【発明の属する技術分野】
本発明は、ユーザーの皮膚を穿刺して、血糖値、その他の検査に使用するために、少量の血液を採取する、刺通要素を有するランセットアッセンブリに関するものであり、特に、刺通要素を包囲する保護要素をツイストオフする際に、刺通要素が曲がらないような構造を持ち、また、使用済みまたは未使用の状態、更に、使用に対して、ランセットアッセンブリの適、不適の状態を目視により確実に認識できる構造を持ったランセットアッセンブリならびにそれらを構成するランセットおよびカバーに関する。

【従来の技術】
世界的に糖尿病患者(インスリン依存タイプおよび非インスリン依存タイプ)が益々増加してきており、このことは、世界糖尿病学会発表資料(1994年)にも発表されている。
WHOや世界糖尿病学会などの関係機関の追跡調査により、糖尿病の治療、あるいは合併症へ進ませないための方法としては、血糖値をできるだけ頻繁に測定して、その結果によりインスリン注射や経口薬を服用すること、食事療法、ダイエット運動などの方法を併用して、血糖値を常に所定のレベルの範囲に維持するということが最も良いということが定説となってきた。

約10年前よりバイエル社(Bayer)、ジョンソン・アンド・ジョンソン社(Johnson & Johnson)、ベーリンガーマンハイム社(Boehringer Mannheim)などの製薬メーカーにはこのような傾向をとらえて、主として、家庭において血糖値を手軽に測定するための試験紙、血糖値計(携帯型)、ランセット(採血針)、ランセットエジェクター(採血針射出機)を市販し、糖尿病患者による自己管理が容易に行えるようになってきた。

最近の試験紙および血糖値計の性能の進歩は著しく、数年前までは、一回の血糖値測定のために、約20〜25マイクロリットルの量の血液を必要としていたのに、現在では、約3〜10マイクロリットルの量の血液で精度よく血糖値を測定できるようになった。
約20〜25マイクロリットルの血液を必要とする以前の試験紙または血糖値計を使用する場合、必要な血液量を得るために用いるランセットの針(または刺通要素)としては、直径が0.8mmのものが必要であった。試験紙および血糖値計の進歩に伴って必要血液量は少なくなり現在では、直径0.65mm、直径0.5mmのランセットが市販され、これらがランセットの主流になっている。

現在では、約2〜10マイクロリットルの血液で血糖値の測定が可能な試験紙および血糖値計が発表されており、これらを用いる測定のためには、直径約0.3〜0.4mmの直径を持つランセットが求められて来ている。
一方、糖尿病の患者からみて、血糖値を正確にコントロールするために、多い場合では、一日に例えば7回程度も採血しなければいけない患者(インスリン依存タイプの患者における例)および一日に数回程度の採血が必要な患者(非インスリン依存タイプの患者における例)にとっては、針の直径が大きいことによる刺通時の痛み、内出血、小さな傷口が常に指先に残るなどの問題があり、この観点からも針の直径が小さいランセットを求める声が出始めていた。

また、幼児または小児に見られる若年性糖尿病患者(インスリン依存タイプが多い)の母親からも刺通時の指先の痛みおよび傷口の発生を最小限にするため針の直径が小さいランセットの上市が望まれていた。
このような、試験紙および血糖計の進歩、患者側からの要望に答えるように、最近、商品名「クリーンレット(Cleanlet)28G」(直径0.4mmの針使用)および「クリーンレット(Cleanlet)3OG」(直径0.32mmの針使用)の二つのランセットが、ゲナーメデイカル社(Gainor Medical、米国)から、また、商品名「ウルトラ−ファインランセット(Ultra−Fine Lancet)」(直径0.36mmの針使用)というランセットがベクトンデイキンソン&カンパニー(Becton Dickinson、米国)より市販された。

これらのランセットは、従来の直径0.8mm、直径0.65mm、直径0.5mmのものに比べて、はるかに細い直径の針を使用しているため、使用した患者からは、刺通時の痛みが激減したこと、長期間にわたって使用しても、内出血や傷口が残ることは殆ど無いなど、一様に歓迎されている。
しかし、これらの小さい直径の針を使用するランセットには、針の直径が小さいために派生する問題がある。

針の直径が、直径0.4mmであるクリーンレット28Gを図1に示す。このランセット11は、ランセットボディ13と保護キャップ15とがノッチ部分により一体に接続され、刺通要素の先端の使用直前まで確実に保持できること、ランセットが使用済みであるか否かの状態(ステイタス)が目視およびランセットボディ13と保護キャップ15との相対的な捻転により確実に判断できること、また、使用済のランセットの刺通要素の先端部を確実に隔離して安全に廃棄できるようにすることというディスポ型のランセットに必要とされる最小限の要件を満足している。

このようなランセットを使用する場合、使用のために保護キャップ15をツイストオフする際に、キャップが刺通要素を軸としてその回りの回転運動により捻られるとは必ずしも言えない。確かに、ランセットボディと保護キャップの向かい合う端面は実質的にフラットであるとも言えるので、刺通要素の軸の回りの回転は比較的確保され易い。

しかしながら、多くの使用者は、刺通要素を軸とする回転と同時に、保護キャップを引き抜く動作も行うので、キャップのフラットな面とランセットボディのフラットな端面が相互に離間していく、ツイストオフ操作の後半部分においては、離間方向(矢印AおよびB)が必ずしも刺通要素の長手方向(矢印C)とは一致せず、その結果、突出する刺通要素が曲がる、特にランセットボディの端面17にて刺通要素が折れ曲がる可能性があることも否定しきれない。刺通要素の直径が小さくなると、この折れ曲がりは生じやすくなる。刺通要素のそのような折れ曲がりは、刺通要素が突出するランセットボディの端面にて特に発生し易い。

そのようなランセットの折れ曲がりが生じても、非常に極端な折れ曲がりを除いて、ランセットの機能には実質的な影響は無いが、穿刺口に刺通要素が真っすぐに浸襲していくと、傷口が最も小さくなり、痛みも少なくなるので、その観点からは、ランセットの使用に際して刺通要素が折れ曲がっていないことが望ましいのは勿論である。
このようなランセットの詳細については、実願平第4−60676号(実開平第6−23505号)の記載および対応する米国特許第5,385,571の記載を参照でき、この引用により、これらの開示内容は、本明細書の一部を構成する。

図2に、刺通要素の直径が0.36mmであるウルトラ−ファインランセット21を示す。このランセットアッセンブリは、図から判るように、針23は、成型されたホールダー25の端面の中心にある小孔に装入されて接着剤より固定されている。従って、刺通要素23はホルダー25から常に露出している。この露出した針先をカバーするために、別に成型された保護キャップ27が刺通要素を含むホールダー先端部29にプレスフィットされている。このような構造であるため、保護キャップは、露出する刺通要素には全く接触しないので、保護キャップ27を取り去る時に、針先を曲げる可能性は少ない。また、採血に使用した後にホールダー先端部に、保護キャップを再びプレスフィットで被せることにより安全に採血針を廃棄できる構造を有している。

しかしながら、図2のランセットアッセンブリの問題点はランセットアッセンブリのステイタス(未使用か、使用済かの状態)を外観から確実に判断できないということにある。
このランセットアッセンブリを採血に使用した後、安全に廃棄するために、ランセットボディの端部に保護キャップを被せてしまうと、使用前と全く同じ状態になる。保護キャップを捻転させても、使用前と同じツイストトルクで捻転するし、保護キャップを(ホルダー25に対して刺通要素の長手方向に)引っ張ってみても、使用前と事実上変わらない引き抜き力を示す。このことを下記実測データ(5測定値の平均)にて示す:

このように採血後のランセットのステイタスが使用前と全く同じであり、ステイタスを判別できないということは、不注意によるランセットの再使用という事故が発生する可能性を否定しきれないことを意味する。エイズ、B型肝炎、C型肝炎などのように体液を媒介とする伝染病が大きな社会問題として取り上げられ、これらの伝染病の防止が要求されている現在、安全な廃棄のために使用後に再キャップしてしまうと、未使用のものと区別できないという機構は問題があるといわざるを得ない。

この点、図1に示すようなクリーンレット28G、クリーンレット30Gを初めとするランセットボディと針先保護キャップを一体で成型し、使用直前に、保護キャップをツイストさせて取り除くという構造のランセットは、たとえ、使用後に保護キャップを刺通要素に、再キャップしたとても、使用のために、保護キャップをツイストしてみると、トルクが実質的に感じられないので、確実にそのランセットが使用されたものであることを認識できる。

更に、図2に示すような針を保持するホールダーおよび針先保護キャップが別ピースであり、一体で成型されていないランセットの場合、プレスフィットでこれらの要素が結合されているだけなので、組立後、自動機で計量され、包装され、滅菌され、更に運送される過程において、保護キャップが、緩んだり外れたりする可能性があり、その意味において、刺通要素の突出先端部の滅菌状態の維持、その保証については劣っている。

【発明が解決しようとする課題】
そこで、ランセット、特に刺通要素の直径が小さいランセットにおいて、図1および図2のランセットの好ましい機能を兼ね備えるランセットを提供することが望まれる。即ち、そのようなランセットには、
1)刺通要素を被覆しているキャップを、針を曲げないでツイストオフできること;
2)成形、滅菌、組立、計量、包装、運搬などの過程を経て、採血のために使用されるまで、刺通要素の滅菌状態を確実に保持できること;
3)ランセットのステイタス(未使用か使用済か、使用されてはいないが、使用に対しては、不適切な状態であるか否か等)を目視により、あるいは例えば針先保護キャップを捻転させることにより確実に判断できること;
4)採血に使用した後に、針先を確実に隔離して、安全に廃棄できるようにすること
を兼ね備えることが望まれる。

【課題を解決するための手段】
第1の要旨において、本発明は、
(1)(i)例えば針のような刺通要素が一端から突出する部分の断面(刺通要素の長手方向に対して垂直な断面)が円形である(従って、円柱形状である)ランセットボディ、および
(ii)突出する刺通要素を包囲してランセットアッセンブリの使用前に刺通要素が露出するのを防止する、断面(刺通要素の長手方向に対して垂直な断面)が実質的に非円形の部分を有して成る保護要素
を有して成るランセット、ならびに
(2)ランセットの保護要素およびランセットボディの円柱状部分の少なくとも一部分を受容し、ランセットボディの断面が円形の部分(円柱状部分)の周囲で刺通要素の長手方向を軸とする回転を可能にする空間(例えば円柱状の空間)および保護要素の周囲で刺通要素の長手方向を軸とする回転を不可能にする空間を有して成る内部空間を有する捻転用カバー
を有して成るランセットアッセンブリを提供する。

本発明のランセットアッセンブリにおいて、捻転用カバーを保護要素の実質的に全部および少なくとも一部分の、好ましくは実質的に全部の円柱状部分に被せることができ、その状態で刺通要素の長手方向軸(以下、単に「長手方向軸」とも呼ぶ)の回りで捻転用カバーとランセットとを相対的に相反する方向に回転させようとすると、両者は円柱状部分の回りでは両者は回転しようとするが、保護要素の回りでは回転に対して抵抗しようとする。従って、回転力がある程度以上になると、抵抗力に優って、捻転用カバーとランセットは相反する方向に回転できるようになっている。

このような回転に際して、ランセットボディの円柱状部分は、その呼び方からも理解できるように、長手方向軸に沿ってある程度の長さ(円柱の高さに相当)を有する。また、円柱状部分の外側表面と捻転用カバーの円柱状空間を規定する内側表面が相反する方向に実質的に同じ軸で回転できる程度にしかクリアランスを有さないようにする。例えば、実質的に容易に摺動するようになっている。

このようにすると、本発明のランセットアッセンブリにおいて、ランセットボディの円柱状部分およびそれを受容する捻転用カバーの内部空間は、捻転用カバーとランセットボディとの間で相対する方向の同軸回転(円柱状部分の中心に刺通要素が存在することが前提であり、従って、軸は実質的に刺通要素となる)を可能にし、また、円柱状部分はある程度の長さを有するため、同軸回転中に長手方向軸に沿って一方または両者が相対的に移動しても同軸回転を維持することができる。これは、ランセットアッセンブリの使用者が、刺通要素を露出させるために、ツイストオフ(ねじ切り)操作をする場合に、回転動作と引き抜き動作を同時に行っても、刺通要素に、特にランセットボディの端面にて、不適切に大きな曲げ力が加わらないことを意味する。

本発明の好ましい態様では、ランセットアッセンブリは、刺通要素を除いたいずれの要素もプラスチック材料により形成されており、円柱状部分と保護要素との間が手動でツイストオフできる強度のノッチ(切り欠き)を有する部分により接続されるように、これらは一体に成形されている。本発明のランセットアッセンブリに適当なプラスチック材料としては、ランセットについては、例えばポリエチレンポリプロピレンであり、捻転用カバーについても例えばポリエチレンポリプロピレンを例示できるが、これらに限定されるものではない。ポリエチレンを使用する場合、γ−線滅菌を実施できるので好都合である。特に好ましい組み合わせの例としては、ランセットにポリエチレンを使用し、捻転用カバーにポリプロピレンを使用する。

上記のようなプラスチック材料を使用することにより、安価に、しかも大量生産できるという利点があり、また、ノッチの形成も容易になる。本発明においてノッチとは、断面を部分的に小さくした部分を意味し、その部分の強度を意図的に弱めてツイストオフによる保護要素とランセットボディとの分離をノッチ部分にて容易に行わしめる機能を有し、ノッチ自体はプラスチック成形に関係する当業者には公知の事項である。

このようにプラスチック材料により一体成形されていると、捻転用カバーとランセットを相反する方向に同軸回転させようとすると、保護要素と捻転用カバーとは回転できないが、捻転用カバーと円柱状部分とは回転できるので、ノッチ部分の破壊強さが回転力より大きい場合は、捻転用カバーとランセットは回転できず、回転力がノッチ部分の破壊強さより大きくなると、ノッチ部分が破壊して捻転用カバーとランセットとの相対的な回転動作が可能となり、破壊後のそれ以上の回転に際しては、破壊に要した回転力より遥かに小さい回転力で両者を回転させることができる。従って、両者の回転に必要な力を感じることにより、使用者は、ノッチ部分が(使用済みのためまたは何らかのトラブルのために)既に破壊されている(従って、そのようなランセットを使用してはいけない)か、あるいは未だ破壊されていない(従って、そのようなランセットをこれから使用してよい)かを容易に判断できる。

一旦、ノッチ部分が破壊すると、保護要素をランセットから分離するには、長手方向軸に沿って両者が相対的に離間するように移動するだけでよく、この移動にはわずかな力しか必要ではないが、厳密に長手方向軸に沿うことは容易なことではない。しかしながら、本発明のアッセンブリでは、円柱状部分がこの長手方向軸に沿う移動をガイドするので、保護要素の脱離に際して刺通要素が折れ曲がるという問題点は実質的に解消される。

本発明の別の好ましい態様では、捻転用カバーの内部空間はその中で保護要素が長手方向軸に沿って移動できるようにする溝を有して成り、また、その移動を目視的に観察できる窓部分(開口部)を捻転用カバーは有する。先に説明したように、捻転用カバーは、保護要素を受容できる内部空間を有するが、その内部空間において、保護要素は長手方向軸に沿った移動を可能にする。この移動は、保護要素とランセットボディが一体に結合している状態(即ち、ツイストオフ前)においては、捻転用カバーをランセットに対して軸方向に移動することによって保護要素および円柱状部分の少なくとも一部分に被せることを可能にする。また、ツイストオフ後にあっては、保護要素のみが捻転用カバー内に残ってその中で制限的に移動できることを可能にする。

そのために、保護要素は、突出部分をストッパーとしてその外側表面に有して成り、上述の保護要素の窓部分を規定する縁はストッパーと協働して、保護要素の長手方向軸に沿った移動を制限する。この窓部分はストッパーを係止して、保護要素が捻転用カバーの開口部に向かって更に移動できないようにする。更に、窓部分は、その中の保護要素の移動を確認できると共に、ランセットの使用後にあっては、突出する刺通要素を有するランセット端部を捻転用カバーに再度挿入してカバーをランセットに被せた場合に、刺通要素が保護要素を押している状態でカバー内で保持されている様子を確認できるようにする。

ランセットの使用後に、ランセットを捻転用カバー内に挿入すると、刺通要素はカバー内に保持されている保護要素を前方に押し、カバー内には保護要素が移動できる空間が存在するので押された保護要素は前方に移動し、露出している刺通要素が全部保護要素の中に入り込むことはない。従って、露出している刺通要素の少なくとも一部分およびそれにより押されている保護要素の一部分を窓部分から目視的に確認できる。
従って、ランセットが捻転用カバー内にセットされている状態であっても、窓部分から内部の様子を確認することによって、使用済みのランセットである(刺通要素が見える)か、使用前のランセットである(刺通要素が見えない)かを容易に確認できる。

より好ましい状態では、捻転用カバーの内部空間は、上記説明の場合より小さく、使用済みのランセットを捻転用カバー内に挿入した場合、刺通要素が保護要素を前方に押すが、ある程度押されるとその移動が止まり、ランセットの更なる挿入動作により刺通要素の先端部が保護要素に貫入するようになっているのが好ましい。そのようにすると、体液が少し付着している刺通要素の先端部が保護要素内に封入されるので好都合である。

更に、本発明の別の好ましい態様では、ランセットボディの円柱状部分は係合手段を有し、この係合手段は捻転用カバーの内側表面に設けた被係合手段と協働してランセットと捻転用カバーとが極めて容易に分離しないようにすると共に、ツイストオフの時の捻転用カバーのランセットボディに対する同軸回転を助長する。即ち、本発明のランセットの使用後に、捻転用カバーをランセットに被せて廃棄するが、被せた状態を維持するために係合手段は存在する。具体例としては、係合手段は、円柱状部分の周囲に設けた円周状の溝であり、被係合手段は、捻転用カバーの内側表面に設けた突出部であり、溝内に突出部がはまりこむようなスナップフィットの関係であってよい。

本発明のランセットアッセンブリにおいて、保護要素の内部空間が非円形の断面を有して成るという場合、内部空間の一部分が円形以外の断面を有することを意味し、そのような断面の形状は特に限定されるものではなく、保護要素が捻転用カバーに対して回転しなければどのような形状であってもよい。例えば四角形(例えば正方形および長方形)、五角形、六角形の断面を例示でき、これらは面取りされていてもよい。また、捻転用カバーの保護要素を受容する部分の断面は、保護要素の断面と必ずしも対応していなくてもよく、あるいは対応していてもよい。尚、円形断面の部分については、捻転用カバーがその部分を受容する部分の断面は、両者が相対的に回転できる程度内で実質的に円形断面の部分に外接してよい。
第2の要旨において、本発明は、上述のようなランセットアッセンブリを構成するランセットおよび捻転用カバーを提供する。これらの特徴は、第1の要旨のランセットアッセンブリに関連して説明した。

【発明の実施の形態】
次に、図面を参照して本発明の好ましい態様を説明する。
図3〜6に本発明のアッセンブリを分解斜視図にて示す。図3は組立前のランセットアッセンブリの状態を、図4はランセット33の斜視図を、図5は捻転用カバー35の部分切り欠き斜視図を、図6は組み立てた後でツイストオフする前の状態の部分切り欠き斜視図を模式的に示す。

本発明のランセットアッセンブリ31は、ランセット33および捻転用カバー35から構成される。ランセット33は、ランセットボディ37および保護要素39を有して成り、これらは、好ましい態様では、一体に成形され、ノッチ部分41により接続されている。図示していないが、刺通要素は、ランセットボディ33および保護要素39内に埋設されており、ノッチ部分41を破壊して保護要素39を前方に(矢印の方向に)移動してランセットボディ37から分離すると、露出するようになっている。ランセットボディ33は、その前方部分にある程度の長さを有する円柱状部分43を有する。

捻転用カバー35は、アッセンブリを組み立てる場合に保護要素39および少なくとも一部分の円柱状部分39が通過する開口部45を有する。捻転用カバー35の内部空間は、溝状(または矩形断面の)空間部分47および円柱状(または円形断面の)空間部分49を有して成る。空間部分47は保護要素39を内部回転不可能な状態で受容でき、また、空間部分49は、少なくとも一部分の円柱状部分39を内部回転可能な状態で受容できる。捻転用カバー35は、更に、矩形断面部分に窓部分51を有して成り、窓部分51を介して保護要素39の状態を見ることができる。窓部分51は、図示するように捻転用カバーの両側もしくは片側に1つまたは複数存在してよい。

ランセットボディの円柱状部分43は、その周囲に係合手段としての溝部分44を有して成る。この溝部分はカバー35の内側に設けた被係合手段53と係合して、カバー内におけるランセットの長手方向軸に沿う移動を制限し、また、カバーとランセットボディの同軸逆方向回転を助長する。図示した態様では、スナップフィットで係合するようになっている。尚、溝に代えてセレーション(例えば幅の小さい複数の溝)を捻転用カバーの内側表面に設けてもよく、この場合、ランセットボディの円柱状部分には、このセレーションと協働して係合する凹凸が設けられる。

保護要素39は、図示するように突出部55を有し、これは、窓部分51の縁57により係止されるストッパーとして機能する。本発明のアッセンブリを販売または使用のために組み立てるために、ランセット33に捻転用カバー35を被せるために矢印の方向に両者を近づける(図3の矢印D)時、突出部は、後方向きに広がるテーパー形状を有するので、また、刺通要素以外がプラスチック材料からできているので、たとえ保護要素と捻転用カバーとの間のクリアランスが小さくても、ランセット33をカバー35内に押し込んで、突出部55が窓部分51の中に位置するようにできる。しかしながら、突出部55が一旦の窓部分51中に入ると、突出部55のテーパー形状のため、突出部55は窓部分51から外れて後方(矢印と反対の方向)に移動することはできない。即ち、一旦突出部55が窓部分51内に入ると、保護要素39の移動は制限され、実質的には捻転用カバー35から脱離することはないようになっている。

図6の状態から、ランセット33と捻転用カバー35を刺通要素を軸63とする回転であって相反する方向に力を加え、その力がノッチ部分41の破壊強度より大きくなると、その部分で破壊が起こり、保護要素39とランセットボディ37との結合関係は無くなる。刺通要素は円柱状部分と同軸である(即ち、円柱の軸が刺通要素である)ので、この回転に際して、ランセットの円柱状部分43がその回りの捻転用カバーの回転をガイドすることになり、カバー(従って、その中に位置する保護要素)の刺通要素を軸とする回転が確保される。また、円柱状部分はある程度の長さを有するので、この回転に際して軸方向からずれた引っ張り力が同時に作用しても、円柱状部分はその力に抗するのでそのような引っ張り力により刺通要素が曲がることはない。

回転によりノッチ部分を破壊した後、ランセットアッセンブリに力を加えて、ランセット33と捻転用カバー35を相反する方向で刺通要素に沿った方向に引き離すと、突出部55が窓部分51から出ることができないので、カバー35内に保護要素39が取り残された状態で刺通要素61が端部から突出した状態になっているランセット33がカバーから抜け出る。この状態を斜視図にて図7に模式的に示す(相反する方向を矢印EおよびFにて示す)。刺通要素61が露出しているランセット33は、その状態で市販のフィンガープリッキングデバイスに装着して、採血に使用する。

In order to mold the lancet of FIG. 3 (the protective element and the lancet body are integrally molded and have a notch portion), a single die was produced.
The material of the piercing element to be inserted is stainless steel 304, two types having a diameter of 0.3 mm and a diameter of 0.4 mm, and the needle tip was ground with an NC needle grinder. The mold was set in a 30-ton vertical molding machine. The needle was molded by inserting it into the mold with tweezers. While holding the lancet with the right hand, the left hand twisted the torsion cover clockwise. The torsion cover was smoothly twisted around the longitudinal axis (piercing element) while being guided by the cylindrical portion of the lancet and a groove provided thereon.
The protective element cannot rotate within the torsional cover, but in this regard, the protective element is rotated integrally with the torsional cover through the window portion of the torsional cover (so the torsional cover is around the protective element). It was confirmed that it cannot be rotated.
When the twisting cover is pulled about 1.5 turns, the rotational torque drops sharply. When the torsion cover is pulled along the longitudinal axis, the pulling operation is guided by the cylindrical portion of the lancet and can be pulled straight out. did it.
