Description

The present invention relates to a treatment instrument for an endoscope with a one-way bending function configured so that a bending portion formed in the vicinity of the distal end of a sheath can be bent in only one direction by remote operation from the hand side.

Since the treatment instrument for endoscope is used through the treatment instrument insertion channel of the endoscope, the sheath that is passed through the treatment instrument insertion channel is extremely thin and long, and its diameter is, for example, The length is about 2 mm and the length is about 1 to 2 m.

When the distal end of such a treatment tool protrudes into the body from the distal end of the endoscope, the curved portion formed in the vicinity of the distal end of the sheath is handed so that the target affected part can be easily sniped. There is one that can be bent arbitrarily by remote control from the side.

In theory, such a bending portion may adopt a mechanism that can be bent at any angle in an arbitrary direction, such as a bending portion of an endoscope, or may be bent in either the vertical direction. It seems that it can be made possible (for example, patent document 1).

However, in reality, the diameter of the sheath of the treatment instrument for endoscope is only about 2 mm as described above, and the lumen necessary for performing the original function as the treatment instrument needs to be secured in the sheath. Therefore, the structure of the bending portion has to be extremely simple and can be bent only in one direction (that is, from a straight state to a specific one direction).

Therefore, a V-shaped cut is formed in the sheath made of a flexible tube in the radial direction from the outer edge side as a curved portion, and the sheath is bent at the cut portion by pulling the operation wire inserted into the main lumen. (For example, patent document 2).

In an endoscopic treatment tool in which the structure of the bending portion has to be very simple, as described above, the bending portion is formed by a V-shaped notch formed radially from the outer edge side to the tube forming the sheath. It is excellent from the viewpoint of flexibility and the like that the sheath is bent at the cut portion.

However, in such a configuration, the sheath bends with a slight wall portion of the sheath left without being cut off as a fulcrum, so that the sheath made of the flexible tube deteriorates and bends in that portion in a short time. Operation may become unstable or break.

Therefore, if the depth of the V-shaped cut is made shallower, the wall portion left uncut by the cut will increase and the strength will be improved, but the resistance of the tube to the bending action will be large, and the curved portion will be sufficiently It cannot be bent.

Although the present invention has a simple configuration using a flexible tube (multi-lumen tube) as a sheath, it can be reliably bent by remote operation, has excellent durability, and has linearity when not bent. An object of the present invention is to provide a good and easy-to-use endoscope treatment tool with a unidirectional bending function.

In order to achieve the above object, a treatment tool for an endoscope with a unidirectional bending function according to the present invention is provided with a distal treatment section at the forefront of a sheath made of a flexible synthetic resin tube, With a one-way bending function configured so that the bending portion formed in the vicinity of the distal end of the sheath can be bent in only one direction by pulling the bending operation wire inserted through the base end from the proximal end side. A treatment instrument for an endoscope, wherein a sheath is formed of a multi-lumen tube having a main lumen and a sub-lumen, and a bending operation wire is inserted and arranged in the sub-lumen so as to be capable of moving forward and backward in an axial direction. A plurality of V-shaped cuts formed in the sheath so as to cut the sub-lumen substantially perpendicularly and formed at a position eccentric from the axial position of the sheath, and the cuts of the V-shaped cuts The gapless cuts formed in the sheath from the direction opposite to the direction are alternately formed by shifting the position in the axial direction of the sheath, and the V-shaped cut is formed in a shape gradually expanding toward the outer edge side of the sheath. Both the V-shaped notch and the gap-shaped notch are cut from the outer peripheral surface of the sheath to a depth that does not reach the axial position of the sheath, and when the bending operation wire is pulled from the proximal end side, the V-shaped notch At the same time the notch is deformed in the closing direction, the gapless notch is deformed in the opening direction, and the sheath is bent in only one direction at the curved portion.

Note that a distal treatment section operation wire for remotely operating the distal treatment section may be disposed through the main lumen.

According to the present invention, a plurality of V-shaped cuts formed in the sheath so as to cut the sub-lumen substantially vertically, and a gapless cut formed in the sheath from the direction opposite to the cut direction of the V-shaped cut. Are alternately formed by shifting the position in the axial direction of the sheath, so that it is a simple configuration using a flexible tube (multi-lumen tube) as the sheath, but can be bent securely by remote operation. And has excellent durability, and also has an effect such as good linearity when not bent and easy to use.

Embodiments of the present invention will be described below with reference to the drawings.
FIG. 5 shows the overall configuration of the endoscope treatment tool. Reference numeral 1 denotes a flexible sheath that is inserted into a treatment instrument insertion channel of an endoscope (not shown), and is formed of, for example, a multi-lumen tube made of a synthetic resin material such as a fluororesin that has a good sliding property. . The diameter (outer diameter) of the sheath 1 is, for example, about 2 mm, and the total length is, for example, about 1-2 m.

At the most distal end of the sheath 1, a distal treatment section 2 of an endoscope treatment tool is disposed. The distal treatment section 2 of this embodiment is a pair of forceps cups for collecting biopsy tissue, and can be opened and closed like a bowl. However, the present invention can be applied even if the distal treatment section 2 is an endoscopic treatment tool other than a forceps cup.

An operation unit 10 is connected to the proximal end of the sheath 1 for an operator to operate by hand. A cup opening / closing slider 12 for remotely opening / closing a forceps cup as the distal treatment portion 2 is provided in the longitudinal direction of the operation portion main body 11 on the elongated operation portion main body 11 to which the proximal end of the sheath 1 is connected and fixed. It is slidably mounted along.

The proximal end of the distal treatment portion operation wire 13 that is inserted through the entire length of the sheath 1 so as to be able to advance and retreat in order to open and close the forceps cup as the distal treatment portion 2 is connected and fixed to the cup opening / closing slider 12. When the cup opening / closing slider 12 is advanced or retracted as indicated by the arrow P, the distal treatment portion operation wire 13 is advanced or retracted thereby opening and closing the distal treatment portion 2 in a bowl shape.

Reference numeral 14 denotes a connection terminal for connecting a high-frequency power cord (not shown), and a so-called hot biopsy can be performed by supplying a high-frequency current to the distal treatment section 2 via the distal treatment section operation wire 13.

A bending operation slider 16 is attached to the operation unit body 11 in a positional relationship in series with the cup opening / closing slider 12 independently of the cup opening / closing slider 12 so as to be slidable along the longitudinal direction of the operation unit body 11. It has been. The bending operation slider 15 has a proximal end of a bending operation wire 15 inserted and disposed in the sheath 1 so as to be able to advance and retreat in the entire length thereof.

As a result, when the bending operation slider 16 is moved forward and backward as indicated by the arrow Q, the bending portion 3 formed in the vicinity of the distal end of the sheath 1 is moved to a specific direction from a predetermined direction (that is, from a straight state). Direction).

FIG. 1 shows the vicinity of the curved portion 3 in an enlarged manner. In the sheath 1 which is a multi-lumen tube, as shown in FIGS. 2 and 3 illustrating the II-II cross section and the III-III cross section, one main lumen 4 and one sub lumen 5 have an axial line over the entire length. It is formed penetrating in the direction. At least the auxiliary lumen 5 is formed at a position eccentric from the axial position of the sheath 1.

In the main lumen 4, a distal treatment section operation wire 13 made of a conductive flexible stranded wire for operating the distal treatment section 2 by an operation from the operation section 10 connected to the proximal end side of the sheath 1 is provided. It is placed loosely over the entire length. The distal treatment section operation wire 13 is schematically indicated by a two-dot chain line in each drawing.

A bending operation wire 15 that is pushed and pulled from the operation unit 10 side is inserted into the auxiliary lumen 5 so as to be movable forward and backward in the axial direction. The distal end (not shown) of the bending operation wire 15 is directly fixed to the sheath 1 at a position closer to the distal end than the bending portion 3, or is indirectly fixed to the sheath 1 via a fixing fitting or the like.

In the bending portion 3, a V-shaped cut 31 is formed in the sheath 1 so as to cut the auxiliary lumen 5 substantially vertically. A plurality of V-shaped cuts 31 are formed at intervals in the longitudinal direction of the sheath 1, and each V-shaped cut 31 is formed in a shape that gradually widens toward the outer edge side of the sheath 1.

Further, a gapless cut 32 is formed in the sheath 1 from the opposite direction to the cut direction of the V-shaped cut 31. Each non-cutting notch 32 that is perpendicular to the axial direction of the sheath 1 is formed by cutting the sheath 1 without cutting the wall member of the sheath 1 at the notch, for example, with a razor blade so that no gap is generated in the sheath 1. ing.

The plurality of gapless cuts 32 are formed alternately with the plurality of V-shaped cuts 31 so as to be displaced in the axial direction of the sheath 1 with respect to the V-shaped cuts 31. In this embodiment, a gapless cut 32 is formed at an exactly middle position between adjacent V-shaped cuts 31 in the axial direction of the sheath 1.

Both the V-shaped notch 31 and the gapless notch 32 are formed so as to have a depth that does not reach the axial position of the sheath 1 from the outer peripheral surface of the sheath 1. In FIG. 2, the bottom of the gap-shaped cut 32 is illustrated by a broken line, and in FIG. 3, the bottom of the V-shaped cut 31 is illustrated by a broken line. As a result, the strength of the sheath 1 in the bending portion 3 is ensured without significantly decreasing.

The distal end of the distal treatment portion operation wire 13 is connected to a known link mechanism (not shown) for opening and closing the distal treatment portion 2, and the distal end of the bending manipulation wire 15 is connected to the bending portion 3 as described above. It is fixed to the sheath 1 at a position near the tip.

In the endoscope treatment instrument with a one-way bending function configured as described above, when the bending operation wire 15 is pulled from the operation unit 10 side, as shown in FIG. At the same time as 31 is deformed in the closing direction, the gapless notch 32 is deformed in the opening direction, and the sheath 1 is pulled in only one direction (the direction in which the bending operation wire 15 is disposed) in the bending portion 3. Bends at an angle corresponding to the amount.

At this time, not only the V-shaped cut 31 is formed at the position on the inner peripheral side of the bending curve, but the gap notch 32 is formed at the position on the outer peripheral side. Without being generated, it bends smoothly in response to the pulling operation of the bending operation wire 15.

When the bending operation wire 15 is pushed from the operation unit 10 side, the bending unit 3 returns to a straight state as shown in FIG. 1, but the gapless notch 32 is closed without a gap. Since excellent linearity is obtained, the bending portion 3 does not shake and is very easy to use.

As described above, the endoscopic treatment instrument with a one-way bending function according to the present invention has a simple configuration using a flexible tube (multi-lumen tube) as the sheath 1 and can be reliably bent by remote operation. It has excellent durability such as being easy to use because it has excellent durability and has good linearity when not bent and does not shake.
