らなる。本発明は、また、新規ですぐれた特性を有する
フリーな微小気泡体(microbubbles)を調製することが
可能な気体(gas)を選定することにより、そのような
像を増強する方法に関する。本発明の方法により選定す
ることが可能とされる気体からなるこれらの微小気泡
は、サイズが極めて小さく、かつ血流中に生存する時間
が長く、これまではフリーな微小気泡が到達できないと
信じられていた心臓血管系、末梢血管系、及びその他の
重要な諸器官の部分を、コントラストを増強してイメー
ジングすることを可能とするものである。

を通るときに反射される。異なるタイプの体組織は、超
音波を異なって反射し、そしてその反射像は、しばし
ば、適切には“エコー”と表現されているが、異なる内
部構造を反射する超音波により生起されるものであり、
当該エコーが検出されて、電子工学的にビジュアルな表
示に変換される。この表示は、外科医や他の診断専門医
にとって、心臓血管の病気の進行具合や腫瘍の存在もし
くは性質などを知ることを初め、色々な点で非常に貴重
なものである。
用な像を得ることはとりわけ難しいことであるが、それ
は、その構造の詳細は、コントラスト増強剤を用いない
超音波の反射によって生起される超音波像においては、
周辺組織と十分に識別できないからである。更に、伝統
的な超音波像は、質、解像度ともに劣っていることは周
知のとおりである。このために、特定の生理学的な状況
の検出や観察は、目的の器官や他の構造に薬剤を注入し
て超音波像におけるコントラストを増強することによっ
て、実質的に改善することができる。他の場合では、コ
ントラスト増強剤そのものの動向を検出することが特に
重要となる。例えば、特殊な心臓血管異常が原因である
ことが知られている明瞭な血流パターンは、血流中に造
影剤を注入して血流を動力学的に観察することによって
のみ識別可能である。
ントラスト増強剤を見い出すべく試みる中で、固体、気
体及び液体を使用することの研究を幅広く行ってきた。
ゼラチンでカプセル化された微小気泡、気体混入リポゾ
ーム、音波処理により部分的に変性された蛋白質、高フ
ッ素化有機化合物を含む乳濁液などの複合物質について
も研究され、主として体内における安定性や超音波像に
おいて著しく増強されたコントラストを与える能力な
ど、理想的な特性を有する薬剤を開発する試みがなされ
てきた。
小さな泡は、標準的な超音波造影営技術を用いて作出さ
れる像において容易に検出される。血流又は体の特定の
箇所に注入されると、微小気泡は、微小気泡を含む領域
とその周辺組織の間のコントラストを増強する。

に急速に溶解することは、それらを超音波コントラスト
増強剤として利用することをきわめて限定的なものにし
ている。最も大きな制限となるものは、微小気泡のサイ
ズとそれが溶液に溶けずに存在する時間の長さである。
気泡は、当然のことながら、気泡の懸濁液が注入される
生体に塞栓症の危険をもたらさないだけの小さなもので
なければならない。同時に、超音波造影に一般的に用い
られている気体からなる極めて小さいフリーな気泡は、
溶液に非常に急速に溶けるので、それらのイメージ増強
能力は、注入部位に隣接した場所のみに存在する。更な
る障害が、心臓血管系の超音波のイメージングに存在す
る。医療研究者らは、通常の空気、純粋窒素、純粋酸素
又は二酸化炭素からなる微小気泡が溶液に溶けるのに要
する時間について研究してきた。これらの気体の直径約
8ミクロン以下で肺を通って左心室に到達し得るほど十
分に小さい微小気泡は、その寿命が約0.25秒以下であ

通り抜けるのに2秒以上かかるので、これらの気体の微
小気泡は、肺を通り抜ける途中で完全に溶けてしまい、
左心室にはとうてい到達しない。上記同書。主として、
このような気泡のサイズと寿命との間のかね合いの困難
さから、多くの研究者らは、フリーな微小気泡は、心臓
血管系の特定の部分の超音波診断に用いるコントラスト
増強剤としては役に立たないという結論を出した。
強剤は、本発明によりその選定が可能な生体適合性の気
体からなる微小気泡であり、十分小さいので、直径約8
ミクロンの肺の毛細血管を通り抜け、それによって左心
室のコントラストの増強された超音波診断を可能とする
ものである。フリーな微小気泡は、血流中に十分長く存
在するので、末梢静脈内に注入されると、溶液に溶ける
ことなく右心室を通過し、肺を通って左心室に到達する
ことができる。また、これらの増強剤のあるものは、溶
液における持続性が極めて長いので、多くの他の器官や
構造のコントラストの増強を可能とする。

することにより、本来フリーな微小気泡を使用する際に
存在すると思われていた多くの制限を克服するものであ
る。従って、ここに開示された物理的、化学的パラメー
ターによって選定された生体適合性の気体あるいはそれ
らの気体の組合わせにより調製される微小気泡の組成物
からなる当該超音波コントラスト増強剤は、十分な時間
存在でき、そして十分小さいサイズであるので、血流中
における当該微小気泡は、安定であり、これまでフリー
な微小気泡が到達できないと考えられていた体内の特定
の構造を、超音波のコントラストを増強してイメージン
グすることを可能にすることが見い出された。

生体物の内外において、必要以上の毒性や生理的又は薬
学的影響をもたらさず許容できる方法で、その機能を発
揮することができる化学物質を意味するものであり、そ
して、それは、生きている生物体の体温において、非常
に低い密度及び粘度、圧力と温度の変化による比較的大
きな拡大及び収縮、そしてどのような容器にも均一に分
布する性向、によって、固体もしくは液体の状態と区別
できるような物質の状態にあるものである。下記の表

良された超音波コントラスト増強剤に関する研究につい
て、その観点で概説することが有用である。

測定されるものでなければならない。超音波像に見られ
る3つの主なコントラスト増強効果がある。すなわち、
逆散乱(backscatter)、ビーム減衰(beam attenuati
on)、及び音差速度(speed of sound differentia
l)である。これらの効果を、それぞれ、順を追って記
述する。

会うと、大きい物質がより大量の超音波を散乱する。

以下に述べることに鏡みて、純粋の液体は、フリーな微
小気泡と比べて、かなり効果の少ない散乱体であると当
業者に認識されている。

在することにより起こる超音波の減衰の結果生じるもの
とみられている。粒子によるエネルギーの吸収は、“相
対運動”と呼ばれるメカニズムで起こる。相対運動によ
って生じた減衰の変化は、粒子濃度と直線的に、そして
粒子とその周辺の媒体の間の密度差の2乗として増加す

泡により生じる逆散乱の著しい増加が、最も劇的な効果
であり、そして、この現象の利点を有するコントラスト
増強剤が、仮りに溶液中における安定性が限られている
という障害が克服されれば、最も望ましいものとなるで
あろう。

の構造もしくは特定の領域に注入される。

いことから、粒子が凝固する傾向を示すような特定の細
胞に限られている。

法は、いずれも、厳密な基準によって気体を選択すれ
ば、肺を通過してコントラストの増強された超音波造影
を可能とするサイズで、安定した微小気泡を造る能力を
生み出せることを何ら示唆するものではない。

を決定する。気泡の安定性に影響を与え、従って、寿命
にも影響を与える系の化学的性質は、気体分子を消費し
たり、変えたり、発生させたりする反応の速度と程度で
ある。
酸化炭素ガスが水中に存在するときに起こる。その気体
が水溶液に溶けると、炭酸が二酸化炭素ガスの水和によ
って生じる。二酸化炭素ガスは、水に高度に溶けるの
で、ガスは急速に溶液に拡散し、気泡サイズは急速に小
さくなる。溶液中に炭酸が存在することで、水溶液の酸
−塩基の化学的性質が変化し、そして、溶液の化学的性
質が気泡体の溶解によって変わると、二酸化炭素の気泡
の安定性が、溶液が飽和するにつれて変化する。この系
においては、気泡の溶解速度は、いくぶんかは、すでに
溶液に溶けている二酸化炭素ガスの濃度に依存する。

泡は、通常の空気を使う従来の技術を使用した調製さ
れ、従来の超音波診断と同じ方法で注入される。

計算する必要がある。特に、それぞれ気体のモルの体積
量と溶液の粘度に依存している、気体の密度、溶液に対
する気体の溶解度、及び溶液における気体の拡散係数が
下記に開示する等式で使用される。かくして、ここに開
示する方法により、与えられた特定の気体−液体系の物
理的性質が評価され、気泡崩壊の速度と程度が見積もら
れ、そして効果的なコントラスト増強剤を構成する気体
が、これらの計算に基づいて選定される。そして、従来
の技術を使用して、実質的に改良されたコントラスト増
強剤が製造されると共に、これを超音波造影の質と有用
性を改良するために使用することができる。

の溶解度(M)、及びDは気体の溶液中での拡散係数
(cm3sec-1)であることを特徴とする超音波像増強のた

超音波造影方法を提供するものである。

する気体のすべてがまだ半径R0の気泡中に含まれている
ことを意味する。

係数を掛けた通常の空気の微小気泡が完全に溶ける時間
に等しい。例えば、気体XのQ係数が10,000ならば、気
体Xの微小気泡は、空気の微小気泡に比べて溶液中に1
0,000倍長く残存する。Q値は、ここにあげた数値が分
かっているかもしくは概算できるとすると、どの溶液中
のどの気体に対しても決定することができる。
値を決定したりもしくは概算したりする別の方法は、気
体の化学的構造によって決める必要がある。これらのパ
ラメーターの値は、ガス・エンサイクロピディアや米国
化学会発行のデータ表などの周知の科学文献から入手で
きる場合がある。ほとんどの気体の密度の値は、Handbo
ok of Chemistry and Physics,CRC Press,72d Ed.(199
1−92)などから容易に入手できる。更に、各種の気体
の水に対する溶解度とモル体積は、正確に測定されてい
る。しかしながら、多くの場合、モル体積と溶解度を求
める数値計算は、上述の方法で個々の気体のQ係数の値
を決定するために使うデータを得るために、計算したり
もしくは概算したりする必要がある。気体を好適に選定
するためにQ値を計算する例は、本発明の方法を如何に
して個々の気体に適用するのかを説明するものである。

の高いQ係数の気体、すなわち、約5%の気体が最適値
である、と混合される。ソルビトールは、市販されてい
る化合物であり、水溶液と混合すると実質的に当該溶液
の粘度を増加する。上記の等式(3)で分かるように、
より高い粘度の溶液は、溶液中の微小気泡の寿命を延ば
す。40−50%ソルビトール溶液は、注入の際軟らかな固
まりに維持されること、すなわち、注入圧力に耐えられ
る範囲で可能なかぎりもとのままに維持することが好ま
しい。微小気泡懸濁液を製造するために、選定された気
体は、一方の注入器に集められる。その同じ注入器に一
定量のソルビトール溶液も入れられる。一定量のソルビ
トール溶液を他の注入器に移して、両者の体積の合計が
目的とする微小気泡の容量パーセントに基づいて適正な
パーセントになるようにする。小口径の穴を有する二つ
の注入器を使って、液体が、ここに記載した目的に適合
するサイズ分布を有する微小気泡懸濁液を製造するため
に、約25回、もしくは必要とされる回数、気体雰囲気中
に噴霧される。この技術は、当然のことながら、目的と
する濃度で目的とするサイズの微小気泡懸濁液を造る方
法に応じて、若干変え得るものである。微小気泡のサイ
ズは、既知の方法で、Butler,B.D.,“Production of Mi
crobubble for Use as Echo Contrast Agents(エコー
コントラスト剤として使用する微小気泡の製造),"J.Cl
in.Ultrasound,V.14 408(1986)、クールター・カウン

の実験のために、190×100mmのパイレックス(登録商
標)(No.3,140)蒸発皿に、37度の水約2,000mLを満た
した。20%ソルビトール溶液5mLを三方活栓につないだ1
0mL注入器に採取した。対象の気体(又は、関連の低沸
点液体)を2立方センチメートル含む10mL注入器を、ソ
ルビトール溶液を含む注入器に取り付けた。ソルビトー
ルと気体又は液体を、微小気泡の懸濁液又は分散液を調
製するために、25回に亘って急速に混合し、次いで、急
速に水に加えた。この方法による微小気泡は、一般的に
約100ミクロンのサイズであり、仮りに空気からなるも
のであれば、持続時間は、31秒(0.5分)と計算され
る。添加の前、添加の間、及び添加の後の超音波走査を
ヒューレット−パッカード・モデル・ソーナス500超音
波スキャナーを5MHzで作動させて行なった。微小気泡が
観察される時間を記録した。結果を下の表Vに示す。実
験によるQ−値は、測定された気体の持続時間を測定さ
れた空気の持続時間で除すことによって得られた。

ーフルオロペンタンとパーフルオロヘキサンの効率を比
較することにより検査した。パーフルオロペンタン(ド
デカフルオロペンタン)は、計算によると、Q−係数が
207,437で、沸点が標準圧力条件のもとでは29.5℃であ
る。パーフルオロヘキサン(PCR社、ゲインズビル、フ
ロリダ)は、計算によると、Q−係数が1,659,496で、
沸点が標準圧力条件のもとでは59−60℃である。従っ
て、ヒトの体温37℃では、パーフルオロペンタンは気体
であり、一方パーフルオロヘキサンは液体である。

水約1,000mLをヒトの代用血液として用意し、実施例5
に示したように、上記各々の分散液のサンプルを添加す
る前と添加した後について、超音波による走査を行なっ

ずに、かつ開示された開発の課題から得られる変更や修
正をなし得るものとみなされるべきである。このよう
に、本発明は、密度、溶解度、モル体積のデータが入手
できれば、当業者が、超音波造影剤としての種々の化合
物の適合性を決定することを可能にするものである。

- 【請求項1】ペルフルオロプロパン、ペルフルオロブタ
ン及びペルフルオロペンタンから成る群から選ばれる気
体の微小気泡を含む、生体適合性超音波造影剤。 - 【請求項2】ペルフルオロプロパンの微小気泡を含む、
請求項1記載の生体適合性超音波造影剤。 - 【請求項3】ペルフルオロブタンの微小気泡を含む、請
