【発明の属する技術分野】
この発明は、人工心肺装置等の体外循環血液回路などで用いられる超音波ドップラ効果を利用した超音波ドップラ血流計に関し、詳しくは、電気メス等からの外来ノイズに強く、誤動作の少ない超音波ドップラ血流計および逆流検出方法に関するものである。

【従来の技術】
超音波ドップラ血流計は、超音波プローブにより血液の流れに対して超音波を照射し、その血液からの超音波エコー信号を受信して、エコー信号のドップラ周波数偏移から血液の流量を測定するものであり、一般に広く使用されているものである。超音波ドップラ血流計では、エコー信号のドップラ周波数偏移を示すドップラ信号に必要ならば平均化処理を施し、そのドップラ周波数から血液の流量を算出している。

人工心肺装置等の体外循環血液回路の場合には、血液の逆流を検出したときに、鉗子等で血液の流れるチューブを挟んで逆流を止める等の処置が必要である。後に詳しく説明するが、エコー信号を直交検波器により検波して、その出力を方向検出部によって処理することにより、+側ドップラ信号と−側ドップラ信号を得る。その−側ドップラ信号の周波数に必要ならば同様の平均化処理を施して、血液の逆流を判定していた。すなわち、−側ドップラ信号の周波数が所定の定数A(例えば、流量に換算して0.3〜0.5[L/min])より大きいと、逆流であると判定して警報を表示するようになっていた。そして、−側ドップラ信号の周波数が、定数A以下となれば、逆流から復帰したものとして逆流警報を解除するようになっていた。

【発明が解決しようとする課題】
従来の超音波ドップラ血流計は、以上のように、−側ドップラ信号から血流の方向を検出していたので、その近傍で電気メス等を使用した場合に、電気メス等からの外来ノイズによって、方向検出部が誤動作して誤った逆流警報が発生することがあった。また、血液の流量が大きくて、かつ、プローブの血液チューブへの取付不良やプローブに性能上の問題がある場合には、方向検出部からの+側ドップラ信号と−側ドップラ信号の分離が悪くなり、誤った逆流警報が発生することがあった。このような、誤った逆流警報は極力減少させる必要がある。

そこで、本発明は、人工心肺装置等の体外循環血液回路などで用いられる超音波ドップラ効果を利用した超音波ドップラ血流計において、電気メス等からの外来ノイズに強く、誤動作の少ない逆流検出方法、および、その検出方法を使用した超音波ドップラ血流計を提供することを目的とする。

【課題を解決するための手段】
上記目的を達成するために、本発明の超音波ドップラ血流測定における逆流検出方法は、超音波を照射する送信部と、前記超音波が反射されたエコー信号を受信する受信部とを備えた超音波プローブと、直交検波を行う直交検波部と、血流の方向を検出するための方向検出部と、血流の方向を判定するための血流方向判定手段とを使用した超音波ドップラ血流測定における逆流検出方法であって、体外循環血液回路の管内を流れる血液に対して、前記送信部により超音波を照射する手順と、前記超音波が血液によって反射されたエコー信号を前記受信部により受信する手順と、前記直交検波部によって前記エコー信号を直交検波して、前記エコー信号の周波数偏移を表すドップラ周波数に対応する第1の検出値を求める手順と、前記エコー信号の前記直交検波部による直交検波信号から、前記方向検出部により、血流が逆方向の場合の前記エコー信号の周波数偏移を表す−側ドップラ周波数に対応する第2の検出値を求める手順と、前記血流方向判定手段により、前記第1の検出値が第1の定数より大きいか否かを判定する第1の判定手順と、前記血流方向判定手段により、前記第2の検出値が前記第1の定数より大きいか否かを判定する第2の判定手順と、前記血流方向判定手段により、前記第2の検出値が第2の定数と前記第1の検出値との積より大きいか否かを判定する第3の判定手順と、前記血流方向判定手段により、前記第1の判定手順、前記第2の判定手順および前記第3の判定手順の判定結果が全て肯定的判定結果である場合には、血流の方向が逆方向であるものと判定する手順と、前記血流方向判定手段により、前記第1の判定手順、前記第2の判定手順および前記第3の判定手順の判定結果の少なくとも一つが否定的判定結果である場合には、血流の方向が正方向であるものと判定する手順とを有するものである。

【発明の実施の形態】
本発明の実施の形態について図面を参照して説明する。図1は、本発明の超音波ドップラ血流計1の構成を示す概略図である。超音波ドップラ血流計1は、血流計本体2と、それに接続された超音波プローブ3とからなるものである。超音波プローブ3は、人工心肺装置等の体外循環血液回路を構成するチューブ9に取り付けられている。超音波プローブ3には、送信部31と受信部32が設けられており、送信部31から血液の流れに対して照射された超音波が、その血液中の血球等によって反射され、超音波エコー信号として受信部32で受信される。

送信部31から照射される超音波は、発振器21からの出力信号によって駆動されたものである。受信部32で受信されたエコー信号には、血流によるドップラ周波数偏移が生じており、この周波数偏移から血液の流速および流量が測定できる。エコー信号は、プリアンプ23によって増幅され、直交検波器22に入力される。また、直交検波器22には、発振器21からの基準信号として、照射超音波信号と同位相の0°信号、および、90°位相の異なる90°信号が入力されている。

直交検波器22からは、エコー信号の周波数偏移に相当する周波数のドップラ信号が得られる。このとき、2種類の基準信号に対応する2種類のドップラ信号が得られる。2種類のドップラ信号は、増幅器24によってそれぞれ増幅される。そして、一方がドップラ信号として流量演算部4に入力される。また、増幅器24から出力された2種類のドップラ信号は、方向検出部25に入力される。方向検出部25は、両方のドップラ信号の位相差により、+側ドップラ信号と−側ドップラ信号に分離して出力する。

この+側ドップラ信号と−側ドップラ信号とから、血流の方向を判定することができる。血流が正方向(正流)の場合は、ドップラ信号と+側ドップラ信号の周波数がほぼ等しくなり、−側ドップラ信号の周波数はほぼゼロとなる。これに対して、血流が逆方向(逆流)の場合は、ドップラ信号と−側ドップラ信号の周波数がほぼ等しくなり、+側ドップラ信号の周波数はほぼゼロとなる。したがって、例えば−側ドップラ信号が定数A(例えば、流量換算で0.3〜0.5[L/min])より大きくなると逆流が発生したものと判定することができる。

ただし、単に−側ドップラ信号だけから判定するようにすると、前述のように、電気メス等からの外来ノイズによって、方向検出部25が誤動作して誤った方向判定を行うことがあり、また、血液の流量が大きくて、かつ、超音波プローブ3のチューブ9への取付不良や超音波プローブ3に性能上の問題がある場合にも誤った方向判定を行うことがあった。本発明は、このような誤った方向判定を減少させるべくなされたものである。

流量演算部4は、ドップラ信号によって血液の流量を演算し、その血流の測定値を表示部5に表示する。また、流量演算部4は、+側ドップラ信号および−側ドップラ信号により、血流の方向(血液の正流・逆流)を判定する。血液の逆流が発生した場合には、逆流警報(逆流が発生した旨の警報表示)を表示部5に出力する。その逆流警報により、操作者が鉗子等で体外循環血液回路のチューブを挟んで逆流を止める等の処置を行い、その後、遠心ポンプの回転速度を上昇させる等の対策を実施してから鉗子等をチューブから取り外す。これにより、逆流状態から復帰すれば逆流警報は解除される。

図2は、流量演算部4の構成を示すブロック図である。流量演算部4には、種々のデータ処理を行うためのCPU40が設けられており、CPU40にはバス41を介してROMやRAM等からなるメモリ42が接続されている。CPU40は、メモリ42に記憶されているプログラムおよびデータに従って動作する。メモリ42には、基本入出力プログラムであるBIOSや、ドップラ信号から血液の流量を演算する流量演算プログラム421や、血流の方向を判定する血流方向判定プログラム422や、表示部5に対して文字や画像の表示を行う表示制御プログラム等が記憶されている。

また、流量演算部4には、タイマ回路43が設けられている。このタイマ回路43に設定された表示更新周期に従って表示部5の測定データ表示を更新する。さらに、流量演算部4には、文字および画像を表示する表示部5がインターフェース回路44を介して接続されている。表示部5としてはCRTや液晶表示板等が使用できる。表示部5には、血流の測定データが数値やグラフ等によって表示され、逆流が発生した場合には逆流警報が表示される。また、流量演算部4には、入力インターフェース回路45が設けられており、この入力インターフェース回路45を介して、ドップラ信号、+側ドップラ信号および−側ドップラ信号が入力されている。

このような構成の流量演算部4により、血流の演算および表示、さらに正流・逆流の判定が行われる。図3は、流量演算部4の動作を示すフローチャートである。ここで、表示部の表示更新周期が1秒、ドップラ信号の平均化時間が3秒の場合について説明する。これらの時間は、これ以外に設定してもよいが、表示更新周期は0.2〜1秒程度、ドップラ信号の平均化時間は1〜3秒程度が好ましい。また、平均化処理は必ずしも行わなくてもよい。

流量演算部4は、手順401で、タイマ回路43に設定された表示更新周期が経過したか否かを判断し、前回表示更新から1秒経過していれば手順402に進み、1秒経過していなければそのまま待機する。次の手順402では、ドップラ信号の周波数のサンプリング値を平均化時間だけ加算して、ドップラ信号を平均化する。ドップラ信号は、例えば50ミリ秒ごとにサンプリングされており、現在から過去3秒間の期間のサンプリング値が保存されている。この場合は、60個のサンプリングデータにより平均化処理が行われる。この平均化されたドップラ周波数をfdとする。

次に、図4または図5のような血流方向の判定を行うことによって、外来ノイズの影響や検出部の性能低下の影響を受けにくくなることを、実例を挙げて説明する。図6は、従来の判定方法と本発明の判定方法の判定結果を比較したものである。図6(a)は、血液の流量が比較的小さい場合であって、直交検波器22の検出信号にノイズが混入した場合の影響を示すものである。正方向の血流はドップラ周波数fdで1000Hzに相当する速度である。このとき、+側ドップラ周波数fdpは950Hz、−側ドップラ周波数fdnは10Hzに相当する信号となる。ドップラ周波数fdで500Hzに相当する逆流が発生すると、+側ドップラ周波数fdpは5Hz、−側ドップラ周波数fdnは475Hzに相当する信号となる。

定数Aを300Hz、定数Bを0.5、定数Cを200Hzとすると、(fd>A)、(fdn>A)、(fdn>B・fd)、(fdn>fdp+C)の各真偽値は図に示すようになる。従来の判定方法(fdn>Aの比較のみによって判定するもの)と本発明の図4の判定方法(方法1)および図5の判定方法(方法2)の判定結果を下段3行に示す。ノイズが混入していない状態では、従来の判定方法でも本発明の判定方法でも同一の判定結果となる。

検出値に電気メス等からの外来ノイズが混入した場合は、ノイズの位相がランダムであるため、fd,fdp,fdnの各検出値にほぼ等しい大きさのノイズ成分が加わることになる。ここでは、400Hz相当の大きさのノイズ成分が混入した場合を示しており、正方向および逆方向のfd,fdp,fdnの各検出値に400Hz相当のノイズ成分が付加されている。図6(a)から、ノイズが混入した場合には、従来の判定方法では、正方向の血流を逆方向であると誤判定してしまうことが分かる。本発明においては、方法1および方法2ともに、このような誤判定は発生せず、正確な判定結果を得ることができる。

図6(b)は、血液の流量が比較的大きい場合であって、超音波プローブ3のチューブ9への取付不良や超音波プローブ3に性能上の問題があり、方向検出部25の各検出信号の分離が悪い場合の影響を示すものである。正方向の血流はドップラ周波数fdで3500Hzに相当する速度である。このとき、方向検出部25の各検出信号の分離が良好な場合には、+側ドップラ周波数fdpは3300Hz、−側ドップラ周波数fdnは35Hzに相当する信号となる。
