Abstract
歯牙及び人工歯根の口腔疾患の予防及び治療のための治療用製剤は、ポリビニルアルコール水溶液に、１種以上のポリ酸又はその塩、１種以上の殺菌剤及び／又は抗炎症剤を混合した混合物を含む。当該製剤は、塗布された部位において粘着性の保護バリアを形成することができ、上述の薬剤、殺菌剤、及び抗炎症剤の生物学的利用能を促進することができる。【選択図】なし
Description
本発明は、概して、口腔疾患、歯牙及び人工歯根（dental implant）の予防及び処置のための治療の分野に関する。より具体的には、本発明は、歯槽骨炎、抜歯後又は大きな歯科治療後の歯根肉芽腫、歯嚢胞又は歯牙感染、固定又は可動性の歯科補綴物の支台（dental prosthesis carrier）を介した感染、歯肉退縮、歯石又は歯垢の蓄積及び口臭の予防及び治療に関する。
本発明は、この種の発明が有用なあらゆる産業に適用可能であるが、特に、口腔病学の分野において有用である。
一般に知られているように、衛生不良によって口腔内に生じる歯牙関連の炎症は、時間の経過とともに症状の深刻度が進行するという点が共通しており、現在の技術で治療を行っても、最終的には、歯牙又は人工歯根の動揺及び喪失などの進行性の損傷が発生する場合が多い。
口腔病学の分野では、従来技術によって現在、多くの種類の予防及び治療用の調剤薬及びその使用が知られている。
米国特許出願公開第２００９／０１４８５８６号には、コラーゲンと、キトサンと、フィブリノゲンと、アルギン酸塩と、塩化カルシウムと、歯髄幹細胞と、少なくとも２種類の抗生物質の組み合わせと、を含む注射液が記載されている。
また、米国特許出願公開第２００９／０１４８４８６号には、メトロニダゾール安息香酸塩及びクロルヘキシジングルコン酸塩を、水性ゲルとして局所投与する歯科疾患治療が記載されている。
さらに、米国特許第６，３６５，１３１号には、安息香酸塩、クロルヘキシジングルコン酸塩、及び局所麻酔剤を含む、歯科疾患用の調剤薬が記載されている。
しかしながら、先行技術の調剤薬の多くは、コンシステンシー（粘稠度：consistency）が液状に近すぎるという欠点を有する。このため、患者の口腔内に薬を塗布しても、患者がこれを飲み込んでしまうので、塗布後、数時間で効果が完全に失われてしまう。
したがって、本発明は、先行技術におけるこのような欠点及びその他の欠点を克服すべく、抜歯後の部位、処置後の部位、歯根清掃、スケーリング（scaling）及びルートプレイニング（root planing）、並びに口腔衛生全般において、局所投与に適した革新的な調剤薬を提供する。
より具体的には、本発明は、軟質ゴムのコンシステンシーまで固化するとともに、その弾性を医療行為者が必要に応じて調整可能な革新的なゲルを提供することを目的とする。
本発明は、さらに、上述のゲルを、口腔病学において一般的な添加剤と有効成分を組み合わせて適切な水溶液中にて調剤することを目的とする。
本発明は、概ねゴム状のコンシステンシーまで固化する性質を備えるとともに、構成材料の種類及び濃度に応じて可塑性を十分に調節可能な革新的なゲルを提供することを主な目的とする。
本発明に係る調剤薬は、製剤とも記載されるが、他の重要な特徴として、前記ゲルは、無菌状態、及び細菌感染及び／又は食物片の侵入に対する保護を必要とする患者の口腔内に、或いは歯牙又は人工歯根の周辺領域に塗布されると、その場に、粘着性の保護バリアを形成するものであり、このバリアは、前記調剤薬に含まれる医療用薬剤、殺菌剤、及び抗炎症剤の生物学的利用能（bioavailability）を革新的に促進する機能を有する。
本発明の実施形態における他の重要な特徴として、前記革新的なゲルが所定の部位でいったん固化すると、当該製剤は、粘膜接着性（mucus adhesiveness）、弾性及び障壁効果、静菌作用、細菌侵入を防止する効果、抗炎症作用、及び殺菌作用などのすべての性質を有する状態になる。これにより、従来技術における類似の製剤のいずれにも勝る優れた有効性を実現することができる。
前記革新的な製剤は、ゲル状であり、ポリビニルアルコール、及び／又は少なくとも１種のポリ酸又はその塩、及び／又は少なくとも１種の殺菌剤、及び／又は少なくとも１種の抗炎症剤、及び／又は少なくとも１種の組織再生成分から成る混合物を水溶液に含む。
他の特徴として、前記革新的な調剤薬は、医療行為者が前記構成成分を混合することで調剤されるので、必要とされる様々な治療に適合するように、当該構成成分の濃度及び種類に基づいて調剤することができる。
本発明の好適な一実施形態において、ポリビニルアルコールは、１０から１，０００，０００の間、好ましくは、１０，０００超の分子量を有するとともに、０．０００１重量％から３０重量％の間、好ましくは４重量％未満の濃度を有する。
本発明の別の好適な実施形態において、ポリ酸及びその塩は、ポリアクリル酸及びその一価の塩からなる群から選択されるとともに、１０から５，０００，０００の間の分子量、及び０．０００１重量％から３０重量％の間の濃度を有する。
本発明のさらに別の実施形態において、前記ポリ酸及びその塩は、アルギン酸及びアルギン酸一価塩からなる群から選択されるとともに、０．０００１重量％から３０重量％の間の濃度を有する。
本発明のさらに別の実施形態では、前記ポリ酸塩は、１．５重量％の溶液中における温度２０℃時の粘度が４０センチポアズ超であるとともに、０．０００１重量％から５重量％の間の濃度を有する高分子アルギン酸ナトリウムである。
本発明のさらに別の好適な実施形態において、前記殺菌剤は、クロルヘキシジングルコン酸塩又はジグルコン酸塩（di-gluconate）、クロルヘキシジン二塩酸塩（chlorhexidine dihydrochloride）、ヨウ素、銀イオン、及びそれらの任意の組み合わせからなる群から選択されるとともに、好ましくは、０．０００５重量％から２５重量％の間、より好ましくは、０．００２重量％から３重量％の間の濃度を有する。
本発明の別の好適な実施形態において、前記抗炎症剤は、アセチルサリチル酸、フルルビプロフェン、及びそれらの任意の組み合わせからなる群から選択されるとともに、好ましくは、０．００１重量％から３０重量％の間、より好ましくは、０．５重量％から１０重量％の間の濃度を有する。
本発明の別の好適な実施形態において、前記組織再生成分（tissue regeneration factor）は、ヒアルロン酸であり、好ましくは、０．００１重量％から３０重量％の間、より好ましくは、０．００１重量％から２重量％の間の濃度を有する。
本発明の特に好ましい実施形態において、前記調剤薬は、理想的には、分子量が約９０，０００で濃度が約２重量％のポリビニルアルコール水溶液に、分子量が４００，０００で濃度が０．５％ｐ／ｐのポリアクリル酸、濃度が約１．５重量％のアルギン酸ナトリウム、濃度が約２重量％のクロルヘキシジングルコン酸塩、及び濃度が約５重量％のフルルビプロフェンを混合した混合物である。
既に説明したように、前記調剤薬は、好ましくは、医療行為者が適切に調合することで得られるものであり、構成成分の種類及び濃度を必要に応じて調整可能であるが、前記調剤薬は、ゲル化用溶液と架橋溶液（cross-linking solution）という二液型の形態で得られるという利点も有する。なお、この場合は、調剤手順を正しく行うために、パラオキシ安息香酸プロピル及び／又はパラオキシ安息香酸メチルなど、何種類かのパラベンが必要である。
既に説明したように、本発明に係る調剤薬は、水性ゲル化用溶液の形態であるので、炎症、歯肉退縮、歯周ポケット又は人工歯根周囲の瘻孔の患者の歯間空隙に塗布すれば、当該歯間空隙を充填し、その形状を整えることができる。加えて、歯肉辺縁の近傍に塗布して自然に架橋させれば、ゴム状に変化させることができる。
本発明に係る調剤薬が、その主要な目的を達成することは、本調剤薬が以下の３つの機能を有するという利点からも明らかであろう。本発明に係る調剤薬は、先ず第１に、静菌性の保護バリアとして機能する。この機能に、塗布部位で生体接着作用を発揮するという第２の機能が組み合わされて、感染及び／又は炎症の発生源となることが多い食物残渣及び細菌の侵入を防止することができる。これに加えて、殺菌剤による殺菌作用があり、当該殺菌剤が時間の経過とともにゴム状ゲルから放出されるので、治療部位及びその周辺における微生物負荷（microbial load）を抑制することができる。
また、ポリビニルアルコールが含有されていることにより、当該薬剤で形成される塊やフィルムは、弾性又はゴム状の固体性（引張及び圧縮に強い耐性を備える）と粘着性とを有するので、治療部位を遮蔽する機能、並びに食物及び細菌の侵入に対する保護機能を実現できる。加えて、ポリビニルアルコール及びポリアクリル酸の静菌性により、投与部位への細菌の拡散を防止できる。また、ポリビニルアルコールの抗炎症性は、自然の生理的状態（natural physiological conditions）の回復に寄与するので、好ましくない衛生状況によりダメージを受けた組織の再生を促すことができる。
加えて、本発明の製剤は、例えば、歯科補綴物用の人工歯根の場合などの術後の組織治癒、歯瘻、及び歯肉退縮など、口腔内の任意の部位において食べ物及び細菌負荷の侵入を遮蔽し、当該部位を保護する作用を実現することができる。
本発明の製剤は、歯根清掃並びにスケーリング及びルートプレイニングを行う際など、歯牙及び口腔の衛生において、歯と歯の間、及び／又は歯と歯茎の間に食べ物や細菌が侵入することを防止し、保護することを必要とする場合に推奨される。
本発明の製剤は、投薬部位における局所濃度が、数日後でも高いまま一定に維持されるという利点を有するので、殺菌剤が細菌負荷を効果的に抑制し、抗炎症剤が投薬部位に作用する。この点に関し、本発明の製剤の有効性は、殺菌剤及び／又は抗炎症剤のみ（１００％）を使用した場合に比べても高いことが、臨床試験によって明らかにされている。
研究及びテストによれば、ポリビニルアルコールは長鎖を有するので、分子量が高ければ、ポリ酸及びその塩の分子間を結合するよう機能する。よって、ゲルを固化及び弾性化して、破断伸びが１００％超となるような高い機械的抵抗及び弾性的な柔軟性を実現する機能は、長い重合鎖の分子量が高いことによって得られるものであり、分子量は、１０，０００以上であることが好ましい。この場合、ポリビニルアルコールの濃度は４％を超えないことが望ましい。このような濃度になると、ゲルが自然に固化するため、経時的な安定性を必要とする用途に本製剤を商用利用することの妨げとなるからである。
高分子量のポリビニルアルコールを使用することには特異性があり、これを使用することで、本発明は、歯の組織に対する生体接着性を実現できるので、殺菌剤及び抗炎症剤の組織への伝達を促進する性質を備えるゲルを得ることができる。さらに、生体接着性を備えることにより、塗布部位を遮蔽して、細菌及び食物残渣の侵入から保護するゲルを得ることができる。
ここまでは、本発明の調剤薬を予防／治療の目的で使用する場合について検討してきたが、本発明は、審美治療（cosmetic treatment）に用いても効果を発揮する。この場合、本製剤は、過酸化物、過酸化水素、及び過酸化カルバミドをベースとする歯面漂白剤をさらに含有するものでもよい。
実施例
以下に、本発明の製剤の調剤、その特徴、及び使用に関する実施例を提示するが、これらは、あくまでも例示を目的とするものであり、添付の請求項によって規定される本発明の権利範囲を何ら限定するものではない。

実施例１：調剤
ポリビニルアルコールを、分子量が９０,０００、濃度が２重量％になるように水に溶解させて、ゲル化用溶液を得た。この水溶液を攪拌して均一溶液とした後に、分子量が４００，０００で濃度が０．５重量％のポリアクリル酸、及び濃度１．５重量％のアルギン酸ナトリウムを追加した。また、混合物全体に対して、濃度２重量％のクロルヘキシジングルコン酸塩と、濃度１０重量％のアセチルサリチル酸を最後に追加した。
ポリビニルアルコールを、分子量が９０,０００、濃度が２重量％になるように水に溶解させて、ゲル化用溶液を得た。この水溶液を攪拌して均一溶液とした後に、分子量が４００，０００で濃度が０．５重量％のポリアクリル酸、及び濃度１．５重量％のアルギン酸ナトリウムを追加した。また、混合物全体に対して、濃度２重量％のクロルヘキシジングルコン酸塩と、濃度１０重量％のアセチルサリチル酸を最後に追加した。
実施例２：投与
実施例１にしたがって調剤された、クロルヘキシジングルコン酸塩を含有するゲル化用溶液を、針先が斜めにカットされた注射器を使って、歯間空隙に注入（又は人工歯根の周辺に投与）した。前記ゲルは、その部位で局所的に固化して、歯間又は人工歯根の周辺に食物残渣が入り込むことを防止する。このような治療は、歯根清掃、歯根の研磨、歯周ポケットの掻爬、歯周ポケットの殺菌、及び／又はスケーリング及びルートプレイニングの後に行うことが推奨され、これにより、同じ症状が頻繁に再発することを予防できる。

ゲル化用溶液の投与に際しては、小型ブラシを使ってゲルの薄膜を（歯と歯茎の間の）歯肉辺縁の高さに塗布することで、遮蔽膜及び殺菌シールを形成することができる。これにより、歯茎及び歯根の健康状態の回復及び維持を促進することができる。
実施例３：プラスチックフィルムの特徴
実施例２に説明した手順で得られるゴム状フィルムは、弾性を備え、また、含有するクロルヘキシジングルコン酸塩を放出する機能を備えることを特徴とする。ゲルのサンプルを用いて測定したところ、１００％を超える破断時の弾性伸びを示す結果が得られた。また、クロルヘキシジングルコン酸塩の放出の持続期間は、７日間超であった。
実施例２に説明した手順で得られるゴム状フィルムは、弾性を備え、また、含有するクロルヘキシジングルコン酸塩を放出する機能を備えることを特徴とする。ゲルのサンプルを用いて測定したところ、１００％を超える破断時の弾性伸びを示す結果が得られた。また、クロルヘキシジングルコン酸塩の放出の持続期間は、７日間超であった。
実施例４：臨床前研究
実施例１の製剤について行われた臨床前研究（preliminary clinical studies）によって、スケーリング及びルートプレイニング、歯根清掃、歯根の研磨、歯周ポケットの掻爬、歯周ポケットの殺菌を行った後に、最適な口内環境が従来よりも長く維持されることが明らかにされた。したがって、同じ治療を短期間のうちに繰り返し行う必要がなくなる。加えて、前記製剤は、歯瘻、嚢胞、或いは、抜歯又は主要な歯科治療の後の歯牙感染、固定又は可動性の歯科補綴物の支台を介した感染の治療又は予防に有用であることが明らかにされた。

実施例５：代替的な調剤
ポリビニルアルコールを、分子量が９０，０００、濃度が２重量％になるように水に溶解させて、ゲル化用溶液を得た。この水溶液を攪拌して均一溶液とした後に、濃度０．２重量％のヒアルロン酸、濃度５重量％のフルルビプロフェン、及び比粘度が４，０００ｃＰで濃度が２重量％のアルギン酸ナトリウムを追加した。また、混合物全体に対して、濃度２重量％のクロルヘキシジングルコン酸塩と、濃度３重量％のクロルヘキシジン二塩酸塩を最後に追加した。
ポリビニルアルコールを、分子量が９０，０００、濃度が２重量％になるように水に溶解させて、ゲル化用溶液を得た。この水溶液を攪拌して均一溶液とした後に、濃度０．２重量％のヒアルロン酸、濃度５重量％のフルルビプロフェン、及び比粘度が４，０００ｃＰで濃度が２重量％のアルギン酸ナトリウムを追加した。また、混合物全体に対して、濃度２重量％のクロルヘキシジングルコン酸塩と、濃度３重量％のクロルヘキシジン二塩酸塩を最後に追加した。
実施例２にしたがって本製剤を投与すれば、歯肉退縮及び歯周ポケットの形成を予防でき、それらの術後の治療にも有効な解決策であることが証明された。
本発明を、いくつかの好ましい実施形態の例示的な実施例を用いて説明してきたが、当業者であれば、下記の請求項に規定される権利範囲を逸脱することなく、様々な変形及び／又は変更が可能であることは理解されよう。
Claims (17)
- ポリビニルアルコール水溶液に、１種以上のポリ酸又はその塩、１種以上の殺菌剤及び／又は抗炎症剤を混合した混合物を含むことを特徴とする、口腔又は歯牙の衛生、審美、治療用製剤。
- ポリビニルアルコールは、分子量が１０から１，０００，０００の間、好ましくは、１０，０００超であり、濃度が０．０００１重量％から３０重量％の間、好ましくは４重量％未満である、請求項１に記載の製剤。
- 前記ポリ酸及びその塩は、ポリアクリル酸及びその一価の塩からなる群から選択されるとともに、１０から５，０００，０００の間の分子量、及び０．０００１重量％から３０重量％の間の濃度を有する、請求項１又は２に記載の製剤。
- 前記ポリ酸及びその塩は、アルギン酸及びそのアルギン酸一価塩からなる群から選択されるとともに、０．０００１重量％から３０重量％の間の濃度を有する、請求項１〜３のいずれか１つに記載の製剤。
- 前記ポリ酸塩は、１．５重量％の溶液中における温度２０℃時の粘度が４０センチポアズ超であるとともに、濃度が０．０００１重量％から５重量％の間である高分子アルギン酸ナトリウムである、請求項１〜４のいずれか１つに記載の製剤。
- 前記殺菌剤は、クロルヘキシジングルコン酸塩、クロルヘキシジン二塩酸塩、ヨウ素、銀イオン、及びそれらの任意の組み合わせからなる群から選択される、請求項１〜５のいずれか１つに記載の製剤。
- 前記抗炎症剤は、アセチルサリチル酸、フルルビプロフェン、及びそれらの任意の組み合わせからなる群から選択される抗炎症剤である、請求項１〜６のいずれか１つに記載の製剤。
- パーフォレーション、食物片の侵入、及び引張に対する耐性を有するゴム状弾性固体に変化する粘性流体状のゲル化用溶液の形態である、請求項１〜７のいずれか１つに記載の製剤。
- 前記ゲル化用溶液を、無菌状態の維持と細菌感染又は食物片の侵入に対する保護とを必要とする患者の口腔内、或いは歯牙又は人工歯根の周辺領域に塗布して、その場に、前記薬剤、殺菌剤、及び抗炎症剤の生物学的利用能を促進する粘着性の保護バリアを形成する、請求項８に記載の使用。
- 前記ゲル化用溶液は、組織再生刺激成分を含有する、請求項８又は９に記載の製剤。
- 前記組織再生刺激成分は、ヒアルロン酸である、請求項１０に記載の製剤。
- 前記ゲル化用溶液は、審美治療に使用される、請求項８〜１１のいずれか１つに記載の製剤。
- 過酸化物、過酸化水素水、過酸化カルバミド、及びそれらの任意の組み合わせをベースとする歯面漂白剤の形態である、請求項１２に記載の製剤。
- 前記ゲル化用溶液は、当該製剤の経時的な安定性を維持するための防腐剤を含有する、請求項８〜１３のいずれか１つに記載の製剤。
- 前記防腐剤は、パラオキシ安息香酸プロピル、パラオキシ安息香酸メチル、及びその任意の組み合わせと同じく、パラベン類に属する、請求項１４に記載の製剤。
- 医療行為者が前記構成成分のすべて又は一部を混合することで調剤されることを特徴とする、請求項１〜１５のいずれか１つに記載の製剤。
- 前記構成成分を二液型の形態、即ち、ゲル化用溶液と架橋溶液の形態で用いることにより調剤されることを特徴とする、請求項１〜１６のいずれか１つに記載の製剤。
