Abstract
Description
The present invention relates to medical image processing in radiographic images such as mammography, and more particularly to a detection method and a detection apparatus for abnormal shadows typified by aneurysm shadows and microcalcification shadows.
Conventionally, in the medical field, it is common practice to discover a lesion using a radiographic image of a subject and observe the state of the lesion to diagnose the presence or progression of a disease. . However, in general, interpretation of radiographic images has been influenced by the experience of the observer and the level of image interpretation ability, and has not always been objective.
Therefore, in recent years, research on computer diagnosis support devices (CAD; Computer Aided Diagnosis; hereinafter referred to as CAD) that automatically detect abnormal shadow candidates using computer processing has been progressing. According to CAD, an abnormal shadow candidate can be detected from a radiographic image based on the characteristics of the density distribution and morphological characteristics of the shadow of the lesion.
However, as in the technique of Patent Document 1, it is difficult to expect a substantial improvement in detection performance simply by changing the sensitivity for each region in accordance with a portion where the possibility of detecting an abnormal shadow is large.
An object of the present invention is to alleviate the density change near the skin line and suppress the occurrence of false detection in an abnormal shadow detection apparatus in a radiographic image.

A specific area calculation process is executed on the input medical image D1 (step S201). The specific area calculation process is a process for calculating an area having a large density change near the skin line to be subjected to the surface correction process. Here, the density is a pixel value of the input medical image D1. Next, a surface correction process is performed on the region obtained in step S201 (step S202). The surface correction process is a process of correcting the area obtained in step S201 using a surface approximated by the least square method.
Next, an abnormal shadow detection process is executed (step S203). The abnormal shadow detection process is a process for detecting a candidate for an abnormal shadow. Next, false positive deletion processing is executed for the abnormal shadow candidate detected in step S203 (step S204). The false positive deletion process is a process for deleting a candidate having a high possibility of false positive among the abnormal shadow candidates detected in step S203.
As in the past, it is not sufficient to improve the detection performance of abnormal shadows only by performing the abnormal shadow detection process and the false positive deletion process on the input medical image D1. Therefore, in the abnormal
In the following description, a case where the medical image D1 is a mammography will be described as an example.
FIG. 3 shows the specific area calculation process.
The specific area calculation process is realized by software processing in cooperation with the
First, a Sobel filter process is performed using each pixel of the input medical image D1 as a target pixel, and a medical image D2 is acquired (step S301). The Sobel filter processing is to calculate a feature amount of a target pixel from a difference between pixel values (density values) of the target pixel and surrounding pixels. Each pixel value of the medical image D2 indicates the calculated feature amount. Next, in the medical image D2 obtained in step S301, a pixel having the maximum feature value for each x coordinate shown in FIG. 4 is defined as a skin-background boundary point (hereinafter referred to as a skin line). Are extracted (step S302).
Next, as shown in FIG. 4, at the point Q that is the coordinate on the skin line that is a fixed distance d1 away from the point P that is the coordinate where the y coordinate in the skin line extracted in the medical image D2 is the minimum, On the other hand, a density profile in the vertical direction is created (step S303). The value of the distance d1 is variable and is input from the
FIG. 5 shows an example of the density profile created in step S303. The horizontal axis indicates the distance in the direction perpendicular to the skin line, and the vertical axis indicates the pixel value. Point A and point B on the horizontal axis correspond to point A and point B in FIG. 4, respectively.
Next, in the obtained density profile, an inclination between points separated by a distance d2 from the point Q side is calculated (step S304). The value of the distance d2 is variable and is input from the
When the inclination is equal to or greater than a predetermined threshold value n (for example, 4.0) (step S305; No), the target coordinates for detecting the inclination are shifted in the direction of point B (step S306), and the process returns to step S304 again. Ask. When the inclination is smaller than the threshold value n (step S305; YES), the target coordinates are stored in the RAM 16 (step S307). Then, a distance L1 between the target coordinates stored in the
Note that the value of the threshold value n is variable, and is input from the
Next, based on the distance L1 calculated in step S308, an area from the skin line to the distance L1 is specified as an area to be subjected to surface correction (step S309). FIG. 6 schematically shows the region specified in step S309. The area indicated by the white part in FIG. 6 is the area specified in step S309.
In the example of FIG. 6, the distance L1 is calculated with d1 being 20 mm, and the region from the skin line to the distance L1 is specified as the surface correction region. However, the distance L1 is calculated by changing d1, and the distance L1 obtained is calculated. A distance image may be created from the average value. For example, it is conceivable to create a distance image by averaging three L1s obtained with d1 being 22 mm, 24 mm, and 26 mm.
FIG. 7 shows the surface correction process.
The surface correction processing is realized by software processing in cooperation with the
FIG. 8 shows an image in the region of interest of size M × N (pixel). Let the pixel value at the coordinates (x, y) be Fxy. FIG. 9 shows an image obtained by approximating the region of interest in FIG. 8 with a two-dimensional quadratic polynomial F (x, y) expressed by the following [Equation 1].
In step S703, the corrected pixel value F′xy is obtained by the following [Equation 4].
In this way, by identifying areas where the density change near the skin is large and performing surface correction, the density change in the area where the influence of the background trend near the skin is relaxed, so that abnormal shadow detection processing in the latter stage can be performed. The occurrence of false detection can be suppressed.
Next, the abnormal shadow detection process will be described.
In the abnormal shadow detection process, a candidate for an abnormal shadow is output by performing a determination using the curvature on the input medical image.
The curvature is obtained by approximating the normal section of the pixel of interest with a circle from a curved surface obtained from the density distribution of a medical image consisting of signal components in three directions: position (x direction, y direction) and density (z direction). Calculated by finding the radius.
FIG. 11 shows a density distribution curved surface E of a medical image composed of signal components in three directions of position (x direction, y direction) and density (z direction). An arbitrary pixel on the density distribution curved surface E is set as a target pixel p, and a plane determined by the normal m at the target pixel p is defined as a normal plane F. An intersection line between the normal plane F and the density distribution curved surface E, that is, a density distribution curved surface E cut out on the normal plane F is defined as a normal section J.
The curvature at the target pixel p can be obtained by approximating the shape of the normal section J with a circle using pixels around the target pixel p. That is, it can be obtained by approximating the shape of the normal section J with a circle. Let the parameter n be the number of pixels used when calculating the curvature. The parameter n is variable, is input by the
FIG. 12 shows an example of image signal distribution in the normal section J determined on the normal plane F at a certain rotation angle θ. Since the medical image is a digital image, it actually shows a distribution as shown in FIG. In FIG. 12, the vertical axis indicates the pixel value, and the horizontal axis indicates the position of the normal plane F in the xy plane direction.
For example, when the parameter n of the region range of the normal section J that approximates a circle is 3, a perfect circle that approximates the image signal of a total of 3 pixels including the pixel of interest p and the pixels located on the left and right sides thereof (hereinafter, approximate circle) Is calculated). When n is 3, since there are three approximation targets, the approximate circle is a perfect circle passing through the three points.
Thus, the region range n approximating the circle is determined, the normal plane F is rotated from 0 to 360 degrees with respect to the rotation angle θ of the normal plane F, and the normal curvature is obtained for each rotation angle. The rotation angle may be rotated by 1 degree, or may be rotated by 5 degrees, for example, to shorten the calculation time.
The curvature is an index indicating whether the curved surface is convex or concave. The larger the curvature in the positive direction, the concave the curved surface, and the larger the curvature value in the negative direction, the convex shape.
Abnormal shadows such as masses are classified as concave shapes. The average value of the obtained curvature, the curvature that becomes the maximum value, the curvature that becomes the minimum value, etc. are calculated as feature quantities, and feature quantities that exceed the preset threshold, that is, large concave areas are detected as abnormal shadow candidates. Is done.
Next, the false positive deletion process will be described.
As a method of deleting false positives, a false positive deletion method using contrast and a false positive deletion method using a concentration concentration filter are generally known. In both methods, an abnormal shadow such as a true mass is determined to be false positive based on the low concentration in the center.

First, a candidate centroid is set for the detected abnormal shadow candidate region (step S1301), and a circle at a certain distance d3 from the candidate centroid is designated as a candidate edge (step S1302). d3 is variable, and is input by the
FIG. 14 shows an example in which the circular region centered on the candidate centroid is divided into an inner region and an outer region according to a multiple of the distance d3 from the candidate centroid to the candidate edge in step S1303. In this example, the inner region is a region where the distance d2 from the candidate centroid is 0.8d3, and the outer region is a region where the distance d5 from the candidate centroid is 1.2d3 to 1.7d3. The distance d4 and the distance d5 are variable, and input is performed by the
Next, the average value of the pixel values in the outer region obtained in step S1303 is calculated (step S1304). Similarly, the average value of the pixel values in the inner area obtained in step S1303 is calculated (step S1305). Next, the difference between the average pixel value of the outer area and the average pixel value of the inner area obtained in steps S1304 and S1305 is calculated as the contrast of the candidate area (step S1306). The contrast obtained in step S1306 is compared with a predetermined threshold value (step S1307). This threshold value is variable, input is performed by the

FIG. 15 shows false positive deletion processing using a concentration concentration filter.
The false positive deletion processing using the concentration concentration filter is realized by software processing in cooperation with the
Gradient information is created for the detected abnormal shadow candidate region (step S1501). This gradient information is created by an edge detection filter. The edge detection filter calculates the gradient of the density of a medical image such as a radiographic image as a gradient vector.
Next, a voting process is performed based on the gradient information obtained in step S1501 (step S1502). A voting method will be described later. Based on the result of voting performed in step S1502, the concentration concentration is calculated (step S1503). Next, the concentration concentration degree obtained in step S1503 is compared with a predetermined threshold value (step S1504). If the concentration concentration degree is equal to or greater than the threshold value (step S1504; YES), the abnormal shadow is concerned. A candidate is detected as a true positive (step S1505). If the concentration concentration is less than or equal to the threshold (step S1504; No), the abnormal shadow candidate is deleted as a false positive (step S1506). The threshold value is variable and can be input by the
FIG. 16 shows a voting method using gradient information. In the example of FIG. 16, for the sake of brevity, the abnormal shadow candidate region will be described as four pixels.
A gradient vector is created based on the gradient information obtained in step S1502. Then, voting is performed for each maximum value of the diameter of the abnormal shadow to be detected in the gradient vector direction from each pixel of the candidate area. Here, voting is to set a constant distance gradient vector from each pixel of the abnormal shadow candidate region. In the example of FIG. 16, the maximum value of the diameter of the abnormal shadow to be detected is 15 mm. The number of times that the vote has been performed is added to each pixel of the abnormal shadow candidate region, and a final vote value is calculated.
That is, the vote value of each pixel is the number of times that the arrow representing the vote shown in FIG. In this example, the voting results of the pixel 1, the
A high density concentration level means that the direction of the gradient vector of each pixel in the candidate area is concentrated on the pixel. The gradient vector of each pixel in the vicinity of the abnormal shadow is substantially directed to the center of the abnormal shadow regardless of the magnitude of the pixel value of the abnormal shadow. Therefore, it can be said that the pixel having a large density concentration degree is a pixel at the center of the abnormal shadow.
The abnormal shadow candidates detected in this way are displayed on the
The above-described abnormal shadow detecting means using the curvature detects the concave sphere with high sensitivity using local information. There are many cases where a location near the skin where the concentration change is large is erroneously detected. Therefore, even if the false positive deletion means shown in FIG. 13 and FIG. 15 is used for the input medical image, it is often impossible to cope with erroneous detection.
