1981年、埜中らによって縁取り空胞型遠位型ミオパチー(Distal Myopathy with Rimmed Vacuoles, DMRV)が報告された(非特許文献1)。DMRVは、15から30歳で発症することが多く、下腿伸筋群が初期より強く障害され(図1)、その後大腿屈筋群の筋力低下、筋萎縮も加わり、発症10年以内に車椅子の生活を余儀なくされることが多い難治性疾患である。2001年、Eisenbergらによって、遺伝性封入体ミオパチー(Hereditary Inclusion Body Myopathy, HIBM)の原因がGNE(UDP-N-acetylglucosamine-2-epimerase/N-acetylmannosamine kinase)遺伝子の変異であることが示唆された(非特許文献2)。2002年、本発明者の浅賀らはHIBMとDMRVは同じ遺伝子の変異に起因する疾患であることを報告した(非特許文献4)。しかしながらGNE遺伝子の欠損は致死性であるため(非特許文献3)、GNE遺伝子ノックアウト動物をDMRVの病態解明と治療方法の開発研究のためにモデル動物として用いることは不可能である。
本発明者の浅賀らは、DMRVの日本人患者家系の解析から、GNE遺伝子の1714番目の塩基がグアニン(G)からシトシン(C)に置換され、キナーゼ部位に含まれるGNEタンパク質のN末端から572番目のアミノ酸残基であるバリン残基がロイシン残基に置換されているという点変異(V572L)を発見した(非特許文献4)(図2)。
GNEタンパク質は、シアル酸の生合成の鍵となる酵素であり、ウリジン2リン酸N-アセチルグルコサミン2エピメラーゼ(UDP-N-acetylglucosamine 2-epimerase)及びN-アセチルマンノサミンキナーゼ(N-acetylmannosamine kinase)の2つの活性を持っている。リコンビナントGNEタンパク質の研究によりV572L変異ヒトGNEはキナーゼ活性が低下し、その患者の骨格筋のO型糖鎖に影響があることが報告されている(Am J Pathol 166: 1211-1130, 2005)。さらに、GNE代謝産物であるN-アセチルマンノサミンやN-アセチルノイラミン酸を加えることで、DMRV患者から採取した細胞のシアル酸化状態が回復したことが報告された(非特許文献5)。
しかしながら、現在においてもDMRVには有効な治療法は見つかっておらず、臨床現場では徐々に悪化していく状態を、手をこまねいて見ているしかないのが現状である。

しかしながら、本願においては下記の実施例に示す様に、GNEタンパク質の機能及び構造を考慮して、好ましくはGNEタンパク質のキナーゼドメイン、さらに好ましくはGNE遺伝子のエクソン10において変異を導入させた。

摘出された筋組織はクリオモルドに対して筋線維を垂直にたてるように配置し、筋組織のほとんどが露出するように少量の包埋剤を流し込んだ。
クリオモルドをピンセットではさみ、あらかじめ十分に冷却したイソペンタン中に沈め、細かく揺すりながら凍結させた。
イソペンタンは、100mlビーカーに70ml程を注ぎ入れ、液体窒素中にビーカーを浸けて冷却した。
クライオスタットを用いて10μmの厚さの切片を作製し、MASコートされたスライドグラスに貼付けた。

7. イオン交換水で軽く洗浄した。
8. グリセリンゼリーで封入した。

GNE点変異(V572L)マウスの表現型
GNE完全破壊マウスは胎生致死であったのに対し、GNE点変異マウスは正常に生まれてきた。雌雄ともに妊よう性に異常はなく、外見上は特に異常は認められなかった。
DMRVは、筋線維の中に縁取り空胞が認められることが特徴であるが、細胞質内に好塩基性封入体が観察されることも特徴であり、これらの病理的変化が診断の決め手となる。本願発明のトランスジェニック非ヒト哺乳動物の骨格筋を、月齢を追って解析したところ、明らかな縁取り空胞の形成は認められないが、患者でも病変が出やすい大腿四頭筋において、10ヵ月齢以降の個体において好塩基性封入体が観察された(図10及び図11)。15ヵ月齢まで観察を続けたところ、多くの筋繊維で封入体が観察された。さらに、電子顕微鏡を用いて15ヵ月齢のGNE点変異マウスの大腿四頭筋の封入体を観察したところ、オートファゴソーム、オートリソソーム、グリコゲノソーム様構造や管状構造が観察された(図12)。これらは、縁取り空胞の出現の前兆であると考えられる。
また、再生筋線維と思われる像も観察されたことから、筋線維の破壊の指標となる血漿クレアチンキナーゼ値を測定したところ、野生型マウスに比べてGNE点変異ホモマウスとヘテロマウスで高い値を示す個体がいることがわかった(図13)。さらに、糸球体障害の指標となり、慢性糸球体腎炎、ネフローゼ症候群、腎硬化症など糸球体に組織変化をきたす腎臓疾患患者の尿中に出現する尿中アルブミン濃度を測定したところ、野生型マウスとGNE点変異ヘテロマウスの間には大きな違いは見られなかったが、GNE点変異ホモマウスでは、有意に高い値を示し、糸球体に障害を有することが推察された(図14)。そこで、腎臓組織を採取し観察したところ、皮質においては尿細管拡張、出血及び炎症性細胞浸潤が見られ(図15)、髄質においては出血が見られ(図16)、腎臓機能が不全となっていることが明らかとなった。
さらに、本発明のGNE点変異ホモマウス、GNE点変異ヘテロマウス及び野生型マウスの500日齢までの生存率を確認したところ、離乳した全てのマウスにおいて1ヵ月や2ヵ月で死亡するマウスは確認されず、80日齢までの生存率がほぼ100%であった(図17)。その後、GNE点変異ホモマウスの生存率は、GNE点変異ヘテロマウス及び野生型マウスと比較して低い傾向を示したが、約40%が500日齢まで生存し、モデル動物として有用であることが明らかとなった。
以上のことから、本発明のGNE点変異マウスは、DMRVの特徴的な病態を発症しており、DMRVモデルマウスであることが明らかとなっただけでなく、腎臓疾患モデルマウスであることも明らかとなった。

Furthermore, when the inclusion body of the quadriceps femoris of a 15-month-old GNE point mutant mouse was observed using an electron microscope, autophagosomes, autolysosomes, glycogenosome-like structures and tubular structures were observed (FIG. 12). These are thought to be precursors of the appearance of rimmed vacuoles.
In addition, since an image that seems to be a regenerated muscle fiber was also observed, the plasma creatine kinase level, which is an index of muscle fiber destruction, was measured. The value was higher in the GNE point mutant homo mouse and hetero mouse than in the wild type mouse. It was found that there were individuals to show (FIG. 13). Furthermore, when measuring the urinary albumin concentration that appears in the urine of patients with kidney diseases that cause tissue changes in the glomeruli such as chronic glomerulonephritis, nephrotic syndrome, nephrosclerosis, etc. Although there was no significant difference between the GNE point mutant hetero mice, the GNE point mutant homo mice showed a significantly high value, and it was inferred that the glomerulus was damaged (FIG. 14). Thus, when kidney tissue was collected and observed, tubular expansion, bleeding and inflammatory cell infiltration were observed in the cortex (FIG. 15), bleeding was observed in the medulla (FIG. 16), and renal function was incomplete. It became clear that.
Furthermore, when the survival rate of the GNE point mutant homo mouse, GNE point mutant hetero mouse and wild type mouse of the present invention up to 500 days of age was confirmed, mice that died in 1 month or 2 months were confirmed in all weaned mice. The survival rate up to 80 days of age was almost 100% (FIG. 17). Thereafter, the survival rate of GNE point mutant homo mice tended to be lower than that of GNE point mutant hetero mice and wild type mice, but about 40% survived to 500 days of age and were useful as model animals.
