Claims (42)
- 少なくとも1つの電極を有する少なくとも1つのリードに接続するための少なくとも1つのポートと、
前記少なくとも1つのポートに接続され、前記少なくとも1つの電極を使用して少なくとも1つの神経刺激治療を少なくとも1つの神経刺激ターゲットに施すように適合された刺激回路と、
前記少なくとも1つのポートに接続され、感知信号を出力するように適合された感知回路と、
前記少なくとも1つの神経刺激治療を施すための前記刺激回路と前記感知回路に接続されたコントローラと
を備え、トリガ・イベントに応答して、前記コントローラは、
前記神経刺激治療が前記感知信号を使用して施される刺激及び感知モードと、
前記神経刺激治療が前記感知信号を使用せずに施される刺激モードと、
前記神経刺激治療が前記神経刺激ターゲットに施されない感知モードとからなる群から選択された少なくとも2つのモードを切り換えるように適合されている埋め込み型機器。 - 前記コントローラは、前記刺激及び感知モード、前記刺激モード、前記感知モードのそれぞれに切り換えるように適合される請求項1に記載の機器。
- 前記トリガ・イベントは、自動的トリガを含む請求項1に記載の機器。
- 前記自動的トリガは、検出された機器変更に基づく請求項1に記載の機器。
- 前記検出された機器変更は、検出された電極障害を含む請求項4に記載の機器。
- 前記検出された機器変更は、機器に電力を供給する電池の寿命末期(EOL)判定を含む請求項4に記載の機器。
- 前記検出された機器変更は、検出されたリード障害を含む請求項4に記載の機器。
- 前記自動的トリガは、環境変化に基づく請求項1に記載の機器。
- 前記環境変化は、前記感知信号に干渉することができる検出された電気的干渉を含む請求項8に記載の機器。

- 前記患者駆動トリガは、モードを切り換えるために使用される患者駆動外部プログラマを含む請求項1に記載の機器。
- 少なくとも1つの電極を有する少なくとも1つのリードに接続するための少なくとも1つのポートと、
前記少なくとも1つのポートに接続され、前記少なくとも1つの電極を使用して少なくとも1つの神経刺激治療を少なくとも1つの神経刺激ターゲットに施すように適合された刺激回路と、
前記少なくとも1つのポートに接続され、感知信号を出力するように適合された感知回路と、
前記少なくとも1つの神経刺激治療を施すための前記刺激回路と前記感知回路に接続されたコントローラと
を備え、トリガ・イベントに応答して、前記コントローラは、少なくとも2つのモードを切り換えて神経刺激ターゲットを切り換えるように適合されている埋め込み型機器。 - 前記トリガ・イベントは、自動的トリガを含む請求項23に記載の機器。
- 前記トリガ・イベントは、患者駆動トリガを含む請求項23に記載の機器。
- 少なくとも1つの電極を有する少なくとも1つのリードに接続するための少なくとも1つのポートと、
前記少なくとも1つのポートに接続され、前記少なくとも1つの電極を使用して少なくとも1つの神経刺激治療を少なくとも1つの神経刺激ターゲットに施すように適合された刺激回路と、
前記少なくとも1つのポートに接続され、感知信号を出力するように適合された感知回路と、
前記少なくとも1つの神経刺激治療を施すための前記刺激回路と前記感知回路に接続されたコントローラと
を備え、トリガ・イベントに応答して、前記コントローラは、少なくとも2つのモードを切り換えて、前記感知信号が送り出される感知部位を切り換えるか、又は前記感知信号を供給するために使用される感知パラメータを切り換えるか、又は感知部位と感知パラメータの両方を切り換えるように適合されている埋め込み型機器。 - 前記コントローラは、少なくとも2つのパラメータを、又は少なくとも2つの複合パラメータを、又はパラメータと複合パラメータとを切り換えるように適合されている請求項26に記載の機器。
- 前記コントローラは、少なくとも3つのパラメータを、又は少なくとも3つの複合パラメータを、又は少なくとも3つのパラメータ若しくは複合パラメータを切り換えるように適合されている請求項27に記載の機器。
- 前記少なくとも2つのモードは、感知部位モードのセット内のモードとして与えられ、前記コントローラは少なくとも2つの感知部位を切り換えるように適合されている請求項26に記載の機器。
- 前記コントローラは、3つ又はそれ以上の感知部位を切り換えるように適合されている請求項29に記載の機器。
- 前記トリガ・イベントは、自動的トリガを含む請求項26に記載の機器。
- 前記トリガ・イベントは、患者駆動トリガを含む請求項26に記載の機器。
- 少なくとも1つの電極を有する少なくとも1つのリードに接続するための少なくとも1つのポートと、
前記少なくとも1つのポートに接続され、前記少なくとも1つの電極を使用して少なくとも1つの神経刺激治療を少なくとも1つの神経刺激ターゲットに施し、心調律管理(CRM)治療を施すように適合された少なくとも1つの刺激回路と、
前記少なくとも1つのポートに接続され、感知信号を出力するように適合された少なくとも1つの感知回路と、
前記少なくとも1つの神経刺激治療と前記CRM治療を施すための前記少なくとも1つの刺激回路と前記感知信号を受信するための前記感知回路に接続されたコントローラと
を備え、トリガ・イベントに応答して、前記コントローラは、
前記神経刺激治療が前記神経ターゲットに施される神経刺激治療モードと、
前記CRM治療が施されるCRM治療モードと、
前記神経刺激治療が前記神経刺激ターゲットに施され、前記CRM治療が施される神経刺激とCRM治療モードとからなる群から選択された少なくとも2つのモードを切り換えるように適合されている埋め込み型システム。 - 前記コントローラは、前記神経刺激治療モード、前記CRM治療モード、前記神経刺激とCRM治療モード、前記コントローラが薬物投与を制御する薬物治療モードからなる群から選択された少なくとも2つのモードを切り換えるように適合されている請求項33に記載のシステム。
- 前記トリガ・イベントは、自動的トリガを含む請求項33に記載のシステム。
- 前記トリガ・イベントは、患者駆動トリガを含む請求項33に記載のシステム。
- さらに、前記少なくとも1つの刺激回路と少なくとも1つの感知回路を収納する単一の機器・ハウジングを備える請求項33に記載のシステム。
- さらに、前記少なくとも1つの刺激回路を収納する第1の機器・ハウジングと少なくとも1つの他の刺激回路を収納する第2の機器ハウジングとを備える請求項33に記載のシステム。
- 埋め込み型機器であって、
少なくとも1つの電極を有する少なくとも1つのリードに接続するための少なくとも1つのポートと、
前記少なくとも1つのポートに接続され、前記少なくとも1つの電極を使用して少なくとも1つの神経刺激治療を少なくとも1つの神経刺激ターゲットに施すように適合された刺激回路と、
前記少なくとも1つのポートに接続され、感知信号を出力するように適合された感知回路と、
前記少なくとも1つの神経刺激治療を施すための前記刺激回路と前記感知回路に接続されたコントローラと
を備え、前記コントローラは前記機器を少なくとも2つのモードで動作させ、トリガ・イベントに応答してモードを切り換えるように適合され、前記少なくとも2つのモードは、動作モードのセット、刺激部位モードのセット、フィードバック・モードのセットからなる群から選択されたモードの少なくとも1つのセットのうちから与えられ、
動作モードの前記セットに対して、前記コントローラは、
前記神経刺激治療が前記感知信号を使用して施される刺激及び感知モードと、
前記神経刺激治療が前記感知信号を使用せずに施される刺激モードと、
前記神経刺激治療が前記神経刺激ターゲットに施されない感知モードとからなる群から選択された少なくとも2つのモードを切り換えるように適合され、
刺激部位モードの前記セットに対して、それぞれの刺激部位モードでは前記神経刺激治療を所定の神経刺激ターゲットに施し、前記コントローラは、少なくとも2つの刺激部位モードを切り換えて神経刺激ターゲットを切り換えるように適合され、
フィードバック・モードの前記セットに対して、前記コントローラは、
それぞれの感知部位モードでは所定の感知部位から感知信号を供給し、前記神経刺激治療のフィードバック制御を施し、前記コントローラは、少なくとも2つの刺激部位モードを切り換えて神経刺激ターゲットを切り換えるように適合される、感知部位モードのセットと、
それぞれの感知パラメータ・モードでは所定のパラメータ又は複合パラメータを感知し、前記コントローラは、少なくとも2つの感知パラメータ・モードを切り換えて、パラメータ同士を、又は複合パラメータ同士を、又はパラメータと複合パラメータとを切り換えるように適合されている、感知パラメータ・モードのセットとの群から選択された少なくとも2つのモードを切り換えるように適合されている埋め込み型機器。 - 前記感知回路は、心調律を感知するように適合され、前記刺激回路は、少なくとも1つの心調律管理(CRM)治療を施すように適合され、
前記少なくとも2つのモードは、動作モードのセット、刺激部位モードのセット、フィードバック・モードのセット、治療モードのセットからなる群から選択されたモードの少なくとも1つのセットのうちから与えられ、
治療モードの前記セットに関して、前記コントローラは、
前記神経刺激治療が前記神経ターゲットに施される神経刺激治療モードと、
前記CRM治療が施されるCRM治療モードと、
前記神経刺激治療が前記神経刺激ターゲットに施され、前記CRM治療が施される神経刺激とCRM治療モードとからなる群から選択された少なくとも2つのモードを切り換えるように適合されている請求項39に記載の機器。 - 神経刺激治療を施すための手段と、
トリガ・イベントに応答して、モードを切り換えるための手段であって、
前記神経刺激治療を制御するために感知信号を使用して神経刺激治療を施すように適合された刺激及び感知モード、感知信号を使用せずに神経刺激治療を施すように適合された刺激モード、神経刺激治療を差し控えるように適合された感知モードのうちの少なくとも2つを切り換えるために使用される動作モードのセットと、
神経刺激ターゲットを切り換えるために使用される刺激部位モードのセットと、感知部位を切り換えるため、又は感知パラメータを切り換えるため、又は感知部位と感知パラメータを切り換えるために使用されるフィードバック・モードのセットとからなる群から選択されたモードの少なくとも1つのセット内のモードを切り換えるための手段を含む手段とを備えるシステム。 - さらに、心調律管理(CRM)治療を施すための手段を備え、モードの前記少なくとも1つのセットは、前記動作モードのセット、前記刺激部位モードのセット、前記フィードバック・モードのセット、治療モードのセットからなる群から選択され、その治療モードのセットは、前記神経刺激治療が前記神経ターゲットに施される神経刺激治療モード、前記CRM治療が施されるCRM治療モード、前記神経刺激治療が前記神経刺激ターゲットに施され、前記CRM治療が施される神経刺激とCRM治療モードを含むる請求項41に記載のシステム。
