Abstract
Description

1つの態様において、本発明は、癌限界決定の方法を提供する。この方法は、放射線データ(radiology data)と病理データ(pathology data)とを組み合わせて、病巣レーザ療法(focal laser therapy)のために癌性組織の場所及び幅を表す3Dモデルを生成する。

他の態様において、本発明は、軟組織の病巣レーザ療法のためのシステムを提供する。システムは、MRI温度測定の使用を必要としない一方で、温度及び治療経過のリアルタイムモニタリング(実時間監視)を依然として可能にして、軟組織の病巣レーザ療法を管理するのに必要とされる時間及び資源を減少させる。
他の態様において、本発明は、マルチモードセンサプローブを提供する。マルチモードセンサプローブは、本発明の方法の実施中に組織切除の強化されたモニタリングを提供する。
他の態様において、本発明は、新規のマルチチャネル針ガイド(multi-channel needle guide)を提供する。マルチチャネル針ガイドは、レーザファイバ及びセンサのような器具の誘導された挿入のためのプラットフォームを提供する。マルチチャネル針ガイドは、レーザ有効範囲(laser coverage)及び治療経過を計算する目的のための精密な寸法を含む。
他の態様において、本発明は、本明細書で提供されるシステム及びデバイスを使用する軟組織のボア外(out-of-bore)病巣レーザ療法の方法を提供する。本方法は、MRI体温測定に依存せずに、外来設定で行うことができる点において、従来技術に対する改良である。本方法は、治療経過のリアルタイムモニタリングのために超音波、光センサ、及び温度センサを使用し、治療の時間及びコストを削減する。
以下の実験例を参照して本発明を更に詳細に記載する。これらの実施例は、例示の目的のために提供されているに過ぎず、特段の断りのない限り、限定的であることを意図されていない。よって、本発明は、以下の例に限定されるものと決して解釈されてならず、むしろ本明細書で提供される教示の結果として明らかになるありとあらゆる変形を包含するものと解釈されるべきである。
病巣レーザアブレーション(FLA)又はレーザ間質性熱療法(LITT)は、手術又は電離放射線を用いずに前立腺癌を治療する方法である(Bomers JGR et al., World Journal of Urology (2016):1-9)。FLAの目的は、隙間に配置された(interstitially placed)拡散レーザファイバの使用によって癌性前立腺組織の凝固壊死を誘発することである(Lee T et al., Reviews in urology 16.2(2014); Stafford RJ et al., The Journal of urology 184.4(2010):1514-1520)。FLAは、1993年に前立腺治療について最初に記載されており(Johnson DE et al., Lasers in surgery and medicine 14.4(1994):299-305)、数多くの最近の研究の対象である(Oto A et al., Radiology 267.3(2013):932-940; Natarajan S et al., The Journal of urology 196.1(2016):68-75)。
中程度のリスクの前立腺癌を有する男性が本研究の対象であった。各々の場合において、癌はMRI/US生検によってMRIで見える関心領域(ROI)内にのみ存在することを確認した。従前にMRI及び生検処置を記載した(Sonn GA et al., The Journal of urology 189.1(2013):86-92)。包含及び除外基準を図12に示す。主要な終点(endpoint)は、6ヶ月のフォローアップ期間中に如何なる治療関連グレード3以上の有害事象(CTCAE,v4.03)が存在しなかったことである。診査の終点は、尿及び性機能の低下、PSAの低下、及び組織構造(histology)又はMR撮像の変化の欠如であった。融合生検にのために利用されるものと類似する部屋設定を使用してFLAを行った(図15)。患者の特性を図13に示す。
各患者はこの研究に登録するに先立ちアルテミス(Artemis)デバイスを介した標的化生検を受けた。それは各ROI及び生検コアの場所を3Dで保存することを可能にする。各患者の撮像及び生検情報を使用して各治療を計画した。この初期段階の研究では、治療限界を意図的に保守的に維持した。
全ての男性に口腔キノロン及びセフトリアキソン又はエルタペネムの注射を用いた洗浄浣腸及び抗生物質予防法を与えた。処置の直前に、全ての患者はケトロラク(30mg)及びミダゾラム(4mg)(最小限の鎮静作用)の単回静脈内投与を受けた。ブピバカインとリドカインの50〜50%混合物を使用して、経直腸式US及び前立腺周囲神経遮断のために、患者を左側臥位置に配置した。前立腺周囲麻酔後、熱プローブの会陰挿入のために、患者を切石術位置に変えた。皮膚内リドカイン(1%)を使用して、2〜3のMR適合性蛍光学熱プローブ(STB, LumaSense, Santa Clara, California)をリアルタイム超音波ガイダンスを使用して前立腺内に会陰式に配置した。他の場所(Natarajan S et al., The Journal of urology 196.1(2016):68-75)に記載したように、前立腺内温度モニタリングのために、少なくとも1つのプローブを直腸壁付近の後部前立腺内に前進させた。バイタルサインを継続的に監視し、各レーザアクティブ化前、中及び後に疼痛スコア(Hawker GA et al., Arthritis care & research 63.S11(2011):S240-S252)を数値的に評価した。
追跡調査を1週間、1ヶ月、3ヶ月及び6ヶ月で行った。各訪問は、詳細な履歴及び身体検査、有害事象のスクリーニング、薬剤調和、PSA、及び健康関連の生活の質(quality
of life)(例えば、IPSS,IIEF)質問表を含んだ。3TmpMRIを6ヶ月で実施し、UCLAで開発されたPI−RADSスコアリング及び基準を用いて解釈した(11,13)。6ヶ月生検で、MRI/US融合(アルテミス)を使用して、元の腫瘍部位、切除ゾーン(ablation zone)、切除ゾーンの限界、任意の新しいROI、及び同側前立腺を通じた6つのテンプレート部位(template sites)をサンプリングした。各前立腺の治療側から12個の生検コア(範囲9〜16)の平均を得た。
6ヵ月の期間中に32個のグレード1及び6個のグレード2の有害な事象を記録した。1人の患者は、試験登録前に計画されていた既存の腰部狭窄の選択的外科的矯正のためのFLA後、2ヶ月間入院した。血尿は、治療後の最も有害な事象であり、全ての患者において介入せずに解決した。全ての患者は、治療及び追跡MRI後に、外来臨床処置室から退院した。治療に関連した重篤な有害な事象(>グレード3,CTCAE)に直面しなかった。
FLA後の1週間、1ヶ月、3ヶ月、及び6ヶ月で、HRQOL質問票を実行した。基線(ベースライン)での中央値IPSSは7であり、5.5まで減少した。基線での中央値IIEF−5スコアは14であり、19まで増加した。変化は有意でなかった。基線での中央値PSAは、基線で7.35ng/mlであり、6ヶ月で2.55ng/mlまで減少した(Wilcoxon符号付き順位検定、p=0.28)。
全ての患者において蛍光光学熱プローブからのデータを成功裡に記録した。経直腸モニタに加えて、平均2つの経会陰プローブを前立腺に挿入した。レーザファイバの先端付近で記録した最大温度は68℃であった。直腸モニタが42℃に近づくならば、レーザアクティブ化は停止し、ファイバは前立腺内に再配置される。全ての場合において、直腸に最も近い熱プローブは42℃未満の温度を記録した。
処置の直後に、MRIは、各患者において、治療領域、即ち、アブレーションゾーンの密閉された局所化された低灌流(confined, localized hypo-perfusion)を明らかにした(図19)。MRIによって決定されるようなアブレーションゾーンの容積の中央値は4.8ccであった。T2又は拡散重み付け撮像における主要な治療関連変化は見られなかった。前立腺容積の中央値は、治療前〜治療後6ヶ月で有意に変化しなかった(33対32cc,p=0.44,Wilcoxon符号付き順位検定)。
追跡生検の結果は、FLAの操作者経験及びエコー源性針の追加に関連した。最初の4人の患者において、生検は、治療ゾーン及び限界の両方に臨床的に有意な疾病が継続的に存在することを明らかにした。次の6人の患者において、生検は、3人(治療ゾーン内に1人、限界内に2人)における微小焦点グリーソン3+3疾病(micro-focal Gleason 3+3 disease)と、他の3人の男性における癌の完全な不存在とを明らかにした(図14)。治療ゾーンからの生検材料は、しばしば、熱効果と一致する間質性線維症、巨細胞反応、ヘモジデリン含有マクロファージ、及び慢性炎症を伴う、良性の前立腺及び間質を明らかにした。図20A〜図20Fは、1人の患者におけるそのような所見を描写している。
放射輝度センサは、組織光学特性における凝固誘発変化に対する感受性が流束量センサよりも高いことを示している(Chin LCL et al., Optics Letters 29,(2004):959-961)。さらに、Chin et al.は、凝固ゾーンが発達するにつれて、(光源に面する)0度の放射輝度が信号において着実に減少することを実証した。対照的に、凝固境界がプローブをひとたび通過すると、(光源から見て外方を向く)180度での放射輝度が信号において増加する。熱誘起凝固は、散乱の増加を引き起こし、凝固フロントがプローブをひとたび通過すると、放射輝度センサに向かって散乱させられる後方散乱光子の増加をもたらす(Chin LCL et al., Optics Letters 29,(2004):959-961)。以下の研究では、熱センサと反対方向に面する2つの放射輝度センサとからなる統合マルチモードセンサが開発されている。そのようなプローブは、ファイバ先端の周りのチャー発達(char development)及び凝固境界の両方を検出することができる。最適なアブレーションゾーンを達成するために、レイジングパラメータを変調させてよい。この技法を使用して、高強度集束超音波及び高周波を含む任意のアブレーションモダリティを監視することができる。それはLETTに特に適している。何故ならば、その進行を監視するために、レーザ誘導凝固を使用することもできるからである。レーザーがひとたび非アクティブ化されると、凝固を監視する能力は失われるが、データは、急速冷却の故に、この段階では最小の損傷しか生じないことを示している(図21)。
凝固境界の伝播をリアルタイムに観察するために、前立腺組織の980nmでの光学特性及び熱特性を模倣するファントム(phantom)を開発する。ファントムは、人間の前立腺について以前に見出された比熱容量(3.779J/(g*K))及び熱伝導特性(0.56W/mK)を含む(Giering K et al., Thermochim. Acta 251,(1995):199-205; Van den Berg CaT et al., Phys. Med. Biol. 51,(2006):809-825)。ファントムの2分の1は、Iizuka et al(Iizuka MN et al., Lasers Surg. Med. 25,(1999):159-169)によって概説される動的光学特性を保有する。以前に得た吸収係数及び減少した散乱係数(μ’s=8.1cm−1、μa=0.66cm−1)が使用される(Bu-Lin Z et al., Int. J. Hyperthermia 24,(2008):568-576)。これはLITT中に光散乱特性が生体内で観察されるように変化することを保証する。ファントムの残余の半分は、光透過性アクリルアミドからなる。Zhang et al(Bu-Lin Z et al., Int. J. Hyperthermia 24,(2008):568-576)によって実証されるように、高速カメラをこの側に配置して、凝固ゾーン発達を記録する。彼らの研究において、凝固領域はアブレーションアプリケータ(ablation applicator)の周りの白色ゾーンとして明確に境界付けられた。更なるアプローチは、LITT中に温度プロファイルを検査する有用な方法として実証されている熱変色インク(thermochromic ink)を追加することである(Mikhail AS et al., Med. Phys. 4304(2016); Negussie AH et al., Int. J. Hyperthermia 6736(2016):1-5)。レーザ拡散器及びマルチモードセンサをファントムの2つの半体の間のインターフェース(界面)に配置する。マルチモードセンサ内の熱プローブ及び光ファイバをフォトダイオード及び温度モニタリングシステムにそれぞれ接続する。このアプローチは、凝固の半径と放射輝度との相関を可能にして、増加した後方散乱の故に凝固フロントがセンサを通過するときに180度での放射輝度が増加することを実証する(Chin LCL et al., Optics Letters 29,(2004):959-961)。
前立腺癌(CaP)及び標的化生検の局所化のためのマルチパラメトリックMRI(mpMRI)の出現は、病巣療法研究のための科学的基礎を提供している(Ahmed HU et al., The Journal of Urology, 2011,185(4):1246-1255; van den Bos W et al., Eur Radiol, 2015, 1-9; Lepor H et al., European Urology, 2015, Epub ahead of print; Oto A et al., Radiology, 2013,267(3):932-940)。理論的には、病巣療法は、治療に関連した病的状態(morbidity)が殆どない癌制御の可能性を提供するが(Ahmed HU et al., The Lancet Oncology, 2012, 13(6):622-632)、これまでにほんの僅かの臨床試験しか実施されていない。Ahmed et al.は、42人の男性におけるMRI特定病変を治療するために高強度集束超音波(HIFU)を使用した(Ahmed HU et al., The Lancet Oncology, 2012, 13(6):622-632)。Oto及び同僚は、8人の男性におけるMRI特定病変を治療するために病巣レーザアブレーション(FLA)を使用した(Oto A et al., Radiology, 2013,267(3):932-940)。Van den Bos et alは、MRI及び造影強化超音波の両方を用いて視覚化された病変を局所的に治療するために不可逆的電気穿孔法(IRE)の使用を最近報告した(van den Bos W et al., Eur Radiol, 2015, 1-9)。
この試験における患者は、臨床ステージが≦T2b CaPであり、グリーソンスコア(GS)≦3+4=7である、58〜72歳の男性8人であった。8人全員を、標的化サンプリング及び系統的サンプリングの療法を含む、MR/US融合生検によって診断し(Sonn GA et al., The Journal of Urology, 2013,189(1):86-92)、それは単一のRM可視病変内にCaPを示し、前立腺内でGS>6を示さなかった。これらの男性は、他の場所に記載したコホート(同齢集団)で融合生検を受けた患者から入院基準毎に選択された(Sonn GA et al., The Journal of Urology, 2013,189(1):86-92)。PI−RADS及び社内で考案した5点評価システムの両方を使用して、全身用コイルを使用した3T MRIを取得し且つ解釈した(Sonn GA et al., The Journal of Urology, 2013,189(1):86-92)。診断の6ヶ月内にFLAを実施した。患者特性を図25に示す。
FLAを使用して、生検で確認した癌を伴う関心領域(ROI)MR増強指標(MR-enhancing index regions of interest)を標的化した。MRIの3Dセグメント化によってROI特性を決定した。前立腺内の各患者のROI幾何学的形状及び場所に従ってMATLAB及びC++を使用して開発したカスタムソフトウェアを使用して、ファイバ場所及び所望の限界を事前に計画した。MRI組織病理学的相関を用いた以前の研究は、MRIが真実な腫瘍容積を1.5cmまで系統的に過小評価することを示している(Priester A et al., Int Symp Focal Therapy Imag 2014, Pasadena, CA, Aug 21-23, PP-24)。以前の生検情報、即ち、陽性コア及び陰性コアの3D場所を使用することによって、この限界を更に精緻化した。かなり大きなボア内経験の間に得られた予備データに基づいて、12〜15Wでの3分のレーザアクティブ化がレーザ先端の周りに径方向に約1cm延在する凝固壊死のゾーンを創り出すと推定した。
患者をMRI管内に配置する前に、全ての被験者は、洗浄食塩水浣腸及び抗生物質、即ち、FLAの前日に開始する5日間の経口シプロフロキサシン及びFLA時の筋肉内セフトリアキソンを受けた。経直腸超音波誘導の下で1%リドカイン及び0.5%ブピバカインの前立腺周辺ブロックを投与した。必要に応じて精神的なベルセド(versed)及びフェンタニル(意識的な鎮静)の静脈内投与によって局所麻酔を補完した。FLAに先立ち、超音波誘導下で経会陰式に配置された小線源療法アプリケータ(Flexi-needle, Best Medical, Springfield, VA)を通じて、2〜3個のMR適合性蛍光光学温度プローブ(STB, LumaSense, Santa Clara, CA)を前立腺内に前進させた。MRTとは無関係に、前立腺内熱変化の評価のために温度プローブを配置した。各患者について、直腸壁付近の後部前立腺内に少なくとも1つのプローブを挿入した。
治療の直後に、mpMRIを取得して評価した。ダイナミックコントラスト強調MRIを使用して治療ゾーンを確認し、それを計画治療ゾーン及びMRTマップと比較した。患者を回復室内で監視し、排便後、全ての人は数時間内に退院した。排出薬物は、キノロン抗生物質及び経口非麻薬性鎮痛薬を含んだ。
9ヶ月の期間に亘って23個のグレード1及び7個のグレード2の有害事象(CTCAE,v4.03)を記録し、それらの全ては自発的に解決した。最も一般的な症状は、血尿(12)、血***症(4)、及び血便(1)であった。全てのグレード2事象は、8日の評価内に解決した。全ての患者は6時間内に退院し、治療後に鎮痛のための麻薬性鎮痛を必要とした者はいなかった。
スクリーニング時に、並びに1週間、1カ月、3カ月及び6カ月の時点に、全ての8人の男性に対してIPSS及びSHIMを収集した。中央値のIPSSは、スクリーニング時に4であり、6ヶ月で3.5に減少した。中央値のSHIMは、スクリーニング時に19.5であり、6ヶ月で20に増加した。健康に関連する生活の質のメトリック(health-related quality of life metric)のいずれにも統計的に有意な変化は6ヶ月後に観察されなかった(それぞれp=0.37,p=0.78)。尿失禁、***障害、又は***の変化は、いずれの患者によっても報告されなかった。
全ての患者についてデータを成功裡に収集したが、これらのデータは患者の動きに非常に敏感であった(図28)。8人の患者のうちの6人について蛍光光学熱プローブからのデータを記録し、最初の2つは技術的に不満足であった。これらの6人の患者では、治療ゾーンの外側の全ての前立腺内場所において平均温度は40℃より下であった(図29A、図29B)。
各患者について、マルチパラメトリックMRI(mpMRI)を診断前、介入直前、及びFLA直後に取得し、再び6ヶ月時に取得した。組織容積及び潅流の変化が注目され、図26に要約されている。T2又はDWIに一貫した変化はなかった。DCEのみを使用して治療直後の効果を決定した(Oto A et al., Radiology, 2013,267(3):932-940)。各患者において、限定的な灌流の領域は、重要な構造から離れて、治療領域内にあり、3ccの中央値容積を有した(図30)。処置中の腫脹及び治療後の有意な収縮(p=0.03、Wilcoxon符号付き順位検定)を含む、腺の形態学的変化は、治療領域の局所化を混乱させた。一般的に、FLA直後のDCEが示すような治療ゾーンは、6ヶ月時にはもはや明らかでなかった。
FLAの6ヶ月後にMR/US融合(Filson CP et al., CA: A Cancer Journal for Clinicians, 2015, 65:265)を使用して追跡標的化生検を行った。生検は、治療領域/元の癌の病巣、治療ゾーンの周りの限界、及び治療された側の系統的な生検を標的とした。各患者から平均15個のコア(範囲:13〜17)を取得した。生検は、如何なる安全性の懸念の証拠も示さなかった(即ち、感染性、外傷性、又は新生物性の有害な変化は見られなかった)。最も一般的な治療関連の所見は、線維症の病巣領域であり、ヘモジデリンを含むマクロファージの存在がしばしば点在し、古い出血の再吸収を示す(図31)。
A et al., Radiology, 2013, 267(3):932-940)。本研究では、8人のうちの7人が3+4=7のグリーソンスコアを有し、4mmを超える中央値の最大コア長を有した。よって、ここでの患者の選択は、中リスクの男性を治療するための現在のコンセンサスの推奨と一致している(Donaldson IA et al., European Urology, 2015, 67(4):771-777)。追跡の6ヶ月時に、8人の患者のうちの5人における元の癌保有病巣の包括的な生検後に癌は検出できず、中リスクの個体における効果的なFLAの可能性を示唆している。
病巣レーザアブレーション(FLA)は、リアルタイムMRI誘導の下で前立腺癌(CaP)を安全に治療するために使用されてきたが、面倒で、時間がかかり、資源集約的であり、放射線処置としてアプローチされる。以下の研究は、広範な標的化生検経験を用いて、MRI/超音波(MRI/US)融合誘導の下で泌尿器科クリニックにおいてFLAを行うための基礎を提供した。
根治的前立腺切除術を受けた65人の男性(中央値61歳、44〜79歳の範囲)の前立腺を、患者特異的な3D印刷モデルを使用して精密に切断した。これらのモデルを術前mpMRI輪郭(図33A)から生成し、各スライスはMR画像平面に対応した(図33B)。腫瘍をホールマウントスライド(whole mount slides)上に描写し(図33C)、デジタル的に再構成し(図33D)、MATLABソフトウェアを使用して対応するMRI病変(UCLAグレード3〜5)と一致させた。全ての患者は以前に治療を受けておらず、平均前立腺容積は40cc(範囲19〜110cc)であった。
マルチパラメトリックMRI(mpMRI)は、前立腺癌(CaP)を撮像し、標的化された介入を誘導する、ロバストな方法である。以下の研究は、MRI可視関心領域(ROI)と既知のCaPの領域との間の空間的関係を調査し、効果的な病巣療法に必要な治療限界を特徴付ける。

- 軟組織における癌限界決定の方法であって、
少なくとも1つのMRI可視病変を有する関心領域の少なくとも1つのMRI画像を取得するステップと、
前記少なくとも1つのMRI画像から前記少なくとも1つのMRI可視病変の3Dモデルを生成するステップと、
前記MRI可視病変を取り囲む組織から少なくとも1つの生検コアを取得するステップと、
前記少なくとも1つの生検コアを、癌含有陽性結節、癌不存在陰性結節、又は未確定癌存在を有する中性結節として分類するステップと、

- 軟組織のための病巣レーザ療法の方法であって、
治療されるべき患者の関心領域のリアルタイム3D超音波モデルを取得するステップと、
該リアルタイム3D超音波モデルの上に少なくとも1つの癌限界3Dモデルをオーバーレイするステップと、
少なくとも1つの予想損傷モデルを生成するステップであって、該少なくとも1つの予想損傷モデルは、前記少なくとも1つの癌限界3Dモデルと少なくとも部分的に重なり合う、ステップと、
前記少なくとも1つの予想損傷モデルに適合するよう、少なくとも1つのアブレーション設定と、前記患者の関心領域内の少なくとも1つのレーザファイバ場所とを計算する、ステップであって、該少なくとも1つのアブレーション設定は、レーザ電力出力と、レーザ曝露持続時間と、レーザ曝露速度と、冷却剤流速とを含む、ステップと、
前記患者の関心領域内少なくとも1つのセンサ場所を計算するステップと、
レーザファイバを前記少なくとも1つのレーザファイバ場所に挿入し、少なくとも1つのセンサを前記少なくとも1つのセンサ場所に挿入するステップと、
前記少なくとも1つのアブレーション設定に従って前記関心領域を切除するステップと、
前記切除される組織損傷の程度をモデル化することによって治療経過を監視するステップとを含む、
方法。 - 前記少なくとも1つの癌限界3Dモデルは、MRI可視病変3Dモデル、MTV3Dモデル、最適化された限界3Dモデル、及び生検コア場所を含む、請求項20に記載の方法。
- 前記予想損傷モデルは、3つの入れ子式の楕円体を含み、最小の楕円体は、最小の予想損傷(minED)を表し、中間の楕円体は、平均の予想損傷(aveED)を表し、最大の楕円体は、最大の予想損傷(maxED)を表す、請求項20に記載の方法。
- 前記予想損傷モデルの前記minEDは、前記MTV3Dモデルの全体をカプセル化する、請求項22に記載の方法。
- 前記少なくとも1つのセンサは、少なくとも1つの熱センサ、少なくとも1つの光センサ、少なくとも1つのマルチモードセンサ、又はそれらの任意の組み合わせを含む、請求項20に記載の方法。
- 前記アブレーション設定は、95℃よりも高い温度を生成することから制約される、請求項20に記載の方法。
- 前記組織損傷の程度は、治療されている前記関心領域に隣接する組織の温度を測定することによってモデル化される、請求項20に記載の方法。
- 前記組織損傷の程度は、前記少なくとも1つのアブレーション設定を実行した直後に組織の冷却速度を測定することによってモデル化される、請求項20に記載の方法。
- 前記組織損傷の程度は、造影剤を用いた外観変化、血管分布変化、機械特性変化、又は組織温度変化の超音波測定によってモデル化される、請求項20に記載の方法。
- 前記組織損傷の程度は、組織光学特性の熱誘導変更のレベルを定量化することによってモデル化される、請求項20に記載の方法。
- 細長い中央熱センサと、
該中央熱センサに隣接し、該中央熱センサと平行である、少なくとも2つの光ファイバと、
各光ファイバの一端に位置付けられるプリズムと、
前記中央熱センサ、前記少なくとも2つの光ファイバ、及び前記プリズムを入れる、ハウジングとを含む、
