ここで、引き抜きまたはCVDリスクは、係数(c)を所与のタンパク質のタンパク質レベル(p)(例えば、正規化されたピーク面積)で乗算したものの加算合計に定数(i)を加えることで決定される。以下の実施例1は、質量分析により決定されるタンパク質値及びモルタンパク質値の両方について、各パネルの各タンパク質に対する係数及び定数を示している。実施例1はさらに、これらの数値のそれぞれに対する95%信頼区間を示している。例えば、CVDリスクスコア及び/またはコレステロール引き抜き値を計算するとき、(c)または(p)の値に、この範囲内の任意の値を使用することができる。
確率=1/(1+exp(−リスクスコア))。
さらに、この確率値に100を掛け、CVDのリスク率を求めることができる。CVDのリスクスコアまたは確率またはリスク率は、医療従事者が、心臓疾患が疑われる患者の治療を決定する際の一助として、または治療もしくは監視を必要とするような患者を除外するために用いることができる。例えば、一般に、CADのリスク率が10%未満である患者は通常、低リスクとみなされ(例えば、心臓疾患の治療または監視を必要としない患者とみなすことができる)、CADのリスク率が10〜20%のリスクである患者は通常、中間リスクとみなされ(例えば、患者に心臓疾患の追加徴候がないか監視することができる)、CADのリスク率が20%を超える患者は通常、高リスクとみなされる(例えば、治療的または手術的介入により処置することができる)。
本実施例では、全体的なコレステロール引き抜き及び冠動脈疾患リスクに関する種々のアルゴリズムを使用し、単一及び複数のHDLタンパク質からなる26のパネルによって全体的なコレステロール引き抜き及び心血管疾患リスクを予測する能力を試験することについて説明する。HDLタンパク質は質量分析法によって測定した。
HDLの精製には以下のプロトコールを使用した。1倍リン酸緩衝生理食塩水(PBS)(pH7.4)中で、15N標識Hisタグ付きアポリポタンパク質A−Iの0.5mg/mL溶液25μLとヒト血清12μLを混合する。血清とApoA−Iの混合物を37℃で15分間インキュベートする間にHisタグ付きApoA−Iが血清試料中のHDL粒子に取り込まれる。次に、ハイスループット自動液体処理プラットフォームに適するピペットチップ形式の固定化金属アフィニティークロマトグラフィーによって、混合物からHDLを精製する。簡潔には、164μLの結合緩衝液(5mMイミダゾール、50mMリン酸ナトリウム、300mM塩化ナトリウム、pH8.0)を添加して、血清とApoA−Iの混合物を200μLに希釈する。アフィニティーカラムチップを400μLの結合緩衝液で平衡化する。次いでアフィニティーカラムを用いて、180μLの血清/ApoA−I混合物を流速200μL/分で6回吸引し、カラムベッド上に分注する。次に、結合緩衝液200μLを吸引して分注することによりカラムベッドを1回洗浄し、続いて強洗浄緩衝液(10mMイミダゾール、50mMリン酸ナトリウム、300mM塩化ナトリウム、pH8.0)200μLで2回目の洗浄を行う。その後、結合したHDL粒子を、溶出緩衝液(300mMイミダゾール、50mM Tris−HCl、25%メタノール、pH9.0)90μLを吸引して分注することにより溶出する。
次に、100mMジチオスレイトール5μL及び50ng/μLエンドプロテイナーゼLys−C10μLを、溶出したHDLに加え、37℃で4時間インキュベートする。Lys−Cが、配列中のリジンアミノ酸残基のC末端側でHDL関連タンパク質を切断し、LC−MS分析及び定量化で予測可能な標的ペプチドを産生する。
25%メタノールを含有する溶出緩衝液中の、Lys−Cで消化したHDL25μLを、ミキシングティーを使用してインラインで注入及び5倍希釈した後、LCカラムに直接装填する。希釈及び充填は、0.1%ギ酸水溶液をミキシングティーへ150μL/分で送液する試料充填ポンプ、及び99%移動相A(0.1%ギ酸水溶液)、1%移動相B(0.1%ギ酸アセトニトリル溶液)をティーへ600μL/分で送液するバイナリポンプを使用して行う。充填後、バイナリポンプをカラムと同一直線上に切り換え、500μL/分で1%から65%までの移動相Bの直線勾配で5分間ペプチドを溶出させる。ダイナミックMRMモードで動作するAgilent 6490トリプル四重極質量分析計を使用してペプチドを検出する。ダイナミックMRMは、スケジュール設定した保持時間枠内でペプチド標的の標的化検出及び定量化を可能にする。インタクトペプチドの質量に対する所定のm/z値(機器のQ1でフィルタリング)、インタクトペプチドが機器のq2で受ける衝突エネルギー、及びペプチド断片に対するフラグメント(m/z)値(Q3でフィルタリング)で構成される所定の「トランジション」で、ペプチドを標的化する。トランジションは、試料内の標的とするペプチド固有であるように選択される。2つのトランジション(後半の定量化に使用される「クオンティファイア」と、品質管理に使用される「クオリファイア」)をペプチドごとにモニタリングし、タンパク質あたり最大2つのペプチドを標的化する。ペプチド標的及びそのトランジションの詳細な一覧を表1に示す。なお、質量分析測定を行う際、列挙したペプチド配列以外の他の部分を使用することができる。
表2に示しているのは、HDLの全体的なコレステロール引き抜きの測定において良好な結果が得られたタンパク質の組み合わせを示す26のパネルである。
ペプチドシグナル強度は、各ペプチドの「クオンティファイア」トランジションのクロマトグラフピークを積分することによって得られる。タンパク質強度は、タンパク質の標的ペプチドそれぞれのクオンティファイアのピーク面積を合計し、1つ以上のHDL関連タンパク質の強度で正規化することによって決定される。
測定されたタンパク質から正規化した強度に基づいて、引き抜き能(探索コホート、及び新鮮時と凍結時の比較)及びCADリスク(総計、イベントの有無を問わない)の多変量アルゴリズムを開発し、これを利用して、元の血清試料のコレステロール引き抜き能またはCADリスクに関連する値を得た。モデル1は、探索試料の全体的なコレステロール引き抜きであり、モデル2は、新鮮試料と凍結試料の全体的なコレステロール引き抜きであり、モデル3は総計の冠動脈疾患(CAD)リスクであり、モデル4はイベントありCADリスクであり、モデル5はイベントなしCADリスクである。これらのモデルの一般式を以下に示す:

ここで、引き抜きは、係数(c)をいくつかのタンパク質の正規化されたピーク面積(p)で乗算したものの加算合計に定数(i)を加えることで決定される。
各パネルに関するデータの統計解析は、R(「www.」の後に「r−project.org」)及びBioconductor(「www.」の後に「bioconductor.org」)を使用して実施した。データは、15N ApoA1タンパク質の総強度で正規化した。MRMで測定した2つのペプチドをもつタンパク質に関して、分析した試料の大半で最高強度を示すペプチドを基準にタンパク質のレベルを算出した。ロバスト線形回帰を用いて、各タンパク質と全体的なコレステロール引き抜きとの相関関係を算出した。最小絶対収縮選択演算子(LASSO:Least absolute shrinkage and selection operator)をすべてのタンパク質に適用し、係数が0にならない特徴を選択した。70の探索試料で測定して選択された特徴に段階的ロバスト線形回帰を適用した。各パネルについて、赤池情報量基準(AIC)が最も低いモデルを選択した。
モル値を得るために、合成ペプチド標準物質を使用したLC−MSにより、表1の各ペプチドに関する測定シグナルの範囲を包含する5点外部検量線を作成し、モニターした。検量線を線形フィットし、1/X(Xはペプチド濃度)の重み付けを行った。その後、算出したモル値を用いてモル量係数を計算した。

アポリポタンパク質A−I関連プロテオームはコレステロール引き抜き能及び冠動脈疾患と関連する
本実施例では、コレステロール引き抜きと関連した、ApoA−I関連血清プロテオーム及びそのHDL機能との関係について考察した。さらに、本研究ではコレステロール引き抜き能(CEC)の予測方法の構築を試みた。Hisタグ付き組換えApoA−Iに関連する血清タンパク質画分をデータ依存型プロテオーム解析にかけ、コレステロール逆転送及び/または冠動脈疾患(CAD)に関連する21種のタンパク質に関する標的定量プロテオーム法を開発した。この標的化方法を細胞系のCEC測定値(N=105)と比較して、プロテオーム予測アルゴリズムを導出し、このアルゴリズムを、健常検体(N=74)ならびに主要有害心血管イベントありCAD検体(N=83)、及び主要有害心血管イベントなしCAD検体(N=74)の症例/対照研究において評価した。ApoA−I関連血清プロテオームと、HDLに観察されるタンパク質との間に有意な共通点がみられた。ApoA−II、ApoC−I、ApoC−II、ApoC−III、ApoD、及び血清アミロイドA(パネル19)で構成されるプロテオーム予測アルゴリズムは、細胞系のCECアッセイ結果と強く相関した(R=0.77)。症例/対照コホートにおけるプロテオーム予測したCEC測定値は、CECとCAD診断との間に有意な逆相関を示し(P=0.0032)、対照とMACEなしCAD検体との間には有意なCECの低下が観察され(P=0.04)、MACEありCAD検体に観察されたCECはさらに減少した。
試験方法はすべて、必要に応じて現地の治験審査委員会による承認を得た。特に明記しない限り、試薬はすべてSigma−Aldrich(St.Louis,MO)またはThermoFisherから入手可能な最高等級品を購入した。この試験で使用した検体は、プールされた血清試料を供給するGolden West biologicalsから入手した。免疫親和性除去によって調製されたリポタンパク質非含有血清は、GenwayBio(San Diego,CA)から購入した。加えて、Cleveland HeartLabから入手したレムナント試料を使用した。冠動脈疾患の症例を呈する臨床検体及び対照は、Fairbanks Institute for Healthy Communitiesから入手した。
組換え発現及び精製した0.5mg/mLの15N標識His6タグ付きアポリポタンパク質A−I(15N−His6ApoA−I)(Genscript,Piscataway,NJ)の1倍PBS(pH7.4)溶液24μLを、12μLのヒト血清に加えた。15N−His6ApoA−I/血清混合物を37℃で30分間インキュベートした。インキュベーション後、製造業者のプロトコールに従って、5mmイミダゾール、20mMリン酸ナトリウム、150mM塩化ナトリウム、pH8.0で総容積200μLに試料を希釈した後、Ni−NTA HisBind Superflow固定相5μLを充填した、PhyTip(Phynexus,San Jose,CA)搭載Tecan Freedom Evo自動液体ハンドラー(Tecan,Mannedorf,Switzerland)を使用して精製した。簡潔には、希釈した試料を、反復ピペットサイクルを用いて250μL/分でPhytipカラムにゆっくりと結合させ、続いて300μLの20mMイミダゾール、20mMリン酸ナトリウム、150mM塩化ナトリウム、pH8.0で引き続き洗浄した。次に、結合したHis6−ApoA−I及び関連種を300mMイミダゾール、50mM Tris−HCl、pH9.0、25%メタノールで溶出した。溶出後、試料は直ちに次の分析に使用するか、または必要になるまで−80℃で保存した。
500ngのエンドプロテイナーゼLysC(Wako Chemicals USA,Richmond,VA)に加えて、ジチオスレイトールを最終濃度5mMの濃縮ApoA−I関連血清画分に添加した。試料をEppendorf PCRサーマルサイクラーにて、37℃で4時間消化した後、4℃に保ち、エンドプロテイナーゼ活性を停止させた。得られたペプチド混合物を、Waters nanoACQUITY超高圧LCシステム(Waters,Inc.,Milford,MA)を使用して分離した。全ペプチド材料500ngを含む10μL容積を逆相Symmetry C18捕捉カラム(180μm内径×20mm、5μm粒子)(Waters,Inc)に、移動相A(0.1%ギ酸、2%アセトニトリル水溶液)を用いて、10μL/分で4分間、注入して捕捉し、洗浄した。次に、1.7μmのBEH130 C18固定相を充填したnanoACQUITYカラム(75μm内径×250mm)で、300nL/分でペプチドを溶出した。その際、移動相B(0.1%ギ酸、2%アセトニトリル水溶液)を2%〜50%の勾配で210分間、その後移動相Bを100%に増加して15分間用い、続いて2%の移動相Bで15分間再平衡化した。分解能120,000のオービトラップ内でのm/z350〜1800のプリカーサーイオンスキャンを使用し、続いて衝突誘起解離を用いてイオントラップして25のデータ依存型MS/MSイベントを行う、LTQ−Orbitrap Velos質量分析計(Thermo Fisher Scientific,Waltham,MA)でペプチドを検出した。プリカーサーは選択幅2.5m/zで分離し、20msの間、35%正規化衝突エネルギーで断片化した。モノアイソトピックなプリカーサーの選択が可能であり、1+及び空電荷状態を有するプリカーサーはMS/MSの選択から除外された。プラスまたはマイナス10ppmの質量を除外枠とするプリカーサーの動的除外を60秒間利用した。イオントラップとオービトラップの自動利得制御限界をそれぞれ1×104と1×106に設定した。RAW形式の質量分析データファイルを、統合型Andromeda検索アルゴリズム15を使用して、MaxQuant(バージョン1.4.1.2)14で処理し、タンパク質を同定した。一般汚染物質データベースに加えて、UniProt Human Database(2013年10月14日にEuropean Bioinformatics Instituteからダウンロード、88,304エントリからなる)に対して検索を行った。プリカーサー質量許容値±7ppm及びフラグメント質量許容値0.6Daで検索を実施した。LysCを切断酵素に設定し、少なくとも6個のアミノ酸を含み、切断エラーが2つまでのペプチドを分析対象とした。メチオニン酸化は可変修飾として許容した。タンパク質及びペプチドの偽陽性率は1%に設定した。タンパク質の同定をさらに高精度化し、少なくとも2つのペプチドが同定されるようにした。
還元剤(100mMジチオスレイトール5μL)及びプロテアーゼ(50ng/μLエンドプロテイナーゼLysC(Wako)10μL)を、濃縮されたApoA−I関連血清画分85μLに添加し、37℃で4時間インキュベートした。その時点の温度は4℃に低下していた。13C6、15N2−リジン標識内部標準ペプチドの規定混合物5μLをタンパク質消化物75μLに添加し、続いて25μLを注入して液体クロマトグラフィー多重反応モニタリング質量分析(LC−MRM)により分析した。注入試料を1.25分間カラムに充填して洗浄した後、移動相Bの直線勾配により500μL/分で溶出した。スケジュール設定した保持時間枠内でペプチド標的の標的化検出が可能である、ダイナミックMRMモードで動作するAgilent 6490トリプル四重極質量分析計を使用してペプチドを検出した。トランジションは、試料内の標的とするペプチド固有であるように選択、最適化して、決定される。2つのトランジションをペプチドごとに、かつタンパク質あたり最大2つのペプチドをモニタリングした。ペプチド標的及びそのトランジションの詳細な一覧を上記の表1に記載しており、パネル19については以下の表67に記載している。
プールされた血清由来のHDL(1.063g/mL<p<1.21g/mL)を、Brewerの方法に多少の変更を加えて用い単離した16。
ヒト血清試料をLDL除去し、J774マクロファージからの3H標識コレステロールの引き抜きを測定する細胞系のアッセイを、de la Llera−Moya17によって記載された方法を用いてVascular Strategies,Inc.,(Plymouth Meeting,PA)に外部委託して実施した。測定値はすべて、引き抜き値を正規化して報告した。
引き抜き相関モデルを開発するための血清試料は、訓練用と試験用のセットをそれぞれ用意するために、6週間空けて2つのバッチで回収されたCHLの未同定のレムナント検体から採取した。各セットに対する候補試料の選択指針として、LDL−c、HDL−c、ApoA、ApoB、トリグリセリド、及び高感度C反応性タンパク質(hsCRP)の定量分析を用い、最終的に好適合の検体セットを得た(表65)。
検体はFairbanks Institute for Healthy Communitiesバイオバンクから選択した。これは、22〜87歳の男女1500人から得た血清試料からなり、内訳は、冠動脈造影によるCAD所見(≧50%閉塞)750人、CAD、陽性ストレス試験、糖尿病、高血圧症、または脂質異常(LDL−C≧130mg/dL、HDL−C≦40mg/dL、総コレステロール≧240mg/dL、またはトリグリセリド≧200mg/dL)に関して陽性所見のない対照対象750人であった。空腹時の血液試料を試験SOPに従って採取した後、−80℃で保存した。CADと診断された対象を評価して、症例とイベントあり症例の2群を決定した。全CAD患者を、主要有害心血管イベント(MACE)を規定するICD−9コードに従って、心筋梗塞(410)、冠動脈バイパス術もしくは血管形成術(36.1、45.82)、または脳卒中(434.91)に分類した。ICD9分類により心筋梗塞と確認された場合、記録を調査し、虚血性疼痛の履歴、異常なECG、異常なトロポニンの3つのうち2つを有する患者を選択した。合計で、イベントなしCAD対象74人と、MACEイベントありCAD対象83人を選択した(表66)。
コンピューター解析の前に、各標的ペプチドのピーク面積存在量を、濃縮過程で使用した15N−His6ApoA−Iの強度で正規化した。LC−MS/MSで測定した2種以上のペプチドを有するタンパク質に関して、分析した試料の大半で最高強度を示すペプチドを用いてタンパク質の相対量を確定した。タンパク質レベルのデータを統計分析に使用した。特性選択のための一連の逐次工程を包含する分析パイプラインを正規化タンパク質レベルデータに適用して、CECに関連するタンパク質を探索した18。一変量分析では、ロバスト線形回帰を各タンパク質に適用し、70個の訓練試料に対するCECを予測し、p値<0.1を有するタンパク質を選択した。elastic netモデルを用いてさらなるフィルタリングを適用した。多変量解析では、部分最小二乗回帰を用いてフィルタリングしたタンパク質でバイオマーカーパネルを構築した。バイオマーカーパネルの性能を評価するために、予測CECと測定CECとの間のスピアマン相関及び中央絶対偏差(cost)を計算した。最初に、未較正のLC−MSピーク強度を用いてバイオマーカーパネルを作成した。プロテオーム法は較正後の絶対モル量が得られるように後に改良し、パネル係数は部分最小二乗回帰を用いて較正後のMRMデータで高精度化した。最後に、パネルを35個の検証試料で試験した。CEC予測バイオマーカーパネルがCADの有無によって対象を区別できるかどうかを評価するために、74人の健常対照と157人のCAD患者のバイオマーカータンパク質を試験した。データの統計分析は、Rバージョン3.2.3を使用して実施した(R Core Team (2013).R:A language and environment for statistical computing,R Foundation for Statistical Computing,Vienna,Austria)。患者コホート累積分布の比較は、コルモゴロフ−スミルノフ(K−S)検定を用いて実施した。
ApoA−Iアフィニティープールの迅速な調製
質量分析に基づくプロテオミクスを用いた、無脂質ApoA−I関連タンパク質の正確かつハイスループットな分析を容易にするために、迅速なアフィニティー濃縮のための戦略を考案した。この戦略では、Hisタグ付きApoA−Iと固定化金属アフィニティークロマトグラフィーを使用した(図2)。N末端ポリヒスチジンタグを有する組換え型の無脂質ApoA−I(His6ApoA−I)をヒト血清に添加し、37℃でインキュベートした。次に、試料をニッケル−NTA固定相に通して、His6ApoA−I及び関連タンパク質と結合させた。低濃度のイミダゾールで固定相を洗浄して、非特異的に結合したタンパク質を抑制した後、複合体化タンパク質及び過剰なHis6ApoA−Iを、高濃度イミダゾールを含む望ましい回収緩衝液に溶出する。次に、最終的に液体クロマトグラフィー質量分析法(LC−MS/MS)で分析するため、標準的な酵素消化ワークフローを、溶出した試料に施した。分析対象の検体に属する内因性タンパク質と区別するため、外因性ApoA−Iの特異的測定が可能である15N標識His6ApoA−I(15N−His6ApoA−I)を使用した。
初期のデータ依存型実験で明らかになったように、無脂質ApoA−Iの親和性画分で同定されたタンパク質の大半がHDL関連であることがわかっている。従来の超遠心分離を用いて単離したHDLと、400人の男性ドナーの市販血清プールを用いたApoA−I関連血清画分との定性的プロテオーム比較により、この観察結果のさらなる明確化を試みた。各濃縮技術あたり3回の繰り返し工程を実施した。総じて、91種のタンパク質が超遠心分離によって精製されたHDL中で同定され、162種のタンパク質が無脂質ApoA−Iアフィニティープールで同定され、そのうち78種のタンパク質は両方の調製方法に共通していた(図3を参照)。
ApoA−Iアフィニティープールとコレステロール引き抜き能(CEC)との関係を調べるため、液体クロマトグラフィー多重反応モニタリング(LC−MRM)に基づく正確な定量方法の使用を試みた。文献検索及び我々のデータ依存型LC−MS結果に基づいて、脂質輸送、コレステロール逆転送及び/または心血管疾患との関連が知られている21種のタンパク質を開発対象として選択した(脂質代謝(アポリポタンパク質A−I、A−II、A−IV、C−I、C−II、C−III、C−IV、D、E、F、J、L−I、M)、酵素(リン脂質輸送タンパク質−PLTP、コレステリルエステル転送タンパク質−CETP、レシチンコレステロールアシルトランスフェラーゼ−LCAT、パラオキソナーゼ1−PON1)、ならびに急性期反応タンパク質(補体C3、ハプトグロビン、血清アミロイドA1及び2−SAA1/2、ならびにSAA4))。アッセイ開発対象とする各タンパク質由来の2つのペプチド(可能な場合)を同定し、ワークフロー全体を最適化した。4時間以内に全タンパク質について安定なペプチド存在量が産生される消化条件を得た。血清原液からの天然ApoA−Iの回収率は、約55%であると推定され、これは、イムノアッセイによって決定された血清中のApoA−Iレベルと強く相関していた(ピアソンr=0.84、補足図6)。比較すると、無脂質の15N−His6ApoA−Iの平均回収率はPBS緩衝液からは85%であり、溶媒除去した血清からは72%であった。
無脂質のApoA−Iアフィニティー濃縮技術を使用して、一連の血清試料を分析し、アフィニティー濃縮タンパク質とCECとの間に関連性が存在するかどうかを調べた。70の訓練試験試料及び35の独立した試験試料(表65)のセットは、どの疾患診断かは問わずに無作為に選択したが、リポタンパク質測定との一致に関しては慎重を期した。各検体について、21種のタンパク質の補体の正規化CEC及び正規化質量分析データを決定した。一変量統計法及び多変量統計法を含む多段階インフォマティクスパイプラインを実施して、相対タンパク質存在量が細胞系のCEC18と相関するバイオマーカーパネルを開発した。一変量解析の後、p値<0.1を有する9種のタンパク質(アポリポタンパク質A−I、A−II、C−I、C−II、C−III、C−IV、D、CETP、及びSAA)をロバスト線形回帰によって同定した。それに続くelastic net回帰で、6種のタンパク質(ApoA−II、ApoC−I、ApoC−II、ApoC−III、ApoD、及びSAA)を選択して、それに部分線形回帰を適用し、最終予測モデルを確定した。
予測CECは、細胞系のCEC測定に関する報告と同様に心血管疾患と逆相関するはずであると仮定し、本発明者らの多変量パネルを使用して、157人のCAD患者(主要有害心血管イベント(MACE)なし74人、MACEあり83人)、ならびに年齢及び性別が一致する外見は健常な対照74人に関してCECを予測した(表66)。
試験セットでのプロテオーム予測CECの性能(r=0.71)は有望であり、コレステロール引き抜きの調節にApoA−I以外のタンパク質が果たす重要な役割を示唆している。蛍光標識コレステロールを用いた修飾細胞系のアッセイを実施する最近のCECの臨床研究によると、前提となる放射性同位体アッセイを用いた正規化引き抜きとやや一致する有意な予測値がみられた(R=0.54)12。それ以外にも引き抜きとの相関が確認されている。予測された通り、多くの研究において、r=0.22〜0.64(本研究ではr=0.57)の範囲でApoA−Iとの正の一変量相関が報告されている11、19。高HDL−C(>68mg/dL)検体では、スフィンゴミエリン/ホスホルエタノールアミン比が引き抜きと有意に関連している(r=0.64)が、正常なHDL−Cレベルでは関連していないことが実証された。引き抜きはまた、HDLサブクラスに依存することが知られており、これは一連の構造的及び機能的相互作用の複雑さをさらに示唆している20。これらの多様性は完全には解明されないままであるが、追加の生化学的及び生物物理学的データをモデルに統合し、CEC予測を改善できる可能性がある。
本発明者らの研究で得たCECのプロテオーム予測因子は、コレステロール輸送と機械的に関連するタンパク質(アポリポタンパク質A−II、C−I、C−II、C−III、D、及びSAA;パネル19)で構成される。引き抜きと正の相関を有するタンパク質では、ApoA−I及びApoA−IIが細胞コレステロールの一次受容体として機能している1、21。同様にApoC−Iは、重要ではないが効率的なコレステロール受容体であり、HDLの成熟に役割を果たすレシチン−コレステロールアシルトランスフェラーゼLCATも活性化する22。ApoC−II及びApoC−IIIは、HDLリモデリングに関与するリポタンパク質リパーゼを調節し、ApoDは、pre−β3−HDL、すなわちHDLの強力な細胞コレステロール受容形態の形成に役割を果たす22、23。血清アミロイドA(SAA)は急性期反応タンパク質であり、HDLからのApoA−Iの変位によりコレステロール引き抜き能に負の影響を与えることが広く特性決定されている24、25。本発明者らはまた、引き抜きの調節にSAAが果たす機能的役割が、引き抜きとhsCRPとの間の負の相関に反映されることにも注目した。hsCRPは、全身性炎症の測定によく用いられる別のバイオマーカーである(ただし、本研究ではApoA−Iの関連性は同定していない)。興味深いことに、プロテオーム解析によって解明された無脂質ApoA−Iと血清タンパク質との相互作用は、Proteome Watchリストに分類されている標準的なHDLプロテオームと高度の重複を示す。

前記バイオマーカーパネル18は、タンパク質ApoA1、ApoC1、ApoC2、ApoC3、及びApoC4を含み、
前記バイオマーカーパネルは、タンパク質ApoA2、ApoC1、ApoC2、ApoC3、ApoD、及びSAA1/2を含む)
のHDL関連タンパク質それぞれのレベルを検出することと;
b)前記対象での心血管疾患(CVD)の概算リスク及び/または前記試料の前記概算コレステロール引き抜き能(CEC)を決定することを含む、前記方法。 - 前記試料が、血清試料、血漿試料、血液試料、及び精製された高密度リポタンパク質(HDL)試料からなる群から選択される、請求項21に記載の方法。
- 前記決定に、第1のアルゴリズムを用いて、心血管疾患(CVD)リスクスコアまたはコレステロール引き抜き能(CEC)スコアを生成することを含み、前記第1のアルゴリズムが、i)各HDL関連タンパク質レベルに所定の係数を乗じて乗算値を生成することと、ii)前記乗算値を合計してパネル固有の定数値を加算することを含む演算を実行し、それによって前記CVDリスクスコアまたは前記CECスコアを生成する、請求項21に記載の方法。
- c)前記CVDリスクスコア及び/または前記CECスコアを示すレポートを生成することをさらに含む、請求項21に記載の方法。
