【考案の詳細な説明】
【0001】
【産業上の利用分野】この考案は心臓外科手術,なかん
ずく冠状動脈バイパス手術において心臓を翻転する用具
に関するものである。
【0002】
【従来の技術】冠状動脈バイパス手術においては,左冠
状動脈回旋技および右冠状動脈後下行枝に移植血管を吻
合するために心臓の後面および下面をよく見ることがで
きるように,心臓を翻転することが必要である。従来一
般的には助手が手を用いて心臓を翻転していた。また用
具として細めのテープを網状に連結した心臓翻転具が考
案され,使用されている(例えば,米国特許3,98
3,863号)。また金属よってフォークのような形状
の外科手術鉤が作成され用いられている(例えば,臨床
医学雑誌「胸部外科」40巻39ページ,南江堂,昭和
62年,東京)。

【考案が解決しようとする課題】従来の技術で述べたも
ののうち,助手が用手的に心臓を翻転する方法では
(1)心臓が滑り落ちやすいので手術の途中で何度も翻
転し直す必要がある,(2)手指が心臓に食込んで心臓
を傷害する危険がある,(3)そのために助手を雇用す
る費用がかかる,等の欠点があり,何らかの方法で人手
をかけずに合理的かつ安全に心臓を翻転する用具が求め
られていた。
【0004】米国特許3,983,863号のような網
状テープを用いると確かに助手の手を必要とせず,心臓
を翻転することができるが,網状に連結されたテープが
心臓の表面を覆うために心臓表面が見にくくなり,目標
とする冠状動脈を発見することが困難となり,また吻合
手術操作を行い難くなるなどの欠点がある。
【0005】またフォークの形状の鉤を用いるにしても
助手を必要とし,また手術中に心臓が滑り落ちる欠点を
免れることができない。
【0006】以上に述べた諸問題を考慮して,この考案
の主目的は助手を必要とせず,安全確実に心臓を翻転
し,心臓の後面と下面の冠状動脈を容易に露出させるこ
とのできる心臓翻転具を提供することにある。
【0007】
【課題を解決するための手段】上記目的を目的を達成す
るための本考案の心臓翻転具は2本の長いテープと,そ
れらをほぼ中央部で連結する短いテープ,および該短い
テープの中央部に連結される長いテープの約2分の1の
長さのテープによって構成されるものである。
【0008】そして短いテープは心臓に装着された時に
心臓の基部を安全に支持し,かつ長いテープを支持する
ために伸縮性のない丈夫な織布から成る。
【0009】そして短いテープの中央部に連結した長い
テープの2分の1長のテープには心臓を翻転する時に心
臓の心尖部を支持固定するようにリングが設けられてい
る。
【0010】上記の4本のテープの相互の連結は縫い込
みによって行ってもよく,また例えば,熱凝固のような
方法によって行ってもよい。

【実施例】この考案の好ましい実施例を図面を参照して
説明する。図1に示すようにこの考案の心臓翻転具は長
さ約80センチメートルの2本のテープ10,20をそ
の中央部において約10センチメートルのテープ30で
並行に連結し,さらにテープ30の中央部に約40セン
チメートルの長さのテープ50をテープ10,20と並
行になるよう連結した構造となっている。テープ50の
中央部に近い部分にはリング55が設けられている。テ
ープ10,20,および50はいずれも幅は約2.5c
mで柔らかい伸縮性のない織布からなり,テープ30は
幅は約2.5cmで柔らかい伸縮性のない織布から成
る。
【0012】この考案の心臓翻転具を冠状動脈バイパス
手術に用いる時にはテープ10の一端11を肺動脈およ
び上行大動脈の下を左から右へ通し,テープ20の一端
21を下大静脈の下を左から右へ通し,テープ30が心
臓の後面の房室間溝部(心臓基部)に位置するようにす
る。そしてテープ10の一端11とテープ20の一端2
1は鉗子で固定しておく(図2)。
【0013】そしてテープ50のリング55内に心臓心
尖部を支持固定してテープ10の端12とテープ50の
端52を右へ牽引することにより心臓の後面の左冠状動
脈回旋枝領域を露出することができる(図3)。
【0014】また同様にテープ50のリング55内に心
臓心尖部を支持固定してテープ50の端52とテープ2
0の端22を右上方へ牽引することにより心臓の下面の
右冠状動脈後下行枝領域を露出することができる。牽引
したテープは鉗子で固定する(図4)。

【考案の効果】この考案の心臓翻転具を用いることによ
り心臓の後面および下面を容易に露出することができ
る。テープ10と50,およびテープ50と20の間隔
は必要に応じて広くとることができるので心臓表面を広
く露出できる。したがって手術を進める上で障害になら
ない。そして心臓は翻転された状態で安定した位置を保
持するので手術を速やかに進めることができる。さらに
助手を必要としない利点がある。

  1. 【請求項1】 織布よりなる2本の長いテープ10,2
    0をそのほぼ中央部において1本の短いテープ30によ
    り連結し,短いテープ30の中央部に長いテープ10,
    20の約2分の1の長さでほば中央部にリング55を設
    けたテープ50を連結することにより成る心臓翻転具。
  2. 【請求項2】 請求項1に記載の各テープは伸縮性のな
    い織布よりなるテープである心臓翻転具。
