Abstract
Description
補強部分を含む主要部及び固定部分を含む2つの端部を有する弧状半月板プロテーゼ本体を含み、
この主要部が、2つの端部間に延在する第1の生体適合性の非吸収性材料から形成された部分を含み、
補強部分及び固定部分が、第2の生体適合性の非吸収性材料から形成され、
補強部分が、固定部分間に延在し、かつ
固定部分が貫通孔を有し、第1の生体適合性の非吸収性材料が、ISO 527−1によって決定される最大でも100MPaの引張係数を有し、かつ第2の生体適合性の非吸収性材料が、ISO 527−1によって決定される少なくとも101MPaの引張係数を有する。
a.第2の材料を成形して補強部分及び固定部分を形成するステップ;
b.固定部分に貫通孔を形成するステップ;及び
c.第1の材料を成形して、補強部分及び任意選択の固定部分を取り囲む、プロテーゼ本体の主要部の部分を形成するステップ、を含む。
[試験方法]
引張係数を、ISO 527−1に従って決定した。使用した試験片は、ISO527−2に従った1BA型の寸法の試料とした。試験片は、特殊なダイを用いて射出成形された80×80×2mmの板から打ち抜いた。試料は、引張試験の前に、pH7.4、37℃の温度の緩衝生理食塩水で少なくとも2週間飽和させた。試験大気は、100%の相対湿度の空気とした。試験中の温度は37℃とした。試験速度は1mm/分とした。引張係数の値は、5つの試験片の平均値である。
健康な半月板の形状を、MRIスキャンを行って決定した。ヒト半月板の形状のコンピュータモデルを、収集したデータに基づいて作成した。アルミニウム型を、平均のヒト半月板のコンピュータモデルに基づいて用意した。コンピュータモデルから、固定部分の直径を決定した。必要な貫通孔と共に、固定部分の断面の面積を決定した。文献によると、ヒト半月板角(meniscus horn)に作用し得る通常の負荷は、60Nであることが分かっている。この負荷、固定部分の表面積、及び40%の安全率により、5.5MPaの応力レベルが得られた。この応力レベルは、固定部分の第2の材料の疲労特性を試験するために選択した。
半月板プロテーゼでは、構成要素の良好な付着が重要である。2つの材料の接触面で、設計上の「弱点」が形成され得る。しかしながら、その接触面に動的な機械的負荷がかけられる半月板プロテーゼの長期間の性能を保障するために2つの部分を互いに強力に接触することが必須である。
溶融温度 210℃、成型温度 50℃、射出時間 0.4秒、環境での5分後のオーバーモールド、予熱なし、溶融滞留時間 4.4分、保持圧力 50MPaとした。

・通常の加工条件の維持により、接触面で驚くほど強い接着が得られた。強度及び伸長特性の減少は一切観察されなかった。
・試料1及び2の破壊時の引張強度及び伸長の値は、大きな差異を示さなかった。従って、標準成形条件下では、接着面の存在は、試料の破壊時の引張強度及び伸長に大きな差異をもたらさないと結論付けることができる。
・成形中の温度をそれぞれ10℃、20℃下げた場合(試料2、3、及び4)、試料の破壊時の引張強度及び伸長が悪化した。成形中の溶融温度の変動が、試料の特性に対して強い影響を有すると結論付けることができる。
・成形温度が50℃から30℃に下げられた場合(試料2と10を比較)、型が110℃の温度で予熱された場合(試料2と11を比較)、これは、試料の破壊時の引張強度及び伸長に明らかな悪影響を有する。
・保持圧力の変動(試料5と試料6)、溶融滞留時間(試料7)、試料の72時間の保存(試料9)、及び長い射出時間(試料8)は、試料2と比較すると試料の破壊時の引張強度及び伸長に対する影響が小さい。
Claims (13)
- 半月板プロテーゼであって、
補強部分(2)を含む主要部(1)及び固定部分(2A、2B)を含む2つの端部(1A、1B)を有する弧状半月板プロテーゼ本体を含み、
前記主要部(1)が、前記2つの端部(1A、1B)間に延在する第1の生体適合性の非吸収性材料から形成された部分を含み、
前記補強部分(2)及び前記固定部分(2A、2B)が、第2の生体適合性の非吸収性材料から形成され、かつ
前記補強部分(2)が、前記固定部分(2A、2B)間に延在し、
特徴として、
前記固定部分(2A、2B)が貫通孔(3A、3B)を有し、
前記第1の生体適合性の非吸収性材料が、ISO 527−1によって決定される最大でも100MPaの引張係数を有し、かつ
前記第2の生体適合性の非吸収性材料が、ISO 527−1によって決定される少なくとも101MPaの引張係数を有する、半月板プロテーゼ。 - 前記第1の生体適合性の非吸収性材料が、ヒドロゲル及び/又は熱可塑性材料を含む、請求項1に記載の半月板プロテーゼ。
- 前記第1の生体適合性の非吸収性材料がポリウレタンを含む、請求項1又は2に記載の半月板プロテーゼ。
- 前記第2の生体適合性の非吸収性材料が熱可塑性材料を含む、請求項1〜3のいずれか一項に記載の半月板プロテーゼ。
- 前記第2の材料の引張係数が最大でも3500MPaである、請求項1〜4のいずれか一項に記載の半月板プロテーゼ。
- 前記プロテーゼ本体の形が、天然の半月板の形に似ている、請求項1〜5のいずれか一項に記載の半月板プロテーゼ。
- 3次元(3D)プロトタイピング技術を使用して天然の半月板の複製を形成する、請求項1〜6のいずれか一項に記載の半月板プロテーゼ。
- 前記第1の生体適合性の非吸収性材料及び/又は前記第2の生体適合性の非吸収性材料が放射線不透過性添加剤を含む、請求項1〜7のいずれか一項に記載の半月板プロテーゼ。
- 前記貫通孔(3A、3B)が、第1の直径を有する第1の部分、及び前記第1の直径よりも大きい第2の直径を有する第2の部分を有する、請求項1〜8のいずれか一項に記載の半月板プロテーゼ。
- 請求項1〜9のいずれか一項に記載の半月板プロテーゼを製造するプロセスであって:
a.前記第2の材料を成形して前記補強部分(2)及び前記固定部分(2A、2B)を形成するステップ;
b.前記貫通孔(3A、3B)を前記固定部分(2A、2B)に形成するステップ;及び
c.前記第1の材料を成形して、前記補強部分(2)及び任意選択の前記固定部分(2A、2B)を取り囲む、前記プロテーゼ本体の主要部(1)の部分を形成するステップ、を含む、プロセス。 - 前記補強部分(2)及び前記固定部分(2A、2B)が単一部品として成形される、請求項10に記載のプロセス。
- 前記貫通孔(3A、3B)が、前記固定部分(2A、2B)及び前記前記端部(1A、1B)の前記第1の材料を貫通して形成される、請求項10又は11に記載のプロセス。
- 請求項1〜9のいずれか一項に記載の半月板プロテーゼによって天然の半月板を置換するための方法。
