Description
本出願は、「動脈瘤の治療用隔離デバイス」と題する、2006年8月17日に出願された米国仮出願第60/822,745号明細書の本出願である。その全体は参照により本明細書中に組み込まれる。
漿果状動脈瘤、特に二分岐部又は他の分岐した血管に位置する漿果状動脈瘤を治療するために、様々な隔離デバイスを提供する。そのような隔離デバイスの実施形態を図3に示す。ここでは、隔離デバイス10は、第1の末端14、第2の末端16及び縦軸18に沿ってそれを通って伸長する管腔17を有する本体12を備える。隔離デバイス10は、少なくとも1つの方向の血流を閉塞するオクルーダ20も含んでいる。この実施形態では、オクルーダ20は第2の末端16付近に位置し、軸オクルーダとして機能するように、縦軸14に沿った血流を閉塞し、流れを縦軸14から遠ざけて迂回させる。
動脈瘤、特に腹部大動脈瘤のエンドリークを治療するために、様々な隔離デバイスを提供する。そのような隔離デバイスは、体内の任意の血管又は任意の管腔状の解剖学的形態を閉塞するためにも使用できることが理解できる。図21は、エンドリークEを有する腹部大動脈瘤AAAを例示する。本発明の隔離デバイス10は、エンドリークEを閉塞するようにエンドリークE内に位置決めされるように示されている。
Claims (18)
- 血管の幹管と複数の分岐枝との交差部付近に位置する動脈瘤を隔離するための隔離デバイスにおいて、当該隔離デバイスが、
第1の末端、第2の末端、及び縦軸に沿って通る管腔を有する本体であって、前記動脈瘤が実質的に前記縦軸と位置合わせされるように前記交差部内に位置決めされるよう構成された本体と;
前記縦軸に沿った前記第1の末端への血流を可能にし、且つ前記動脈瘤を隔離するために流れを前記縦軸から遠ざけて迂回させるように、前記管腔を横切って配置されるオクルーダとを具える隔離デバイス。 - 前記本体が前記第1及び第2の末端間に延在する本体壁を具え、この壁が、前記縦軸から遠ざけて流れを迂回させるのを容易にするように構成された開口部を有する、請求項1に記載の隔離デバイス。
- 前記本体壁が、前記第1の末端付近で前記縦軸に沿って通る流れを容易にするように配置されたより小さい開口部と、前記縦軸から遠ざけて流れを迂回させるのを容易にするように配置されたより大きな開口部とを有するメッシュを具える、請求項2に記載の隔離デバイス。
- 前記オクルーダが、開口部のないシート、開口部を有するシート、格子、支柱、スレッド、ファイバー、フィラメント、生体適合性充填物、接着剤又はこれらの組合せを具える、請求項1に記載の隔離デバイス。
- 前記オクルーダが、円錐形状及び先端部を有する分流器を具え、前記先端部が、前記縦軸から遠ざけて流れを迂回させるのを容易にするように前記第1の末端と向かい合っている、請求項4に記載の隔離デバイス。
- 前記オクルーダが、前記第2の末端から前記動脈瘤へと伸長可能な袋を具える、請求項1に記載の隔離デバイス。
- 前記本体がコイル形状を有する、請求項1に記載の隔離デバイス。
- 前記第1の末端が、前記幹管内で前記隔離デバイスを係留するように構成され、前記第2の末端が、前記動脈瘤の頚部付近に非侵襲的に常駐する、請求項1に記載の隔離デバイス。
- 血管の幹管と複数の分岐枝との交差部付近に位置する動脈瘤を隔離するための隔離デバイスであって、ボール形状を有する本体を具え、前記本体が、前記幹管を通る前記複数の分岐枝への血流を可能にしつつ、流れを前記動脈瘤から遠ざけて迂回させるように前記交差部内に位置するよう構成された、隔離デバイス。
- 前記本体が、編組み、メッシュ、格子、コイル、支柱、又はこれらの組合せを具える、請求項9に記載の隔離デバイス。
- 前記本体が関節支柱を具える、請求項10に記載の隔離デバイス。
- 前記本体の一部の上に配置されたカバーを更に具え、前記カバーは流れを前記動脈瘤から遠ざけて迂回させるように構成される、請求項9に記載の隔離デバイス。
- 前記本体が、前記ボール形状を共に形成する別々の部品で構成される、請求項9に記載の隔離デバイス。
- 被覆を有する少なくとも1つの本体を備え、前記被覆は、前記本体の接触部分が各接触点で互いに対して移動できる非結合状態から、前記本体の接触部分が各接触点で互いに対して移動するのが制限される結合状態へと転移可能である、隔離デバイス。
- 前記本体がコイル形状を有する、請求項14に記載の隔離デバイス。
- 前記被覆が熱に反応して転移可能である、請求項14に記載の隔離デバイス。
- 前記被覆が高周波エネルギーに反応して転移可能である、請求項16に記載の隔離デバイス。
- 前記被覆がポリマーを含む、請求項16に記載の隔離デバイス。
