Abstract
Description
The present invention relates to a method for producing a sterilized infusion preparation.
Patients who are unable or inadequate for oral and enteral feeding are given high-calorie infusions via the intravenous vein. As infusion preparations used at this time, sugar preparations, amino acid preparations, electrolyte preparations, mixed vitamin preparations, fat emulsions and the like were commercially available, and were used by being appropriately mixed at the hospital at the time of use depending on the disease state etc. . However, such a mixed injection operation in a hospital is not only complicated, but also has a problem that the possibility of bacterial contamination is high at the time of the mixing operation and it is unsanitary. For this reason, a chemical solution containing sugar, amino acids, electrolytes, vitamins, etc. is stored separately in a plurality of chambers partitioned by a partition wall that can be communicated, and can be administered after opening the partition wall and mixing at the time of use. The formulation is used.
Vitamins blended in such general infusion preparations are broadly classified into water-soluble vitamins and fat-soluble vitamins, but fat-soluble vitamins are insoluble in water by themselves, so they can be used in water when stored in infusion preparations. It is necessary to make it a solubilized and fat-soluble vitamin-solubilized preparation. Surfactants are generally used for solubilizing fat-soluble vitamins, and nonionic surfactants having polyoxyethylene chains are frequently used as surfactants that can be used intravenously.
By the way, a nonionic surfactant having a polyoxyethylene chain, when dissolved in water, the hydrophilic oxygen is outward, the hydrophobic group is inward, and the water molecule is loosely bonded to the oxygen atom of the outer ether bond by a hydrogen bond. It becomes a state to do. When a chemical solution containing a nonionic surfactant is heated, water molecules that have been loosely bonded are peeled off and the hydrophilicity gradually decreases. When a certain threshold is exceeded, the entire solution suddenly becomes cloudy. This is a phenomenon called a cloud point phenomenon, and the temperature at this time is called a cloud point.
When a chemical solution containing a nonionic surfactant having a polyoxyethylene chain, such as the above fat-soluble vitamin-solubilized preparation, is heated to a cloud point or higher, the cloud point phenomenon occurs to cause cloudiness. When the temperature is further increased, a haze-like step occurs. When the temperature is further increased, the nonionic surfactant having the most polymerization degree is completely dehydrated, aggregates and precipitates as particles (Patent Document 1).
In many cases, the temperature at the time of heat sterilization of an infusion preparation is 100 ° C. or higher, and in this case, most nonionic surfactants having a degree of polymerization are completely dehydrated in the chemical solution. Normally, when cooling to room temperature after heat sterilization, the aggregated particles generated at the time of heat sterilization become moire as it cools, and the moire precipitates produced at room temperature disperse and dissolve over time, and the chemical solution is It will eventually return to a transparent and uniform appearance. However, when the container containing the chemical solution has a complicated shape, a highly viscous precipitate is generated in a part of the container, and further, the precipitate may not be dissolved over time. Such a deposit is not harmful, but the deposit may contain a drug, which may result in the amount of drug required at the time of infusion administration not being administered to the patient. Moreover, since the infusion container is usually transparent or translucent, it may cause a problem in appearance.
In order to suppress the formation of such precipitates, a stabilizer such as sugar alcohols is blended in the chemical solution and the blending amount is appropriately set, or a stabilizer such as polyethylene glycol or propylene glycol is blended. A solution by a pharmaceutical treatment such as adjusting the cloud point to be higher than the sterilization temperature has been proposed (Patent Document 2).
However, when a chemical solution containing a nonionic surfactant containing a stabilizer such as polyethylene glycols or propylene glycol is sealed in a transparent plastic container, the content may decrease due to adsorption of the drug on the container Occurs. In particular, olefin polymers such as polypropylene and polyethylene do not contain any harmful components to the human body, are safe, chemically stable and have excellent chemical resistance, and are strong, transparent, heat-sealable, and communication ports ( It is widely used as a container for infusion preparations because it has excellent bonding properties for members such as port) and is inexpensive. However, when it is sealed in these containers, there is a problem of content reduction.
The object of the present invention is to prevent the formation of highly viscous precipitates when an infusion preparation having a compartment containing a chemical solution containing a nonionic surfactant having a polyoxyethylene chain is sterilized by heating and then cooled. And providing a method for producing a sterilized infusion preparation that does not require the addition of a stabilizer such as polyethylene glycols or propylene glycols, which may cause a problem of a decrease in drug content.

Hereinafter, the present invention will be described based on preferred embodiments of the present invention.
The method for producing an infusion preparation of the present invention comprises a soft bag having a peripheral portion, a cylindrical discharge port attached so as to be able to communicate with the inside of the soft bag, and a weakness for communication inhibition formed so as to surround the discharge port. A polyoxyethylene chain accommodated in the chemical mixture chamber for use formed by the seal portion, the weak seal portion for inhibiting communication, the discharge port, and the peripheral portion; A step of preparing an infusion preparation having an in-use mixing chemical solution containing a nonionic surfactant, and 70% or more of the in-use mixing chemical solution being accommodated in the discharge port internal space A sterilization step of performing the heat sterilization by installing the infusion preparation is characterized.
First, the process of preparing an infusion preparation is demonstrated based on an Example.
The
The soft bag 2 is made of a soft resin sheet. The flexible bag 2 is preferably formed into a cylindrical shape by an inflation molding method. The soft bag 2 is manufactured by various methods such as a T-die method, a blow molding method, a dry laminating method, a hot melt laminating method, a coextrusion inflation method, a coextrusion T die method, and a hot press method. But you can. And when the flexible bag 2 of FIG. 1 is manufactured by the inflation molding method, two side edges are formed by being bent in the manufacturing process. Moreover, you may form the bent side by lightly pressing as needed.
The soft bag 2 preferably has a water vapor barrier property. As the degree of water vapor barrier property, the water vapor permeability is preferably 50 g / m 2 · 24 hrs · 40 ° C. · 90% RH or less, more preferably 10 g / m 2 · 24 hrs · 40 ° C. · 90% RH or less. More preferably, it is 1 g / m 2 · 24 hrs · 40 ° C. · 90% RH or less. This water vapor permeability is measured by the method described in JISK7129 (Method A).
Thus, since the soft bag 2 has a water vapor barrier property, the evaporation of moisture from the inside of the
The soft bag 2 is preferably made of a resin having a certain degree of heat resistance. Examples of such resins include polyethylene, polypropylene, cyclic polyolefin, ethylene-α olefin copolymer such as ethylene-propylene copolymer, propylene-α olefin copolymer, a mixture of polypropylene and polyethylene or polybutene, or the like. Partially crosslinked products, ethylene-vinyl acetate copolymers, polyesters such as polyethylene terephthalate and polybutylene terephthalate, and soft vinyl chloride resins. Preferred are polyolefin resins such as polyethylene, polypropylene, and cyclic polyolefins. A resin composition that has been softened by adding various elastomers or the like as necessary as a component is used.
The soft bag 2 may have a single-layer structure (single-layer body) made of the material as described above, or a plurality of layers (particularly layers of different materials) stacked for various purposes. It may be a multilayer laminate. In the case of a multilayer laminate, a plurality of resin layers may be stacked, or a metal layer may be stacked on at least one resin layer.
The thickness of the sheet material (single layer or multilayer laminate) constituting the flexible bag 2 is appropriately determined according to the layer configuration and the characteristics of the material used (flexibility, strength, water vapor permeability, heat resistance, etc.). Although it is not particularly limited, it is usually preferably about 100 to 500 μm, more preferably about 200 to 360 μm.
Moreover, when the soft bag 2 uses a cylindrical film, the upper end side seal part 9 is provided in an upper end part, and the lower end
The partition
The partition

A known port can be used as the discharge port 6 attached to the lower end side of the soft bag 2. For example, as shown in FIG. 1, the tubular member 6a, the
The constituent material of the cylindrical member 6a and the
Moreover, the elastic member 6c of the discharge port 6 preferably has a self-occlusion property, and after the needle tube is extracted from the elastic body, the puncture hole thereof is closed to prevent the chemical liquid from leaking. Examples of the constituent material of the elastic member 6c include polyolefin such as polyethylene, polypropylene, ethylene-propylene copolymer, ethylene-vinyl acetate copolymer, and cross-linked ethylene-vinyl acetate copolymer, polyester, polyvinyl chloride, and polyurethane. , Flexible polymer materials such as polyamide, thermoplastic resins such as polyamide elastomer and polyester elastomer (thermoplastic elastomer), natural rubber, isoprene rubber, silicone rubber, butadiene rubber, various rubber materials such as styrene-butadiene rubber, etc. The elastic material, or a combination of any two or more of these, may be mentioned, and those containing an elastic material are preferable from the viewpoint of piercing property and reocclusion property.

These amino acids do not necessarily need to be used in the form of free amino acids, but are inorganic acid salts (for example, L-lysine sulfite, L-lysine hydrochloride, L-cysteine hydrochloride (monohydrate), etc.), organic Acid salts (for example, L-cysteine malate, L-lysine acetate, L-lysine malate, etc.), ester bodies that can be hydrolyzed in vivo (for example, L-tyrosine methyl ester, L-methionine methyl ester) , L-methionine ethyl ester, etc.), N-substituted products (for example, N-acetyl-L-tryptophan, N-acetyl-L-cysteine, N-acetyl-L-proline, etc.) and the like. In addition, dipeptides obtained by peptide bonding of the same or different amino acids (for example, L-tyrosyl-L-tyrosine, L-alanyl-L-tyrosine, L-arginyl-L-tyrosine, L-tyrosyl-L-arginine, etc.) You may use in the form of. The amino acid content can be appropriately determined according to the purpose of use such as the administration route, but after mixing all the solutions of the infusion preparation, which is the sterilized product of the present invention, separated and accommodated in a plurality of chambers. The amino acid concentration in the mixed solution (infusion solution) is preferably 1 to 10 w / v%, more preferably 1 to 6 w / v%.
The amino acid solution is adjusted to pH 6.0 to 8.5, preferably pH 6.0 to 7.5 using a pH adjusting agent as necessary. When the pH of the amino acid solution is in the above range, aggregation, precipitation, alteration, and the like of various components contained in the amino acid solution can be prevented. The pH adjuster of the amino acid solution is not limited as long as it can be used as a pharmaceutical additive. Examples of the pH regulator include citric acid, acetic acid, tartaric acid, carbonic acid, lactic acid, fumaric acid, propionic acid, boric acid, phosphoric acid, sulfuric acid and their compounds, adipic acid, hydrochloric acid, gluconic acid, succinic acid, water Examples thereof include potassium oxide, calcium hydroxide, sodium hydroxide, magnesium hydroxide, sodium hydrogen carbonate, maleic acid, malic acid, and the like, and one or more of these can be used.

ビタミンDとしては、ビタミンD2(エルゴカルシフェロール)、ビタミンD3(コレカルシフェロール)、これらの活性型などを挙げることができる。 The third chemical solution preferably contains a fat-soluble drug that is solubilized by a nonionic surfactant having a polyoxyethylene chain. Examples of such a fat-soluble drug include fat-soluble vitamins. Here, the kind of fat-soluble vitamin is not particularly limited, and vitamin A, vitamin D, vitamin E, vitamin K, and the like can be used. For example, when the infusion preparation produced according to the present invention is a parenteral nutrition infusion, it contains at least three kinds of vitamin A, vitamin D and vitamin E, or 4 of vitamin A, vitamin D, vitamin E and vitamin K. It is desirable to contain at least seeds. Examples of vitamin A include retinol palmitate and retinol acetate having vitamin A (retinol) activity. Examples of vitamin D include vitamin D2 (ergocalciferol), vitamin D3 (cholecalciferol), and active forms thereof. Examples of vitamin E include tocopherol acetate, tocopherol oxalate, and dl-α-tocopherol. Examples of vitamin K include phytonadione, menatetrenone, menadione, and derivatives thereof.
The content of the fat-soluble vitamin is not particularly limited, and can be determined according to the purpose of use, the form of use, the age and condition of the salary for the third chemical solution, and the like. Generally, the daily vitamin intake required for adults is 2000 to 5000 IU for vitamin A, 200 to 1000 IU for vitamin D, 2 to 20 mg for vitamin E, and 0.1 to 10 mg for vitamin K. You may decide in consideration of quantity.
Moreover, the compounding quantity of the nonionic surfactant which has the said polyoxyethylene chain is 1 weight part of the weight of the fat-soluble vitamin contained in a chemical | medical solution (when containing 2 or more types of fat-soluble vitamins, the total weight). On the other hand, it is preferably 2 to 12 parts by weight from the viewpoint of good solubilization of the fat-soluble vitamin and excellent storage stability, and more preferably 2.5 to 10 parts by weight. If the amount of the nonionic surfactant having a polyoxyethylene chain is less than 2 parts by weight, the solubilization may be insufficient. On the other hand, if the amount is more than 12 parts by weight, The effect of just matching is not expected.
In this invention, the 3rd chemical |

Next, the sterilization process will be described.
The sterilization step of the present invention is a step of heat-sterilizing the
In the heat sterilization step, 70% or more of the
The method of installing the

(Comparative Example 1)
(1) The infusion preparation produced in the same manner as in Example 1 above was placed horizontally on a non-tilted sterilization chamber member and autoclaved according to a conventional method. At this time, the fat-soluble vitamin-containing liquid was in a state where about 1.5 mL of 3 mL, that is, about 50% of the total amount was accommodated in the space inside the discharge port.
(Example 1 and Comparative Example 1)
When the container with the fat-soluble vitamin solubilizing solution is sterilized in a flat state as in the production method of Comparative Example 1, a haze-like precipitate is generated when the container is cooled to room temperature after heat sterilization. And remained as a highly viscous precipitate. On the other hand, according to the manufacturing method of Example 1, such a highly viscous precipitate did not remain.
