(Background of the Invention)
The identification of any publication in this or any section of this application does not admit that such publication is prior art to the present invention.
Weakly basic and poorly water soluble (intestinal pH), azole functionalized drugs often exhibit low bioavailability or irregular absorption. While these drugs are often soluble in low pH environments (eg, stomach (pH 1-2)), these drugs are primarily insoluble in higher pH environments such as those found in the small intestine (pH 5-7). is there. As a result, such drugs generally precipitate out of solution as they pass from the low pH environment of the stomach to the higher pH environment of the small intestine. Examples of such weakly basic and poorly water-soluble azole drugs include difenamazole, epirizole, fluconazole, itraconazole, ketoconazole, letrozole, posaconazole, ravconazole, saperconazole, saperconazole, And voriconazole.
Some of these weakly basic and poorly water-soluble azole drugs (eg but not limited to itraconazole, posaconazole and terconazole) are for the treatment and / or prevention of fungal infections (eg invasive fungal infections) Has been developed. So far, the development of these drugs is because their solubility in aqueous solutions is very pH dependent, which poses difficulties in providing sufficiently and easily controlled bioavailability. Have a problem.
U.S. Patent No. 6,057,031 describes a pharmaceutical composition of posaconazole, a nonionic surfactant, and a diluent. U.S. Patent No. 6,057,031 describes a pharmaceutical composition of posaconazole and a soluble or insoluble polymer (eg, povidone or crospovidone), the components of which are present in the stated ratios. U.S. Patent No. 6,057,031 describes a pharmaceutical capsule composition of posaconazole coated on inert beads and a binder. None of Patent Document 3, Patent Document 4, and Patent Document 5 describe a composition that can provide an oral dosage form with low bioavailability variability across a patient population.
US Pat. No. 6,057,033 describes a fine powder of posaconazole combined with a surfactant and a thickener in the form of a liquid suspension (eg, an oral suspension marketed under the trade name NOXAFIL ™). A pharmaceutical composition is described. NOXAFIL ™ suspension provides sufficient bioavailability, but NOXAFIL ™ is administered in combination with food or nutritional supplements to ensure proper plasma concentration in humans, and NOXAFIL It is recommended that TM be administered several times a day. In addition, liquid oral suspensions are convenient for some patients (eg, patients who have difficulty swallowing pills), but solid dosage forms are desirable and / or necessary in other situations. Is done. In particular, solid dosage forms are desirable for ease of portability and storage, as well as to promote patient compliance by avoiding the need for additional dosing devices. Furthermore, there remains a need for a solid dosage form of posaconazole that provides sufficient bioavailability, is less variable across the patient population, and avoids the need to be administered in combination with food. Similarly, solid dosage forms with sufficient bioavailability provide a treatment regimen that can reduce the number of doses administered per day to achieve the desired therapeutic plasma concentration.
A composition with enhanced bioavailability containing pharmaceutical compounds and various polymers is described in US Pat. In one example, U.S. Patent No. 6,057,031 describes glycogen phosphorylase inhibitors in hydroxypropylmethylcellulose and hydroxypropylmethylcellulose derivative polymers. The composition described in Patent Document 7 is prepared by spray drying a solution containing a phosphorylase inhibitor and hydroxypropyl methylcellulose acetate succinate (HPMC-AS) dissolved in a common solvent. U.S. Patent No. 6,057,034 issued to Hayes et al. On April 19, 2005 generally describes compositions containing azole antifungal compounds and polymers. The described composition dissolves the azole compound and polymer in a common solvent (eg, methylene chloride, chloroform, ethanol, methanol, isopropanol, ethyl acetate, or acetone, or a mixture of two or more thereof). This solution is prepared by spray drying the solution using conventional spray drying equipment to form a solid particulate composition. An example of an azole-containing composition described in US Pat. No. 6,053,097 involves a hydroxypropyl methylcellulose phthalate (HPMC phthalate) polymer derivative prepared by spray drying a solution containing an active pharmaceutical ingredient (API) and a polymer. Itraconazole. These compositions are reported to show an improvement in the bioavailability of itraconazole and the elimination of the food effects associated with the administration of itraconazole.
There remains a need for pharmaceutical compositions with enhanced bioavailability that contain weakly basic and poorly water-soluble azoles (eg, posaconazole). Such compositions reduce the dose and / or absorption variability required for the same or better therapeutic effect, reduce the cost of goods for the product, and / or reduce the dosage regimen. . Such a solid dosage form of the drug provides greater convenience to the patient and therefore promotes patient compliance.

(Molecular solid dispersion pharmaceutical composition)
In some particularly preferred embodiments, the present invention provides an oral pharmaceutical composition containing a solid molecular dispersion of the present invention containing posaconazole and HPMCAS. When maintained in an aqueous environment having a more acidic pH than about pH 2 (eg, as found in the stomach), this pharmaceutical composition releases a limited amount of posaconazole and about pH 5 (eg, found in the small intestine). When maintained in an aqueous environment having a less acidic pH (as is the case), the pharmaceutical composition releases a large amount of posaconazole constituting the composition. In a particularly preferred embodiment, for the molecular solid dispersions provided herein, the stable amorphous solid state posaconazole is dissolved in HPMCAS. The molecular solid dispersions provided herein exhibit the excellent mechanical and physical properties necessary for subsequent grinding, mixing, and tablet compression. Such molecular solid dispersions can be utilized directly as powders or granules. Alternatively, such molecular solid dispersions can be used to prepare solid dosage forms such as capsules, tablets, granules, powders, and unit dose packets. Indeed, the molecular solid dispersions provided herein are suitable for high drug content dosage forms containing ≧ 100 mg of drug per unit dosage form. The pharmaceutical composition of the present invention is believed to reduce the food effect compared to the food effect observed when a commercially available suspension is administered orally and when compared to a commercially available oral dosage form , Which would reduce the observed variability in bioavailability across the patient population.

The dissolution profile of the molecular solid dispersion of posaconazole in capsule A and capsule B represented in FIG. 1 was determined as follows. Briefly, USP-II Apparatus Paddle Stirrer was used for dissolution testing. Sample dissolution profiles were obtained in 900 ml of dissolution medium at
The dissolution profiles of two molecular solid dispersions of posaconazole with HPCMAS (grade L or grade M) according to the present invention, described herein as capsule A and capsule B, respectively, are shown in FIG. As shown in FIG. 1, “% recovery” refers to the amount of posaconazole released from the sample relative to the total amount of posaconazole in the sample being assayed. Controlled release of posaconazole from both molecular solid dispersions was observed at pH1. Even at
Surprisingly, the molecular solid dispersion containing HPMCAS also prevented the precipitation of posaconazole at pH> 4 (where posaconazole has a solubility of less than 1 μg / mL in aqueous solution). Furthermore, the dissolution profile of the molecular solid dispersion at pH 6.4 lasted for a long time (see FIG. 1). As a result, the inventors have surprisingly found that the molecular solid dispersions of the present invention increase both the rate and extent, or the rate or extent, of posaconazole absorption in the gastrointestinal tract, thereby increasing posaconazole.
(X-ray powder diffraction of molecular solid dispersion)
The presence of the crystalline form of posaconazole was evaluated using X-ray powder diffraction as described in US Pat. No. 6,713,481. A molecular solid dispersion of posaconazole with HPMCAS at a ratio ranging from 1: 1 to 3: 1 (by weight) after storage for 3 months under RH4 conditions (ie, 40 ° C. and 75% relative humidity) or ambient After storage for 1 year under conditions (ie, 15-25 ° C. and 50-60% relative humidity), the presence of crystalline posaconazole was evaluated. Specifically, molecular solid dispersions of HPMCAS grade M: posaconazole with ratios of 1: 1, 2: 1, and 3: 1 (by weight) were tested. Similarly, molecular solid dispersions of HPMCAS grade L: posaconazole with ratios of 2: 1 and 3: 1 were also tested. At no time point crystalline posaconazole was detected in any of these molecular solid dispersions. In particular, when using this assay, the limit of detection for crystalline posaconazole is less than 3%. These results indicate that the amorphous form of posaconazole is stable within these molecular solid dispersions.
(Comparison of bioavailability of posaconazole compositions)
Adult male monkeys (6 monkeys / group) were administered after overnight fasting with one of three different oral compositions as shown in Table 2. Food was returned 4 hours after dosing and body weight was recorded on the day of dosing. Blood from each monkey before administration, as well as 0.25 hours, 0.5 hours, 1 hour, 2 hours, 4 hours, 8 hours, 24 hours, 48 hours, 72 hours after administration 96 hours, and 120 hours later, to determine the concentration of posaconazole in plasma and to calculate pharmacokinetic (PK) parameters for each composition.
The three different posaconazole compositions tested were oral suspensions (described in US Publication 2003/0055067) and molecular solid dispersion capsule dosage forms (Capsule A and Capsule B).
