Description

【発明の詳細な説明】
【０００１】
【産業上の利用分野】本発明は、癌によって損傷したヒ
ップボーンのための内部人工補綴体に関するものであ
る。
【０００２】
【従来の技術】損傷した大腿骨のジョイントボール及び
／又は、ヒップボーンのジョイントソケットが、適当に
機能しなくなった場合、その代用として使用される数々
の内部人工補綴体が知られている。癌によって損傷した
ヒップボーンにおいては、多くの場合、ヒップボーンの
骨質構造の大部分が罹患し、このため通常の方法におい
ては人工補綴体ヒップソケットを支持することが出来な
い。このような場合、末端部分（心臓から離れた部分）
に人工補綴体ヒップソケットを受け入れるための窪みを
有する、独特の骨代用物が形成されなければならない。
この種の内部人工補綴体を個々に形成するには、多くの
費用と時間を要する。
【０００３】初めにＸ線特にコンピューター断層写真を
使用することにより代用されるべき骨部分のモデルを形
成し、次いでそのモデルから内部人工補綴体を形成する
方法は知られている。
【０００４】
【発明が解決しようとする課題】本発明の目的は、癌に
よって損傷したヒップボーンのための内部人工補綴体で
あって、様々な種類の個別の形態を有するヒップボーン
に使用出来る内部人工補綴体を提供することである。
【０００５】
【課題を解決するための手段】本発明は、癌によって損
傷した部分が本来のヒップソケット若しくはその付近の
領域にある場合に、写真検査が必然であることを前提と
したものである。更に、人工補綴体ヒップソケットを受
け入れる人工補綴体の窪みが充分な支持部を有し、ヒッ
プボーンに安全に固着されることが重要である。
【０００６】
【作用】本発明による内部人工補綴体は、独特の構造を
有しているが、ヒップボーンの寸法に関して個々の調整
が可能なものである。本発明によれば、この内部人工補
綴体は基端部及び末端部からなり、これらがネジによっ
て互いに固く締め付けられる。基端部及び末端部は共
に、ヒップボーン若しくは脊椎にそれぞれネジによって
固着されるための取り付け用突出部即ち伸長部を有して
いる。基端部は、第５脊椎とサトラム（ｓａｔｒｕｍ）
に固着されることが望ましい。
【０００７】本発明による内部人工補綴体の２つの部分
構造は、ダイカスト工程で容易に作成される。更に、外
科的処置による人工補綴体部分の取り付けはこれらの２
部分構造のために容易化されている。例えば、骨盤のあ
る骨部分を切断した後、初めに基端部が固定され、次い
で末端部が基端部へ取り付けられ、それから、基端部が
脊椎にネジ止めされる。
【０００８】末端部と基端部との間の安全な接続の為
に、本発明は組み合わせ結合を提供している。本発明の
別の実施例によれば、双方の組み合わせ結合部分は、第
２部分の対応する窪みに関して係合する第１部分に形成
された、例えば凸部のような、相互係合面を含んでい
る。該凸部は両部分を結合するのに１個しかネジを使わ
ない場合でも両部分に関して起こる相対回転を阻止す
る。好ましくは一対のネジを使用し、その頭部を埋め込
むことが好ましい。
【０００９】更に、本発明の別の実施例によれば、人工
補綴体の末端部は一方が恥骨枝に、もう一方が坐骨枝に
それぞれ係合する、一対の支持部分を有するように形成
されており、それぞれの支持部分は取り付けブラケット
を含んでいる。こうして、これらの支持部分は、恥骨枝
若しくは残っている坐骨枝のそれぞれと、人工補綴体ヒ
ップソケットを受け入れるように窪みが形成されている
人工補綴体の末端の部分と、の間にブリッジ部分を画定
している。
【００１０】更に、本発明の別の実施例によれば、基端
部は一対の取り付けブラケットを有しており、そのブラ
ケットのＢＣＳ面を有する一方は、仙骨に側面係合し、
又、そのブラケットのもう一方は、神経の通路を残しつ
つ前部において脊椎に係合している。それぞれのブラケ
ットは、好ましくは関連する脊椎に固着される海綿質ネ
ジを受けるための数多くのネジ穴を有している。

【実施例】図１から図４に示された内部補綴体１０は、
基端部分１２（骨の心臓に近い方の部分）と、末端部分
１４（骨の心臓から離れた部分）と、を有している。両
部分１２及び１４は共にダイカスト成型され、身体に適
応可能な素材で作られている。型は、例えば、代用され
るべき骨の部分を調べるためにコンピューター断層写真
を使用し、こうして得られたそのデータにより型を成型
する公知の方法で作成される。
【００１２】基端部分１２は板状部分１６を含んでい
る。この板状部分１６は、図示していない骨クギを受け
入れるための４個の穴２２を含んでいる平坦な取り付け
ブラケット２０に曲げ部分１８を通って併合している。
該平坦な取り付けブラケット部分２０は板状部分１６の
面に関して僅かな角度を持って伸びている。取り付け部
分２０は曲げ部分２４を越えて第２のカーブした取り付
けブラケット部分２６まで伸びている。この取り付けブ
ラケット２６は骨クギ（図示せず）を受けるような４個
の穴２８を有する。板状部分１６は、２個の穴３２を備
えた伸長部分３０を有する。板状部分１６の自由端に
は、円形の凸部３４が設けてある。該円形の凸部３４に
噛み合う凹部３６が伸長部分３０の肩部に形成されてい
る。
【００１３】末端部分１４は板状部分４０を有してい
る。この板状部分４０は２個の穴４４を備えた伸長部分
４２を有している。これらの伸長部分３０、４２は互い
に補完しあう形状を有しており、板状部分４０の凸部４
６は伸長部分３０、４２が重なり関係に載置されるとき
に凹部３６へ係合するようになっている。該板状部分１
６と４０とを一緒に固定するため、ネジ４８が使用され
ている。ここでこのネジの頭は穴４４内に受け入れられ
ている。
【００１４】末端部分１４の中央部には、適当な例えば
プラスチック素材で作られた人工補綴体ヒップソケット
を受けるための窪み５０が設けられている。更に該窪み
０の両側に対抗する板状部分４０には、第１の支持部分
５２と第２の支持部分５４とが設けてある。これらの支
持部分５２、５４にはそれぞれ骨ネジを受け入れるため
の穴６０、６２を有している側方取り付けブラケット５
６、５８が設けてある。更に、これらの支持部分５２、
５４は、骨へ対する固着を容易にするための微小球形構
造を有している鈍角端と表面部分とを有している。数多
くの穴が開けられている。支持部分５４は穴を有してお
り、この穴の端部６４は窪み５０の中に開いている。図
２に示すように、支持部分５２は２個の穴を有してい
る。これらの穴は、骨盤の適当な骨の部分に末端部分を
取り付けるためのネジを受け入れている。
【００１５】図２によると、骨盤の骨７０は正面図で示
されており、恥骨及び坐骨は左側面が示されている。従
って、恥骨枝７２及び坐骨枝７４は左側である。外科的
切開が選択され、それにより支持部分５２、５４の鈍角
端が、枝部分７２、７４に鈍角状態で接触する。この取
り付けは、取り付けブラケット５８及び５６を介して行
われる。更に、これらの支持部分は窪み５０の中に開口
している穴を介して挿入されているネジによって固定さ
れている。
【００１６】外科手術において、初めに末端部分１４が
前述の方法で取り付けられ、それから末端部分１４が基
端部分１２にネジ４８を使用して固定される。こうして
取り付けブラケット２０は対応するように形成されねば
ならない関連した脊椎（第５腰部脊椎）に側面係合す
る。次いで、取り付けブラケットがネジによって該脊椎
に固定される。図２及び４に示したように、他の取り付
けブラケット２６が、脊椎の前部に係合し、かつネジに
よって同様に固定される。アーチ状をなす曲げ部分２４
は神経溝７６のために空間を残している。

    (57)【特許請求の範囲】
  1. 【請求項１】 人工補綴体ヒップソケットを受けるため
    の窪みを末端部分に有している、癌によって損傷したヒ
    ップボーン用内部人工補綴体であって、 内部人工補綴体が、ネジ係合部材（４８）によって互い
    に固定されている、独立した末端部分（１４）と、独立
    した基端部分（１２）と、を有し、 これらの末端部分と基端部分とが、それらの部分をヒッ
    プボーン（７０）若しくは、脊椎（７８）のそれぞれに
    ネジ止めするための取り付けブラケット（５６、５８、
    ２０、２６）を含んでおり、 前記基端部分（１２）が２個の取り付けブラケット（２
    ０、２６）を有しており、第１のブラケットが脊椎（７
    ８）に対して側方に係合し、他のブラケットが脊椎の前
    面に係合し、その中に神経通路（７６）が残されてい
    る、 ことを特徴とする、癌によって損傷したヒップボーン用
    内部人工補綴体。
  2. 【請求項２】 脊椎（７８）の側面に係合している取り
    付けブラケット（２０）が、平坦形状をなし、他方の取
    り付けブラケットがアーチ形状をなしていることを特徴
    とする、請求項１に記載の内部人工補綴体。
  3. 【請求項３】 末端部分（１４）の支持部分（５２、５
    ４）端部が鈍角の形状をなし、好ましくは、複数の球形
    面を有することを特徴とする、請求項１〜２のいずれか
    １項に記載の内部人工補綴体。
  4. 【請求項４】 支持部分（５２、５４）が各ネジを受け
    入れるため窪み（５０）の中に開口している、少なくと
    も１つの貫通開口を有していることを特徴とする、請求
    項１〜３のいずれか１項に記載の内部人工補綴体。
