Abstract
Description
本発明はバイオマーカーに関し、特に、がんなどの様々な状態を診断するための新規な生物学的マーカーに関するが、これに限定されるものではない。特に、本発明は、膵臓がんおよび/または大腸がんなどのがんを検出するためのアッセイにおける診断マーカーおよび予後マーカーとしてのこれらの化合物の使用、ならびに対応する検出方法に関する。本発明はまた、これらの疾患を治療剤で治療する効力を決定する方法、ならびにこのアッセイおよび方法を実施するための装置に関する。アッセイは定性的および/または定量的であり、大規模スクリーニングおよび臨床試験に適応可能である。

を含み、ここで、前記装置は、被験者由来の身体サンプル中のC2またはC3アルデヒド、またはその類似体もしくは誘導体から選択された特徴的化合物の濃度の増加を特定するために使用され、それにより、被験者が大腸がんに罹患しているか、またはその素因を有するか、または被験者の状態の否定的な予後を提供することを示唆する。

(ii)参照と比較して、被験者由来の身体サンプルにおいて、C2もしくはC3アルデヒド、またはその類似体もしくは誘導体から選択される特徴的な化合物の濃度の増加、を同定する。これにより、治療薬または特殊食による治療計画が無効であることを示唆する。
(i)被験者由来のサンプル中の特徴的な化合物の濃度を決定すること。ここで、参照と比較して、C2またはC3アルデヒド、またはその類似体もしくは誘導体から選択される特徴的な化合物の濃度の増加は、被験者が大腸がんに罹患しているか、またはその素因を有するか、または被験者の状態の否定的な予後を提供することを示唆し、および、
(ii)治療薬を被験者に投与することもしくは投与したこと、または被験者に特殊食を与えることにより、前記治療剤または特殊食が大腸がんの進行を予防、低減または遅延させること。
本発明者らは、膵臓がんおよび大腸がんを含む様々ながんを検出するためのVOCの使用を調査した。
膵臓がんは、治癒的治療が不可能な進行期に診断される患者がほとんどであるため、予後は非常に不良である。呼気揮発性有機化合物は、他のがん型を検出するための新規なバイオマーカーとしての可能性を示している。本研究では、膵臓がんの存在と関連する独特の呼気揮発性有機化合物プロファイルを同定し、検証したことから、膵臓がん診断経路に組み入れるための呼気分析の可能性が示唆された。
2件の研究が実施された。最初のプロファイリング研究では、呼気を収集して分析し、がん患者と対照患者の間で濃度の異なるVOCを同定した。これらの化合物を用いて、膵臓がんの診断モデルを開発した。次に、別の独立に収集した患者コホートを用いて、このモデルの妥当性を検証した。
登録患者はすべて、2016年3月から2016年12月まで、インペリアルカレッジNHSトラストから採用された。地域倫理承認が付与された(REC ref: 14/LO/1136)。研究の詳細は適格患者全員に説明し、登録前に十分なインフォームドコンセントと文書による同意を取得した。人口統計学的および臨床的情報を収集した。
全ての症例を標準参照試験で確認した。膵臓がんは腹部CT(Computerized Tomography)または超音波内視鏡検査で確認し、組織学的には穿刺吸引生検で確認した。腹部CTまたは超音波は、対照群内の患者の膵臓を検査した。
呼気収集は、VOC測定に対する呼気操作および病院環境の影響についての本発明者らの調査によって通知された、以前に検証された方法論[11]を使用して実施した[17、18]。全ての患者は口腔汚染または交絡因子として作用する食事摂取のリスクを最小限にするために、呼吸サンプリング前に最低4時間絶食させた。サンプル収集室および実験室からの大気もまた、収集された呼気サンプルに対するバックグラウンドVOCの影響を調査するために分析された。呼吸サンプリングの方法は、最初のプロファイリング研究では不活性アルミニウムバッグ(Bedfont Scientific Ltd.、英国メードストン)から検証研究ではReCIVA呼吸サンプルシステム(Owlstone Medical Inc.、英国ケンブリッジ)に変更した。
全ての空気サンプルを収集の48時間以内に分析した。分解研究からのデータは、揮発性物質が48時間、呼気バッグ内で安定なままであることを示した[19]。TD−GC−MSは、揮発性および半揮発性化合物の同定および定量に使用される分析方法である。装置に入ったVOCはクロマトグラフィーカラム(7890B GC、 Agilent technologies、チーダル、英国)を通って移動し、固定相とのそれらの親和性に従って分離され、特定の保持時間でカラムを出る。次いで、VOCは質量分析計(5977A MSD、 Agilent technologies、英国)に入り、そこでイオン化され、加速され、偏向され、それらの質量/電荷(m/z)比に基づいて検出される。ガスクロマトグラフィーと質量分析の両方の組み合わせは、いずれかの成分を個別に使用するよりも改善された化合物同定を可能にする。
最初のオーブン温度を40℃で4分間保持し、次に1分間保持で100℃に5℃/分で上昇させ、1分間保持で110℃に5℃/分で上昇させ、200℃に5℃/分で上昇させ、最後に4分間保持で240℃に10℃/分で上昇させた。総GC分析時間は47分であった。質量分析計は、電子衝突イオン化モード、走査質量イオン20〜250m/z、5.9走査/秒で操作した。質量分析計四重極および線源の温度はそれぞれ230℃および150℃であった。水からの干渉を最小限にするために、実験の開始時に3分間の溶媒遅延を使用した。
クロマトグラムおよび質量スペクトルデータを定性分析ソフトウェア(Agilent Masshunter Qualitative Analysis、英国)で抽出した。次いで、3〜47分の間の保持時間を有するすべてのピークの化学的同一性を、NISTデータベースで確認した。同定されたVOCからの保持時間および特徴的なm/zイオンを、全てのクロマトグラムにわたるそれらの存在量(Agilent Masshunter Quantitative Analysis、英国)の定量に使用した。定量化には±0.1分の保持時間範囲を使用し、0.2分の範囲からの特徴的なイオンのみを定量化することを確実にした。
すべての統計学的分析は、IBM SPSS 24(IBM corp.,Armonk,NY)を用いて行った。0.05未満のP数値を有意とみなし、すべての統計学的検定は両側性であった。がん疾患の状態および交絡因子は独立変数と考えられ、VOC量は従属変数と考えられた。Shapiro−Wilk統計検定を行った。
患者
計68例(表1参照)をモデル開発コホートに募集した。限局性腺がん(n=7)、限局性神経内分泌腫瘍(NET)(n=4)、転移性腺がん(n=10)、転移性NET(n=4)、陽性対照(n=20)、正常膵臓(n=23)を含むがん(n=25)群と非がん(n=43)群に患者を割り付けた。
クロマトグラムの定性分析により、NISTデータベースから同定可能な66のVOCが得られた。これらのVOCのうちの22は、バックグラウンド空気中で高濃度であることが判明したか、または内因的に産生される可能性が低いと考えられたかのいずれかであるため、以降の分析から除外した。残りの44のVOCの識別、ならびにそれらの保持時間および特徴的なm/z比を、その後、VOC存在量の定量化に使用した。
ROCプロット(図2および3参照)は、開発コホートのがん患者の呼吸に有意な変化が認められたVOCのみを用いて、両コホートについて構築した。
呼気中のVOCのガスクロマトグラフィー質量分析定量は、膵臓がんの存在によって有意に変化した合計13の化合物を同定した。有意なVOCは、3つの主要な化学基、すなわちアルデヒド、脂肪酸およびアルコールからのものであった。すべてのROCモデルは、0.7を超えるAUCとの良好な識別を示した。また、腺がんと非がんを区別するモデルでは識別がより強かった。これらの結果は、膵臓がんの診断ツールとしての呼気VOC試験の可能性をさらに確立することができる、より大きな多施設試験の基礎を提供する。
呼気揮発性物質は、膵臓がん患者と非がん患者を区別する明確な潜在能力を有する。

例2−大腸がん
大腸がん(CRCa)が最も早期に診断された場合、CRCa患者の10人に9人以上が5年以上生存するのに対し、最後の病期で診断された場合は10人に1人未満である[1]。しかし、CRCaの主要診断根拠としての腸症状の利用は、陽性的中率が非常に不良であることが示されている[2]。本研究では、大腸がんの存在と関連する独特の呼気揮発性有機化合物プロファイルを同定し、検証したことから、大腸がん診断経路に組み入れるための呼気分析の可能性が示唆された。
サンプルサイズ:
研究(i)−150例におけるプロファイリング診断調査(大腸がん50例;炎症性腸疾患、ポリープおよび憩室疾患を含む陽性対照50例および正常下部消化管(LGI)内視鏡検査を受けた陰性対照50例)。
倫理承認:NHS保健研究所(NRES Committee London−Camden and Islington)2014年7月16日に取得された承認(REC参照14/LO/1136)。治験登録番号:UKCRN18063。
(i)試験−プロファイリング診断試験
150例(大腸がん50例、下部消化管の他の病態を有する患者(陽性対照)50例、下部消化管が正常な患者(陰性対照)50例)。陽性対照患者のうち、15例(30%)に炎症性腸疾患が認められ、21例(42%)に下部消化管内のポリープが認められ、14例(28%)に憩室疾患が認められた。憩室疾患および炎症性腸疾患を有する全患者は、呼気サンプリング時に活動性憩室炎または大腸炎を有さなかった。
総計79名の患者を被験者とし、CRCa患者25名、対照患者54名を被験者とした。対照患者54例のうち、31例(57.4%)は内視鏡検査で正常な下部消化管を有し、12例(22%)はポリープ、7例(13%)は憩室疾患、4例(7%)は炎症性腸疾患であった。患者の医学的共存症、投薬の使用および主症状を比較分析したところ、70歳以上の患者の割合が増加し、大腸がん群で貧血を呈する割合が増加したことを除いて、群間に有意差は認められなかった。
計40例を被験者に、再発の証拠のない術後患者19例と再発が証明された患者21例について検討した。再発例のうち5例は孤立性肝再発であり、16例は腹膜再発のみ、あるいは肝・腹膜再発を同時に認めた。患者の医学的共存症の比較分析、および投薬の使用は、群間で有意差を明らかにしなかった。
本研究の結果は、プロパナールがCRCaの診断のための単一呼気バイオマーカーとなる可能性を有することを示唆する。プロパナールは、CRCaを有する良好な診断精度の患者を、他の大腸疾患を有する患者および内視鏡的に正常な下部消化管を有する被験者と区別するようであった。これらの所見は、独立した診断研究および大腸がん再発患者において検証されている。多変量解析で示されたように、プロパナール濃度の上昇は患者因子に影響されなかった。プロパナールは食道胃がんのVOC呼気モデルの成分ではなく[9]、上部および下部消化管のがんは将来の臨床診療においてがん特異的分析を可能にする可能性のある、明確な非重複呼吸プロフィールを有することを示唆している。
本研究は、1回の呼気バイオマーカー(プロパナール)とCRCaの原発性存在および再発との関連を示唆する。これらの知見を検証するためには、更なる多施設共同検証試験が必要である。

Claims (26)
- 膵臓がんに罹患している被験者、またはその素因を診断するための、または被験者の状態の予後を提供するための方法であって、前記方法は、被験者由来の身体サンプルにおける特徴的な化合物の濃度を分析することと、この濃度を膵臓がんに罹患していない個体における特徴的な化合物の濃度についての参照と比較することと、を含み、ここで、参照と比較して、(i)被験者由来の身体サンプルにおいて、第1の炭素原子が=O基で置換され、第2の炭素原子が−OH基で置換されているC1−C3アルデヒド、C1−C3アルコールおよびC2−C10アルカン、またはそれらの類似体もしくは誘導体から選択される特徴的な化合物の濃度の増大、または(ii)被験者由来の身体サンプルにおいて、C1−C20アルカン、C4−C10アルコール、C1−C6カルボン酸、およびC4−C20アルデヒド、またはそれらの類似体もしくは誘導体から選択される特徴的な化合物の濃度の減少は、被験者が膵臓がんに罹患しているか、または膵臓がんに対する素因を有するか、または被験者の状態の否定的な予後を提供していることを示唆する。
- 膵臓がんに罹患している被験者を治療薬または特殊食で治療する効力を決定するための方法であって、前記方法は、被験者由来の身体サンプルにおける特徴的な化合物の濃度を分析することと、この濃度を膵臓がんに罹患していない個体における特徴的な化合物の濃度についての参照と比較することと、を含み、ここで、(i)参照と比較して、被験者由来の身体サンプルにおいて、第1の炭素原子が=O基で置換され、第2の炭素原子が−OH基で置換されているC1−C3アルデヒド、C1−C3アルコールおよびC2−C10アルカン、またはそれらの類似体もしくは誘導体から選択される特徴的な化合物の濃度の減少、または(ii)参照と比較して、被験者由来の身体サンプルにおいて、C1−C20アルカン、C4−C10アルコール、C1−C6カルボン酸、およびC4−C20アルデヒド、またはそれらの類似体もしくは誘導体から選択される特徴的な化合物の濃度の増大は、前記治療薬または特殊食による治療計画が有効であることを示唆し、または、ここで、(i)参照と比較して、被験者由来の身体サンプルにおいて、第1の炭素原子が=O基で置換され、第2の炭素原子が−OH基で置換されているC1−C3アルデヒド、C1−C3アルコールおよびC2−C10アルカン、またはそれらの類似体もしくは誘導体から選択される特徴的な化合物の濃度の増加、または(ii)参照と比較して、被験者由来の身体サンプルにおいて、C1−C20アルカン、C4−C10アルコール、C1−C6カルボン酸、およびC4−C20アルデヒド、またはそれらの類似体もしくは誘導体から選択される特徴的な化合物の濃度の減少は、前記治療薬または特殊食による治療計画が無効であることを示唆する。
- 前記特徴的な化合物がC1−C3アルデヒドである場合、前記化合物はC1アルデヒドである、請求項2または3に記載の方法。
- 前記特徴的な化合物がC1−C3アルコールである場合、前記化合物はC1アルコールまたはC3アルコールである、請求項1〜3のいずれか1項に記載の方法。
- 前記特徴的な化合物が第1の炭素原子が=O基で置換され、第2の炭素原子が−OH基で置換されているC2−C10アルカンである場合、前記=O基で置換された炭素原子は末端炭素原子ではない、請求項1〜4のいずれか1項に記載の方法。
- 前記特徴的な化合物が第1の炭素原子が=O基で置換され、第2の炭素原子が−OH基で置換されているC2−C10アルカンである場合、前記化合物は、第1の炭素原子が=O基で置換され、第2の炭素原子が−OH基で置換されているC3−C6アルカンまたはC4アルカンである、請求項1〜5のいずれか1項に記載の方法。
- 前記特徴的な化合物がC1−C20アルカンである場合、前記化合物はC3−C15アルカンまたはC5−C14アルコールである、請求項1〜6のいずれか1項に記載の方法。
- 前記特徴的な化合物がC1−C20アルカンである場合、前記化合物はC5アルコール、C6アルコールまたはC14アルコールである、請求項1〜7のいずれか1項に記載の方法。
- 前記特徴的な化合物がC4−C10アルコールである場合、前記化合物はC4−C7アルコールまたはC14アルコールである、請求項1〜8のいずれか1項に記載の方法。
- 前記特徴的な化合物がC1−C6カルボン酸である場合、前記化合物はC2−C4カルボン酸またはC3カルボン酸である、請求項1〜9のいずれか1項に記載の方法。
- 前記特徴的な化合物がC4−C20アルデヒドである場合、前記化合物はC5−C15アルデヒドまたはC7−C13アルデヒドである、請求項1〜10のいずれか1項に記載の方法。
- 前記特徴的な化合物がC4−C20アルデヒドであるとき、前記化合物はC8アルデヒド、C9アルデヒド、C10アルデヒド、またはC11アルデヒドである、請求項1〜11のいずれか1項に記載の方法。
- 膵臓がんに罹患している被験者、またはその素因を診断するための、または被験者の状態の予後を提供するための装置であって、前記装置は、
(i)被験者由来のサンプル中の特徴的な化合物の濃度を決定するための手段と、
(ii)膵臓がんに罹患していない個体由来のサンプル中の特徴的な化合物の濃度の参照と、を含み、前記装置は、参照と比較して、(i)被験者由来の身体サンプルにおいて、第1の炭素原子が=O基で置換され、第2の炭素原子が−OH基で置換されているC1−C3アルデヒド、C1−C3アルコールおよびC2−C10アルカン、またはそれらの類似体もしくは誘導体から選択される特徴的な化合物の濃度の増大、または(ii)被験者由来の身体サンプルにおいて、C1−C20アルカン、C4−C10アルコール、C1−C6カルボン酸、およびC4−C20アルデヒド、またはそれらの類似体もしくは誘導体から選択される特徴的な化合物の濃度の減少、を同定するために使用され、これにより、被験者が膵臓がんに罹患しているか、またはその素因を有するか、または被験者の状態の否定的な予後を提供することを示唆する。 - 治療薬または特殊食を用いて膵臓がんに罹患している被験者を治療する効力を決定するための装置であって、前記装置は、
(a)被験者由来のサンプル中の特徴的な化合物の濃度を決定するための手段と、
(b)膵臓がんに罹患していない個体由来のサンプル中の特徴的な化合物の濃度のための参照と、
を含み、
ここで、前記装置は、以下を同定するために使用される:
(i)参照と比較して、被験者由来の身体サンプルにおいて、第1の炭素原子が=O基で置換され、第2の炭素原子が−OH基で置換されているC1−C3アルデヒド、C1−C3アルコールおよびC2−C10アルカン、またはそれらの類似体もしくは誘導体から選択される特徴的な化合物の濃度の減少、または参照と比較して、被験者由来の身体サンプルにおいて、C1−C20アルカン、C4−C10アルコール、C1−C6カルボン酸、およびC4−C20アルデヒド、またはそれらの類似体もしくは誘導体から選択される特徴的な化合物の濃度の増大、を同定し、これにより、治療薬または特殊食による治療計画が有効であることを示唆すること、または、
(ii)参照と比較して、被験者由来の身体サンプルにおいて、第1の炭素原子が=O基で置換され、第2の炭素原子が−OH基で置換されているC1−C3アルデヒド、C1−C3アルコールおよびC2−C10アルカン、またはそれらの類似体もしくは誘導体から選択される特徴的な化合物の濃度の増加、または参照と比較して、被験者由来の身体サンプルにおいて、C1−C20アルカン、C4−C10アルコール、C1−C6カルボン酸、およびC4−C20アルデヒド、またはそれらの類似体もしくは誘導体から選択される特徴的な化合物の濃度の減少、を同定し、これにより、治療薬または特殊食による治療計画が無効であることを示唆する。 - 前記特徴的な化合物が、請求項3〜12のいずれか1項に定義されるものである、請求項13または14に記載の装置。
- 膵臓がんに罹患している被験者、またはその素因を診断するための、または被験者の状態の予後を提供するための、第1の炭素原子が=O基で置換され、第2の炭素原子が−OH基で置換されているC1−C3アルデヒド、C1−C3アルコールおよびC2−C10アルカン、ならびにC1−C20アルカン、C4−C10アルコール、C1−C6カルボン酸、およびC4−C20アルデヒド、またはそれらの類似体もしくは誘導体からなる群より選択される特徴的な化合物の、バイオマーカーとしての使用。
- 前記特徴的な化合物が、請求項3〜12のいずれか1項に定義されるものである、請求項16に記載の使用。
- 大腸がんに罹患している被験者、またはその素因を診断するための、または被験者の状態の予後を提供するための方法であって、前記方法は、被験者由来の身体サンプル中の特徴的な化合物の濃度を分析することと、この濃度を大腸がんに罹患していない個体中の特徴的な化合物の濃度についての参照と比較することと、を含み、ここで、参照と比較して、被験者由来の身体サンプル中のC2もしくはC3アルデヒド、またはその類似体もしくは誘導体から選択される特徴的な化合物の濃度の増大は、被験者が大腸がんに罹患しているか、またはそれに対する素因を有するか、または被験者の状態の否定的な予後を提供することを示唆する。
- 大腸がんに罹患している被験者を治療薬または特殊食で治療する効力を決定するための方法であって、前記方法は、被験者からの身体サンプル中の特徴的な化合物の濃度を分析することと、この濃度を大腸がんに罹患していない個体中の特徴的な化合物の濃度についての参照と比較することと、を含み、ここで、(i)参照と比較して、被験者からの身体サンプル中のC2もしくはC3アルデヒド、またはその類似体もしくは誘導体から選択される特徴的な化合物の濃度の減少は、治療薬または特殊食による治療計画が効果的であることを示唆し、または(ii)参照と比較して、被験者からの身体サンプル中のC2もしくはC3アルデヒド、またはその類似体もしくは誘導体から選択される特徴的な化合物の濃度の増加は、治療薬または特殊食による治療計画が効果的でないことを示唆する。
- 前記特徴的な化合物がC2アルデヒドである、請求項18または19に記載の方法。
- 前記特徴的な化合物がC3アルデヒドである、請求項18または19に記載の方法。
- 大腸がんに罹患している被験者、またはその素因を診断するための、または被験者の状態の予後を提供するための装置であって、前記装置は、
(i)被験者由来のサンプル中の特徴的な化合物の濃度を決定するための手段と、
(ii)大腸がんに罹患していない個体由来のサンプル中の特徴的な化合物の濃度の参照と、
を含み、ここで、前記装置は、被験者由来の身体サンプル中のC2またはC3アルデヒド、またはその類似体もしくは誘導体から選択された特徴的化合物の濃度の増加を特定するために使用され、それにより、被験者が大腸がんに罹患しているか、またはその素因を有するか、または被験者の状態の否定的な予後を提供することを示唆する。 - 大腸がんに罹患している被験者を治療薬または特殊食で治療する効力を決定するための装置であって、前記装置は、
(a)被験者由来のサンプル中の特徴的な化合物の濃度を決定するための手段と、
(b)大腸がんに罹患していない個体由来のサンプル中の特徴的な化合物の濃度のための参照と、
を含み、前記装置は、以下を同定するために使用される:
(i)参照と比較して、被験者由来の身体サンプルにおいて、C2もしくはC3アルデヒド、またはその類似体もしくは誘導体から選択される特徴的な化合物の濃度の減少、を同定し、これにより、治療薬または特殊食による治療計画が有効であることを示唆すること、または、
(ii)参照と比較して、被験者由来の身体サンプルにおいて、C2もしくはC3アルデヒド、またはその類似体もしくは誘導体から選択される特徴的な化合物の濃度の増加、を同定し、これにより、治療薬または特殊食による治療計画が無効であることを示唆する。 - 前記特徴的な化合物が、請求項20または請求項21に定義されるものである、請求項22または請求項23に記載の装置。
- 大腸がんに罹患している被験者、またはその素因を診断するための、または被験者の状態の予後を提供するための、C2もしくはC3アルデヒド、またはその類似体もしくは誘導体から選択される特徴的な化合物の、バイオマーカーとしての使用。
- 前記特徴的な化合物が、請求項20または請求項21に定義されるものである、請求項25に記載の使用。
