本明細書に記載されるバイオマーカーは、メタボロームプロファイリング技術を使用して発見された。そのようなメタボロームプロファイリング技術は、以下に示される実施例において、ならびに米国特許第7,005,255号および第7,329,489号および米国特許第7,635,556号、米国特許第7,682,783号、米国特許第7,682,784号、および米国特許第7,550,258号により詳細に記載され、それらのすべての全内容は、参照により本明細書に組み込まれる。

本明細書に提供されるバイオマーカーはまた、疾患または状態を治療するのに有用であり得る、ベータ細胞機能不全等の疾患または状態と関連するバイオマーカーを調節することにおける活性に関して、組成物のスクリーニングも可能にする。例示的な方法は、表4に記載される1つ以上のバイオマーカーのレベルを調節することにおける活性に関する試験化合物をアッセイすることを含む。スクリーニングアッセイは、インビトロ(例えば、ベータ細胞アッセイ)、および/またはインビボで実施してもよく、試験組成物の存在下でのそのようなバイオマーカーの調節をアッセイするのに有用な当技術分野で知られている任意の形態でもよい。例示的なアッセイ方法は、例えば、細胞培養アッセイ、器官培養アッセイ、およびインビボアッセイ(例えば、動物モデルを必要とするアッセイ)を含む。別の例示的な方法は、2−ヒドロキシ酪酸塩またはリノレオイル−LPCのレベルを調節することにおける活性に関して、試験化合物をアッセイすることを含む。
別の態様において、ベータ細胞機能障害等の疾患または状態を治療するための方法が提供される。本方法は、概ね、有効量のL−LPC、または疾患または状態を有しない健常な対象と比較して、疾患または状態を有する対象において低下する1つ以上のバイオマーカー(複数可)を用いて、ベータ細胞機能障害等の疾患または状態を有する対象を治療することを含む。投与され得るバイオマーカーは、L−LPC、またはL−LPC、およびその疾患または状態を有しない対象と比較して、疾患または状態の状況において減少する表4の1つ以上のバイオマーカーを含み得る。そのようなバイオマーカーを、バイオマーカー化合物の識別(即ち、化合物名)に基づいて単離することが可能である。
本明細書で論じられるバイオマーカーを用いる他の方法もまた、企図される。例えば、米国特許第7,005,255号、第7,329,489号、第7,550,258号、第7,550,260号、第7,553,616号、第7,635,556号、第7,682,782号、および第7,682,784号に記載される方法は、本明細書に開示されるバイオマーカーのうちの1つ以上を含む小分子プロファイルを用いて実施することができる。
実施例
A.代謝プロファイルの特定
それぞれのサンプルを分析して、数百の代謝産物の濃度を測定した。GC−MS(ガスクロマトグラフィー−質量分光測定)およびLC−MS(液体クロマトグラフィー−質量分光測定)等の分析技術を使用して、代謝産物を分析した。複数のアリコートを同時に、かつ並行に分析し、適切な品質管理(QC)の後、それぞれの分析から導かれた情報を再統合した。最終的には数百の化学種になる数千の特性に応じてすべてのサンプルを特徴付けた。使用される技術は、新規で化学名称のない化合物を特定することが可能であった。
いくつかの統計的手法を使用してデータを解析して、確定可能な集団間(例えば、インスリン抵抗性および対照、インスリン抵抗性およびインスリン感受性、インスリン抵抗性および2型糖尿病)で区別するのに有用である、確定可能な集団または小集団(例えば、対照生体サンプルまたはインスリン感受性患者と比較したインスリン抵抗性生体サンプルに対するバイオマーカー)に異なるレベルで存在する分子(既知の名前付き代謝産物または無名の代謝産物)を特定した。確定可能な集団または小集団における他の分子(既知の名前付き代謝産物または無名の代謝産物)も特定した。
ベータ細胞機能は、正常な耐糖能(NGT)を有する対象において最も高く、耐糖能異常(IGT)を有する対象においてより低く(即ち、障害を来たす)、2型糖尿病(T2D)を有する対象において最も低い。ベータ細胞機能のバイオマーカーは、NGT、IGT、およびT2Dの対象からの血漿サンプルの分析から特定された。対象は、2つの研究群のうちの1つの群の参加者から選択された。第1の対象群(群1)は、以下の試験対象患者基準(性別、30〜60齢、臨床的に健常である)に従って、ヨーロッパ13カ国にある19の施設で採用された予想される観察コホートから選択され(Hills et al.Diabetologia(2004))、10歳群に従って性別および年齢別に階層化した。初期除外基準は、肥満、高血圧、脂質障害もしくは糖尿病、妊娠、心臓血管疾患もしくは慢性肺疾患、先月内で5kg以上の体重変化、癌(過去5年間)、および腎不全の治療であった。スクリーニング後の除外基準は、動脈圧≧140/90mmHg、空腹時血漿グルコース>7.0mmol/L、2時間血漿グルコース(標準75gの経口グルコース負荷試験[OGTT]において)≧11.0mmol/L、血清総コレステロール≧7.8mmol/L、血清トリグリセリド≧4.6mmol/L、およびECG異常であった。これらの群1の対象のベースライン試験は、2002年に開始し、2005年に完了した。3年間のフォローアップ検査は、2005年に開始し、2008年に終了した。
データは、中央値および[四分位範囲]および平均値および標準偏差として与えられる(表1〜3)。多重線形回帰、相関、およびロジスティック回帰分析を、未変換データおよび対数変換データにおいて実行した。多重線形回帰は、空腹時血漿グルコースおよび食後2時間のOGTTグルコース結果によって定義されるように、糖代謝異常(グルコース調節異常、IGR、または2型糖尿病、T2D)への進行を有する代謝産物の独立した関連性を試験した。結果を、オッズ比および95% 信頼区間(C.I.)として示す。統計的分析を、JMP(JMP、バージョン8.SAS Institute Inc.,Cary,NC,1989−2009)を用いて実施した。
試験を行った群1の参加者間で、ベースライン測定から3年後、経口グルコース負荷は、依然として、779人の対象(安定したNGT)において正常であり、123人の対象(進行者)において悪化した。ベースラインで、進行した対象は、ベータ細胞グルコースの感受性がさらに低くなり、血漿2−HBがより高く、LGPC濃度がより低くなった(表2A)。フォローアップ後、2−HBレベルは、安定したNGTの対象において減少し(0.38[2.01]μg/mL)、進行者において増加し(0.42[2.12]μg/mL)、差異は、p=0.0003のレベルで有意であった。対照的に、LGPCレベルは、安定したNGTにおいて増加し(0.83[6.4]μg/mL)、進行者において減少し(0.2[4.8]μg/mL)、この差異は、統計的に有意であった(p<0.05)(表2A)。

研究は、実施例1および2に記載される3つの独立したコホートにおいて実施され、ベータ細胞機能と関連するバイオマーカーが現在使用されるベータ細胞機能指数に対してサロゲートを提供することができるかどうかを判定した。これらの研究において、バイオマーカーAHBおよびLGPCは、現在利用可能なベータ細胞指数:ベータ細胞グルコース感受性測定(pmol.min−1.m−2.mM−1)、インスリン分泌指数(0〜30分および0〜120分でのインスリン濃度の変化(ΔI)(AUC)をグルコース濃度の変化(ΔG)(AUC)で割ったものとして計算される)、またはIS/IRの処分指数(OGTT時、インスリン分泌/インスリン抵抗性(処分)指数(ΔI0−120/ΔG0−120×Matsuda指数)(MI)として計算される、ΔISR0−120/ΔG0−120×MI)によって測定されるベータ細胞機能と相関することを示す。
ベータ細胞バイオマーカーを用いた肥満手術後のベータ細胞機能の変化の監視
治療的介入後のベータ細胞機能の変化を監視するためのベータ細胞機能のバイオマーカーの有用性を示すために、バイオマーカーレベルを、2型糖尿病の治療のために治療的肥満外科手術を受けた対象において測定した。
別の研究では、薬物療法後のIS/IRの処分指数の変化とベータ細胞機能バイオマーカーAHBおよびLGPCのレベルの変化との間の関係が判定された。研究コホートは、ピオグリタゾン(45mg/日)またはプラセボに無作為化され、2.4年間追跡された431人のIGTの対象(FPG=105、2時間PG[OGTT]=168、HbA1C=5.5%)から成った。AHB、LGPC、インスリン、HbA1c、およびMatsuda指数に対して、ベースラインおよび研究終了時に、空腹時血漿サンプルを分析した。AHBレベルの変化は、薬物療法に応答した患者に対してPIOに応答して減少し、応答せず、2型糖尿病になった対象において増加した。L−GPCのレベルは、薬物療法に応答した患者に対してPIOに応答して増加し、応答せず、2型糖尿病になった対象において減少した。データを表3に示す。Matsuda指数との相関は、AHBに対して−0.178(p<0.005)であり、LGPCに対して0.127(p=0.009)であった。
EDTAヒト血漿中の、表4に記載されるバイオマーカーのそれぞれを測定するための方法を展開した。ヒト血漿サンプルを内部標準でスパイクし、以下に記載されるようにタンパク質沈殿させた。遠心分離後、上清を除去し、4つの異なるクロマトグラフィーシステム(カラム/移動相の組み合わせ)を使用してWaters Acquity/Thermo Quantum Ultra LC−MS−MSシステムに注入した。

細胞培養
親ラットインスリノーマINS−1細胞株(University of Geneva,Geneva,Switzerland)から由来し、選択されるクローンINS−1e細胞を、11.1mMのグルコースを含有するRPMI1640培地中の単層培養において増殖した。培養培地に、10%の加熱不活性化ウシ胎仔血清、1mMのピルビン酸ナトリウム、10mMのHEPES、2mMのグルタミン、50μmol/リットルのβ−メルカプトエタノール、100U/mLのペニシリン、および100μg/mLのストレプトマイシンを添加した。細胞を、加湿した95%空気−5%CO2雰囲気下で、37℃で培養した。インスリン分泌実験において使用する少なくとも96時間前に、細胞を、1.5×105細胞/ウェルの密度でウェルに播種した。組織培養試薬は、Life Technologies,Inc.Invitrogen(Basel,Switzerland)から得た。

- 前記1つ以上のバイオマーカーは、2−ヒドロキシ酪酸塩および/またはリノレオイルリゾホスファチジルコリンを含む、請求項13に記載の方法。
- 治療的介入に応答した膵ベータ細胞機能障害の進行または退行を監視する方法であって、
対象からの生体サンプルを分析して、表4から選択される1つ以上のバイオマーカーのレベル(複数可)を決定することと、
前記1つ以上のバイオマーカーのレベルを決定することと、
前記対象に治療処置を施すことと、
治療的介入に応答した前記対象における膵ベータ細胞機能障害の進行または退行を監視するために、前記サンプル中の前記1つ以上のバイオマーカーのレベル(複数可)を、前記1つ以上のバイオマーカーの膵ベータ細胞機能障害の進行および/または膵ベータ細胞機能障害の退行の基準レベルと比較することと、を含む、方法。 - 前記1つ以上のバイオマーカーは、2−ヒドロキシ酪酸塩および/またはリノレオイルリゾホスファチジルコリンを含む、請求項16に記載の方法。
- 前記治療的介入は、外科的介入、薬物治療介入、およびライフスタイルの介入からなる群から選択される、請求項16に記載の方法。
