Abstract
The present invention relates to recombinant human alpha I-antitrypsin (rAAT) dry powder composition.
Description
The present invention relates to dry protein formulations and in particular to alpha 1-antitrypsin (AAT).
AAT and recombinant alpha 1-antitrypsin (rAAT) are effective therapeutic agents for a number of clinical indications. rAAT is a 44 kD 395 amino acid protein that is an aglycosylated protein and has an amino acid sequence that is identical to human plasma protein (AAT) except for the amino-terminal N-acetylmethionine. It is desirable that this is a dry, stable preparation of AAT or rAAT that can be immediately reconstituted in water and used immediately.
Excipients were typically used in dry protein formulations, mainly including buffers, saccharides, and surfactants (see, eg, Carpenter et al, Pharm. Biotechnol. 13: 109-133, 2002). See Other effective stabilizing excipients include fillers, chelating agents, antioxidants, reducing agents, and amino acids.
US Pat. No. 5,780,001A describes a dry powder formulation of AAT for administration by inhalation. Various drying techniques are presented.
Prolastin (Bayer) is a lyophilized formulation of glycosylated AAT derived from human plasma. With 1 g of alpha 1-antitrypsin functional active per 40 mL of sterile water, when reconstituted as directed, the liquid composition is> 20 mg / ml AAT, 100-210 mEq / L Na, 60-180 mEq / L Cl, 15-25 μM sodium phosphate, <5 ppm PEG, and <0.1% sucrose. The lyophilized formulation should be stored under freezing.
Vemuri et alは、Stability and Characterization of Protein and Peptide Drugs:Case Histories,ed.Wang and Pearlman,Plenum Press,NewYork(1993)の9章において、rAATの製剤、主に液体型を記載する。 Vemuri et al., Stability and Characterization of Protein and Peptide Drugs: Case History, ed. In Chapter 9 of Wang and Pearlman, Plenum Press, New York (1993), formulations of rAAT, mainly liquid forms, are described. Stability is promoted by increasing its salt content, for example at pH 7.5. However, salts are generally considered unsuitable for lyophilized formulations because of their lower glass transition temperature.
Travis et al.,(J.Biol. rAAT stabilization has certain problems compared to natural proteins. Travis et al. , (J. Biol. Chem. 260: 4384-4389, 1985) describe a comparison of the thermal stability of rAAT from yeast and AAT from natural plasma. The half-life of the non-glycosylated rAAT, which is against its heat stress response activity, is significantly shorter than the half-life of its natural glycosylated counterpart.
SUMMARY OF THE INVENTION The present invention is based on the discovery of a dry formulation of rAAT, where the rAAT has a defined concentration of rAAT and salt, and if not lyophilized (ie, 5 ° C. or less) Excellent stability for up to 2 years or more. This can be achieved without compromising other desired properties, such as quick reconstitution and synthesis solutions that do not contain unwanted ones. Excipient content, particularly those that can potentially promote microbial growth, is minimized and unapproved or unofficial drugs can be avoided. The formulation does not have a bad smell or taste. The formulation can be modified for convenient lyophilization.

The salt used will typically be NaCl. However, it is readily understood by those skilled in the art that other salts can have the same effect whether the cations are different (eg, KCl) or the anions are different (eg, NaBr) or both. Will be done.
The dry powder composition of the present invention may be free of sugar. It usually contains less than 1% and preferably less than 0.5% water.

The composition may be retained with at least 80% monomeric rAAT, preferably> 95% monomer during storage at 50 ° C. or equivalent for 3 months.
Stability criteria (retention activity) and denaturation are demonstrated in the art by so-called tests by those skilled in the art. The activity test was performed by Beatty et al, J Biol. Chem. 255, 3931, 1980, based on the porcine pancreatic elastase inhibition test. Denaturation is investigated by molecular sieve chromatography (SEC) HPLC method by aggregate formation and evaluation of unmodified rAAT reported as% of monomer. The equivalence of the given conditions can be understood by the so-called person skilled in the art, ie based on the Arrhenius equation.
In order to produce the formulations of the present invention, solutions or other compositions containing the desired ingredients were dried. Suitable methods of drying include, but are not limited to, lyophilization, spray drying, spray lyophilization, fluidized bed technology, and supercritical fluid drying.
Preferred drying procedures are freeze drying and spray drying. Both procedures can be performed by standard techniques in the field known to those skilled in the art. For example, spray drying consists of a three step procedure that results in dry particle formation. The process begins by atomizing the liquid raw material and using compressed air to form a fine droplet spray, and then drying the droplets by evaporating the mist consisting of the droplets. Heat. The final particles in the formation of the dry powder are collected as a product. The gas and excess fine dust are removed. These steps are performed using three components: a nozzle-shaped sprayer; a drying chamber; and a collection system known as a cyclone and pot.
The dry formulation or reconstituted liquid composition is suitable for administration to a patient in need thereof. Suitable routes of administration include, but are not limited to, inhalation, topical, subcutaneous, and intravenous delivery.

FTIR spectra were collected for these formulations to assess the structural stability of rAAT in the dry state. It has been shown that the retention of the native structure in the solid can predict the long-term storage stability of the dried protein (Carpenter et al, 2002, supra). FIG. 1 shows the FTIR of liquid and solid rAAT in the 917-1 formulation. It should be noted here that the amide I region (1700-1600 cm −1 ) is sensitive to changes in the secondary structure and that all peaks in the second derivative spectrum are negative. Each peak in the amide I region corresponds to a different secondary structure type. There is a clear perturbation in the rAAT conformation before and after lyophilization. The peak near 1655 cm −1 corresponds to the α helix structure, the band near 1635 cm −1 corresponds to the β sheet structure, and the peak at 1688 cm −1 is attributed to the extended β strand structure or β sheet. The random coil structure is assigned to a band around 1644 cm −1 .
Liquid samples representing the natural structure exhibit a significant amount of beta sheet and alpha helix structure. With respect to lyophilization, if there is no stabilizer (917-11 formulation), there is an important structural perturbation as shown in FIG. The α-helix band disappears almost completely, while there is a significant increase in the band above 1680 cm −1 , corresponding to an expanded and loop structure. FIG. 3 shows the effect of salt on rAAT in the solid state. The 917-1, -3, and -4 formulations added 175 mM, 100 mM, and 50 mM NaCl to 20 mM sodium phosphate, 5 mM citrate, 2.5 mM NAC, and 3 mM L-Met, respectively. Included.
The lower salt concentrations, formulations 3 and 4, appear to have the greatest degree of structural perturbation, and all three formulations are less perturbed than without the stabilizer. Eventually, lyophilization appears to cause some structural perturbations compared to the natural structure. The degree of change is minimized by the addition of excipients containing salt. NaCl concentrations above 50 mM appear to cause more natural-like structures, with 50-100 mM being the optimal condition. The results are unexpected because sugars are usually required or used to maintain the native protein structure in the dry state. The structural stability of these formulations was evaluated by FTIR to reveal subtle differences between rAAT structures in the dry state. FIG. 4 shows FTIR spectra of rAAT formulated in sugar-based formulation (1008-1) and salt-based formulation (1008-2). The secondary structure of rAAT in both these formulations can be superimposed. The fact that salt can achieve the same level of stability as protein at high concentrations is remarkable and not obvious. During reconfiguration, the original rAAT secondary structure is retained as shown in FIG.
Based on the surprising observation of freeze-dried rAAT formulations containing a high concentration of NaCl, the stability analysis has shown that the addition of a common stabilizer in the lyophilized protein formulation is the structural stability and acute stability of rAAT (60 To systematically evaluate whether to promote 3 months at ℃. Saccharides are commonly used in protein formulations to stabilize the molecules, possibly by hydrogen bond replacement and subsequent removal of water molecules around the protein during lyophilization. Sugar is also shown to be a non-crystalline environment in the dry state, promoting the structural stability of the protein, and effectively replaces the water of hydration that is removed during drying. Surfactants are also often used in protein formulations to reduce surface adsorption that can damage the protein. Since the possible route of administration of rAAT is pulmonary delivery via aerosolization, the effect of surfactants should be particularly noted. Therefore, the role of polyoxyethylene sorbitan such as polysorbate 80 (Tween 80) is also evaluated at various concentrations. These formulations are shown in Table II.
The lyophilized formulations were evaluated for short-term stability (1 and 3 months) under accelerated storage conditions at 60 ° C. This storage temperature is particularly stringent for assessing protein stability and can affect the results for trehalose-based formulations with a particularly high glass transition temperature (T g ). The rationale for choosing this temperature was based on previous stability tests that evaluated the stability of rAAT in a shorter frame. Reconstituted activity and monomer percentages were measured over a maximum of 3 months storage at 60 ° C. and are shown in Tables III and IV, respectively.
No significant differences were observed in any of the formulations tested after 1 month, suggesting that both sugar-containing and sugar-free formulations show comparable stability. Stability data after 3 months storage at 60 ° C. showed more extensive results. Formulations containing both sugar and salt resulted in a more stable analytical result than when either sugar or salt was included. These data are consistent with the FTIR test showing a high degree of secondary structure retention in these formulation types. The low specific activity seen in the 1008-2 formulation could be attributed to the moisture content in the formulation, which was about 1% higher than that measured in the other selected formulations. This suggests that a stable lyophilized rAAT formulation should preferably have a moisture content of less than 1%.
These results suggest that rAAT is a relatively stable protein and does not appear to require sugars for stabilization in the lyophilized state.
