本発明による、医療用ナビゲーションシステムにおけるレジストレーション用テンプレートにより、手術治具が正確にレジストレーションするために、型押し状のマーキング又はガイドホールにて示される3以上のレジストレーションポイントの位置情報が正確に医療用ナビゲーションシステムに読み込まれてDICOMデータ上にこれを反映させることができる。
このため、「Mimics」により、三次元画像上において作製された型押し状マークのマーキング又はガイドホールにて示される3以上のレジストレーションポイント用のピンの形状データを0.01mm単位の精度にて該患者の手術対象部位のDICOMデータに付与し、出力することができる。
手術用ナビゲーションシステムにおいては、レジストレーションに先立ち、この「Mimics」より出力された、型押し状のマーキング又は、ガイドホールにて示される3以上のレジストレーションポイント用のピンの形状データが正確に付与されたDICOMデータを読み込み、しかる後、当該DICOM上の3以上のレジストレーションポイントと直接該患者の手術対象部位に正確に面接合されたレジストレーション用テンプレート上の3以上のレジストレーションポイント間においてレジストレーションが容易にしかも高精度にて行うことができる。

According to the registration template in the medical navigation system according to the present invention, in order to register the surgical jig accurately, the positional information of three or more registration points indicated by the embossed marking or the guide hole is accurate. 本願発明テンプレートを粉末焼結積層造形法又は光造形法等によって作製する際に、原材料として単なる樹脂等の透明材料を用いれば、透明なレジストレーション用テンプレートを作製することができる。
透明なテンプレートは、レジストレーションの際に嵌合すべき骨表面の状態が容易に視認できる利点がある。
粉末焼結積層造形法および光造形法に用いる三次元形状データは、生体の精密な立体形状モデルを製作するに際して、その原型となる人体の各部位の形状を、磁気共鳴映像法(MRI)、X線コンピュータ断層撮影(X線CT)、超音波撮影などのいずれか1種又は2種以上の複合方法を用いて透視計測又は外形計測したものである。得られた人体から実測した各部位の三次元的立体形状と寸法データは、例えば、医用デジタル画像を一度二次元のDICOMデータに変換し、さらにサーフェスタイプのSTLフォーマットデータに変換することにより、粉末焼結積層造形法および光造形法等に適用することができる。

実施例1
粉末焼結積層造形法により、手術対象の側頭部位模型及び側頭部位に密着するテンプレートを作製した。
粉末焼結材料として、平均粒径58μmの球状のナイロン11の粉末70重量%と、平均粒径60μmのガラスビーズ30重量%の混合物を用い、造形装置として、100W炭酸ガスレーザを備えた粉末焼結積層造形装置[3D−Systems社、「Vanguard−HS」]を用いた。
成人男子の側頭部位の骨をX線コンピュータ断層撮影したデータから側頭部位及びテンプレートのデータを作成した。テンプレートのデータには、3ヵ所のガイドホールの位置及び寸法のデータを追加した。
ソフトウェア[Materialise社、「Mimics」]を用いて造形用データに変換した。このデータを造形装置に入力し、積層ピッチ0.10mmで粉末焼結材料を逐次積層焼結し、実寸大の側頭部位模型及び図1に示すような寸法40×40mmの最大厚さ15mmのガイドホール2の付いたテンプレート1を作製した。
図2に示すように、作製した側頭部位模型3の表面にテンプレートを当接すると両者の面同士は精度良く接合して、前後左右にゆすっても移動しない安定な接合が得られた。
この安定な接合状態において、ガイドホールをレジストレーションポイントとして手術治具を当ててレジストレーションすることも可能であり、また、図2に示すガイドホール2を経由して図3のように側頭部位模型3の表面にレジストレーションポイントとして着色マーク4を付けることもできた。
作製した側頭部位模型3を、ドリルメスで切削したところ、自然骨と同一の感触で模型を切削することができた。
実施例2
粉末焼結積層造形法により側頭部位に密着する透明なテンプレートを作製した。
粉末焼結材料として、平均粒径58μmの球状のナイロン11の粉末を用い、造形装置として、100W炭酸ガスレーザを備えた粉末焼結積層造形装置[3D−Systems社、「Vanguard−HS」]を用いた。
成人男子の側頭部位の骨をX線コンピュータ断層撮影したデータからテンプレートのデータを作成した。テンプレートのデータには、3ヵ所のガイドホールの位置及び寸法のデータを追加した。また、側頭部位の骨の表面に正確に面接合する面の反対面には、側頭部位の骨の凹凸面のデータを用いた。
