Abstract
Description
The present invention relates to an embolus penetrating device for use in the treatment of an obstruction site in a body cavity.
An embolization device for use in the treatment of a hardened occlusion site in a blood vessel (for example, chronic total occlusion (Chronic Total Occlusion)) has been proposed.
The case where the embolus removal catheter is passed through the blood vessel in order to move the rigid head to the occlusion site in the blood vessel will be exemplified. Here, when the blood vessel is bent, the embolus removal catheter cannot be passed through the blood vessel unless the rotation drive wire and the balloon catheter are bent following the blood vessel. In the case of the above-described embolization removal catheter, since the rotation drive wire is made of stainless steel and the balloon catheter is made of plastic, the flexibility of the rotation drive wire may be lower than that of the balloon catheter. For this reason, especially with respect to the rotational drive wire, it may not be possible to bend following the blood vessel. In this case, there is a problem that the balloon catheter is caught in the blood vessel and the rigid head cannot be moved to the occlusion site.
An object of the present invention is an embolus penetrating device for treating an occlusion site in a body cavity by using a cutting means, and is capable of appropriately moving the cutting means to an occlusion site in a body cavity. Is to provide a device.
The embolus penetrating device of the present invention has a cutting means capable of cutting a closed site in a body cavity, a flexible and elongated shape, the cutting means being connected to one end, and the other end The driving means is connected to the cutting means, and the cutting means can be rotated in response to the driving of the driving means. At least a part of the cutting means is inserted into the lumen of the catheter and is one side with respect to the catheter. Rotating means that is movable to both sides of the distal end side and the proximal end that is the other side, and capable of projecting the cutting means from the distal end side end portion of the catheter when moved to the distal end side The hardness of the rotating means is softer than the hardness of the catheter.
The embolus penetrating device has rotating means that is softer than the catheter. The rotating means rotates according to the driving of the driving means, and rotates the cutting means. The cutting means cuts a closed site in the body cavity by rotating. Here, in order to cut the occlusion site by the cutting means, it is necessary to bring the cutting means close to the occlusion site through the catheter and the rotation means in the body cavity. In this case, since the rotation means is softer than the catheter, when the catheter can be bent following the bent body cavity, the rotation means can also follow the body cavity and bend. For this reason, the embolus penetrating device can appropriately move the cutting means connected to one end of the rotating means to the occlusion site in the body cavity.
In the present invention, the rotating means is N coils (where N is an integer of 2 or more) wound so that the respective central axes extend in the same direction, and the n-th (where n is The (n + 1) -th coil is wound along the outer peripheral surface of the coil of any one of 1 to “N−1”, and the n-th coil and the “n + 1” -th coil are N coils wound in the reverse direction may be provided. In this case, the rotating means can appropriately transmit the driving force of the driving means to the cutting means to rotate the cutting means, and at the same time the hardness of the rotating means can be made softer than the hardness of the catheter. Therefore, a rotating means softer than the catheter can be easily produced.
In the present invention, the diameters of the windings of the N coils may be substantially the same. In this case, for example, the diameter of the rotating means can be minimized by minimizing the diameter of the winding. Therefore, a rotating means capable of passing through a thin body cavity can be easily manufactured.
In the present invention, the rotating means may include three coils. In this case, the rotating means can minimize the diameter of the rotating means while appropriately transmitting the driving force of the driving means to the cutting means.
In the present invention, the end portion on the distal end side of the cutting means has a flat portion perpendicular to a rotation axis when the cutting means is rotated by the rotating means, and the flat portion cuts the blocking portion. Possible uneven portions may be provided. In this case, the cutting means can perform cutting by appropriately bringing the uneven portion of the flat portion into contact with the closed portion that gradually closes from the proximal end side toward the distal end side. Further, by providing the concavo-convex portion on the flat surface portion, it is possible to prevent the concavo-convex portion of the flat surface portion from contacting the catheter and damaging the catheter when the rotating means moves to the distal end side or the proximal end side with respect to the catheter.
In the present invention, the cutting means has a substantially circular cross section when cut by a plane orthogonal to the rotation axis, and the diameter of the plane portion is closer to the base end side than the plane portion. Or larger than the diameter. In this case, it can suppress that the part of a base end side rather than a plane part contacts a catheter. Therefore, the embolus penetrating device can more appropriately suppress the cutting means from coming into contact with the catheter and damaging the catheter.
In the present invention, the concavo-convex portion may include at least one groove portion that is recessed from the planar portion toward the proximal end and extends along the planar portion. In this case, the shavings when the closing member is cut by the cutting means can be moved to the proximal end side through the groove. For this reason, the device for penetrating the embolus portion can suppress the cutting by the cutting means from being inhibited by the shavings.
In the present invention, the at least one groove portion may have two groove portions extending in a straight line and orthogonal to each other. In this case, it is possible to easily form the two groove portions in the flat portion of the cutting means.
In the present invention, the catheter and a connector having an inlet connected to the proximal end of the catheter and capable of injecting liquid from the outside into the lumen of the catheter may be further provided. . In this case, the embolus penetrating device can exclude the gas from the catheter by injecting the liquid into the catheter through the injection port. Moreover, when the temperature of the obstruction | occlusion part rises at the time of cutting by the cutting means, it can cool with the liquid inject | poured from an injection port.
In the present invention, the catheter may have a first lumen through which the rotating means is inserted and a second lumen through which a guide wire is inserted. In this case, the first lumen through which the rotating means is inserted in the catheter can be separated from the second lumen through which the guide wire is inserted. For this reason, the embolus penetrating device can smoothly move the cutting means along the guide wire, so that the cutting means can be appropriately brought close to the occluded site in the body cavity.
In the present invention, the catheter is connected to the first catheter having the first lumen, the second lumen, and the vicinity of the distal end of the outer peripheral surface of the first catheter. A second catheter may be provided. In this case, the diameters of the first catheter and the second catheter can be minimized. Accordingly, the embolus penetrating device can pass the catheter even in a narrow body cavity.
In the present invention, the torque transmission performance of the rotating member may be higher than the torque transmission performance of the catheter. In this case, twisting of the rotating means when the rotating means is rotated by the driving means is suppressed. Therefore, the embolus penetrating device can cut the occluded portion more effectively by rotating the rotating means by the driving means.

An example of a method for using the

The user moves the
In the process in which the cutting member 4 gradually moves to the tip side while excavating the CTO, sanitary saline is injected from the
<Main functions and effects of the present invention>
The
The rotating
Conventionally, in a debulking device such as a rotablator, the diameter of a member (such as a wire) that rotates a cutting member increases, and a thin blood vessel may not pass therethrough. On the other hand, in the embolus
In the
The cutting member 4 has a cylindrical shape, and the diameter of the cut surface is the same regardless of the position in the stretching direction. In this case, as shown in FIG. 8, when the rotating
The

In a conventional debulking device, shavings during cutting may clog the cutting member, obstructing the cutting operation, or the temperature of the closed part may increase during cutting. On the other hand, the

In the conventional debulking device, the diameter of the catheter is increased, and there are cases where a thin blood vessel cannot be passed. On the other hand, the
