Description
Embodiments of the present invention relate to analytical methods, kits, and detection devices.
近年、microRNA(miRNA)と疾患との関係が注目されている。 In recent years, the relationship between microRNA (miRNA) and disease has been attracting attention. miRNA has the function of regulating gene expression, and it has been reported that the type and expression level of miRNA changes from the early stages of various diseases. That is, in patients with a certain disease, the amount of a specific miRNA is increased or decreased compared to healthy individuals.
As shown in FIG. 1A, the analysis method includes, for example, preparing a sample derived from a subject (preparation step (S11));
and quantifying any one of the target miRNAs in a sample derived from a subject (quantification step (S12)).
First, a sample derived from a subject is prepared (preparation step (S11)). The sample can be collected using a general method according to the type of sample. The sample may be used as is after collection, or may be processed so as to be in a state that does not inhibit the reaction for quantifying nucleic acids or to be in a state that is more suitable for the reaction. Processing may be performed by any known means, such as chopping, homogenization, centrifugation, precipitation, extraction and/or separation.
For example, extraction may be performed using a commercially available nucleic acid extraction kit. Examples of nucleic acid extraction kits include NucleoSpin (registered trademark) miRNA Plasma (manufactured by Takara Bio), Quick-cfRNA Serum & Plasma Kit (manufactured by Zymo Research), miRNeasy Serum/Plasma Kit (manufactured by Qiagen), miRVana PARIS isolation kit (manufactured by Thermo Fisher), PureLinkTM Total RNA Blood Kit (manufactured by Thermo Fisher), Plasma/Serum RNA Purification Kit (manufactured by Norgen Biotech), microRNA Extractor (registered trademark) SP Kit (manufactured by Wako Pure Chemical Industries, Ltd.), and High Pure miRNA Isolation Kit (manufactured by Wako Pure Chemical Industries, Ltd.). A commercially available kit (Sigma-Aldrich) or the like can be used. Alternatively, without using a commercially available kit, extraction can be performed, for example, by diluting the sample with a buffer solution, heating it at 80 to 100°C, centrifuging it, and collecting the supernatant.
Next, any one of the target miRNAs in the sample is quantified (quantification step (S12)). The quantification step (S12) can be performed using a general method for quantifying RNA, particularly short-chain RNA such as miRNA. The method is not limited, but examples include a method in which the target miRNA is reverse transcribed to generate cDNA, the obtained cDNA is amplified, and the amplified product is detected and quantified. When the RNA is short-chain, it is generally practiced to extend the cDNA obtained by reverse transcription so as to add an artificial sequence to the end in order to facilitate amplification. In addition, rolling circle amplification is known as a technique for directly amplifying RNA in a sample and detecting and quantifying the amplified product without going through reverse transcription. For amplification, for example, PCR, qPCR, or LAMP can be used. Detection and quantification may be performed after amplification or over time during amplification. For detection and quantification, for example, a measurement method using a signal based on turbidity or absorbance, a measurement method using an optical signal, or a measurement method using an electrochemical signal, or a combination thereof, can be used. For example, the target miRNA can be quantified from the intensity or change in the signal obtained according to the amount of amplified product, or the time it takes for the signal to reach a threshold (rise time), or the number of cycles (rise cycle number) when using a PCR method. Furthermore, when the concentration of the target miRNA in the sample is relatively high or when a device capable of highly sensitive measurement can be used, it is also a common method to directly detect the target miRNA (or its cDNA) without amplifying the target miRNA. An example of a device capable of direct detection is a microarray equipped with a nucleic acid probe that specifically binds to the target miRNA.
Detection and quantification can also be performed using next-generation sequencing (NGS). In this case, the target miRNA can be quantified from detection results such as the number of reads aligned to the target miRNA.
The quantitative value of the target miRNA may be determined using a calibration curve that shows the relationship between the detection result of the above signal and the amount of the target miRNA present. The calibration curve can be created by preparing multiple standard samples (each sample has a different target miRNA concentration) containing the target miRNA at a known concentration and detecting various signals for these samples. By comparing this calibration curve with the detection result of the signal obtained for a sample derived from a subject, the amount of the target miRNA present in the sample can be calculated. The amount of the target miRNA present in the sample can be calculated, for example, as the number of copies of the target miRNA per unit amount of sample.
Quantification in the quantification step (S12) may be performed, for example, using a commercially available kit. Examples of commercially available kits include TaqMan® Advanced miRNA Assays (manufactured by Thermo Fisher, catalog No. A25576), miRCURY LNA® miRNA PCR Assays (manufactured by Qiagen, catalog No. 339306, SYBR® Green qPCR microRNA Detection System (manufactured by Origin Technologies), etc., and may be used together with primers that specifically amplify the target miRNA.
A more preferred example of the quantification step (S12) is described in the fourth or fifth embodiment.

For example, the analysis method of the first embodiment of the further embodiment may further include a determination step (S13) that may be performed after the quantification step (S12), as shown in FIG. 1(b). Examples of the highly expressed target miRNAs include miR-1307-5p, miR-3152-5p, miR-3189-3p, miR-3915, miR-4267, miR-4417, miR-4427, miR-4433b-5p, and miR-452. -5p, miR-4632-3p, miR-548y, miR-5694, miR-571, miR-6733-3p, miR-6782-3p, miR-887-5p, miR-3937, miR-444 9, miR-4674, miR-4707-3p, miR-564, miR-615-5p, miR-6510-5p, miR-7108-5p, miR-1225-5p, miR-1269b, miR-2467-3p, miR-297, miR-3175, miR-3177-3p, miR-3666, miR-4736, miR-6825-3p, and miR-6845-5p. When the quantitative value is higher than the control, the patient is judged to have breast cancer.
Hereinafter, when a quantitative value is described as "high or low," it means that it is high for highly expressed target miRNAs or low for low expressed target miRNAs.
For example, the determination can be made based on a previously obtained quantitative value or threshold value of the target miRNA in a control.
The control is, for example, a healthy individual. A healthy individual refers to an individual that is at least not afflicted with cancer. A healthy individual is preferably a healthy individual that has no disease or abnormality. The individual selected as the control may be an individual different from the subject analyzed by the present method, but is preferably an individual belonging to the same species, i.e., if the subject is human, it is preferably a human. In addition, the physical conditions, such as age, sex, height, weight, etc., or the number of individuals of the control are not particularly limited, but it is preferable that the physical conditions are the same or similar to those of the subject to be examined by the present analysis method.
The quantitative value of the target miRNA in the control is, for example, a quantitative value of the target miRNA obtained using the same or similar sample from the control and the same method as that used in the quantification step (S12). Here, the term "similar sample" refers to a sample that is substantially equivalent to the sample from the subject, although the collection method is different. For example, when the sample from the subject is a body fluid secreted from a certain organ, the similar sample from the control is, for example, a secretion secreted from tissue or cells collected and cultured from the corresponding organ. Alternatively, since the body fluid may contain tissue or cells detached from the organ, and the cytoplasmic matrix released by crushing the same cells, the similar sample from the control may be, for example, tissue (or cells) collected from the corresponding organ.
The threshold is, for example, the presence of the target miRNA that can distinguish between a quantitative value in a subject with breast cancer and a quantitative value in a control without breast cancer. The quantitative value of the target miRNA in a control without breast cancer is, for example, derived from a standard subject whose presence or absence of breast cancer is known, and is obtained by applying a method similar to the subject quantification step (S12) to a sample that is the same as or similar to the subject or a sample containing a breast cancer cell line. Such a quantitative value in a control without breast cancer can be determined, for example, as the threshold. However, the threshold is not limited and can be determined according to the quantification method used, the type of sample, the measurement conditions, etc.
Alternatively, the threshold may be determined for each subject. For example, if the target miRNA quantitative value of a healthy subject is monitored periodically during a health checkup or the like, the threshold value can be used to issue an alarm indicating the possibility of breast cancer when the quantitative value is higher or lower than the threshold value. The threshold value may vary from person to person. For example, in subject A, where the quantitative value of the target miRNA usually remains at about 10 3 copies, if it becomes 10 4 copies at some point, it can be determined that there is a possibility of breast cancer. On the other hand, in subject B, where the quantitative value remains at about 10 2 copies, if it becomes 10 3 copies at some point, it can be determined that there is a possibility of breast cancer.
The quantitative value or threshold value in the control may be determined from previous findings such as those in the literature.
Here, a determination that a person has the disease includes a high probability of having the disease. Conversely, a determination that a person does not have the disease includes a low probability of having the disease.
According to a further embodiment, determining that the subject is afflicted with breast cancer also includes determining the prognosis or recurrence of breast cancer in the subject. For example, as shown in FIG. 1(c), the analysis method includes a prognosis/recurrence determination step (S14) of determining the prognosis or recurrence of breast cancer in the subject from the results of the quantification after the quantification step (S12). In the prognosis/recurrence determination step (S14), for example, if the quantification value is high or low, it can be determined that the prognosis of breast cancer in the subject is poor, or that breast cancer has recurred, or that there is a high possibility of recurrence. The quantification value and threshold value in the control can also be used to determine the prognosis and recurrence. In particular, it may be preferable to use a threshold value determined for each subject.
In addition, after the determination step (S13) and/or the prognosis recurrence determination step (S14), it is also possible to select and assist in the selection of a type of therapy or a type of drug to be applied to the subject in accordance with the determination results. For example, as shown in FIG. 1(d), the analysis method includes a selection step (S15) of selecting a type of therapy or a type of drug to be applied to the subject from the determination results after the determination step (S13) and/or the prognosis recurrence determination step (S14). Here, the therapy or drug is for the treatment of breast cancer. The type of therapy or type of drug includes the amount, timing, or duration of use of the therapy or drug.
According to the analysis method of the first embodiment described above, by quantifying any one of the target miRNAs in a sample derived from a subject, it is possible to easily determine whether or not a subject has breast cancer. In other words, this method makes it possible to easily distinguish between subjects with breast cancer and subjects without breast cancer.
The method of this embodiment can detect breast cancer early because it is possible to use serum or plasma, which can be easily collected during health checkups, etc. By using serum or plasma, etc., the physical and economic burden on the subject can be greatly reduced compared to cytology, etc., and the procedure is simple, so the burden on the examiner is also small. In addition, since the miRNA concentration contained in serum or plasma is stable, more accurate testing can be performed.
According to a further embodiment, there is also provided an analytical method for assisting in determining whether or not a subject has cancer, which includes quantifying target miRNA in a sample derived from a subject (quantification step (S12)). "Assisting in determination" includes, for example, obtaining information regarding the possibility that the subject has breast cancer. "Information" is, for example, information regarding the analysis results of a sample, which may be, for example, a quantitative value. According to this method, it is possible to obtain more accurate information for determining whether or not a subject has cancer, determining the prognosis, determining the presence or absence of recurrence, or selecting a treatment or drug to be applied to the subject.
In a further embodiment, the present analytical method can also be used to detect breast cancer cells in samples not derived from a subject. For example, when artificially producing breast cancer cells, the method can be used to determine whether or not breast cancer cells are present in a cell-containing solution produced.
(marker)
According to the first embodiment, a marker for detecting breast cancer is provided, which comprises any one of target miRNAs.
Here, "marker" refers to a substance whose presence or concentration in a sample can be detected to determine whether the sample and/or the subject from which it is derived is in a particular state.
The breast cancer detection marker of the first embodiment can be used to determine the presence or absence of breast cancer in a subject, the prognosis or recurrence, or to select a treatment method or drug to be applied to the subject, for example, by measuring the amount (quantitative value) present in a sample derived from the subject, as described above.
(kit)
According to a first embodiment, a kit for detecting breast cancer is provided.
The kit includes reagents that can be used in a general method for quantifying RNA, particularly short-stranded RNA such as miRNA, and a nucleic acid capable of specifically binding to a target miRNA (i.e., hybridizing to the target miRNA). When the qPCR method is used to detect the target miRNA, the nucleic acid capable of specifically binding to the target miRNA may be a reverse transcription (RT) primer for reverse transcribing the target miRNA, an extension (EL) primer for extending the target miRNA, or an amplification primer set for amplifying the target miRNA.
The RT primer is a primer for obtaining cDNA of a target miRNA. The RT primer contains a sequence complementary to at least a portion of the sequence of the target miRNA. The RT primer may further contain an artificial sequence that is added to the cDNA to facilitate amplification of the cDNA of the target miRNA.
EL primers are primers for adding artificial sequences to cDNA to facilitate amplification of the cDNA of a target miRNA. EL primers may contain a sequence complementary to at least a portion of the sequence of the cDNA of the target miRNA and a sequence to be added for elongation of each cDNA.
For example, if the amplification primer set is for the PCR method, it includes at least a forward primer and a reverse primer. Alternatively, the amplification primer set may be for the LAMP method, and may include primers having a sequence corresponding to the base sequence of the target miRNA, which are used in general LAMP methods. Alternatively, the amplification primer set may be for, for example, the NGS method, and may include a forward primer including an artificial adapter sequence and a reverse primer including its complementary sequence. The amplification primer set for the NGS method may include multiple combinations of forward primers and reverse primers including different barcode sequences in order to analyze multiple samples simultaneously. When the amplification primer set is used for the rolling cycle amplification method, the kit further includes a circular single-stranded DNA to which the amplification primer hybridizes to serve as a template for amplification.
Each primer included in the amplification primer set may be designed to bind to the cDNA of the target miRNA or its complementary sequence, or may be designed to bind to an artificial sequence added by the RT primer and/or EL primer. The preferred sequences of the RT primer, EL primer, and amplification primer set will be described in the fourth embodiment.
Furthermore, when the target miRNA in the sample is directly detected by a microarray, the nucleic acid capable of specifically binding to the target miRNA is a nucleic acid probe provided in the microarray. The nucleic acid probe has at least a partial sequence of the target miRNA, its cDNA, or its amplification product, or a complementary sequence thereof.
The above nucleic acids included in the kit may be provided individually or in any combination in a container together with a suitable carrier. Suitable carriers include, for example, water, a physiological solution, or a buffer solution. The container may be, for example, a tube or a microtiter plate. Alternatively, these nucleic acids may be provided immobilized on a solid phase such as a microfluidic chip.
In addition to the nucleic acid, the kit may also include reagents for reverse transcription, extension and/or amplification, such as enzymes, substrates and/or labeling substances that generate optical or electrical chemical signals for detection. Labeling substances are, for example, fluorescent dyes such as SYBR Green, EvaGreen (registered trademark), and SYTO (registered trademark) 82, or indicators such as metal complexes such as ruthenium hexaamine in the case of current detection.
The kit can be used, for example, to determine whether a subject has breast cancer, to determine prognosis, to determine whether recurrence has occurred, to select a type of treatment or a type of drug, etc., as described above.
According to further embodiments, a kit for detecting breast cancer is provided as a diagnostic composition or diagnostic agent for breast cancer. Also provided according to embodiments is the use of at least one of the above nucleic acids in the manufacture of a diagnostic composition or diagnostic agent for breast cancer.
Second embodiment (analysis method)
As shown in FIG. 2A, the method of the second embodiment includes, for example, preparing a sample derived from a subject (preparation step (S21));
and quantifying multiple types of target miRNA in a sample derived from a subject (quantification step (S22)).
When quantifying multiple types of target miRNA, a separate reaction system may be prepared for each type of target miRNA, and amplification, detection, and quantification may be performed for each reaction system, for example, by PCR, qPCR, microarray, or LAMP. Alternatively, multiple types of target miRNA may be detected and quantified in the same reaction system using a flow channel chip or the like capable of simultaneously detecting multiple nucleic acids. For example, by using the NGS method, it is also possible to simultaneously amplify, detect, and quantitate multiple types of target miRNA in the same reaction system.
The quantitative value of each target miRNA can be determined using a calibration curve that shows the relationship between the detection result of the above signal and the abundance of each target miRNA, but it is preferable to create a calibration curve for each target miRNA. By comparing this calibration curve with the detection result of the signal obtained for a sample derived from a subject, the abundance of each target miRNA in the sample can be calculated. The abundance of each target miRNA in the sample can be calculated, for example, as the number of copies of each target miRNA per unit amount of sample.
The data obtained in the quantification step (S22), including the quantitative values of multiple target miRNAs, can be used to determine or assist in determining whether or not a subject has breast cancer. For example, the analysis method of the second embodiment may further include a determination step (S23) performed after the quantification step (S22), as shown in FIG. 2(b).
In the determination step (S23), a standard is set, for example, in the same manner as in the first embodiment. That is, the presence or absence of breast cancer in a subject can be determined by comparing the quantitative value of the subject-derived sample obtained in the quantification step (S22) with the standard. The standard for each target miRNA is preferably the quantitative value or threshold value of the corresponding target miRNA in a control. Each threshold value is preferably determined for each target miRNA.
When data on the quantitative values of multiple types of target miRNAs is obtained by the quantification step (S22), the subject may be determined to have breast cancer if the quantitative value of one of the target miRNAs is higher or lower than a standard such as a threshold value, or the subject may be determined to have breast cancer if the quantitative values of two or more target miRNAs are higher or lower than a standard. Also, the subject may be determined to have breast cancer if the quantitative values of all of the above target miRNAs, i.e., the target miRNA group, are higher or lower than a standard.
According to a further embodiment of the method, after the quantification step (S22), a prognosis recurrence determination step (S24) similar to that of the first embodiment is included. In addition, after the determination step (S23) and/or the prognosis recurrence determination step (S24), it is also possible to select and/or assist in the selection of a type of therapy or a type of drug to be applied to the subject according to the determination result. Furthermore, the method includes a selection step (S25) similar to that of the first embodiment after the determination step (S23) and/or the prognosis recurrence determination step (S24). Here, the therapy or drug is for the treatment of breast cancer. The type of therapy or drug includes the amount, timing, or duration of use of the therapy or drug.
The multiple types of target miRNAs are selected from any of SEQ ID NOs: 1 to 112. A preferred combination of multiple types of target miRNAs may vary depending on conditions such as the type of subject, the type of sample, and the type of detection and quantification method. By investigating combinations of target miRNAs with good detection and quantification sensitivity using a specific subject, sample, and detection and quantification method, it is possible to determine a preferred combination under specific conditions. It is also possible to combine a highly expressed target miRNA with a low expressed target miRNA.

Third embodiment (analysis method)
As shown in FIG. 3A, the analysis method of the third embodiment includes, for example, preparing a sample derived from a subject (preparation step (S31));
Quantifying any one of the target miRNAs described in the first embodiment (i.e., any one type of target miRNA) in a sample derived from a subject (quantification step (S32));
Determining whether the subject is a breast cancer patient, a cancer patient other than breast cancer, or a healthy subject (determination step (S33));
This is a method for determining whether or not breast cancer is present.
Cancers other than breast cancer are collectively referred to below as "other cancers." For example, other cancers include colorectal cancer, stomach cancer, lung cancer, ovarian cancer, pancreatic cancer, biliary tract cancer, esophageal cancer, liver cancer, brain tumors, bladder cancer, prostate cancer, sarcoma, uterine cancer, kidney cancer, and cervical cancer.
The quantification step (S32) of the third embodiment can be performed in the same manner as the quantification step (S12) of the first embodiment. The subject and sample can be the same as those of the first embodiment, but the subject may be an animal that may be afflicted with at least one of breast cancer and other cancers, or an animal that has previously been afflicted with at least one of breast cancer and other cancers. In other words, the subject may be an individual randomly selected from a population that includes healthy individuals, individuals afflicted with breast cancer, and individuals afflicted with other cancers.
In the determination step (S33) of the third embodiment, if the quantitative value of the target miRNA is high or low, the subject may be determined to be suffering from breast cancer and not from another cancer. The high or low quantitative value may be determined, for example, based on the quantitative value or threshold value of the target miRNA in a control previously obtained as in the first embodiment. In the reverse transcription step (S1-1), the reverse transcription solution is mixed with the sample and is preferably maintained at, for example, about 10°C to 55°C. As a result, the
The dissociation step (S1-2) can be carried out, for example, by heating the reaction solution after reverse transcription to 80°C to 100°C. By maintaining these conditions, the

The amplification step (S1-4) is carried out using the LAMP method.
In the amplification step (S1-4), a mixture of the solution containing the
The detection step (S1-5) preferably involves detecting a signal that changes over time during the amplification step (S1-4) in response to an increase in the amplified product, measuring the rise time of the signal, and quantifying the target miRNA as a result. "Over time" may mean either continuous or detection at multiple points in time at desired time intervals.
As the signal, it is preferable to use, for example, the turbidity of the reaction solution, or an optical signal or an electrochemical signal from the reaction solution. When an optical signal is used, it is preferable to carry out the amplification reaction in the presence of a labeling substance that generates an optical signal that changes in response to an increase in the amplified product. In this case, the labeling substance is, for example, an indicator such as a fluorescent reagent or an intercalator. When an electrochemical signal is used, it is preferable to carry out the amplification reaction in the presence of a labeling substance that generates an electrochemical signal that changes in response to an increase in the amplified product. In this case, the labeling substance is, for example, a redox probe.
Detection is preferably performed using, for example, a detection device. The detection device includes, for example, a chip. The chip is an amplification detection unit that amplifies the target miRNA and detects the amplification product, and includes, for example, a substrate and one or more detection regions provided on one surface of the substrate. For example, the reverse transcription step (S1-1) to the amplification step (S1-4) can be performed on the one surface, and if an amplification product is present, a signal can be detected in the detection region. In addition, the detection device may further include a calculation unit that calculates a quantitative value of the target miRNA from the detection result obtained in the amplification detection unit.
When an optical signal is used, the detection region is, for example, an optical sensor. When an electrochemical signal is used, the detection region is, for example, an electrode. The electrode is preferably, for example, gold, since it has good sensitivity.
A reverse transcription primer for reverse transcribing the target miRNA, an extension primer for extending the target miRNA, and/or an amplification primer set for amplifying the target miRNA may be releasably immobilized near the detection region.
A nucleic acid probe may also be used in the detection step (S1-5). The nucleic acid probe has a sequence that hybridizes with the amplified product, and the amplified product can be detected by detecting hybridization between the nucleic acid probe and the amplified product. For example, the nucleic acid probe may be fixed near the detection region of the detection device.
Next, the target miRNA in the sample is quantified based on the rise time of the signal obtained in the detection step (S1-5). For example, it can be determined that the earlier the rise time, the greater the amount of target miRNA present. For example, it is preferable to quantify the target miRNA in the sample using a calibration curve that shows the relationship between the rise time and the amount of target miRNA present.
According to the quantification process of the fourth embodiment described above, the target miRNA can be reverse transcribed, extended, amplified, and detected more specifically and efficiently. Therefore, by performing the quantification process (S12) of the first embodiment, the quantification process (S22) of the second embodiment, and the quantification process (S32) of the third embodiment in the quantification process of the fourth embodiment, it is possible to more accurately determine the presence or absence of breast cancer, or to assist in the determination, and to distinguish it from other cancers.
In a further embodiment, the overall activity level of miRNA may differ for each individual, and the yield of extraction and amplification reactions in sample processing may differ for each type of cell or sample. Therefore, it is preferable to correct the quantitative value obtained in the quantification step using the quantitative value of a miRNA (hereinafter referred to as "standard miRNA") that is commonly present in the subject-derived sample and the control-derived sample before using it in the determination step. Specifically, the quantitative value of hsa-miR-486-5p (SEQ ID NO: 320), which is known to be highly expressed in cancers other than breast cancer and healthy subjects, may be used for correction. In detail, the concentrations of both are calculated from the quantitative values of each target miRNA and hsa-miR-486-5p, and further, the concentration normalized by subtracting the concentration of hsa-miR-486-5p from the concentration of each target miRNA is corrected to be an index of expression level, and the expression level ratio is obtained between the groups of breast cancer, healthy subjects, and cancers other than breast cancer.
In the method according to the fourth embodiment, a kit for determining the presence or absence of breast cancer is provided. This kit preferably includes, for example, a nucleic acid capable of specifically binding to a target miRNA, and includes, for example, any of the above-mentioned
The kit according to the fourth embodiment may also include a primer set or a nucleic acid probe for quantifying the correction miRNA.
According to a further embodiment, in the extension step (S1-3), multiple types of target miRNAs with different base sequences in one sample are extended. In this case, primer sets for 1st to nth target miRNAs are used. The primer sets for 1st to nth target miRNA extension each include extension primers 11 to 1n and extension primers 21 to 2n for extending sequences 11 to 1n contained in the 1st to nth target miRNAs, respectively. Here, n is an integer of 2 or more.
In this case, for example, the one sample may be dispensed into n equal portions, extension primers 11 to 1n are added thereto to prepare reaction solutions 11 to 1n , a first extension reaction may be performed, and then extension primers 21 to 2n may be used in each reaction solution to perform a second extension reaction. Alternatively, a first extension reaction solution containing the sample and all of extension primers 11 to 1n may be prepared, and after the first extension reaction is performed, the reaction solution may be dispensed into n equal portions, extension primers 21 to 2n are added thereto to prepare reaction solutions 21 to 2n , and then a second extension reaction may be performed. Alternatively, the first extension reaction may be carried out in a first extension reaction solution containing the sample and extension primers 11 to 1n , and the second extension reaction may be carried out in the reaction solution using extension primers 21 to 2n without dispensing the reaction solution. As will be described in detail later, when the reaction solution is not dispensed in this manner, the n types of target miRNA can be detected in a differentiated manner by dispensing the reaction solution into n equal portions in the step of amplifying the extension product or by using an electrochemical detection device.
According to a further embodiment, in the detection step (S1-5), multiple types of target miRNAs having different base sequences contained in one type of sample are detected. In this case, first to nth target miRNA detection primer sets are used. The first to nth target miRNA detection primer sets each include a first to nth target miRNA extension primer set for extending the 1 1 to 1 n sequences contained in the first to nth target miRNAs, respectively, and a first to nth LAMP amplification primer set for amplifying the first to nth extension products obtained, respectively, using the first to nth target miRNA extension primer sets. Here, n is an integer of 2 or more.
For example, if n types of reaction solutions each containing n types of extension products are prepared in the extension step (S1-3), the first to nth amplification products are obtained using the first to nth LAMP amplification primer sets in each reaction solution, and the amount of each amplification product is detected, allowing the n types of target sequences to be distinguished and detected.
Alternatively, when a single reaction solution containing n types of extension products is prepared in the extension step (S1-3), the n types of extension products can be detected in a differentiated manner, for example, using an electrochemical detection device. The electrochemical detection device includes a chip having a substrate with at least n electrodes on one surface. On the one surface near each electrode, the first to nth LAMP amplification primer sets are releasably fixed. A labeling substance that generates an electrochemical signal is added to a single reaction solution containing the n types of extension products, the reaction solution is brought onto the one surface of the substrate, and the reaction solution is maintained under isothermal amplification conditions, whereby an amplification reaction of the corresponding extension product is carried out at each electrode using the first to nth LAMP amplification primer sets. Therefore, by measuring the rise time of the electrochemical signal at each electrode, the n types of target miRNA can be detected and quantified in a differentiated manner.
Fifth embodiment In the fifth embodiment, a preferred example of a step of quantifying a target miRNA other than that shown in the fourth embodiment will be described. In this example, the target miRNA is specifically reverse transcribed, and an artificial sequence is added to elongate the miRNA to obtain an extension product, which is then detected by analyzing the base sequence using the NGS method.
The next-generation sequencer used may be, for example, a sequencer such as the MiSeq (registered trademark) series or NextSeq (registered trademark) 550 (manufactured by Illumina), or a single molecule sequencer (manufactured by Pacific Biosciences, etc.), but is not limited to these.
By calculating the number of reads aligned to the target miRNA, it is possible to quantify and compare the amount of miRNA. In other words, it is possible to comprehensively quantify reads that have the sequence of the target miRNA and compare the number of reads. When comparing samples, the number of reads can be normalized using the TMM method or RPKM normalization method, etc., and then compared.
For the analysis, a method of simply comparing the number of reads or normalized values and performing a significance test, a transcriptome analysis platform such as DESeq or EdgeR, medical statistics software such as EZR (Easy R) or JMP (registered trademark), or a method of creating a discriminant equation using Fisher's discriminant analysis, nonlinear discriminant analysis using Mahalanobis distance, neural networks, random forests, or other machine learning can be used, but the methods are not limited to those mentioned above.
Prior to analysis with a next-generation sequencer, targeted sequencing analysis can also be performed to enrich only the desired miRNA sequences.
The NGS method is a base sequence analysis method that can analyze the base sequences in a sample that is a mixture of extracts from multiple subjects in a massively parallel manner, and can further identify which specimen the base sequence originates from in a single analysis. Therefore, according to the analysis method of the fifth embodiment, one or more types of target miRNAs in multiple subjects can be quantified and determined, making it possible to more simply and quickly analyze the presence or absence of breast cancer in a subject.

From the NGS analysis results of a total of 376 samples, 2558 types of miRNAs were identified.
4.0.2(R Development Core Team (2013).R:A language and environment for statistical computing.R Foundation for Statistical Computing,URL http://www.R-project.org/)を用いて実施した。
(Statistical Selection Method for miRNA Markers)
Calculations and statistical analyses using the read counts for each sample of the identified 2558 miRNAs were performed using R language 4.0.2 (R Development Core Team (2013). R: A language and environment for statistical computing. R Foundation for Statistical Computing, URL http://www.R-project.org/).
First, to select miRNAs whose expression levels differed significantly between breast cancer samples and other samples, the miRNAs were standardized using the DESeq2 pipeline (http://bioconductor.org/packages/release/bioc/html/DESeq2.html) and then compared between the two groups. The results of the comparison between the two groups are shown in Figure 6. There were a total of 319 types of miRNAs whose absolute expression ratio (Log2 Fold Change) was 0.5 or more (points located within the region (6a) in Figure 6 where the miRNA quantity ratio between the two groups was 0.5 or more, and points located within the region (6b) where the miRNA quantity ratio between the two groups was -0.5 or less).
2段階目として、まず、p値<0.01を満たすmiRNAを用いた乳がんの検出感度及び特異度を算出し、その感度及び特異度に基づいたROC(受信者動作特性試験、receiver operating characteristic)曲線を作成した。 For these 319 miRNAs, the following two-stage numerical criteria were set as a criterion for whether the change rate was statistically significant. In the first stage, miRNAs that satisfied a p-value of <0.01 were selected using padj, which corrects the increase in the probability of type I error associated with multiple comparisons performed repeatedly using the Wald test. The p-value for each miRNA was calculated using the DESeq2 pipeline. In the second stage, the detection sensitivity and specificity of breast cancer using miRNAs that satisfied a p-value of <0.01 were first calculated, and a ROC (receiver operating characteristic) curve was created based on the sensitivity and specificity. Next, the AUC (area under the curve) value of the ROC curve was calculated, and genes that satisfied an AUC>0.7 were selected as candidates for breast cancer detection markers.
感度、特異度及びAUC値は、EZRを使用し、Kanda.Y(Bone Marrow Transplantation 2013: 48, 452-458)に倣って算出した。 The sensitivity, specificity and AUC values were calculated using EZR following Kanda. Y (Bone Marrow Transplantation 2013: 48, 452-458). The threshold was set using two methods: the Youden Index, which selects the threshold that maximizes (sensitivity + specificity), and the minimum distance method, which selects the threshold that minimizes the distance from the upper left corner of the ROC curve (= {(1-sensitivity) 2 + (1-specificity) 2 }).
(Selection results of miRNA markers) The calculation results of the absolute expression ratio, p-value, AUC value, threshold, specificity, and sensitivity of each of the 319 types of miRNAs are shown in Tables 3-1 to 3-12. The miRNAs of SEQ ID NOs: 113 to 319 did not meet the selection criteria based on p-value and AUC value. On the other hand, the miRNAs of SEQ ID NOs: 1 to 112 met the selection criteria based on p-value and AUC value, and therefore were found to be capable of significantly detecting specimens affected by breast cancer. In addition, the sensitivity of each of the miRNAs of SEQ ID NOs: 1 to 112 exceeded the sensitivity of CEA, an existing tumor marker (=15.73%), demonstrating that the detection sensitivity of the miRNAs of SEQ ID NOs: 1 to 112 is excellent.
For example, when comparing the expression level measurements of hsp-miR-3609 (SEQ ID NO: 97) between breast cancer samples, healthy subjects, and 15 other cancer samples, the expression level measurements of the breast cancer patient group were found to be significantly (p = 1.21E-16) lower than the other groups (see Figure 7). In addition, according to the ROC curve (see Figure 8) created based on the expression level measurements of this hsp-miR-3609, the AUC value was 0.942. When the threshold was determined by the Youden Index, the threshold was 5.67, and the sensitivity and specificity were 100% and 81.8%, respectively. When the threshold was determined by the minimum distance method, the threshold was 5.67, and the sensitivity and specificity were 87.5% and 89.2%, respectively. These results indicate that hsp-miR-3609 is an excellent marker for distinguishing breast cancer patients from healthy individuals and patients with other cancers.
These results show that by quantifying at least one miRNA selected from the miRNAs of SEQ ID NOs: 1 to 112, breast cancer samples can be distinguished from a sample group containing healthy subjects and 15 types of other cancer samples with a sensitivity higher than that of CEA, an existing tumor marker.
Example 2. Comprehensive analysis of miRNA markers for distinguishing early-stage breast cancer from early-stage patients of other cancers and healthy subjects In this example, we analyzed whether the miRNAs of SEQ ID NOs: 1 to 112, which were shown in Example 1 to be excellent markers for distinguishing breast cancer patients from healthy subjects and patients with other cancers, can distinguish even early-stage breast cancer, clinical stage 0 or 1.
(Preparation of specimen)
Among the samples shown in Table 2, data from stage 0 or 1 breast cancer and other cancer samples, as well as data from healthy individuals, were used to verify whether it was possible to distinguish patients in the early stages of breast cancer from a population that included healthy individuals and patients in the early stages of cancers other than breast cancer, in a manner similar to that of Example 1.
The number of samples consisted of 11 serum samples from breast cancer patients, 30 serum samples from healthy subjects, and 82 serum samples from patients with other cancers. The breakdown of the serum samples from patients with other cancers was as follows: 2 colon cancer, 5 gastric cancer, 7 lung cancer, 2 ovarian cancer, 4 pancreatic cancer, 9 biliary tract cancer, 4 esophageal cancer, 10 bladder cancer, 7 sarcoma, 12 uterine cancer, 13 kidney cancer, and 7 cervical cancer (see Table 2).
(Statistical Selection Method for miRNA Markers)
The sensitivity and specificity when each of the miRNAs of SEQ ID NOs: 1 to 112 was used alone as a marker were calculated in the same manner as in Example 1, and the results are shown in Tables 4-1 to 4-5. As shown in Tables 4-1 to 4-5, the sensitivity exceeded the sensitivity (=15.73%) of the existing tumor marker CEA, which was based on
Therefore, it was shown that quantifying miRNAs selected from miRNAs of SEQ ID NOs: 1 to 112 can distinguish patients in the early stages of breast cancer from healthy individuals and patients in the early stages of other cancers with a sensitivity higher than that of the existing tumor marker CEA.
