本発明は、概略的には、医療用カテーテルに関する。さらに、詳細には、胃および/或いは小腸、または、膀胱に、腹壁の瘻すなわち造孔を通してアクセスし、これを行う際に目立たない状態を維持するカテーテルに関する。

【従来の技術】
食べられない或いは食べられなくなるであろう患者の消化器に食物を人為的に導入する必要性が、今世紀を通じ、そして、前世紀から知られている。1970年代中期より前には、飼育すなわち栄養補給は、赤いゴムチューブまたはポリ塩化ビニルの供給チューブを用いて、鼻から胃に行われていた。鼻から胃へのチューブによる経腸飼育の使用は、シリコンゴムまたはポリウレタンで構成されたチューブの導入とともに、70年代後期に劇的に増加した。これらのチューブは、より強い材料で作られているので、壁がより薄く、したがって、外径がより小さくなっている。これらの小さなチューブは、挿入し易く、かつ、患者の苦痛が少なく、そして、これらチューブの採用により、鼻から胃への経路を介しての経腸栄養摂取が大変急速に発達し、そして、一般的に、経腸栄養摂取への関心が高まった。

6.投与セットを交換するとき、又は、ひどい咳が過度の背圧を引き起こしたときに、患者からの胃酸の漏れを防止する逆流防止弁。
7.保持装置と内部ボルスタとの間のチューブの長さを調節して腹壁の厚さに適合するように、患者の腹壁厚を測定する測定システム。
胃瘻チューブが経腸飼育に使用されるのと同様に、恥骨カテーテルチューブ組立体は、膀胱に薬剤を投与し、又は、膀胱から尿を排出するの使用される。このようなチューブ組立体は、胃瘻チューブ組立体の項で上述したのと同じ7つの特徴または構成要素を備えている。しかしながら、これらは、膀胱または恥骨の領域の上方で腹壁に形成された瘻を通して膀胱にアクセスする。

【発明が解決しようとする課題】
可撓性、および、耐よじれ性は、耐潰れ性と同様に、胃瘻チューブ部分および恥骨上カテーテル法のチューブ部分において、特に重要である。これらのチューブ部分が、皮膚と直交して身体を出るため、これらのチューブ部分が皮膚のすぐ近く位置するように、これらチューブ部分を折り曲げることが望ましい。この問題は、クイン等(Quinn et al) の米国特許第4、834、712号明細書で対処されている。加えて、胃瘻チューブ部分の形態の幾つかは、胃から十二指腸または空腸まで飼育する延長部を含んでいる。これらのチューブ部分延長部は、これらが十二指腸に配置されるか更に空腸に配置されるかに応じた1乃至5の円弧状の曲がり角を通り抜けることができなければならない。高い係数を備えた即ち可撓性が低いチューブ部分は、腸の中を通る際に、腸の側壁に突っ込み、所望のきつい湾曲を作りにくい。

本発明のさらにもう一つ目的は、改良アクセスチューブ部分セットを提供することである。
本発明のさらにもう一つの目的は、身体アクセスチューブ組立体用改良コネクタを提供することである。

【課題を解決するための手段】
上述およびその他の目的は、胃または膀胱アクセス組立体用の補強されたシリコーンチューブ部分であって、補強されていないシリコーンチューブ部分と同じ弾性率を有し、且つ、潰れず又は捩れない壁を有するシリコーンチューブ部分を提供することによって、本発明にしたがって達成される。このチューブ部分は、比較可能なウレタンチューブとほぼ同じ厚さであり、補強された部分を含む従来のシリコーンラバーチューブ部分より薄い。したがって、この新規なチューブ部分は、上述した組立体に使用されているウレタンのチューブ部分および公知のシリコーンチューブ部分の両者より優れている。
本発明のチューブ部分は、比較的柔軟なシリコーンラバー、すなわち、標準ジュロメーターAスケールで測定して硬度35を有するラバーを用いて作られている。比較として、PEGで使用された従来のシリコーンラバーチューブは、Aスケールで60のジュロメーター硬度を有しており、中間から硬いと考えられる硬度を有する。硬いシリコーンラバーは、通常、Aスケールで80乃至90のジュロメーター硬度読みを有している。ジュロメーター硬度読取りは、ASMEスタンダードデズィグネーションD−2240−97の1−4頁に説明されている標準実施に従って行われる。

【発明の実施の態様】
図面、特に図1を参照すると、本発明の一形態の特徴を具現化した交換胃瘻チューブ組立体が、全体を参照番号10で示されている。交換胃瘻チューブ組立体10は、適所で、患者の腹壁Aの外側の飼育物質供給チューブ11から、患者の瘻Sの中を通り、患者の胃STまで延びているのが、示されている。瘻Sは、上述したいくつかの公知の方法の一つによる従来の方法で形成するのがよい。
チューブ組立体10は、上述した意味での交換チューブ組立体である。チューブ組立体10は、従来の飼育物質供給チューブ11に、後述する方法で、容易に着脱できるように設計されている。
本発明の特徴は胃瘻チューブ組立体に例示されているが、後述するように、これらの特徴は、例えば、PEG、および、空腸造瘻術チューブ組立体、または、恥骨カテーテル組立体への応用が同様に有利であることが判る。

ボーラス16のノーズ部分33に、軸線方向の探り針受入れポケット39が形成されている。この点で、ボーラス16は、上述の米国特許第5、451、216号明細書に開示されたものとは異なる。このポケット39は、探り針が採用されている方法とその目的の両方について下記に詳述する方法で、(ここでは図示しない)探り針の先端を受け入れるように設計されている。

チューブ部分15を製造するために、コイルスプリング42が、押し出されたシリコーンチューブに挿入される。液体シリコーンが、チューブの長さに沿って流れるように、チューブ内に導入され、スプリング42のワイヤを被覆しかつ覆い、スプリングのワイヤをチューブの内側に付着させる。液体シリコーンは硬化し、元のチューブと、コイルスプリング42と、被覆とを結合して、内側面46と外側面47とを備えた略円筒形状の壁を有するチューブ部分15にする。コイルスプリング42は、チューブ部分15のシリコーンボディ41の骨組みになるものを形成する。

交換胃瘻チューブ組立体110は、主としてシリコーンラバーで形成され且つ本発明の特徴を備えた短いチューブ部分115を備えているのが示されている。下記で詳述するように構成されている胃瘻チューブ部分115は、チューブ部分の排出端117でチューブ部分を通る液体流路114と流体連通して連結されたボーラス(塊)116と、通路の入口端119で通路114と流体連通して連結されているセットコネクタ118とを有している。
タイヤ型バルーン120が、ボーラス116の先端に隣接して、排出端117の近くでチューブ部分115を囲んでいる。バルーン120は、空気または水のような流動媒体で満たされる。空気が好ましく、この例では、空気(エアー)が採用される。

Claims (9)
