The present invention comprises a composition comprising a monoacetyldiacylglycerol compound as an active ingredient for treating fatty liver disease, more particularly a monopoly for oral administration that prevents and / or alleviates the symptoms of fatty liver disease. The present invention relates to a composition containing an acetyldiacylglycerol compound.
The liver is one of the most important metabolic organs in the body and is responsible for important functions such as bile secretion, nutrient storage and detoxification. When the liver is in an abnormal state, metabolic disorders such as glucose metabolism, lipid metabolism, protein and nitrogen metabolism, amino acid metabolism, protein metabolism and hepatic brain disease, vitamin metabolism, and poor absorption may occur. In addition, liver disease may be exacerbated by infections, fatty liver, cirrhosis and the like. Known liver diseases to date include acute hepatitis caused by drugs, alcoholic fatty liver, obesity, diabetes, non-alcoholic fatty liver caused by hyperlipidemia, acute hepatitis caused by viral infection, chronic hepatitis, and cirrhosis. It is related to such things.
Among them, non-alcoholic fatty liver disease (NAFLD) is the most common chronic liver disease associated with obesity, diabetes, and hyperlipidemia, with or without alcohol, and is excessive in the liver. It is characterized by the accumulation of lipids (mainly triglycerides). Generally, in NAFLD, more than 5% by weight of total liver weight is lipid. NAFLD can develop into steatohepatitis, fibrosis, cirrhosis, and even hepatocellular carcinoma.
There are many studies on NAFLD, but the relationship with
Non-alcoholic steatohepatitis (NASH) is one of the non-alcoholic steatohepatitis (NAFLD). The root cause of NASH is unknown. However, when analyzing patients suffering from NASH, NASH is obese,
In addition, non-alcoholic fatty liver disease (NAFLD) increases the risk of developing
Lipoprotein lipase (LPL) acts as a lipase for the pancreas, liver, and endothelium and is predominantly distributed in skeletal muscle, heart, and adipose tissue. Lipoprotein lipase (LPL) is an ultra-low density lipoprotein that contains chylomicrons (CM) that bind to ApoE (APO protein) and distribute triglycerides to muscle and adipose tissue and hydrolyze triglycerides (triglycerides, TG). be. They play an important role in hydrolyzing triglycerides (TGs) and removing triglycerides (TGs) from peripheral tissues such as muscle and adipose tissue for energy or storage sources. Therefore, when lipoprotein lipase (LPL) is reduced (lost), triglyceride (TG) circulates excessively, causing hypertriglyceridemia.
Streptozotocin (STZ) is a selective pancreatic β-cell toxin that induces rapid and irreversible necrosis of cells and is often used to induce diabetes. STZ is also a good candidate for modeling fatty liver. STZ-induced diabetes is characterized by hyperglycemia, dyslipidemia, weight loss and fatty liver.
Therefore, it would be desirable to have new therapies for fatty liver disease.

About 10% of patients with total fatty liver disease are known as steatohepatitis. Non-alcoholic steatohepatitis (NASH) is caused by elevated triglycerides and decreased fat metabolism due to hyperinsulinemia. If non-alcoholic fatty liver and non-alcoholic steatohepatitis are not treated, liver function may worsen and cirrhosis may occur via liver fibrosis.
Fibrosis is a type of wound healing process in liver tissue. When liver tissue is damaged by hepatitis virus, non-alcoholic fatty liver, drugs, etc., hepatocytes are continuously damaged and the vascular structure is disrupted in the hepatic lobule as it is regenerated. The result is a function lost by the regenerative nodule, which can lead to a fibrotic reaction in the liver, leading to cirrhosis.
Damaged hepatocytes secrete free radicals and inflammatory substances, activating Kupffer cells and inflammatory cells, leading to activation of hepatic stellate cells. Hepatic stellate cells, which are normally stores of vitamin A, are activated when liver injury occurs and can synthesize and secrete various extracellular matrices such as collagen to cause liver fibrosis. .. With chronic liver injury, damaged hepatocytes can no longer be regenerated and are gradually replaced by extracellular matrix such as collagen, which can lead to liver fibrosis and cirrhosis.
That is, non-alcoholic fatty liver disease (NAFLD) is a continuous disease that progresses from simple non-alcoholic fatty liver to cirrhosis via fatty liver disease and liver fibrosis.

In the examples of the present disclosure, the use of monoacetyldiacylglycerol compounds increased by (1) recovery of body weight lost by streptozotocin (STZ) and (2) excessive accumulation of triglyceride (TG) in the liver. It has been shown that the weight of the liver was reduced (Example 1). Therefore, it can be seen that administration of the monoacetyldiacylglycerol compound improves fatty liver.
Triglycerides (TG, triglycerides) are absorbed in the small intestine as exogenous fat and contain the apolipoprotein of ApoB48. Triglyceride (TG) is recombined with chylomicrons (CM) along with ApoB48. High triglyceride CM is transported through the lymphatic system into the blood while circulating throughout the body, and triglyceride (TG) is transferred into peripheral tissues such as muscle, adipose tissue and monocytes for energy or storage. Be absorbed. The triglyceride (TG) contained in the CM is then hydrolyzed by lipoprotein lipase (LPL) so that it can be transported to the liver via the portal vein, resulting in smaller CM residual particles. Therefore, when lipoprotein lipase (LPL) is reduced, unused CM in peripheral tissues such as muscle, adipose tissue and immune cells is transported to the liver, resulting in excessive circulation of triglyceride (TG) to the liver. Bring. This causes hypertriglyceridemia and can lead to fatty liver disease.
ApoB48 is a lipoprotein that transports blood cholesterol and triglycerides. When ApoB48 is in excess, triglyceride (TG) is high, which means that it contributes to dyslipidemia. Therefore, uptake of triglyceride (TG) in the liver may promote fat accumulation in hepatocytes that cause fatty liver (see FIG. 4).
In another example of the disclosure, the liver of the experimental group was observed, hematoxylin and eosin (H & E) was used to image the fat of the follicle, and Oil Red O (ORO, Sigma Aldrich, St. Louis, Missouri, USA) was used. It was used to measure the degree of liver fat accumulation. (1) The concentration of triglyceride (TG) in hepatocytes and plasma was reduced (Examples 2 and 3), (2) the expression of lipoprotein lipase (LPL) was improved (Example 4), and (3) portal. It has been shown that the expression of apolipoproteins, including ApoB48, in the veins was reduced (Example 3). It was confirmed that the monoacetyldiacylglycerol compound achieves the functional improvement of peripheral tissues that produce and secrete lipoprotein lipase (LPL). These results also show that monoacetyldiacylglycerol compounds are effective in treating fatty liver.
In another example of the present disclosure, the use of monoacetyldiacylglycerol compounds reduces NAFLD activity score (NAS), fatty degeneration, lobular inflammation and hepatocellular balloon-like enlargement for non-alcoholic fatty liver disease (NAFLD) (NAFLD). It has been shown that Examples 10) and (2) the fibrotic portion of the liver tissue was reduced (Example 11).

The amount of monoacetyldiacylglycerol in the pharmaceutical composition is not particularly limited and can be widely varied. In order to confirm the efficacy of 1-palmitoyle-2-linole oil-3-acetylglycerol (EC-18 or PLAG) in the treatment of fatty liver disease, a streptozotocin (STZ) -induced fatty liver disease model was used in this experiment. used.
Experimental Example 1: Preparation of control group and experimental group to confirm the effectiveness of treatment for non-alcoholic fatty liver disease (NAFLD)
FIG. 1 shows the experimental protocol of the present invention. As shown in FIG. 1, mice were randomly divided into a control group and a streptozotocin (STZ) treated group. All STZ-treated groups received an intraperitoneal injection of STZ at a dose of 200 mg / kg body weight (BW) (1 day), which dose was dissolved in sodium citrate buffer (pH 4.5).
The day after the administration of streptozotocin (STZ), the induction of diabetes was confirmed by a glucometer (ACCU-CHEK, Roche diabetes Inc.), and acute diabetes was induced. All mice with fasting blood glucose levels above 200 mg / dL were considered to have acute diabetes. After confirming induction, mice in the experimental group were further randomized into three groups, the STZ monotherapy group and the low and high dose PLAG + STZ treatment groups. 1-Palmitoyle-2-linole oil-3-acetyl-rac-glycerol (PLAG) represented by the
Mice were administered PLAG by oral gavage for 3 days. The intended oral dosages were 250 and 50 mg / kg BW in mice, respectively. The same PBS was orally administered to the control group and the STZ monotherapy group for 3 days, and the mice were sacrificed on the 4th day.
Example 1: Analysis of body weight and relative liver weight
All mice in the control group, streptozotocin (STZ) alone, streptozotocin (STZ) + low dose PLAG treatment group, and streptozotocin (STZ) + high dose PLAG treatment group prepared in the previous experiment were weighed and recorded daily at the same time. The results are shown in A of FIG. In addition, the relative liver weights of the control group and the experimental group were calculated using the following equations, and the results are shown in B of FIG.
(Formula 1)
(Absolute liver weight) / (BW on the day of slaughter) x 100
As shown in A of FIG. 2, all STZ-treated mice showed a statistically significant reduction in BW compared to the control group. However, the high-dose PLAG group showed less reduction in BW compared to mice treated with STZ alone. There was no difference in BW between the STZ group and the low dose PLAG group. This suggests that oral PLAG administration attenuated the weight loss due to diabetes.
As shown in B of FIG. 2, the relative liver weight (per gBW) of the STZ alone group was significantly larger than that of the control group. However, the PLAG-treated group had a smaller average liver weight than the STZ group, indicating that PLAG supplementation improved fatty liver.
Example 2: Histopathological analysis
At the end of the 4-day experimental period, mice were fasted for 6 hours and then sacrificed. FIG. 3 is a photograph of liver sections stained with H & E and ORO. Specifically, in order to measure the degree of liver fat accumulation, the livers of the control group and the experimental group were observed and imaged using hematoxylin eosin (H & E) and Oil Red O. Liver samples were immediately fixed in 10% buffered formalin at room temperature, then the tissue was embedded in paraffin, sectioned and stained with hematoxylin and eosin (H & E). Frozen liver sections were fixed with 4% paraformaldehyde for 20 minutes and then stained with ORO for 15 minutes at room temperature. The sample was then counterstained with Meyer's hematoxylin for 10 seconds and loaded. Images were taken under an optical microscope (Olympus, Tokyo, Japan).
As shown in FIG. 3, as expected, H & E staining dramatically increases hepatic fatty degeneration in the STZ alone group, as evidenced by the increased number of fatty vacuoles (large drop fat degeneration and small drop fat degeneration). Evidence was shown. However, PLAG at a dosage of 250 mg / kg / day visibly reduced microvesicular fatty degeneration. To a lesser extent, the low-dose PLAG-treated mice also showed a decline.
ORO staining revealed a higher content of liver TG in the STZ alone group, which was reduced by PLAG. These results suggest that PLAG alleviates hepatic fatty degeneration in a dose-dependent manner.
Example 3: Analysis of TG levels in liver and plasma
To measure the concentration of triglyceride (TG) in the plasma of the control group and the experimental group, blood was collected by eye bleeding, and the plasma was separated by centrifugation at 3,000 rpm for 10 minutes. Plasma samples were left frozen until assay.
FIG. 4 is an example of the mechanism by which fat absorbed in the small intestine affects the liver, and FIG. 5 shows the level and content of TG in the liver and plasma when the composition according to the present invention is used. It is a chart (A in FIG. 5) and a chart showing apoB48 protein expression in portal vein plasma (B in FIG. 5). Specifically, in order to measure the triglyceride (TG) content of hepatocytes, liver tissue (20 mg) was homogenized in isopropanol using a tissue crusher. The homogenate was centrifuged at 5,000 rpm for 10 minutes and the supernatant was collected. The TG content of the supernatant was measured using a triglyceride H kit (Wako Diagnostics, Richmond, Virginia, USA). To confirm the uptake of hepatic TG, we measured ApoB48 expression in portal blood as a marker for the transport and uptake of high TGCM in the body.
As shown in FIG. 5, the ApoB48 level of STZ-treated mice was higher than that of the control group. However, PLAG treatment reduced ApoB48 levels in a dose-dependent manner, indicating that PLAG improved lipid metabolism by enhancing the uptake of TG into peripheral tissues or the whole body.
Example 4: Immunohistochemical analysis
FIG. 6 is a chart showing the TG content in muscle tissue (A in FIG. 6) and a chart showing the relative mRNA expression of LPL in muscle tissue (B in FIG. 6) when the composition according to the present invention is used. ), And a photograph (C in FIG. 6) showing a representative immunohistochemical image of LPL staining of muscle sections. Specifically, lipoprotein lipase (LPL), the rate-determining enzyme of TG clearance, controls the catabolism of high TG lipoproteins, including CM. The levels of LPL protein and mRNA in muscle tissue were examined to examine TG clearance by peripheral tissue after PLAG treatment.
As shown in FIG. 6, the muscle TG content was not significantly different between the groups (A in FIG. 6). The RT-PCR results showed that LPL mRNA expression was reduced in the STZ group compared to the control group, whereas it was increased in the high-dose PLAG group (FIG. 6B).
Immunohistochemical properties were abundantly expressed in the control group and decreased in STZ-treated animals. However, LPL protein expression in the PLAG-treated group was similar to the control. Overall, mRNA and protein findings suggest that PLAG induces TG clearance by restoring this function in peripheral tissues (Fig. 6C).
Example 5: Western blotting
検出は、Immobilon Western Chemiluminescent HRP Substrate(Millipore Corporation,米国マサチューセッツ州ビレリカ町)を用いて行った。
FIG. 7 shows a chart showing the weight of muscle specimens and skeletal muscles derived from mice (A in FIG. 7), and the results of relative mRNA expression of
As shown in FIG. 7, muscle atrophy is defined as loss of muscle mass and occurs in the context of STZ-induced diabetes. Similar to BW, muscle weight was also significantly reduced by STZ treatment. PLAG treatment slightly improved muscle mass, but there was no significant difference between the STZ alone group and the PLAG group (Fig. 7A).
It investigated the effect of PLAG on muscle function by measuring the differentiation of myoblasts into myotubes. This transformation induces changes in morphology and muscle-specific gene expression. We found that the mRNA levels of CAV3 and myogenin were reduced in STZ-treated C2C12 cells, but PLAG treatment restored the levels to normal (C in FIG. 5). This is consistent with in vivo results from muscular tissue, including the quadriceps and gastrocnemius muscles (FIG. 5B). These results indicate that STZ impairs normal myoblast differentiation and PLAG may help normalize muscle function.

FIG. 8 shows the structural chemical formulas of PLAG and PLG (A in FIG. 8), a chart showing weight changes in each group (B in FIG. 8), a photograph of a macroscopic liver specimen (C in FIG. 8), and H & E staining. It is a photograph of a liver section. Specifically, the selectivity of PLAG was investigated by comparing the effects of PLAG and PLG in an STZ-induced mouse model. PLG is a prototype of diacylglycerol, and PLAG is a kind of acetylated diacylglycerol. As shown in FIG. 8 below, PLAG-treated mice showed less reduction in BW than mice treated with STZ alone. There was no difference in BW between the STZ group and the PLG-treated mice (B in FIG. 8). In addition, the results showed that PLAG attenuated fatty liver. In contrast, liver injury was not reduced by PLG (C & D). These results suggest that PLAG plays a specific role in protecting the liver from STZ-induced fatty liver.

The OCA and EC-18 250 mg / kg groups showed a significant reduction in the inflamed area (F4 / 80 positive area) compared to the vehicle group. The inflamed areas in the EC-18 30 mg / kg group and the EC-18 100 mg / kg group tended to be lower than those in the vehicle group.
