Abstract
Description

Claims (28)
- 心機能不全を治療する方法であって、前記方法は、
心臓の少なくとも1つの心室に刺激パルスの刺激パターンを送達する工程を含み、
前記刺激パルスの少なくとも1つが、前記心臓における収縮末期容積(ESV)および拡張末期容積(EDV)の少なくとも1つを減少させるように構成される第1刺激設定を有し、前記刺激パルスの少なくとも1つが、前記第1刺激設定とは異なる第2刺激設定を有し、
前記刺激パターンが、前記収縮末期容積(ESV)および拡張末期容積(EDV)の少なくとも1つを少なくとも5%減少させ、前記収縮末期容積(ESV)および拡張末期容積(EDV)の少なくとも1つを、少なくとも1時間の期間、平均して、このような減少された容積に維持するように構成される、方法。 - 前記期間が、少なくとも3ヶ月間である、請求項1に記載の方法。
- 前記刺激パターンが、心負荷を少なくとも2%減少させ、前記心負荷を、前記期間の間、平均して、このような減少された負荷に維持するように構成される、請求項1に記載の方法。
- 前記刺激パターンが、前記期間の間、治療前血圧値と比較して血圧を平均圧の±10%以内に維持するように構成される、請求項1に記載の方法。
- 前記刺激パターンが、前記患者における心臓リモデリングを予防するように構成される、請求項1に記載の方法。
- 前記第1刺激設定が、少なくとも1つの心室における心房収縮を減少させるかまたはそれを予防するように構成され、前記刺激パルスの少なくとも1つが、前記第1刺激設定とは異なる第2刺激設定を有する、請求項1に記載の方法。
- 前記第1刺激設定が、前記収縮末期容積(ESV)および拡張末期容積(EDV)の少なくとも1つを少なくとも5%減少させるように構成され、前記第2刺激設定が、前記収縮末期容積(ESV)および拡張末期容積(EDV)の少なくとも1つにおける前記減少に対する圧反射反応または圧反射適応を減少させるように構成される、請求項1に記載の方法。
- 前記刺激パターンが、前記収縮末期容積(ESV)および拡張末期容積(EDV)の少なくとも1つにおける前記減少に対する圧反射反応または圧反射適応を減少させるように構成される、請求項1に記載の方法。
- 前記第1刺激設定が、前記収縮末期容積(ESV)および拡張末期容積(EDV)の少なくとも1つを少なくとも5%減少させるように構成され、前記第2刺激設定が、前記収縮末期容積(ESV)および拡張末期容積(EDV)の少なくとも1つにおける前記減少に対するニューロン反応またはニューロン適応を減少させるように構成される、請求項1に記載の方法。
- 前記刺激パターンが、前記収縮末期容積(ESV)および拡張末期容積(EDV)の少なくとも1つにおける前記減少に対するニューロン反応またはニューロン適応を減少させるように構成される、請求項1に記載の方法。
- 前記第1刺激設定が、前記収縮末期容積(ESV)および拡張末期容積(EDV)の少なくとも1つを少なくとも5%減少させるように構成され、前記第2刺激設定が、ホルモン分泌を増加させるように構成される、請求項1に記載の方法。
- 前記刺激パターンが、ホルモン分泌を増加させるように構成される、請求項1に記載の方法。
- 前記第1刺激設定が、心房興奮後に、前記心臓の心室を40〜90ミリ秒刺激することを含む、請求項1に記載の方法。
- 前記第2刺激設定が、前記心臓の心房を刺激し、それによって心房刺激を引き起こすことを含む、請求項13に記載の方法。

- 前記刺激パターンが、前記第1および第2刺激設定とは異なる第3刺激設定を有する少なくとも1つの刺激パルスを含む、請求項1に記載の方法。
- 心機能不全が、収縮末期容積(ESV)および拡張末期容積(EDV)の少なくとも1つにおける増加に関連する、請求項1に記載の方法。
- 前記心機能不全が、うっ血性心不全である、請求項1に記載の方法。
- 第1期間の間、1つの刺激パターン構成で前記刺激パターンを適用し、
前記第1期間の間、収縮末期容積(ESV)、拡張末期容積(EDV)、心負荷および血圧の少なくとも1つを示す少なくとも1つのパラメータを感知し、
前記感知に従って、前記刺激パターン構成を調整することをさらに含む、請求項1に記載の方法。 - 前記刺激パターンの前記調整が、前記第1刺激設定および前記第2刺激設定の少なくとも1つの調整を含む、請求項22に記載の方法。
- 前記刺激パターン構成の調整が、前記刺激パターン内の前記第1刺激設定を有する刺激パルスおよび前記第2刺激設定を有する刺激パルスの少なくとも1つの数および比率の少なくとも1つを調整することを含む、請求項22に記載の方法。
- 心機能不全を治療するシステムであって、前記システムは、
患者の心臓の少なくとも1つの心室に刺激パルスを送達するように構成される刺激回路、および
前記少なくとも1つの心室に刺激パルスの刺激パターンの送達を実行するように構成される少なくとも1つの制御器を含み、
前記刺激パルスの少なくとも1つが、前記心臓における収縮末期容積(ESV)および拡張末期容積(EDV)の少なくとも1つを減少させるように構成される第1刺激設定を有し、前記刺激パルスの少なくとも1つが、前記第1刺激設定とは異なる第2刺激設定を有し、
前記刺激パターンが、前記収縮末期容積(ESV)および拡張末期容積(EDV)の少なくとも1つを少なくとも5%減少させ、前記収縮末期容積(ESV)および拡張末期容積(EDV)の少なくとも1つを、少なくとも1時間の期間、平均して、このような減少された容積に維持するように構成される、システム。 - 前記少なくとも1つの制御器が、前記期間の間、収縮末期容積(ESV)、拡張末期容積(EDV)、心負荷、および血圧の少なくとも1つを示す少なくとも1つの感知されるパラメータに関連する入力データを受信し、前記少なくとも1つの感知されるパラメータに従って前記刺激パターン構成を調整するように構成される、請求項25に記載のシステム。
- 前記少なくとも1つの感知されるパラメータを感知し、前記少なくとも1つの制御器に前記入力データを通信するように構成される少なくとも1つのセンサをさらに含む、請求項26に記載のシステム。
- 前記少なくとも1つの制御器、前記刺激回路および前記少なくとも1つのセンサが、単一の装置に組み合わされる、請求項27に記載のシステム。
