【発明の属する技術分野】
本発明は、車いす使用者を車いすに載せたまま歯科、眼科、耳鼻咽喉科、整形外科等の治療を行えるチェアーユニットに関する。

【従来の技術】
社会の高齢化に伴い、車いすを常用する人の数が増加している。これらの人が医療機関において医師の治療を受ける場合に、車いすから治療台への移動が必要となることがある。しかし、車いす使用者の中にはその障害、症状の程度により自力では車いすから治療台への移動ができない場合があり、その際には医師や、看護婦等の補助スタッフの大きな負担となっていた。また、車いすから治療台への移動中に事故が起きることも考えられる。

これらの問題を解決するため、診療所内に車いす専用スペースを設け、ここに車いす用電動リフト等を設置して治療を行っている場合があった。前記車いす用電動リフトは、車いすの車輪を専用台に載置してストッパーにて固定しており、車いすを最大約30度チルトさせることができる。また、チルトした際の車いす使用者の頭部を支えるため、車いすの後方から前方に向けて張り出すようにヘッドレストが設けられている。

【発明が解決しようとする課題】
しかし、一般に市中のクリニック等の医療施設において上記のように専用スペースを設けることは、使用頻度が比較的低いこともあり、困難である場合がある。

また、車いすを30度チルトさせた程度では、術者は立位の診療を余儀なくされ、通常の診療姿勢、いわゆるホームポジションをとることができない。さらに、前記した車いすの後方から前方に向けて張り出すように設けられたヘッドレストも、術者がホームポジションを取るのに邪魔な存在である。

そこで、本発明は、歯科、眼科、耳鼻咽喉科、整形外科等において、健常者の治療に用いられるものと兼用できて、車いす専用スペースを必要とせず、十分な安全を確保しつつ車いすに車いす使用者を乗せたまま術者がホームポジションを取ることができる、診療用車いす用チェアーユニットを提供することを目的とする。

あわせて本発明は、車いす専用スペースが確保できる場合には、十分な安全を確保しつつ車いすに車いす使用者を乗せたまま術者がホームポジションを取ることができる車いす専用の診療用チェアーユニットを提供することをも目的とする。

また、本発明はその性質上、上記の医療用チェアーユニットに適用できる他、理容、美容用のチェアーユニット、さらには、交通機関の座席に対しても適用が可能である。

【課題を解決するための手段】
以下、本発明について説明する。なお、本発明の理解を容易にするために添付図面の参照符号を括弧書きにて付記するが、それにより本発明が図示の形態に限定されるものではない。

請求項1の発明は、着脱可能な健常者用いす(2)と、着脱可能な車いすホルダー(1)と、健常者用いすおよび前記車いすホルダーを着脱すべき本体(3)とを備えた診療用車いす用チェアーユニットであって、健常者用いすおよび車いすホルダーには同一形状の連結部材(11、12)が備えられており、本体には連結部材を連結すべき被連結部材(14)を備えたリフター(15)が備えられており、診療時には、前記健常者用いす(2)と前記車いすホルダー(1)のいずれかを、前記連結部材(11、12)で前記本体(3)の前記被連結部材(14)に連結して使用することを特徴とする、診療用車いす用チェアーユニットにより前記課題を解決する。

この発明によれば、健常者用いすと車いすホルダーはそれぞれ本体に着脱可能に設けられているので、健常者の治療には健常者用いすを、車いす使用者には車いすホルダーを本体に装着することにより、一つのチェアーユニット本体で健常者と車いす使用者の両者の治療等を行うことができる。したがって、健常者専用ユニットと、車いす用専用ユニットとを別々に備える場合と比較して、診療所内スペース等の有効利用を図ることができる。また、健常者用いすと車いすホルダーの連結部材は同一形状であるので、同一手順にて本体の被連結部材に連結を行うことができる。さらに、被連結部材はリフターに備えられているので、リフターを上下動させることにより連結を行うことができる。

【発明の実施の形態】
以下本発明を図面に示す実施形態に基づき、医療機関における車いすを使用する患者の治療行為を想定しつつ説明する。本発明にかかる第一実施形態の車いす用チェアーユニットは、図1に示すように、車いすホルダー1と、健常者用いす2と、本体3の3つの主要部分により構成されている。車いすホルダー1と健常者用いす2には、同一形状の連結部材11、12が備えられている。一方、本体3には垂直なガイドにそって上下動するリフター15、チェアーユニット全体の動作制御を司る制御盤16、術者等の操作意志を入力する操作パネル17等が備えられている。リフター15には、リフトモーター18、位置センサ19(図9参照)などと共に、連結部材11、12と嵌合する形状に形成された被連結部材14が設けられている。車いすホルダー1または健常者用いす2を本体3に隣接する所定位置に配置し、リフター15を上昇させることによって、被連結部材14は連結部材11、12に嵌合し、車いすホルダー1と本体3、または健常者用いす2と本体3は一体となる。さらにリフター15を所定高さまで上昇させて、車いす使用者の治療を行う。一方、車いす使用者の治療が終了したら、一体となった車いすホルダー1と本体3、または健常者用いす2と本体3は、リフター15により所定の高さまで下降させる。そして、車いすホルダー1または健常者用いす2の下方に専用の台車を配置した後、さらにリフター15を下降させることにより、両者を分離することができる。

車いすホルダー1は車いす使用者を車いすに乗せたままホルダーに載置して、上昇、チルト動作にて所定のポジションまで姿勢変更させて、治療を行うものである。一方健常者用いす2は、健常者を座らせて上昇、チルト動作にて所定のポジションまで姿勢変更させて、治療を行うものである。したがって、健常者用いす2を本体3に連結して一体としたものは、基本的には通常の健常者用チェアーユニットと同様のものである。

通常時は、健常者用いす2を本体3に装着して一体とし健常者の治療に使用する。一方、車いすの使用者が治療に訪れた場合には、健常者用いす2にかえて車いすホルダー1を本体3に装着したうえで、車いすホルダー1上に車いすを載置して車いす使用者の治療を行う。このようにすることにより一つのチェアーユニット本体3で、健常者と車いす使用者の両者の治療を行うことができる。

次に、車いすホルダー1の連結部材11と本体3の被連結部材14の構成を図2を参照しつつ説明する。健常者用いす2の連結部材12も基本的に車いすホルダー1の連結部材11と同一構造であるので、ここでは車いすホルダー1の連結部材11に代表させて説明する。連結部材11は、車いすホルダー1の台41の下面に溶接あるいはボルト締めなどにより堅固に取り付けられている。連結部材11は截頭円錐形を倒置した形状をしており、円錐底面が台41の下面に取り付けられている。連結部材11の截頭円錐頂部近くの外周に沿っては、一条の溝21が形成されている。

一方本体3のリフター15に取り付けられている被連結部材14には、連結部材11の円錐形状に嵌合する形状にて凹部13が形成されている。さらに被連結部材14には抜け防止機構20が設けられている。これについては後に詳述する。連結部材11の上下方向の長さは、被連結部材の凹部13の長さに等しいか、あるいはわずかに長く形成されている。このように連結部材11と被連結部材14とを構成することにより、両者を嵌合させたときに、本体3と車いすホルダー1とは堅固に連結され、一体となる。

図3には、抜け防止機構20の詳細が示されている。被連結部材14の外部から、凹部13の連結部材11の溝21に対応する位置に向けて、水平な孔23が設けられている。孔23には孔23の内径よりわずかに小さな外径を有する丸棒22が水平方向に摺動可能に挿入されている。丸棒22の全長は孔23より大きく設けられており、丸棒22の先端28は溝21の凹部に達することができる。丸棒22の他端側には丸棒22を蓋うように円筒部材24が被連結部材14に取り付けられている。また、丸棒22は、孔23および円筒部材24の内部でその軸心のまわりに回転自在となっている。丸棒22の同方向他端部にはつまみ27が取り付けられており、つまみ27を回動することにより丸棒22をその軸心のまわりに回転させることができる。丸棒22のつまみ27の近傍にはストッパー25が取り付けられている。丸棒22は円筒部材24に設けられた不図示のばねにより付勢されてその先端28が、連結部材11の溝21の溝に嵌入可能となっている。これにより連結部材11と被連結部材14からなる連結機構10の抜け防止がはかられ、連結部材11が被連結部材14の凹部13から抜けることが防止される。この際には丸棒22に設けられたストッパー25とつまみ27の凹部26が嵌合している。つまみ27を回転させて、ストッパー25につまみ27の平坦部29を乗り上げることにより、丸棒22が図3の右側の方向に引き寄せられて、丸棒22の先端28の溝21への嵌入が解け連結部材11と被連結部材14との抜け防止が解除される。なお、被連結部材14には抜け防止機構20の丸棒22が連結部材11の溝21に挿入されているか否かを感知するセンサ30が設けられている。これについては後に詳述する。

図5(a)は、背もたれ80を取り付けた車いすホルダー1の基部を示している。車いすは、図で右側を前、左側を後ろの姿勢にて車いすホルダーに載置保持される。したがって以降の説明においてはこの車いすの姿勢を基準に、図5(a)の右側を前方、左側を後方と呼ぶこととする。背もたれ80の取り付け部材81a、81bは、フレームホルダー71a、71b上に車いすのフレームが載置される際の車いす使用者の姿勢を考慮して、U字パイプ64上に配置し、取り付けられる。2つの取り付け部材81、81bの間には軸82が略水平に取り付けられている。背もたれ80の支柱85は、軸82のまわりに回転自在に取り付けられている。U字パイプ64の取り付け部材81a、81baの後方上面には、調整ハンドル取り付け台83が取り付けられている。調整ハンドル取り付け台83の頭部にはスクリューナット89が形成されている。このスクリューナット89には背もたれ角度調整ハンドル84のボルト部84aが螺合している。ボルト部84aの先端は支柱85に当接している。したがって調整ハンドル84を回動させて背もたれ80のチルト角度を調整することができる。支柱85には、腰当て86、背当て87、ヘッドレスト88などが適宜取り付けられる。このように背もたれ80は後方への張り出しなく構成されているので、治療の際に医師がホームポジションをとる妨げとはならない。

図5(b)は、背もたれ80がU字パイプ64上にチルト可能に取り付けられた、他の実施形態を示している。この実施形態においてはチルト機構としてテレスコーピック機構91が採用されている。テレスコーピック機構91の一端は取り付け具93を介して支柱85に水平な軸のまわりに回転自在に取り付けられている。テレスコーピック機構91の他端側も取りつけ具92を介してU字パイプ64に水平な軸のまわりに回転自在に取り付けられている。テレスコーピック機構91そのものは、外筒94と外筒94に内接摺動する内筒95と、内筒95の摺動を固定するストッパ96とを備えている。

ストッパ96のつまみ97、98をつまむとストッパ96の固定が解けて内筒95は外筒94内を軸心方向に摺動自由となる。そこで支柱85を好みの角度にチルトさせてストッパ96のつまみ97、98を放すと、内筒95、外筒94の位置関係はそこで固定されて背もたれ80のチルト角度が保たれる。

図6は、車いすホルダー1の基部に取り付けられたチルト機構100を示す。チルト機構100は、台41に取り付けられた取り付け金具103と、取り付け金具103に回転自在に取り付けられモーター107を備えた本体106と、モーター107により回転駆動されるスクリュー108と、スクリュー108と螺合するスクリューナット111とを供えている。取り付け金具103には複数の孔104が設けられていて、台41にボルト締にて固定されている。取り付け金具103の上部には水平な軸105が設けられている。軸105には本体106が軸105のまわりに回転自在に取り付けられている。本体106の下部にはモーター107が備えられている。本体106の後方にはスクリュー108が、さらに後方に伸長するように取り付けられている。モーター107の駆動力は、不図示の動力変換機構によりスクリュー108に伝達されるように構成されている。スクリュー108の先端部はスクリューナット111に螺合している。スクリューナット111は、前記したスクリューナット取り付け台63に、水平な軸110のまわりに回転自在に取り付けられている。

2つの軸105、110の間の距離は、スクリュー108が回転することにより伸縮する。一方、これらの軸を支えるL字パイプ50とU字パイプ64は蝶番機構55により連結されており、L字パイプ50は不動である。したがってスクリュー108が回転すると、U字パイプ64は、蝶番機構55の軸62の周りに回転、すなわちチルトする。これによってU字パイプ64の先端に取り付けられた車いすフレーム保持部70もチルトし、それに載置された車いすのチルト動作が可能となる。本発明の車いすホルダー1に載置された車いすは、車いすフレーム保持部70が確実に車いすのフレームを支えるので最大45度チルトさせることができる。

台41の前方端部には、ワイヤープレート102が台41に鉛直に取り付けられている。ワイヤープレート102には、電源供給や、センサ類の信号の送受信を司る集合ケーブル101が取り付けられている。集合ケーブル101の一端側先端部にはソケット101aが設けられており、ソケット101aは、本体3の差し込み(不図示)に着脱可能となっている。本体3の差し込みからは、電源や制御盤16へ所定の配線がなされている。集合ケーブル101の他端側配線はワイヤープレート102にて車いすホルダー1のそれぞれの供給先/送受信先に分岐している。電源ケーブル、および制御信号を授受するケーブルの一部はチルト機構100のモーター107に導かれている。一方浮き上がり感知センサ74およびセンサ90からの信号を本体3の制御盤16に送信するケーブル類は、前記した孔104に挿入されてU字パイプ64の内部を通り、所定位置に配置された各センサまで導かれている。

図7は、車いすフレーム保持部70に車いす120を載置した状態を示す。一般に、車いす120には、ホイール122の軸125と補助輪124の軸126の間の高さに、両側面に配置されている2つのホイール122、122の内側に前後方向に水平平行な2本の下部フレーム121が設けられている。本実施形態の車いすホルダー1においては、この下部フレーム121を車いすフレーム保持部70のホルダー71a、71b上に載置して車いす120全体を支える構造となっている。2本の下部フレーム121、121の幅は、車いすの大きさによって異なるが、前記したようにホルダー71a、71bの幅は伸縮ステム75、77、76、78により調整可能なので、フレーム121、121の幅にあわせて伸縮ステム75、77、76、78の長さを調節することにより、安定して車いす120を保持することができる。また、ホイール122をささえるのではなくフレーム121を直接支えるので、より安定して車いす120を保持することができる。車いす120を車いすホルダー1に載置した後リフター15にて車いすホルダー1をわずかに上昇させ、チルト機構100を作動させて車いす120の前方(図7で右側)を上昇させるようにすればさらに安定して保持することが可能である。
