以下に、本発明の一実施形態に係る骨補填材1および骨補填材1の製造方法について図1〜図3を参照して説明する。
本実施形態に係る骨補填材1は、図1に示されるように、骨欠損部に移植される基材2と、該基材2の表面に付着した多数の微粉体3とを備えている。
Below, the manufacturing method of the
As shown in FIG. 1, the
The
微粉体3は、基材2と同様に、60〜80%の気効率を有するβ−TCPの多孔質体からなり、50μm以下の直径寸法を有している。微粉体3の直径が50μmより大きくなると、微粉体3による骨形成を促進する作用が低下する可能性がある。微粉体3は、基材2の外表面および連通孔4の内面に付着している。基材2表面における微粉体3の密度は、3.0g/cm3以下が好ましい。
The
このように構成された本実施形態に係る骨補填材1は以下の製造方法によって製造される。
本実施形態に係る骨補填材1の製造方法は、図2に示されるように、基材2を形成する基材形成ステップS1と、基材2に微粉体3を付着させる付着ステップS2と、微粉体3を付着させた基材2を焼成する焼成ステップS3とを備えている。
The
As shown in FIG. 2, the manufacturing method of the
基材形成ステップS1は、例えば、リン酸水素カルシウム、炭酸カルシウムおよび水を適切な割合で配合したスラリーを十分に反応させた後に乾燥させ、乾燥させて得られた固形物を粉砕し、粉砕して得られた粉末を鋳型内で焼結させることにより行われる。このときに、鋳型の形状を適宜選択することにより、連通孔4も形成される。 In the substrate forming step S1, for example, a slurry obtained by mixing calcium hydrogen phosphate, calcium carbonate and water in an appropriate ratio is sufficiently reacted and then dried, and a solid obtained by drying is pulverized and pulverized. The powder obtained is sintered in a mold. At this time, the communication hole 4 is also formed by appropriately selecting the shape of the mold.
付着ステップS2は、β−TCPの多孔質体からなる微粉体3を緩衝液に混合した混合液を基材2の外表面および連通孔4内面に付着させた後、基材2を乾燥させることにより行われる。これにより、微粉体3を、基材2の外表面および連通孔4の内面に容易に略均一に付着させることができる。
In the attaching step S2, a mixed liquid obtained by mixing
焼成ステップS3は、微粉体3が付着させられた基材2を非酸素雰囲気下、例えば、窒素雰囲気下またはアルゴン雰囲気下で加熱することにより行われる。焼成時の温度は、β−TCPの融点より低ければよいが、300〜500℃が好ましい。焼成により、基材2に付着した微粉体3が基材2の表面と共有結合を形成し、微粉体3がより堅固に基材2表面に保持される。
The firing step S3 is performed by heating the
According to the
また、基材2の気孔内にまで血液が十分に供給されない環境であっても基材2表面においては効率良く骨形成が進むので、例えば、人工股関節などの血流が十分に得られない部位であっても良好な骨治癒を得ることができるという利点がある。
In addition, even in an environment where blood is not sufficiently supplied into the pores of the
In the above embodiment, the
For example, as shown in FIG. 3, when the
また、上記発明においては、基材2の材料としてβ−TCPを用いることとしたが、基材2の材料はこれに限定されるものではなく、生体適合性の高い材料であればよい。
基材2の材料としては、例えば、HAP(ハイドロキシアパタイト)などのリン酸カルシウムであってもよく、チタンやステンレスなどの金属でもよい。骨補填材1の製造方法において焼成ステップS3を行う場合には、焼成に耐えられる材料が用いられる。
In the above invention, β-TCP is used as the material of the
The material of the
このように、基材2の材料にβ−TCP以外のものを用いた場合でも、基材2の表面に微粉体3を付着させることにより基材2表面における骨形成を促進し、基材2と該基材2に隣接する骨との骨癒合の早期化を図ることができる。また、従来骨癒合が難しいために骨補填材としての使用が困難であった材料、例えば、無機材料からなる基材2であっても、その表面に微粉体3を付着させることにより移植後の良好な骨癒合を図ることができる。
As described above, even when a material other than β-TCP is used as the material of the
また、上記実施形態においては、微粉体3としてβ−TCPの多孔質体からなるものを用いることとしたが、他のリン酸カルシウム系の材料からなるものを用いても、上述した実施形態と同様に、骨形成を促進することができる。
Moreover, in the said embodiment, although it decided to use what consists of a porous body of (beta) -TCP as the
Next, examples of the bone prosthetic material and the bone prosthetic manufacturing method according to the embodiment described above will be described.
[Example 1]
The relationship between the size of the fine powder and the bone forming ability was examined by examining the phagocytic effect of osteoclasts on the fine powder of β-TCP having different sizes.
In order to form new bone, it is thought that bone formation by osteoblasts and bone resorption by osteoclasts must proceed in parallel. That is, the effect of the fine powder on the bone forming ability can be evaluated from the degree of phagocytosis of osteoclasts to the fine powder of β-TCP.
まず、ヒト末梢血から分離した破骨細胞を、細胞数が2×106個/mlになるようにHank’s緩衝液で調製した。次に、マイクロプレートの各ウェルに、作成した細胞懸濁液50μlと、β−TCP多孔質体の顆粒(微粉体)および5×10−5モルのルミノールを含んだ懸濁液50μlとを添加し、37℃でインキュベートした。 First, osteoclasts separated from human peripheral blood were prepared with Hank's buffer so that the number of cells was 2 × 10 6 cells / ml. Next, 50 μl of the prepared cell suspension and 50 μl of a suspension containing β-TCP porous body granules (fine powder) and 5 × 10 −5 mol of luminol are added to each well of the microplate. And incubated at 37 ° C.
ここで、β−TCP多孔質体の顆粒は、β−TCPの最終濃度が0、0.01、0.1、0.5および1mg/mlになるように、各ウェルに添加した。また、β−TCP多孔質体の顆粒として、直径が25〜75μmのものと直径が75〜105μmのものとを使用した。 Here, the granules of the β-TCP porous body were added to each well so that the final concentrations of β-TCP were 0, 0.01, 0.1, 0.5, and 1 mg / ml. Moreover, as a granule of the β-TCP porous body, those having a diameter of 25 to 75 μm and those having a diameter of 75 to 105 μm were used.
そして、インキュベート中に、マイクロプレートリーダを用いてルミノールからの発光を経時的に測定した。破骨細胞がβ−TCPを貧食することにより活性酸素が産生され、該活性酸素とルミノールが反応することによりルミノールが発光する。ルミノールの発光強度の測定結果を図4(a),(b)に示す。 During the incubation, light emission from luminol was measured over time using a microplate reader. When osteoclasts prey on β-TCP, active oxygen is produced, and luminol emits light when the active oxygen reacts with luminol. The measurement results of the luminous intensity of luminol are shown in FIGS. 4 (a) and 4 (b).
図4(a),(b)は、横軸がインキュベート時間を示し、縦軸がルミノールの発光強度を相対光単位(RUL)で示している。この実験結果から、直径寸法が75〜105μmの顆粒よりも、直径寸法が25〜75μmの顆粒を使用した方が効率的に破骨細胞によるβ−TCPの貪食が進むことが確認された。また、破骨細胞がβ−TCPと接触してから約5分間でその貧食作用が最大となることが確認された。 4A and 4B, the horizontal axis indicates the incubation time, and the vertical axis indicates the luminol emission intensity in relative light units (RUL). From this experimental result, it was confirmed that the phagocytosis of β-TCP by osteoclasts progressed more efficiently when granules having a diameter of 25 to 75 μm were used than granules having a diameter of 75 to 105 μm. It was also confirmed that the phagocytosis was maximized in about 5 minutes after the osteoclasts contacted with β-TCP.
以上の結果から、β−TCP多孔質体からなる直径25〜75μmの微粉体を基材の表面に付着させておくことより、破骨細胞を基材表面に効率的に捕捉してβ−TCPの吸収を促進させ、その結果として骨形成を促進することができることが示唆された。 From the above results, it is possible to efficiently capture osteoclasts on the surface of the base material by attaching a fine powder having a diameter of 25 to 75 μm made of a β-TCP porous body to the surface of the base material. It was suggested that the bone resorption can be promoted and as a result, the bone formation can be promoted.
〔実施例2〕
実施例2に係る基材および微粉体を、以下の手順で製造した。
リン酸水素カルシウム二水和物と炭酸カルシウムとを、Ca:P=1.5:1となるように混合し、これに純水を添加して10重量%のスラリーを調整した。調整したスラリーをボールミル内で24時間摩砕しながら反応させた後、バット内に収容して80℃で乾燥させた。次に、乾燥して得られた固形物を粉砕し、750℃で1時間仮焼した。
[Example 2]
The base material and fine powder according to Example 2 were produced by the following procedure.
Calcium hydrogen phosphate dihydrate and calcium carbonate were mixed so that Ca: P = 1.5: 1, and pure water was added thereto to prepare a 10 wt% slurry. The prepared slurry was reacted while being ground in a ball mill for 24 hours, and then contained in a vat and dried at 80 ° C. Next, the solid obtained by drying was pulverized and calcined at 750 ° C. for 1 hour.
次に、仮焼して得られた粉末60gに、ポリアクリル酸アンモニウムエチレンノニルフェニルエーテル5mlを添加して撹拌することにより発泡体を生成した。次に、生成した発泡体を鋳型内に収容して80℃で24時間乾燥させた後、1時間に100℃ずつ昇温させ、1050℃で30分間焼結することによりβ−TCP多孔質体からなる基材を製造した。 Next, 5 ml of ammonium polyacrylate ethylene nonylphenyl ether was added to 60 g of the powder obtained by calcination, and a foam was produced by stirring. Next, the produced foam is accommodated in a mold and dried at 80 ° C. for 24 hours, and then heated at 100 ° C. per hour and sintered at 1050 ° C. for 30 minutes to form a β-TCP porous body. A substrate consisting of
製造された基材の気孔率を粉末X線回折法で測定したところ、60〜80%であった。
微粉体は、製造した基材を粉砕して粉末状にした後、得られた粉末を孔径が25μmのふるいにかけて選別することにより得た。
It was 60 to 80% when the porosity of the manufactured base material was measured by the powder X ray diffraction method.
The fine powder was obtained by pulverizing the produced base material into a powder and then screening the obtained powder through a sieve having a pore size of 25 μm.
Example 3
Next, the bone forming ability of the bone grafting material comprising the base material and fine powder according to Example 2 was evaluated by the following experiment.
First, the bone grafting material according to this example was manufactured by the following procedure. A substrate having a porosity of 75% was used. The substrate used was a substantially cylindrical one having an outer diameter of 3.7 mm and a height of 10 mm, and one end face having a hole with a depth of 8 mm and an inner diameter of 1.3 mm. After a solution prepared by mixing fine powder in PBS at 0.1 g / ml was attached to the base material, the base material was sufficiently dried to produce the bone grafting material according to this example.
次に、24週齢のハイブリット犬の下顎の骨を抜歯し、抜歯から2ヶ月経過後に、抜歯により空いた下顎骨の穴内に本実施例に係る骨補填材を移植した。移植から4ヶ月後、下顎骨の移植部位を採取し、骨補填材の略中心位置において頬舌的に切り出して病理組織片を作成した。作成した病理組織片を、HE(ヘマトキシリン・エオジン)染色を施した後、光学顕微鏡で観察した。 Next, the mandibular bone of a 24-week-old hybrid dog was extracted, and after 2 months from the extraction, the bone filling material according to this example was transplanted into the hole of the mandible vacated by the extraction. Four months after the transplantation, the transplanted portion of the mandible was collected, and a buccal tongue was cut out at a substantially central position of the bone filling material to create a pathological tissue piece. The prepared pathological tissue piece was stained with HE (hematoxylin and eosin) and then observed with an optical microscope.
本実施例に対する比較例1として、抜歯により空いた穴の内部を空洞の状態のままで4ヶ月経過させた下顎骨について、本実施例と同様に病理薄切片を作成・染色し、観察を行った。また、本実施例に対する比較例2として、切削により基材に孔を形成して切削粉をそのまま基材表面に残すことにより製造した骨補填材を用いたことを除いて本実施例と同様に移植、病理組織片の作成・染色、観察を行った。 As Comparative Example 1 with respect to the present example, a thin pathological section was prepared, stained, and observed for the mandible for which 4 months had passed while the inside of the hole vacated by tooth extraction was left hollow. It was. Further, as Comparative Example 2 with respect to the present example, the same as in the present example except that a bone prosthetic material manufactured by forming a hole in the base material by cutting and leaving the cutting powder as it is on the base material surface was used. Transplantation, preparation, staining, and observation of pathological tissue pieces were performed.
以上の3種類の観察結果を図5(a),(b)〜図7(a),(b)に示す。図5(a)、図6(a)および図7(a)は、実体顕微鏡により1倍の倍率で観察した写真である。図5(b)、図6(b)および図7(b)はそれぞれ、図5(a)、図6(a)および図7(a)の骨補填材の移植部位または下顎骨の穴の部位をシステム顕微鏡により10倍の倍率で拡大観察した写真である。 The above three types of observation results are shown in FIGS. 5 (a) and (b) to FIGS. 7 (a) and 7 (b). 5 (a), 6 (a) and 7 (a) are photographs observed at a magnification of 1 with a stereomicroscope. 5 (b), FIG. 6 (b) and FIG. 7 (b) respectively show the implantation site of the bone filling material or the hole of the mandible of FIG. 5 (a), FIG. 6 (a) and FIG. 7 (a). It is the photograph which expanded and observed the site | part with the magnification of 10 time with the system microscope.
5 (a) and 5 (b) show the results using the bone grafting material according to this example. New bone B stained in pink was observed in the hole A of the mandible, that is, in the site where the bone filling material was transplanted. Symbol C indicates the alveoli, symbol D indicates cancellous bone, and symbol E indicates fine powder. Fine powder E was observed as agglomerated in part. This is thought to be because most of the fine powder E was not observed because it was absorbed and replaced with autologous bone, and only a part of the fine powder E remained without being absorbed and autogenous.
図6(a),(b)は、比較例1の結果を示している。下顎骨の穴A内は繊維性のコラーゲン組織Fによって満たされているとともに、炎症が惹起していることが確認された。
図7(a),(b)は、比較例2に係る骨補填材を用いた結果を示している。下顎骨の穴A内には、本実施例に係る骨補填材を移植した結果と同様に、新生骨Bが観察できた。また、切削粉はほとんど観察されなかった。これは、切削粉が吸収および自家骨に置換されたためと考えられる。
6 (a) and 6 (b) show the results of Comparative Example 1. FIG. It was confirmed that the hole A of the mandible was filled with fibrous collagenous tissue F and that inflammation had occurred.
7A and 7B show the results using the bone prosthetic material according to Comparative Example 2. FIG. In the hole A of the mandible, new bone B could be observed as in the result of transplanting the bone grafting material according to this example. Moreover, almost no cutting powder was observed. This is probably because the cutting powder was absorbed and replaced by autologous bone.
以上の結果から、基材の表面に微粉体が付着した骨補填材を用いることにより、十分に早期に骨形成が進行することが確認された。 From the above results, it was confirmed that bone formation proceeds sufficiently early by using a bone grafting material having fine powder adhered to the surface of the base material.
〔実施例4〕
次に、実施例2により製造された直径25μm以下の微粉体の粒度分布を以下の手順で調べた。
実施例2により製造された微粉体を、トリトンX−100を添加した蒸留水に混合し、超音波バス内で出力150Wの超音波を照射することにより蒸留水内で十分に分散させ、分散溶液を調整した。次に、分散溶液の粒度分布を、日機装社製マイクロトラックHRAを用いて測定した。その測定結果を図8に示す。
Example 4
Next, the particle size distribution of the fine powder having a diameter of 25 μm or less produced according to Example 2 was examined by the following procedure.
The fine powder produced in Example 2 was mixed with distilled water to which Triton X-100 was added, and was sufficiently dispersed in distilled water by irradiating ultrasonic waves with an output of 150 W in an ultrasonic bath. Adjusted. Next, the particle size distribution of the dispersion solution was measured using a Microtrac HRA manufactured by Nikkiso Co., Ltd. The measurement results are shown in FIG.
図8から、実施例2に係る微粉体の大きさは、1〜10μmnの範囲内に分布していたことが確認された。すなわち、直径が1〜10μmの微粉体により、骨補填材の骨形成を促進することができることが示唆された。 From FIG. 8, it was confirmed that the size of the fine powder according to Example 2 was distributed within the range of 1 to 10 μm.
