Abstract

Description
開示の背景
生物薬剤、特に、治療用抗体およびそれらのフラグメントの大多数は、非経口経路、例えば、静注または皮下注射によって投与される。これらの投与経路は、多くの場合、利便性がなく、痛みを伴い、特に1日に何回も注射が必要な場合には患者のコンプライアンスを引き下げる。それらはまた、スタッフの時間、保管および器具の点で、医療提供者にとってもコストがかかることになる。

カモスタットメシル酸塩(CAS No:59721−29−8)の化学名は、4−[[4−[(アミノイミノメチル)アミノ]ベンゾイル]オキシ]ベンゼン酢酸2−(ジメチルアミノ)−2−オキソエチルエステルメタンスルホネートであり、例えば、Sequoia Research Productsから入手できる。カモスタットメシル酸塩(CM)は、膵炎および術後逆流性食道炎の治療用として日本および韓国で認可されている経***性セリンプロテアーゼ阻害剤である(Foipan Product information sheet; Takasugi et al., Digestion 1982, 24:36-41; Kono et al., Am J Surg. 2005 Sep, 190(3): 412-7)。CMは、トリプシン、トロンビン、カリクレインおよびプラスミンを含む広域の阻害を有する(Tamura et al., 1977, Biochimica et Biophysica Acta 484, 417-422)。胃腸管内でのCMの代謝は明らかではないが、CMの代謝産物であるGBPAはそれ自体活性がある(Beckh et al., Res Exp Med, 1987, 187: 401-406)。
用語「単一可変ドメイン」は、抗体可変ドメインに特徴的な配列を含んでなる折り畳まれたポリペプチドドメインを意味する。従って、単一可変ドメインには、VH、VHHおよびVLなどの完全な抗体可変ドメイン、および例えば1以上のループが抗体可変ドメインに特徴的ではない配列で置換されている改変された抗体可変ドメイン、または末端切断されたもしくはN末端もしくはC末端延長を含んでなる抗体可変ドメイン、ならびに全長ドメインの少なくとも結合活性および特異性を保持する可変ドメインのフラグメントが含まれる。単一可変ドメインは、異なる可変領域またはドメインとは独立に抗原またはエピトープに結合することができる。「ドメイン抗体(商標)」または「dAb(商標)」は、「単一可変ドメイン」と同じと見なすことができる。単一可変ドメインはヒト単一可変ドメインであり得るが、齧歯類(例えば、WO00/29004号に開示の通り)、テンジクザメおよびラクダ科VHH dAb(商標)などの他種由来の単一可変ドメインも含む。ラクダ科VHHは、天然に軽鎖を欠いた重鎖抗体を産生する、ラクダ、ラマ、アルパカ、ヒトコブラクダ、およびグアナコを含む種に由来する免疫グロブリン単一可変ドメインである。このようなVHHドメインは、当技術分野で利用可能な標準的技術に従ってヒト化してもよく、このようなドメインは「単一可変ドメイン」と見なされる。本明細書で使用する場合、VHはラクダ科VHHドメインを含む。
抗標的単一可変ドメイン、例えば、抗TNFα単一可変ドメインは、前記標的、例えば、TNFαと結合する単一可変ドメインを意味する。標的はいずれの好適な標的であってもよい。一実施形態では、本開示の単一可変ドメインは、下記:TNFα、IL−23、LAG−3、IL−6、IL−13、IL−18、TSLP、CD3または前記のいずれか1つの受容体、例えば、TNFRαRIもしくはTNFRαRIIなどのTNFα受容体、IL−23受容体、LAG−3受容体、IL−6受容体、IL−13受容体、IL−18受容体、TSLP受容体、またはCD3受容体のうちいずれか1つを標的とする。一実施形態では、本開示の単一可変ドメインは、ケモカインまたはケモカイン受容体、例えば、グルタミン酸−ロイシン−アルギニン受容体、すなわち、ELR受容体、例えば、配列番号12および19〜22に示されるアミノ酸配列を含んでなるものを標的とする。
親和性は、ある分子、例えば、本開示の単一可変ドメインの、別の分子、例えば、その標的との、単一の結合部位での結合の強度である。単一可変ドメインの、その標的との結合親和性は、平衡法(例えば、酵素結合免疫吸着アッセイ(ELISA)もしくはラジオイムノアッセイ(RIA))、または動態(例えば、BIACORE(商標)分析)によって決定することができる。
一実施形態では、単一可変ドメイン−標的相互作用の平衡解離定数(KD)は、100nM以下、10nM以下、2nM以下または1nM以下である。あるいは、KDは、5〜10nMの間、または1〜2nMの間であり得る。KDは、1pM〜500pMの間、または500pM〜1nMの間であり得る。当業者ならば、KDの数値が小さいほど結合が強いことが分かるであろう。KDの逆数(すなわち、1/KD)は、M−1を単位とする平衡会合定数(KA)である。当業者ならば、KAの数値が大きいほど結合が強いことが分かるであろう。
解離速度定数(kd)または「解離速度」は、単一可変ドメイン−標的複合体の安定性、すなわち、1秒当たりに減衰する複合体の画分を表す。例えば、kd 0.01秒−1は、1秒当たりに1%の複合体が減衰することに相当する。一実施形態では、解離速度定数(kd)は、1×10−3秒−1以下、1×10−4秒−1以下、1×10−5秒−1以下、または1×10−6秒−1以下である。kdは、1×10−5秒−1〜1×10−4秒−1の間、または1×10−4秒−1〜1×10−3秒−1の間であり得る。
用語「中和する」は、本明細書で使用する場合、in vitroまたはin vivoにおいて、標的の生物活性が本明細書に記載されるような単一可変ドメインの存在下で、単一可変ドメインの不在下での標的の活性に比べて低くなることを意味する。中和は、標的のその受容体への結合を遮断すること、標的がその受容体を活性化するのを妨げること、標的もしくはその受容体をダウンレギュレートすること、またはエフェクター機能に影響を与えることのうちの1以上によるものであり得る。一実施形態では、本開示の単一可変ドメインは、その標的を中和する。
「遷移中間点」または「Tm」は、単一可変ドメインの50%がその天然コンフォメーションにあり、他方の50%が変性している温度である。一実施形態では、単一可変ドメインは高いTmを有する。特に、Tmは約66℃以上である。Tmを含む単一可変ドメインの熱安定性は、示差走査熱量測定(DSC)を用いて測定することができる。
「経口投与」は、本明細書で使用する場合、口による本明細書に開示される組成物の投与を意味する。本開示の組成物は一般に嚥下され、それらが作用する胃腸(GI)管に移行する。少量が腸管粘膜から循環中に吸収され、全身作用に向けられる。吸収は口(口腔)および胃で始まり得るが、通常、小腸で起こる。
「胃腸(GI)管」は、上方GI管:口、咽頭、食道および胃;ならびに下方GI管:小腸、十二指腸、空腸、回腸、大腸(盲腸、結腸−上行結腸、横行結腸、下行結腸およびS字結腸を含む)、直腸および肛門;ならびに胆嚢、肝臓および膵臓を含む。本開示の組成物は、GI管の上述の領域のいずれか1以上を標的とし得る。一実施形態では、組成物は小腸を標的とする。一実施形態では、組成物は大腸を標的とする。
本明細書に開示される医薬組成物は、本明細書に記載のヒト疾患のいずれか1以上の処置を目的とし得る。一実施形態では、本医薬組成物は、単一可変ドメインを場合により1種類以上の薬学上許容される担体および/または賦形剤と組み合わせて含んでなる。
このような組成物は、許容医薬実務により知られている、また求められるような薬学上許容される担体を含んでなる(例えば、Remingtons Pharmaceutical Sciences, 16th edition (1980) Mack Publishing Co.参照)。このような医薬組成物の製造方法は、当業者に周知である。
一実施形態では、本開示の医薬組成物は、経口投与される。液体(溶液、懸濁液(水性または油性)、およびエマルション)、半固体(ペースト)、フィルムおよび固体(錠剤、トローチ剤、カプセル剤、散剤、結晶および顆粒剤)を含む様々な投与形が企図される。
経口投与用の液体分散剤は、シロップ、エマルションおよび懸濁液であり得る。シロップは、担体、例えば、サッカロースまたはサッカロースをグリセリンおよび/またはマンニトールおよび/またはソルビトールとともに含有し得る。

単一可変ドメインを投与するための有効用量および処置計画は、患者の年齢、体重および健康状態、ならびに処置される疾患などの因子によって決まり得る。このような因子は担当の医師の裁量の範囲内である。適当な用量を選択する指針は、例えば、Smith et al (1977) Antibodies in human diagnosis and therapy, Raven Press, New Yorkに見出すことができる。
本開示の組成物中の単一可変ドメインとカモスタットメシル酸塩の比は、約1:0.1、1:1、1:10、1:25、1:50、または1:100であり得る。一実施形態では、本開示の組成物中の単一可変ドメインとカモスタットメシル酸塩の比は、約1:100である。一実施形態では、本開示の組成物中の単一可変ドメインとカモスタットメシル酸塩の比は、約1:10である。
本医薬組成物は、他の薬剤を伴う単一可変ドメインのパーツキットを場合により使用説明書とともに含んでなり得る。好都合には、本キットは、所定量の試薬を使用説明書とともに含んでなり得る。
本開示は、本開示の組成物をそれを必要とする患者に投与する工程を含んでなる、本明細書に開示される疾患の処置方法を提供する。
本開示はまた、本明細書に挙げられている疾患および障害の処置用薬剤の製造における、本明細書に記載されるような本開示の組成物の使用を提供する。本開示の組成物により処置可能な疾患および障害には、胃腸管障害が含まれる。
「胃腸管障害」は、GI管を侵す障害であり、腸炎、直腸炎、クローン病を含む炎症性腸疾患(IBD)、潰瘍性大腸炎を含む大腸炎、セリアック病、ベーチェット症候群(Behet’s syndrome)および口腔粘膜炎が含まれる。一実施形態では、胃腸管障害はIBDである。一実施形態では、胃腸管障害はクローン病である。一実施形態では、胃腸管障害は潰瘍性大腸炎である。
GI管を標的とすることにより処置され得る他のいずれの疾患も、本開示の方法により処置される疾患の範囲内に包含される。例えば、GI管内の標的に結合する本開示の単一可変ドメインは、GI管を超える効果、および全身性疾患の治療をもたらし得る。
用語「個体」、「被験体」および「患者」は、本明細書互換的に使用される。被験体は一般にヒトである。被験体はまた、マウス、ラットまたは霊長類(例えば、マーモセットまたはサル)などの哺乳類でもあり得る。被験体は、非ヒト動物であり得る。
処置は、治療的、予防的または防止的であり得る。被験体は、それを必要とするものである。処置を必要とするものとしては、特定の医学的疾患にすでに罹患している個体に加え、将来その疾患を発症する可能性のある個体を含み得る。本明細書に記載の単一可変ドメインの治療上有効な量は、その疾患の1以上の症状を改善もしくは軽減する、またはその疾患を予防もしくは治癒するために有効な量である。
プロテアーゼ豊富な溶液中で単一可変ドメインを安定化させる方法が提供される。この方法は、カモスタットメシル酸塩を含んでなる組成物中に単一可変ドメインを配合した後にその組成物をプロテアーゼ豊富な溶液に曝すことを含んでなる。
「プロテアーゼ豊富な」溶液は、プロテアーゼ、特に、GI管内に見られるプロテアーゼを、例えば、生理量で含んでなる溶液である。プロテアーゼは、ポリペプチド鎖の1以上のペプチド結合を加水分解することによってタンパク質分解を行う酵素である。ヒトでの空腹期トリプシンの生理量は20〜50U/mlである。ヒトでの食後早期トリプシンの生理量は、60〜100U/mlである。ヒトでの食後後期トリプシンの生理量は、500〜1500U/ml(McConnell et al., International Journal of Pharmaceutics 364: 213-226 (2008))。一実施形態では、プロテアーゼ豊富な溶液中のトリプシン量は、上述の範囲のいずれであってもよい。一実施形態では、プロテアーゼ豊富な溶液は、トリプシンを下記の量:20U/ml、30U/ml、40U/ml、50U/ml、60U/ml、70U/ml、80U/ml、90U/ml、100U/ml、200U/ml、300U/ml、400U/ml、500U/ml、600U/ml、700U/ml、800U/ml、900U/ml、1000U/ml、1100U/ml、1200U/ml、1300U/ml、1400U/mlまたは1500U/mlのいずれかよりも高い量で含んでなる。一実施形態では、プロテアーゼ豊富な溶液は、キモトリプシンおよび/またはパンクレアチンをさらに含んでなり得る。一実施形態では、プロテアーゼ豊富な溶液は、トリプシン、キモトリプシンおよび/またはパンクレアチンを含んでなる。一実施形態では、プロテアーゼ豊富な溶液は、人工腸液(SIF)である。SIFは、胆汁、パンクレアチンおよびトリプシンを含んでなる。SIFはまた、塩化ナトリウム、塩化カリウムおよび塩化カルシウムも含んでなり得る。一実施形態では、SIFは、実施例に記載のように、例えば、プロテアーゼを実施例1に明示される量で含んでなる。
本明細書内で、本開示は、実施形態に関して、明瞭かつ簡潔な明細を記載できる方法で記載してきた。実施形態は本開示から逸脱することなく、様々に組み合わせたり分離したりし得ることが意図され、理解されるべきである。
実施例1:人工腸液(SIF)中でのドメイン抗体(商標)パネルの固有安定性
人工腸液(SIF)は、下記のように容量を実質的にスケールダウンしたこと以外は、TNO−TIM(商標)胃腸管モデル系に使用される処方箋に基づいて処方した。

人工腸液(SIF)の調製:
胆汁溶液は、2.0g(+/−0.02g)の胆汁粉末を250g(+/−5g)の精製水に、透明な溶液が得られるまで、絶えず撹拌しながら徐々に加えることにより調製した。

パンクレアチン溶液は、2.1g(+/−0.2g)のパンクレアチン粉末を150g(+/−3g)の精製水に加えることにより調製した。スターラーを用い、泡立ちを最小限にするように留意した。均質な混合物が得られたところで、溶液を3500rpmで20分間遠心分離した後、上清を氷上で保存した。
小腸電解質溶液(SIES)25%(濃縮液)は、精製水を250g(+/−5g)の塩化ナトリウム、30g(+/−0.5g)の塩化カリウム、および15g(+/−0.3g)の脱水塩化カルシウムを加えて合計2174gとすることにより作製した。これらの塩が溶けたところで、pHを1M水酸化ナトリウムでpH7.0(+/−0.5)に調整した。
次に、SIES希釈液を、43.5(+/−1g)のSIES濃縮液を用いて総重量1000gとなるまで精製水に加えて調製した。
トリプシン溶液は、200mg(+/−5mg)のトリプシンを100g(+/−2g)のSIES希釈液に溶かすことにより調製した。次に、この溶液を1.5mlのエッペンドルフ管(1ml/管)にピペットで移し、−20℃で冷凍した。
次に、SIFを、25g(+/−0.3g)の胆汁溶液、12.5g(+/−0.3g)のパンクレアチン溶液および12.5g(+/−0.5g)のSIES希釈液(2:1:1比の胆汁/パンクレアチン/SIES希釈液)を混合することにより調製した。その後、溶液使用する直前に1mlのトリプシン溶液を加えた。
ドメイン抗体(商標)の調製
検討下のドメイン抗体(商標)(dAb(商標))を、Vivaspin(商標)500 3kD MWCOカラムを用いておよそ20mg/mlまで濃縮した。サンプル回収率を最大にするためにカラムを使用前にPBSで予備洗浄した。濃度は、Nanodrop(商標)により、モル吸光係数および分子量オプションを用いて確認した。
検討下のドメイン抗体(商標)(dAb(商標))を、Vivaspin(商標)500 3kD MWCOカラムを用いておよそ20mg/mlまで濃縮した。サンプル回収率を最大にするためにカラムを使用前にPBSで予備洗浄した。濃度は、Nanodrop(商標)により、モル吸光係数および分子量オプションを用いて確認した。
反応アセンブリ
SIF中でのdAb(商標)のインキュベーションは、終容量250μlで行った。混合物中に添加されたdAb(商標)の容量は、終濃度1mg/mlとなった。

25μlのアリコートをすぐに取り出し、ドライアイス上で保存した(0時点)。反応混合物を振盪しながら(100rpm)37℃でインキュベートした。次に、0.25時間、0.5時間、1時間、2時間、4時間、6時間および一晩で25μlアリコートを取り出した。サンプルをドライアイス上で急速冷凍し、分析まで−80℃で保存した。
SDS−PAGE分析
種々の時点でSIFに残ったdAb(商標)の量をSDS−PAGEおよび濃度計により測定した。簡単に述べれば、サンプルを水とサンプルローディングバッファーの混合物で1/10希釈し、5分間80℃に加熱した。サンプルを急冷した後、10μlを調製済みの標品(水中dAb(商標))および分子量マーカーとともに、4〜12%Novex(商標)ビス−トリスゲルにのせた。このゲルを1×MESバッファー中、150V一定で45分間泳動させ、Instant Blue(商標)で一晩染色することによりタンパク質バンドを可視化した。得られたバンドの濃度測定は、Odyssey Li−Cor(商標)ゲルイメージングシステムを用いて行い、存在するdAb(商標)の量を0時点のバンド(出発量)の濃度と比較して計算した。dAb(商標)の出発量のパーセンテージに対する時間の指数曲線を作成し、dAb(商標)の開始量の50%が存在する時間を半減期と見なした。

上記の方法を使用し、表1に示されるように、遷移中間点(Tm)の異なるdAb(商標)のパネルをSIF中でインキュベートし、SDS−PAGEおよび濃度計により分析した。これらの例に関して、高Tm dAb(商標)はTm≧66℃のdAb(商標)を意味し、低Tm dAb(商標)はTm≦56℃のdAb(商標)を意味する。
結果を図1に示す。このグラフは、異なる3日に行ったSIF試験を組み合わせたものである。DOM4、最高TmのdAb(商標)は、明らかに、検討下の他のdAb(商標)よりもはるかに安定であった。これが閾値であるかどうかを調べるために、表2に示されるように、さらに4つの高Tm dAb(商標)を上記の方法を用いて試験した。
高Tm dAb(商標)のパネルの結果を図2に示す。
他のdAb(商標)のうちDOM8は、SIF中で極めて安定であった。他の3つのdAb(商標)は安定ではなかった。しかしながら、試験したこれら5つの高Tm dAb(商標)のうち4つは、Tm66℃未満のdAb(商標)より安定であった。最も安定な2つのdAb(商標)(DOM4およびDOM8)は両方ともVκフレームワークを有していた。しかしながら、DOM11もVκフレームワークを有していたが安定性ははるかに低かったことから、このフレームワークは安定性上、それほど重要ではない可能性がある。DOM11は、ここで試験した他の3つの高Tm dAb(商標)とは異なる機会にSIF中でインキュベートした。
実施例2:カモスタットメシル酸塩を用いたin vitroドメイン抗体(商標)の安定化
実施例1で試験したdAb(商標)のパネルをまた、SIF中、カモスタットメシル酸塩(CM、Sequoia Research Products)の存在下でもインキュベートし、プロテアーゼの阻害がdAb(商標)をさらに安定化させる助けとなるかどうかを調べた。SIF調製の節に上述した電解質溶液にCMを350mg/mlの濃度で加え(CMは高濃度ではあるが、飽和点未満であった)、50℃に温めて溶解させた。CMを終濃度10mg/mlでSIF/dAb(商標)に加えた。使用した時点とその後の分析を実施例1の通りに行った。
実施例1で試験したdAb(商標)のパネルをまた、SIF中、カモスタットメシル酸塩(CM、Sequoia Research Products)の存在下でもインキュベートし、プロテアーゼの阻害がdAb(商標)をさらに安定化させる助けとなるかどうかを調べた。SIF調製の節に上述した電解質溶液にCMを350mg/mlの濃度で加え(CMは高濃度ではあるが、飽和点未満であった)、50℃に温めて溶解させた。CMを終濃度10mg/mlでSIF/dAb(商標)に加えた。使用した時点とその後の分析を実施例1の通りに行った。
結果は、図3および4に、比較のために実施例1からの結果の横に示す。SIF/dAb混合物へのCMの添加は、試験したdAb(商標)のうち1つ以外の総ての半減期を延長した。この半減期の延長は、試験した総ての分子で同じではなく、このことはdAb(商標)の固有の特性がそれらの安定化能に寄与することを示唆している。さらに、試験した高Tm dAb(商標)の総てで半減期は24時間を超えるまでに安定化したことから、高Tm dAb(商標)は、それらの様々な半減期にも関わらず、CMによる安定化に本質的により従順であるものと思われる。
実施例3:dAb(商標)安定性のモデル化およびドメイン抗体(商標)の固有安定性に対するTmの重要性
トリプシンおよびキモトリプシン(パンクレアチン中に存在)切断部位のタンパク質配列をスキャンするためにperlスクリプトを欠いた。次に、半減期を推定切断部位と、またTmと相関させた。

弱い正の相関がTmと半減期の間に見られた(Spearman、0.58;Pearson、0.31)。しかしながら、両相関測度を用い、CMの存在下でTmと半減期の間に強い正の相関が見られた(Spearman、0.78;Pearson、0.90)。このことは、Tmが高いほどdAb(商標)はCMによる安定化に従いやすいことを示唆する。推定切断部位と半減期の間には、CMの存在下でも不在下でも明らかな相関は見られなかった。
モデル化プロセス中、2つのVκフレームワークdAb(商標)は、推定トリプシン切断部位は同じであるが、SIF中での半減期が異なりTmも異なることが見出された。これらはDOM4(半減期6.1時間、Tm72.8℃)およびDOM1(半減期0.1時間、Tm55℃)であった。これら2つのdAb(商標)を、SIFの場合に用いたものと同じ濃度で、胆汁酸塩もパンクレアチンも含まずに、トリプシンとともにインキュベートした。半減期に見られる違いは、この場合にはTmによるものであろう。トリプシン/dAb(商標)混合物にCMもまた加えた。半減期を従前のように算出し、結果を図5に示す。
トリプシン単独の存在下では、DOM4の半減期は、DOM1の半減期よりもかなり長かった。この場合、Tmの違いは、おそらく分子の安定性の増強によるものである。
実施例4:カモスタットメシル酸塩の使用は、絶食させたハン・ウェスターラットの十二指腸に直接投与したTNFR1特異的dAb(商標)DOM101(配列番号6)を安定化させる
ハン・ウィスターラットに100mgCMを含むまたは含まない1mgのDOM101を投与し、CMが胃腸管内でdAb(商標)を保存するかどうかを調べた。簡単に述べれば、ラットをイソフルラン麻酔薬によって麻酔し、投与処方物の直接十二指腸内注射(500μl)のために十二指腸の位置決定を助けるために正中腹部切開を行った。投与後、腹部切開を閉じ、ラットを回復させた後に、試験ケージに戻した。十二指腸への直接投与は、胃内の胃液の酸性条件を迂回し、腸管の薬物動態の直接的分析を可能とした。
ハン・ウィスターラットに100mgCMを含むまたは含まない1mgのDOM101を投与し、CMが胃腸管内でdAb(商標)を保存するかどうかを調べた。簡単に述べれば、ラットをイソフルラン麻酔薬によって麻酔し、投与処方物の直接十二指腸内注射(500μl)のために十二指腸の位置決定を助けるために正中腹部切開を行った。投与後、腹部切開を閉じ、ラットを回復させた後に、試験ケージに戻した。十二指腸への直接投与は、胃内の胃液の酸性条件を迂回し、腸管の薬物動態の直接的分析を可能とした。
動物は、下記の時点:0.5、1.5、3、5、7および18時間で屠殺した(3匹/群)。
血液サンプルを採取し、腸管を摘出し、その構成部分:十二指腸(×2)、空腸(×6)、回腸、盲腸、結腸(×2)、直腸に分けた。
GentleMACS(商標)分離装置を用い、洗剤およびプロテアーゼ阻害剤を含有する溶解バッファー中で腸管サンプルをホモジネートした。TNFR1特異的MSD(商標)アッセイを用い、サンプルをDOM101に関してスクリーニングした。要するに、MSDプレートをTNFR1−Fcでコーティングした。プレートを洗浄し、ウシ血清アルブミンでブロッキングした。組織サンプルを希釈し、dAb(商標)の標準曲線とともにプレートに加えた後、室温でインキュベートして結合させた。プレートを洗浄し、スルホタグ結合抗Vh抗体を各ウェルに加えた。インキュベーション後、プレートを洗浄し、MSDリードバッファーでインキュベートした。得られた電気化学的発光シグナルをSector Imager 6000で読み取った。
結果を図6に組織1グラム当たりのナノグラムとして表す。
図6(a)のCMの不在下、dAb(商標)は、十二指腸では0.5時間にのみ、空腸では1.5時間までのみ検出できた。最高量は空腸で検出でき、回腸では少量で検出できるに過ぎなかった。
図6(b)のCMの存在下では、dAb(商標)は、投与7時間後にはGI管の至る所で検出可能であった。dAb(商標)は、後期の時点では、回腸、盲腸、結腸および直腸で検出できただけであった。従前のように、最高量のdAb(商標)は空腸から回収された。胃腸管移行の見込みにも関わらず、dAb(商標)は7時間目に十二指腸および空腸でも検出可能であり、dAb(商標)が胃腸管組織へ浸透したこと示唆された。十二指腸内投与後、DOM101は血漿中に低レベル(総用量の0.1%未満)で検出可能であり、dAb(商標)が組織へ浸透可能であることが確認された(データは示されていない)。
実施例5:カモスタットメシル酸塩の使用は、絶食させたハン・ウィスターラットの結腸に直接投与したTNFR1特異的dAb(商標)DOM101(配列番号6)を安定化させる
ハン・ウィスターラットに、CMを含むまたは含まない1mgのDOM101を投与し、カモスタットメシル酸塩が大腸管でもdAb(商標)を保存するかどうかを調べた。簡単に述べれば、ラットをイソフルラン麻酔薬によって麻酔し、結腸の位置決定を助けるために正中腹部切開を行い、500μl用量の投与処方物を直接結腸に注射した。投与後、腹部切開を粗く閉じ、イソフルラン麻酔下でラットを維持し、試験期間中モニタリングした。本実施例では、動物1匹当たり100mg(実施例4の通り)および10mgの2種類のCM用量を試験した。
ハン・ウィスターラットに、CMを含むまたは含まない1mgのDOM101を投与し、カモスタットメシル酸塩が大腸管でもdAb(商標)を保存するかどうかを調べた。簡単に述べれば、ラットをイソフルラン麻酔薬によって麻酔し、結腸の位置決定を助けるために正中腹部切開を行い、500μl用量の投与処方物を直接結腸に注射した。投与後、腹部切開を粗く閉じ、イソフルラン麻酔下でラットを維持し、試験期間中モニタリングした。本実施例では、動物1匹当たり100mg(実施例4の通り)および10mgの2種類のCM用量を試験した。
動物を0.5および3時間で屠殺した(各時点3匹)。血液サンプルを採取し、腸管を摘出し、以下のような構成部分に分けた:盲腸、結腸(×2)、直腸。
実施例4で述べたように、サンプルをホモジネートし、スクリーニングした。結果を図7に組織1グラム当たりのナノグラムとして表す。
高レベルのdAb(商標)が、盲腸、結腸および直腸(10mgカモスタット群を除く)において、CMの存在下または不在下の0.5時間で検出できた。GI管の下部ほど消化酵素のレベルが低いということで、これを説明できる。10mgカモスタット群における直腸の0.5時間でのdAb(商標)の欠如は、おそらくこれらの動物の盲腸の湿重が高いことによると思われ(データは示されていない)、従って、dAb(商標)はこの切片にも保持されている可能性がある。しかしながら、CMの不在下での3時間時点のdAb(商標)レベルは、特に盲腸および直腸では、2つのCM群で見られたものに比べて実質的に低かった。低用量のCM(10mg)ほど、広範なGI管において保存するdAb(商標)が高用量であるので有効であると思われた。
実施例1〜5の要約
これらの実施例は、カモスタットメシル酸塩とドメイン抗体(商標)の併用投与がこれらの分子の経口送達の新規なプラットフォームとして使用可能であることを実証する。in vitroで試験した11種のdAb(商標)のうち10種がCMの添加により様々な程度で安定化された。in silicoでモデル化したところ、CMの存在下での半減期とTmの間に強い相関が見られ、Tmが高いほど、dAb(商標)はCMによる安定化に従いやすいことが示唆される。同一の推定トリプシン切断部位を有する2種類のdAb(商標)の比較でも、SIF中でのdAb(商標)の固有安定性に関するTmの重要性が示される。

in vitroにおける結果は、in vivo試験によって裏付けられ、カモスタットメシル酸塩とDOM101の併用投与は、十二指腸に送達した場合でも結腸に送達した場合でも、GI管から回収可能なdAb(商標)の量を実質的に増す。CMの処方物への添加は、クローン病または潰瘍性大腸炎などの胃腸管病態の治療を目的とするdAb(商標)の十二指腸または結腸への局所送達を可能とするはずである。

Claims (17)
- カモスタットメシル酸塩と単一可変ドメインとを含んでなる組成物。
- 医薬組成物である、請求項1に記載の組成物。
- 経口投与される、請求項1または請求項2に記載の組成物。
- 単一可変ドメインが抗標的単一可変ドメインであり、標的がTNFα、IL−23、LAG−3、IL−6、IL−13、IL−18、TSLP、CD3、前記のいずれか1つの受容体またはELR受容体である、請求項1〜3のいずれか一項に記載の組成物。
- 単一可変ドメインがTNFα、IL−23、LAG−3、IL−6、IL−13、IL−18、TSLPまたはCD3を中和する、請求項1〜4のいずれか一項に記載の組成物。
- 単一可変ドメインが約66℃以上の遷移中間点(Tm)を有する、請求項1〜5のいずれか一項に記載の組成物。
- 単一可変ドメインとカモスタットメシル酸塩の比が約1:0.1、1:1、1:10、1:25、1:50または1:100である、請求項1〜6のいずれか一項に記載の組成物。
- 腸溶コーティングされる、請求項1〜7のいずれか一項に記載の組成物。
- 薬剤として使用するための請求項1〜8のいずれか一項に記載の組成物。
- 薬剤の製造のための請求項1〜9のいずれか一項に記載の組成物の使用。
- 薬剤が胃腸管病態を処置するためのものである、請求項9に記載の組成物または請求項10に記載の使用。
- 胃腸管病態がクローン病、潰瘍性大腸炎を含む大腸炎、セリアック病、ベーチェット症候群および口腔粘膜炎である、請求項11に記載の組成物または使用。
- 請求項1〜8のいずれか一項に記載の組成物をそれを必要とする患者に投与する工程を含んでなる、胃腸管病態を処置する方法。
- プロテアーゼ豊富な溶液中で単一可変ドメインを安定化させる方法であって、カモスタットメシル酸塩を含んでなる組成物中に単一可変ドメインを配合した後にその組成物をプロテアーゼ豊富な溶液に曝すことを含んでなる、方法。
- 単一可変ドメインとカモスタットメシル酸塩の比が約1:0.1、1:1、1:10、1:25、1:50または1:100である、請求項14に記載の方法。
- プロテアーゼ豊富な溶液が絶食下の人工腸液である、請求項14または15に記載の方法。
- プロテアーゼ豊富な溶液が、トリプシン、キモトリプシンおよび/またはパンクレアチンを含んでなる溶液である、請求項14または15に記載の方法。
