実施例1:患者背景と検体数
様々な手術対象者(心臓手術、弓部大動脈人工血管置換術部分切除術、冠動脈バイパス移植術、胸部大動脈人工血管置換術、肝部分切除、膵頭部十二指腸切除術、食道亜全摘手術、頸椎椎弓形成術)に対し、手術前、手術直後(0h)、術後1時間後(1h)、2時間後(2h)の各点にて採血を実施し、各種パラメーターを測定しその変動の確認を行った。症例により採血できなかったものもあり、採血ポイントでの検体数は表1に示す通りである。パラメーターとして、SOFAスコア、tAM、mAM、Presepsin、IL−6(インターロイキン6)、CRP(C反応性ペプチド)、WBC(白血球数)を測定した。

実施例1の検体において、術例に関わらずSOFAスコアに対する各パラメーターの相関性(Spearman’s rho)を検証した。表2に示す通り、SOFAスコアとCRP間では相関性が認められないものの、他のパラメーターは有意差をもって相関性を示した。tAMならびにmAMにおいては順位相関係数がそれぞれ0.771、0.755と良好な相関性を示すものであった。
実施例1の検体において、SOFAスコアごとの各パラメーターの濃度あるいは数量の分布を図1、図2、図3、図4、図5、図6に示す。SOFAスコアごとの例数はSOFAスコア0/1/2/3/4/5/6/7/8/9/10/11それぞれ8/25/8/10/8/8/16/7/8/7/3/3検体の計111検体である。
実施例4:各パラメーターのSOFAスコア診断能比較
実施例1の検体において、それぞれのSOFAスコアの弁別能をROC(Receiver Operating Characteristic)解析によりAUC(曲線下面積)にて比較した。例えばSOFAスコア5以上の弁別能の検証ではSOFAスコアが4以下を陰性群、5以上を陽性群としROC解析を行った。その結果を図7に示す。なお横軸は、当該スコア以上の弁別能の検証を行ったものである。SOFAスコア1から11までの全域を通してtAMが最も優れたAUCを示し0.874以上を示した。続いてmAMが優れたAUCを示し0.821以上を示した。AUC一覧を表3に示す。またSOFAスコア5以上のROC解析図ならびにSOFAスコア9以上のROC解析図をそれぞれ図8、図9に示す。

実施例6:アドレノメデュリン測定値によるSOFAスコアの弁別
実施例1の患者を含めて、SOFAスコア、tAM、mAMの3パラメーターが取得された患者背景を表4に示す。405検体のtAM、mAMの測定値を用いSOFAスコアの弁別能をROC解析した。AUC、カットオフ値、感度、特異度を表5(tAM)ならびに表6(mAM)に示す。なおカットオフ値は(1−感度)2+(1−特異度)2が最小値となる濃度を求めた。各SOFAスコアとカットオフ値の関連性を検証した結果、tAM(r=0.982)、mAM(r=0.984)それぞれ高い相関係数を示し、またその比例関係はそれぞれtAMカットオフ値=2.1981×SOFAスコア+9.2007、mAMカットオフ値=0.1781×SOFAスコア+1.0159で示されるものであった(図11、図12)。本結果より、tAM、mAMそれぞれは、カットオフ値の一定の濃度変動をもってSOFAスコアが1ポイントずつ変動することを示している。すなわちtAMを経時的に2点以上測定した場合、その濃度変動約2.2pmol/L濃度ごとにSOFAスコアが1ポイント変動すること、またmAMを経時的に2点以上測定した場合、その濃度変動約0.18pmol/L濃度ごとにSOFAスコアが1ポイント変動することを示している。
