぞれはほぼ等しい形状となっている。また、開口24もステント本体21の側面を形成するように、複数、具体的には、6個形成されている。なお、切欠部および開口は上記の形状および個数に限定されるものではなく、切欠部としては、3〜10個、開口としては、3〜10個程度が好適である。ステント本体21では、上記のような形状のステント部材が連接部26cにより2つつながった形状となっている。

ステント本体21の形成材料としては、合成樹脂または金属が使用される。合成樹脂としては、ある程度の硬度と弾性を有するものが使用され、生体適合性合成樹脂が好ましい。具体的には、ポリオレフィン(例えば、ポリエチレン、ポリプロピレン)、ポリエステル(例えば、ポリエチレンテレフタレート)、フッ素樹脂(例えば、PTFE、ETFE)などである。また、金属としても生体適合性を有するものが好ましく、例えば、ステンレス、タンタルチタン、ニッケルチタン合金などがある。特に、超弾性金属が好ましい。ステント本体21は、全体において物性の急激な変更点が形成されることなく一体に形成されていることが好ましい。ステント本体は、例えば、留置される生体内部位に適合した外径を有する金属パイプを準備し、金属パイプの側面を、切削加工、化学エッチングなどにより部分的に除去して、側面に複数の切欠部または複数の開口を形成することにより作製される。

ステント本体21を形成する超弾性金属としては、超弾性合金が好適である。ここでいう超弾性合金とは一般に形状記憶合金といわれ、少なくとも生体温度(37℃付近)で超弾性を示すものである。好ましくは、49〜53原子%NiのTiNi合金、38.5〜41.5重量%ZnのCu−Zn合金、1〜10重量%XのCu−Zn−X合金(X=Be,Si,Sn,Al,Ga)、36〜38原子%AlのNi−Al合金等が好適に使用される。特に好ましくは、上記のTiNi合金である。また、Ti−Ni合金の一部を0.01〜10.0%Xで置換したTi−Ni−X合金(X=Co,Fe,Mn,Cr,V,Al,Nb,W,Bなど)とすること、またはTi−Ni合金の一部を0.01〜30.0%原子で置換したTi−Ni−X合金(X=Cu,Pb,Zr)とすること、また、冷間加工率または/および最終熱処理の条件を選択することにより、機械的特性を適宜変えることができる。さらに、上記のTi−Ni−X合金を用いて冷間加工率および/または最終熱処理の条件を選択することにより、機械的特性を適宜変えることができる。

そして、使用される超弾性合金の座屈強度(負荷時の降伏応力)は、5〜20kg/mm2(22℃)、より好ましくは、8〜150kg/mm2、復元応力(除荷時の降伏応力)は、3〜180kg/mm2(22℃)、より好ましくは、5〜130kg/mm2である。ここでいう超弾性とは、使用温度において通常の金属が塑性変形する領域まで変形(曲げ、引張り、圧縮)させても、変形の解放後、加熱を必要とせずにほぼ元の形状に回復することを意味する。

ステント本体21の形状は、挿入時に縮径可能であり、かつ、体内放出時に拡径(復元)可能なものであればよく、上述の形状に限定されるものではない。例えば、コイル状のもの、円筒状のもの、ロール状のもの、リング状のもの、異形管状のもの、高次コイル状のもの、板バネコイル状のもの、カゴまたはメッシュ状のものでもよい。

ステント本体21は、熱可塑性樹脂により被覆されていることが好ましい。熱可塑性樹脂としては、熱可塑性フッ素樹脂、ポリオレフィン(例えば、低密度ポリエチレン、低密度ポリプロピレン)、塩化ビニル樹脂、エチレン酢酸ビニル共重合体、ポリエステル(低融点ポリエステル)、ポリカーボネート、ABS樹脂、シリコーンゴム(RTVゴム)、熱可塑性ポリウレタンなどが使用できる。そして、溶剤可溶型の熱可塑性樹脂が作業性および均一なコーティングが容易な点より好ましい。溶剤可溶型の熱可塑性樹脂としては、熱可塑性樹脂フッ素樹脂であるフッ素系エラストマー、エチレン酢酸ビニル共重合体、塩化ビニル樹脂、シリコーンゴム(RTVゴム)、ポリウレタンなどがある。熱可塑性樹脂としては、融点が120〜200℃程度のものが好適であり、熱可塑性樹脂のコーティング厚としては、10〜100μm程度が好適である。

筒状カバー22は、ステント本体21の側面(側壁)を封鎖するように、言い換えれば、ステント本体21の外周または内周もしくはその両者を塞ぐように形成されている。そして、ステント本体21との接触部(熱可塑性樹脂との接触部)全体において固着されている。このため、ステント本体の変形に対するカバーの変形の追従性が高く、カバーがステント本体の変形の障害となることが少ない。また、カバーとステント本体の固着部分がステント本体の全体に分散しているため、使用時および留置時にストレスが一部分に強くかかることがなく、固着部分でのカバーの破断の危険性も少ない。

筒状カバー22としては、図3に示すように、筒状にあらかじめ形成されたものが好適であるが、帯状のものをステント本体21に巻き付けて全体として筒状に形成したものでもよい。あらかじめ筒状に形成されたものとしては、チューブ状に接続部がなく形成されたものが好適であるが、帯状のものを巻いて端部を熱融着して筒状としたもの、また、帯状のものをスパイラルに巻いて筒状としたものでもよい。この実施例では、押し出し成形などによりチューブ状に接続部がなく形成されたものが用いられている。

筒状カバー22の形成材料としては、熱可塑性樹脂より融点が高く、かつ、熱可塑性樹脂と熱融着可能な合成樹脂が好適に使用される。より好ましくは、可撓性もしくは、弾性を有するものが好適である。融点は20℃以上、上述の熱可塑性樹脂より高いことが作業性などの点より好ましい。筒状カバー22の形成材料の具体例としては、フッ素樹脂(例えば、PTFE、ETFE)、ポリオレフィン(例えば、ポリエチレン、ポリプロピレン)、ポリエステル、熱可塑性ポリウレタンなどが使用される。

筒状カバー22としては、ステント本体との熱融着時に収縮しないものが好適である。熱融着時に収縮しないような素材(フィルム)としては、製造時の熱履歴をあまり持っていないもの、製造時に延伸されていないものなどが考えられる。また、筒状カバーとして用いる前に、熱融着時程度の温度に加熱し収縮をさせた後に使用することにより対応してもよい。筒状カバーとしては、厚さが0.01〜10mm程度のものが好適である。

また、上述のステント2では、ステント本体の外側から筒状カバーを固着しているが、筒状カバーは、ステント本体の側壁を封鎖できればよく、ステント本体の内側(内面側)にカバーを固着したものでもよい。
内側シース3は、図1および図2に示すように管状体であり、先端および後端は、開口している。内側シース3の先端部は、内部に収納したステント2により拡張可能なものとなっている。具体的には、内側シース3の先端部(全体でもよい)は、延伸性もしくは伸縮性を有する材料に形成されている。このため、内側シース3の先端部は、ステント2の自己復元力により押し広げられ、拡張する。また、内側シース3の先端部は、伸縮性を備えず、折り畳み可能なものとしてもよい。この場合には、内側シース3の先端部は、ステント2の自己復元力により折り畳まれる前の形状に押し広げられる。

また、内側シース3の先端部の拡張程度は、ステント2の最大外径(非圧縮時の外径)未満であることが望ましい。つまり、内側シース3の拡張時の外径は、ステント2の最大外径(非圧縮時の外径)より小さいことが望ましい。このようにすることにより、後述するステント吐出用部材5を用いて、内側シース3の先端より、ステント2を吐出させたときに、吐出されたステント2はさらに拡張するため、血管などの体腔の目的部位への配置が容易である。内側シース3の先端部の拡張程度としては、ステント2の最大外径(非圧縮時の外径)の1/3〜4/5程度であることが好ましい。

さらに、内側シース3の先端部は、少なくとも内側シース3の先端より若干基端側となる位置に血液流通用開口31を備えている。この実施例では、4つの血液流通用開口(側孔)31を備えている。具体的には、内側シース3では、ステント収納部に近接し、かつそれより基端側となる位置に、開口31が形成されている。このように、ステント収納部よりも基端側となる位置に開口31を設けることにより、後述する図6に示すように、外側シース4より内側シース3を突出させた状態(言い換えれば、非ステント突出時かつ内側シース拡張時)においても、内側シース3内に流入する血液を開口31より末梢側に流通させることができる。なお、開口31は、内側シース3の先端部全体に形成してもよい。

内側シース3としては、全体を同一樹脂により成形してもよく、また、先端部のみ別材料により成形したものを接続することにより作製してもよい。
内側シース3の全体を同一樹脂により成形する場合および先端部を別部材で成形する場合の本体部(先端部を除く部分)の形成材料としては、ポリオレフィン(例えば、ポリエチレン、ポリプロピレン、エチレンープロピレン共重合体、エチレン−酢酸ビニル共重合体など)、ポリ塩化ビニル、エチレン−酢酸ビニル共重合体、ポリアミドエラストマー、ポリウレタン等の熱可塑性樹脂、シリコーンゴム、PFA、PTFE、フッ素系エラストマーなどのフッ素系樹脂等が使用でき、好ましくは上記の熱可塑性樹脂である。

また、内側シース3の先端部の形成材料としては、先端部を伸縮性を有するものとする場合には、ウレタンゴム、シリコーンゴム、ブタジエンゴムなどの合成ゴム、ラテックスゴムなどの天然ゴム、オレフィン系エラストマー(例えば、ポリエチレンエラストマー、ポリプロピレンエラストマー)、アミド系エラストマー(例えば、ポリアミドエラストマー)、スチレン系エラストマー(例えば、スチレン−ブタジエン−スチレンコポリマー、スチレン−イソプレン−スチレンコポリマー、スチレン−エチレンブチレン−スチレンコポリマー)、ウレタン系エラストマー(例えば、熱可塑性ポリウレタンエラストマー)などの熱可塑性エラストマーが好適である。

また、内側シース3の先端部の形成材料としては、先端部を折り畳み可能なものとする場合には、ある程度の可塑性と剛性を有するものが好ましく、例えば、ポリエチレン、ポリプロピレン、ポリブテン、エチレン−プロピレン共重合体、エチレン−酢酸ビニル共重合体、アイオノマーなどのポリオレフィン、さらにはこれらの架橋もしくは部分架橋物、ポリエチレンテレフタレートなどのポリエステル、ポリフェニレンサルファイド、ポリアミド、フッ素樹脂などの高分子材料などが使用できる。また、これら高分子材料を適宜積層した積層フィルムも使用できる。

また、内側シース3を構成する材料には造影剤を含有させてもよい。含有させる造影剤としては、例えば、硫酸バリウム、ビスマス、タングステン等が好適である。
さらに、内側シース3の先端には、図1および図2に示すように、マーカー35を設けることが好ましい。マーカーは、コイルスプリングあるいはリングにより形成することが好ましい。マーカーの形成材料としては、X線造影性の高い材料、例えば、Pt、Pt合金、W、W合金、Au、Au合金、Ir、Ir合金、Ag、Ag合金などを用いることが好ましい。
さらに、内側シース3の後端部32には、後述するステント吐出用部材5のワイヤー部52をほぼ液密にかつ摺動可能に挿通する弁体33が設けられている。この弁体33は、内側シース3の後端部32内に挿入されるとともにリング状保持部材34により後端部32に固定されている。

内側シース3としては、使用対象の血管部位により相違するが、外径が、0.5〜20mm、好ましくは、0.7〜8mm、内径が、0.3〜19.8mm、好ましくは、2.5〜7.8mm、長さが、100〜1500mm、好ましくは、150〜1000mm、先端部の拡張時の外径が、0.6〜40mm、好ましくは、1.2〜24mmである。
また、内側シース3としては、上述したような形状のものに限定されるものではなく、例えば、内側シース3の先端部をメッシュ状に形成してもよい。

さらに、内側シース3のステント収納部内面には、ステント2の吐出を容易にするために、低摩擦性物質を被覆してもよい。また、外側シース4との間の摺動を容易にするために、内側シース3の外面にも、低摩擦性物質を被覆してもよい。低摩擦性物質としては、シリコーンオイル、フッ素樹脂(PTFE、ETFEなど)などが使用できる。

外側シース4は、少なくとも内側シースの先端部全体を収納し、かつ後端部が体外より操作可能となる長さを備え、さらに、内側シース3を摺動可能に被包する管状体である。そして、外側シース4は、ステント2を収納した内側シース3を格納した際に、外径が変化しない程度の硬度を有している。さらに、生体内に挿入する際の押し込み性を発現させる程度の硬度を有していることが好ましい。

外側シース4としては、保形性の高い先端部を備え、かつ本体部は柔軟性を備えるものであることが好ましい。また、外側シース4の先端部の形状保持性を向上させるために、コイルやメッシュ等の補強用構造物を設置してもよい。このような構造物を構成する材料としては、例えば、ステンレス鋼、アルミニウムまたはアルミニウム合金、超弾性合金、形状記憶合金等の各種金属材料や、ポリ塩化ビニル、ポリウレタン、ポリエチレン、ポリプロピレン、PTFE、ポリカーボネート、ABS樹脂、アクリル樹脂、ポリイミド等の各種合成樹脂が好ましい。

本発明の体腔内病変部治療用器具は、略円筒形状に形成され、生体内挿入時には外径が圧縮され、生体内留置時には外径が拡張して元の形状に復元する自己拡張型体腔内留置用ステントと、該ステントの外径を圧縮させた状態で先端部内面に保持した内側シースと、該内側シースを収納する外側シースと、前記ステントを生体内にて吐出させる吐出機構とを備える体腔内病変部治療用器具であり、前記ステントは、側面が膜により被覆されており、前記外側シースの先端より前記内側シースの先端部を吐出可能であり、さらに、前記内側シースの先端部は、該外側シースの先端より吐出後に、前記ステントにより拡張可能であるとともに、少なくとも先端より若干基端側となる位置に血液流通用開口を備えている。

In contrast, the self-expandable stent has a contraction and expansion function. [Problems to be solved by the invention]
Some self-expandable stents have a membrane on the side, and by providing a membrane on the side, the area of contact with the inner wall of the lesion, such as a stenosis of a body cavity such as a blood vessel, increases, The improvement effect is high. For this reason, a self-expandable stent having such a membrane is used, and the self-expandable stent (self-expanding stent) having this membrane can be easily inserted to the target indwelling site, and the target indwelling can be performed.
