以上、スネア20又はスネアループ25bの伸張方向を好適に屈曲させ、操作性に優れる把持鉗子10について説明した。本把持鉗子10のスネア20又はスネアループ部25bのシース12の先端から突出する長さは、5~50mmとすることができる。図4(a)にスネア20が拡張した状態を示す。スネア20は、ハンドライン30のシース12の連結部302の方向に屈曲しており、大きく拡張している。本把持鉗子10は、スネア20を縁枠とし、または、スネアループ部25bを縁枠とする袋状のネット27を装着することができる。これにより異物等の回収を容易に行うことができる。把持鉗子10のスネア12にネット27を装着した例を図4(b)に示す。ネット27は袋状になっており、崩壊しやすい塊状のものを好適に回収することができる。また、図1又は図3に示す形態の操作具であって、例えばスネアに高周波電流を供給する端子を有する操作具においては、その端子を電気手術器に接続することにより操作性に優れた医療用スネアを構成することができる。

また、本発明に係る把持鉗子10は、消化器官や胆管などに沿って長い棒状の組織や異物を好適に把持することができる。かかる把持鉗子を図4に示す。本把持鉗子10は、操作具11から延伸し、本体管状部12aから先端に小径管状部12bが突出し、本体管状部12aの先端部にスネア開口部12cを有する管状のシース12と、本体管状部12aの内腔を進退する操作ワイヤ15と、その操作ワイヤ15に連結片16を介して接続され、スネア開口部12cから小径管状部12bを跨がってシース12の外周を取囲むように突出するスネア20と、一端がスネア20の頂部に連結され本体管状部12aを貫通し操作具11の手元操作部116に保持されてなる糸状のハンドライン30を有する。この把持鉗子10においては、ガイドワイヤ28が使用されている、ガイドワイヤ28は、シース12の小径管状部12bから突出し、小径管状部12b及び本体管状部12aを貫通して手元操作部115に保持されている。手元操作部115においてガイドワイヤ28の進退操作をすることができる。また、本例のハンドライン30は、一端がシース12の本体管状部12aの先端部分のハンドライン開口部12dから突出してスネア20に連結され、他端が手元操作部116において進退操作することができるようになっている。

本把持鉗子10は、スネア20が小径管状部12bを跨がってシース12の外周を取囲むように突出しているのが特徴である。操作ワイヤ15を操作して図5(a)に示すようにスネア20を拡張して、棒状の組織や異物をスネア20のループ内に取り囲んだ後、図5(b)に示すように、スネア20を引き絞ると当該組織や異物を確実に把持することができる。小径管状部12bの長さは、5~50mmとすることができる。

かかる棒状物把持型の把持鉗子においても、上記スネア20及び操作ワイヤ15を使用しない形態のスネアループ部25bを有するスネアワイヤ25を使用することができる。すなわち、操作具11の手元操作部115からシースの本体管状部12aを貫通し、スネア開口部12cから小径管状部12bを跨がってシース20の外周を取囲むように突出してスネアループ部を形成し、折り返して本体管状部12aを貫通し、操作具11の手元操作部115に戻って手元操作部115に保持され、手元操作部115の操作によりスネアループ部のスネア開口部12cからの突出長さが調整可能なスネアワイヤ(図示せず)とすることができる。
