Abstract
Description
The present invention relates to a balloon tube device that is used to stop bleeding by pressing a bleeding that is difficult to stop bleeding from a peeled surface generated after the placenta in a cesarean section operation with a pre-placenta, for example.
Usually, the placenta is attached above the uterine wall and peels off after the fetus is delivered, causing bleeding from the peeled surface. In many cases, there is no particular difficulty in hemostasis because it contracts to stop bleeding. However, in the case of the placenta previa, the placenta is attached to the lower uterus and the exit of the uterus is blocked, so the fetus is delivered by caesarean section. Since there are few myometriums, rapid uterine contraction is difficult to occur, and as a result, the frequency of cases in which hemostasis is difficult has increased.
In order to cope with this bleeding, hemostasis such as performing uterine wall sutures, uterine contractors, or blocking the blood flow of the uterine arteries is attempted while blood transfusion is performed. Sometimes hysterectomy is required.
As an instrument for controlling bleeding from the uterus after the delivery of a fetus, which is applied in such a case, a tamponade device has been proposed in which a balloon inserted into the uterus is inflated and brought into contact with a release surface to stop bleeding. (Patent Document 1) This device is provided with an inner discharge pipe that discharges blood and the like accumulated in the womb that is open at both ends to the outside of the body, and an outer sheath that surrounds the inner discharge pipe over its entire length. The sheath is provided with a balloon that partially encloses the inner drainage tube and an inlet provided with a valve that injects fluid into the balloon. By applying uniform pressure to the balloon, the sheath is brought into close contact with the bleeding uterine wall. And try to control bleeding.
(特許文献2) In addition, as a tube device provided with an inflatable balloon for application in the uterus, a first balloon that is inserted into the uterus and inflated, and a second balloon that is inflated in contact with the outer uterine ostium, There has been proposed a uterine fixation device in which the uterus is sandwiched and held by first and second balloons. (Patent Document 2)
According to the device of the above-mentioned document, bleeding can be controlled by inflating a balloon in the uterus and closely contacting and covering the placenta peeling surface of the uterus, and hemostasis can be expected by indwelling for an appropriate time (about 12 hours). . However, when the balloon part of these conventional devices is introduced into the uterus, all of them are inserted transvaginally from the outside of the body, and a blind procedure is required to insert or inflate. In addition, in order to grasp the indwelling status of the balloon in the uterus, confirmation by ultrasonic waves or the like may be necessary.
In the case of caesarean section, since the uterus is opened, it is possible to insert the tube from the uterus side to the vagina side under direct vision, but there is no such idea in the past. In the tube device, there is a concern that the passage through the cervix or the vagina may be hindered when the tube is inserted or removed due to a branched fluid injection port.
Therefore, the present invention is particularly suitable as a device for stopping bleeding after exfoliation of the placenta by cesarean section by pressing the balloon, and it is safe and reliable to install and remove the balloon in the uterus and the like. An object was to provide a tube device.
The balloon tube device of the present invention includes a tube shaft provided with a passage communicating from one end side to the other end side, a balloon provided in the middle of the tube shaft, a fluid route for injecting and discharging fluid to the balloon, and the fluid A fluid injection / discharge port provided at the end of the route is provided, and the fluid route is provided in parallel with the tube shaft, and is formed with fluid injection / discharge ports at both ends of the tube shaft. At that time, one end of the fluid inlet / outlet provided at both ends is mainly a fluid inlet and the other end is mainly a fluid outlet.
Further, in addition to the above configuration, a second balloon adjacent to the balloon with a predetermined distance may be provided on the tube shaft. In this case, the second balloon is attached to the tube shaft in the same manner as the balloon. A second fluid route for injecting and discharging fluid is provided in parallel with the tube shaft, and a second fluid inlet / outlet is provided on both ends of the tube shaft. Moreover, it is preferable to provide the side hole for ensuring the channel | path connected from the one end side to the other end side in the edge part vicinity of a tube shaft.
The fluid route (including the second fluid route) is formed by being inserted into the tube shaft as a route tube parallel to the tube shaft, or in parallel with the passage formed integrally with the tube shaft. It is preferably formed as a separate lumen.
Furthermore, the fluid injection / discharge port (including the second fluid injection / discharge port) is preferably formed to have an outer diameter equal to or smaller than the outer diameter of the tube shaft, each of which is entirely or partially formed. It is preferable to take a configuration that can be maintained in a form that can be accommodated in the passage of the tube shaft and that does not protrude from the outer periphery of the tube shaft when accommodated. For that purpose, each specific fluid inlet / outlet preferably takes one of the following forms.
1. A fluid injection / discharge port is formed in the end portion of the tube shaft.
2. The fluid inlet / outlet is formed so as to be variable between a state of branching and projecting from the axis of the tube shaft and a state of being accommodated in the passage of the tube shaft.
3. The fluid outflow / injection port is formed to be variable between a state in which it protrudes from the tip of the tube shaft together with the end of the fluid route and a state in which it is engaged with the end of the tube shaft and accommodated in the end.
The fluid inlet and the fluid outlet are either provided with a connector having a check valve on both sides, or the fluid outlet is not provided with a check valve and is formed by closing the end of the fluid route. It is preferable to do it.
(Function)
According to this means, the fluid injection / discharge port to the balloon or the injection port and the discharge port (hereinafter referred to as “fluid injection / discharge port”) are located on both ends of the tube shaft. Therefore, during the caesarean section, the device is introduced from the opened uterus side to the vagina while confirming directly, and fluid is injected from the fluid inlet located on the end side of the uterus. The balloon can be inflated in the inside, the inflated balloon can be brought into close contact with the bleeding surface, and the situation can be visually confirmed. In addition, the device can be inserted and fluid can be injected into the balloon from the clean uterus side, not from the vaginal cavity or the lower limb side, which is the unclean side during surgery. Even if the device is left in the uterus, the uterine wound is sutured and the operation is completed, the device is removed after an appropriate time of placement, and the fluid outlet is the end of the vagina opposite to the fluid inlet. Since it is provided in the side, the fluid in a balloon can be discharged | emitted from this discharge port, and this instrument can be extracted.
In addition, a second balloon is provided adjacent to the balloon, and the second balloon is disposed in an inflated state in the outer uterine ostium, so that the balloon in the uterus becomes a bleeding part in the placenta of the placenta. It can hold | maintain in the state closely_contact | adhered reliably. As a result, when a single balloon device such as that disclosed in
Also, if the fluid route is inserted into the tube shaft as a root tube, or formed integrally as a separate lumen with the tube shaft passage, the tube device is inserted by being formed into a cylindrical shape with no irregularities other than the balloon part, Removal is smooth.
Furthermore, if the fluid injection / discharge port is formed smaller than the outer diameter of the tube shaft and can be maintained in a form that does not protrude outward from the outer circumference, it can be inserted because there is no branching part with the tube shaft like conventional devices. Smooth operation that can be passed without being caught in the cervix or vagina when removing. In addition, even when the second balloon is provided, there are two fluid injection / discharge ports, etc. on one side and one of them must be branched. By adopting a configuration that can be accommodated in the passage, no protruding portion is generated in the circumferential direction of the tube shaft. In addition, even if the fluid outflow / injection port protrudes from the tube shaft tip together with the end of the fluid route in the axial direction, it can be accommodated by engaging the tube shaft end so that it protrudes in the circumferential direction of the tube shaft during insertion. No part is produced. Furthermore, if a fluid injection / discharge port or the like is set in the end of the passage of the tube shaft or in the middle of the passage and the opening such as the end of the passage is blocked, a side hole is provided in the vicinity of the end. The blood in the uterus can be discharged outside without blocking the passage of the shaft lumen.
Both the fluid inlet and outlet may be a connector equipped with a check valve, but if the outlet used exclusively for discharging, which is required only when the instrument is removed, has a closed fluid route, the connector becomes unnecessary. Can be simplified. Even in that case, there is no functional problem because the fluid in the balloon can be discharged by cutting the blocking portion. On the other hand, if a connector with a check valve is provided on the discharge port side, the amount of fluid after closing can be adjusted, for example, fluid can be reinjected into the balloon. Can wither.
According to the balloon tube device of the present invention, the tube can be introduced from the uterus side to the vagina side while visually observing the caesarean section, and the balloon can be inflated and installed. The balloon can be safely and reliably attached to the balloon.

FIG. 2 shows that the inlet is accommodated in the tube shaft, and the balloon is An expanded state is shown. FIG. 3 shows the tube shaft as a cross-sectional view, and also shows the configuration inside the lumen passage of the shaft tube, FIG. 4 is an enlarged view thereof, A is the uterine distal end portion, B is the balloon portion, C indicates the vaginal distal end portion.
For example, in the cesarean section operation using the pre-placenta, the device of this embodiment controls the bleeding that is difficult to stop bleeding that occurs frequently due to placenta separation after delivery. It is a balloon tube device particularly suitable for compression hemostasis, which is used for procedures such as indwelling (for about 12 hours) and performing compression hemostasis. In addition, in this description, in order to facilitate a specific understanding, it is described as a balloon tube that is applied to hemostasis after this placenta exfoliation.
The instrument of the present embodiment includes a
Next, each part will be described in detail.
The
Further, in this configuration, since the
The
A
The
The
A

On the other hand, the
The
In order to discharge the fluid from the
The
Here, it is desirable that the
Next, a method for using the balloon tube device of the present embodiment in the case of the placenta previa will be described. FIG. 5 shows a state in which the device of this embodiment is placed in the uterus.

FIGS. 6 to 8 show a second embodiment of the present invention, FIG. 6 is a configuration diagram in a state where a balloon is inflated, and FIG. 7 is a partial cross-sectional view in a state in which the balloon is deflated. The structure of is shown. FIG. 8 shows a form in which the device is inserted into the vagina from the uterus. Note that in the following description, the description of components that are the same as those in the present embodiment is omitted, and the names and symbols of the components are the same as those in the above embodiments.
The tube device of the present embodiment is a female luer taper provided with a check valve similar to the
The
When the fluid outlet from the
The present embodiment is a tube device that stops bleeding caused by the cesarean section of the pre-placental placenta by bleeding from the peeled surface by pressing the balloon, but according to the configuration of the present invention, the tube is provided from both sides of the tube. Since fluid can be injected and discharged, it can be widely applied to applications where injection and discharge are desired from different directions.
