Abstract
Description
a)血管の2次元画像データに対応する画像データを取得するステップと、
b)画像データ内の血管の進路及びその進路に沿った血管の幅を決定するステップと、
c)画像データ内の血管の幅に基づいて、血管の狭窄評価用の機能的指数を決定するステップとを含む。
−血管の少なくとも一部の形状モデルを決定する。形状モデルは、血管の進路に沿った内径及び内径の変化を含む。
−形状モデルに基づいて、血管の少なくとも一部の少なくとも1つの抵抗値を決定する。
−少なくとも1つの抵抗値に基づいて、血管又は血管の一部を流れる流体の圧力降下を決定する。
−血管の少なくとも一部の形状モデルを決定する。形状モデルは、血管又は血管の一部の内径と血管の進路に沿った内径の変化とを含む。これは、例えば血管6の一部を複数の区画を有するように分割し、各区画について直径又は平均直径を決定することによって行われる。区画数が多いほど、狭窄評価用の機能的指標はより正確となる。
−形状モデルに基づいて、血管の少なくとも一部の少なくとも1つの抵抗値を決定する。図2を参照すると、左から右へ流れる模擬流体は、例えば狭窄のために、W2がW1よりも小さい場合に、増加する流体力学的抵抗を経験する。具体的には、血管又は血管セグメントは、少なくとも1つの線形又は非線形抵抗器、好適には一連のそのような抵抗器によって近似される。
−少なくとも1つの抵抗値に基づいて、血管又は血管の一部を流れる流体の圧力降下を決定する。例えば圧力降下は、1つ以上の決定された抵抗値及び幾つかの境界条件を含む集中素子流体力学モデルを使用して決定される。例えば血管又は血管の一部の入口及び/又は出口における(ボリュメトリック)流量及び/又は圧力に関して仮定を行い、当該仮定を、集中素子モデルにおいて境界条件として使用する。或いは、大動脈圧測定値といった物理的測定値を境界条件として使用してもよい。
−例えば画像に関連付けられるDICOMデータから決定される血管造影X線画像の解像度。
−投影された血管樹のサイズ(例えばコントラスト強調された血管が占める画像領域)。投影血管樹が大きいほど、より高品質の画像を表す。
−例えば背景領域、即ち、血管樹以外の画像の一部におけるノイズ測定によって決定される投影内のノイズ量。ノイズ量は少ないほど、より高品質の画像を表す。
−例えばエントロピー分析又は勾配測定によって決定される投影の鮮明さ。
−例えばシーケンスにおける隣接画像を用いた対応分析によって決定される冠動脈の動き。動きが激しいと輪郭がぼやけ、これにより、低品質の画像がもたらされる。
Claims (15)
- 血管の狭窄評価用の機能的指標を決定する装置であって、
入力インターフェースと、
処理ユニットと、
を含み、
前記入力インターフェースは、血管の2次元表現を表す画像データを取得し、
前記処理ユニットは、
血管の少なくとも一部の形状モデルであって、前記血管の進路に沿った内径及び前記内径の変化を含む当該形状モデルを決定し、
前記形状モデルに基づいて、前記血管の前記少なくとも一部の少なくとも1つの抵抗値を決定し、
前記少なくとも1つの抵抗値に基づいて、前記血管又は前記血管の一部を流れる流体の圧力降下を決定する、
装置。 - 前記2次元画像は、前記血管の投影である、請求項1に記載の装置。
- 前記処理ユニットは、前記血管の幅を決定する際に、前記画像データにおける遠近効果を補償するように、前記画像データに濃度測定法を適用する、請求項1又は2に記載の装置。
- 前記処理ユニットは、前記画像データ内の前記血管の幅にスケーリング係数を適用し、前記スケーリング係数を乗じた前記幅に基づいて、狭窄評価用の前記機能的指標を決定する、請求項1乃至3の何れか一項に記載の装置。
- 前記処理ユニットは、前記画像データの画像面からの前記血管の距離を受信し、前記距離に基づいて、前記スケーリング係数を決定する、請求項4に記載の装置。
- 前記処理ユニットは、複数の血管セグメントがあるように、前記血管を前記血管の進路に沿ってセグメント化し、前記複数の血管セグメントそれぞれの幅に、特定のスケーリング係数を適用する、請求項4に記載の装置。
- 前記処理ユニットは、前記血管の1つのセグメントが、前記投影面から実質的に同じ距離を有するように、前記血管をセグメント化する、請求項6に記載の装置。
- 前記処理ユニットは、前記画像データ内の基準要素を検出し、前記基準要素の既知のサイズと、前記画像データ内の前記基準要素のサイズとに基づいて、前記スケーリング係数を決定する、請求項4乃至7の何れか一項に記載の装置。
- 前記機能的指標は、前記血管の中心線に沿った圧力降下、仮想部分冠血流予備量比、前記血管を通る血流の関数としての前記圧力降下の曲線、流体力学的抵抗値、血流速度プロファイル、血流速度分布の1つである、請求項1乃至8の何れか一項に記載の装置。
- 狭窄評価用の前記機能的指標は、複数の2D画像から別々に導出される、請求項1乃至9の何れか一項に記載の装置。
- 様々な画像からの前記機能的指標間の変化が決定され、所定の最大変化と比較され、前記比較に基づいて、合否基準が生成される、請求項10に記載の装置。
- 前記処理ユニットは、狭窄評価用の前記機能的指標を計算するためのベースとして使用される1つ以上の2D画像の品質スコアを計算する、請求項1乃至11の何れか一項に記載の装置。
- 狭窄評価用の機能的指標を決定する方法であって、
a)血管の2次元画像データに対応する画像データを取得するステップと、
b)前記血管の進路に沿った内径及び前記内径の変化を含む、血管の少なくとも一部の形状モデルを決定するステップと、
c)前記形状モデルに基づいて、前記血管の前記少なくとも一部の少なくとも1つの抵抗値を決定するステップと、
d)前記少なくとも1つの抵抗値に基づいて、前記血管又は前記血管の一部を流れる流体の圧力降下を決定するステップと、
を含む、
方法。 - 処理ユニットによって実行されると、請求項13に記載の方法のステップを行う、請求項1乃至12の何れか一項に記載の装置を制御する、コンピュータプログラム。
