図7は、ファイバ2の入射前にレーザ光のNAを可変させる手段について説明する図である。図7(a)は入射NAが0.08のときを示し、図7(b)は入射NAが0.16のときを示した図である。図7(a)では、レーザ光源10から出射されたレーザ光が集光レンズ12で集光され、ファイバ2へと入射されている。図7(b)では、スポットサイズの設定によるNA変更信号を受けた制御部70が駆動ユニット60を動作させ、ガラス板61をレーザ光源10と集光レンズ12の間に挿入している。ガラス板61はレーザ光源10から出射したレーザ光の中心光軸に対して傾けられているため、ガラス板61を透過するレーザ光は屈折される(横ずれが起きる)。また、ガラス板61はレーザ光を中心光軸から偏心させる程度の厚み、ここでは1.6mmを持っているため、図7(a)のときに対して集光レンズ12に入射するレーザ光の光軸をずらすとができる。ガラス板61で光軸に対して横ずれしたレーザ光は集光レンズ12により、ファイバ2へと入射される。このとき、ファイバ2へと入射されるレーザ光は集光レンズ12の周辺側を通るため、ファイバ2への入射角(入射NA)はガラス板61がないときよりも大きくなる。これにより、ファイバ2の出射端面2aでの出射角(出射NA)も大きくなる。このときの出射NAは、入射NAに対応している。つまり、入射NA0.08のならば、ほぼ出射NA0.08に、入射NA0.16ならば、ほぼ出射NA0.16となる。このようにして、コントロール部3の操作によって、ファイバの出射NAを切り換えることができる。図7の例のようにファイバ入射NA(ファイバ出射NA)の切換えを2段階で行う場合、その入射NAの切換えは、レーザ光がケラレないスポットサイズで行うことが好ましい。例えば、スポットサイズ100μm以上で入射NAを広くする。

このようなNAの切り換えを以下に説明する。術者がスポット径調節ツマミ5aを調節し、所望のスポットサイズを選択する。選択されたスポットサイズに応じて、ズームレンズ22、24が移動される。このとき、ポテンショメータ25からスポットサイズの設定信号が制御部70へ入力される。制御部70はスポットサイズの設定信号に応じてガラス板61を光路に挿入するか否かを決定する。例えば、スポットサイズが100μm未満の場合は、ガラス板61を光路から外したままとする。スポットサイズが100μm以上の場合は、ガラス板61を光路に挿入してファイバ出射NAを0.16に変える。これにより、導光光学系(特にSp値)を変更することなく、また、導光光学系でのレーザ光のケラレを回避しつつ、角膜や水晶体でのエネルギ密度を低減したレーザ照射が行える。

そこで、先に述べた本発明の実施形態で、広視野倒像レンズ100を用いた光凝固時にどの程度まで照射レーザ光のスポットサイズが拡大可能であるかを説明する。まず、ファイバ出射NAを変化させると、透光体でのビーム径がどのような変化が生じるかを、図10を使用して説明する。図10では、照射レーザ光のNAを5段階で変化させた場合の透光体上でのビーム径を計算した結果を示している。ここでは、広視野倒像レンズ100を用いた場合のビーム径を算出している。図10では、NA0.08からNA0.4まで、NAを0.08ずつ変化させている。図10(a)、(b)のいずれの図でも横軸は照射レーザ光の眼底でのスポットサイズを表している。ここでは、2倍程度の広視野倒像レンズ100を用いるいるため、眼底でのスポットサイズは装置本体1で設定したスポットサイズの2倍程度となっている。図10(a)、(b)の縦軸は各透光体上でのビーム径を表している。図10(a)は角膜上、図10(b)は水晶体上のビーム径についてのグラフである。

図10(a)、(b)に示されるように、照射レーザ光のスポットサイズが大きくなるに従って、各透光体でのビーム径が小さくなる傾向が分かる。図上の「角膜上=網膜上」、「水晶体上=網膜上」のラインは、透光体上でのビーム径が眼底上でのスポットサイズと同じ大きさになる部分を示している。このラインは、一般的に言われている目安であり、先に挙げた日特許文献2にも記載されている。図示するように、NAを大きくするに従って、各透光体での光束(ビーム径)が大きくなる傾向にある。ビーム径が大きくなることで、透光体上でのエネルギー密度が低下する。すなわち、NA0.08の場合は適用可能な眼底でのスポットサイズは400μm(多くとも500μm、設定スポットサイズで200〜250μm)程度までであったが、2倍のNA0.16にした場合は600μm(設定スポットサイズで300μm)まで適用可能になり、3倍のNA0.24にした場合は700μm(設定スポットサイズで350μm)程度まで適用可能となる。さらに、4倍のNA0.32にした場合は800μm(設定スポットサイズで400μm)まで適用可能になり、5倍のNA0.4にした場合は900μm(設定スポットサイズで450μm)まで適用可能となる。

しかしながら、導光光学系の可動ミラー26等のサイズの問題から、NAを大きくするには限界がある。図11は、NAを変えたときの照射レーザ光のスポットサイズと可動ミラー26位置での光束径の関係を示す図である。図11の縦軸は照射レーザ光のビーム径であり、横軸は可動ミラー26位置での光束の半径を表している。図11でも図10と同様にNAを5段階で変化させている。図11の横軸に水平なラインは可動ミラー26の限界サイズを示したものである。例えば、可動ミラー26の限界サイズを20mmとすれば、この限界を示すラインは10mmとなる。このラインを超えるとレーザ光はケラレてしまう。図示するように、NAを大きくするに従って適用できる設定スポットサイズも大きくなる。すなわち、NA0.08の場合は最小の50μm(眼底で100μm)までカバーできているが、2倍のNA0.16では100μm(眼底で200μm)以上が適用可能であり、NA0.24では150μm(眼底で300μm)以上が適用可能であり、NA0.32では200μm(眼底で400μm)以上が適用可能であり、NA0.4では250μm(眼底で500μm)以上が適用可能である。ここで説明したNAとスポットサイズの対応は、本実施形態の光学系に関してのものであり、他の光学系であれば、これらNAとスポットサイズの対応は多少異なる。

以上のようなことから、ファイバ出射NAは、ファイバより後の導光光学系、特に、可動ミラー26でレーザ光がケラレることを回避する範囲との関係で変更することが必要となる。図7で示したように、NA0.8とNA0.16の2段階の切換えの構成においては、設定スポットサイズ50μm以上〜100μm(眼底で100〜200μm)未満まではNA0.08とし、100μm(眼底で200μm)以上の場合にNA0.16に切換える。さらにNA0.24が追加された3段階の切換えの構成においては、さらに設定スポットサイズが150μm(眼底で300μm)以上の場合にNA0.32に切換える。同様に、さらにNA0.32が追加された4段階の切換えの構成においては、設定スポットサイズが200μm(眼底で400μm)以上の場合にNA0.24に切換える。さらにNA0.4が追加された5段階の切換えの構成においては、設定スポットサイズが250μm(眼底で500μm)以上の場合にNA0.4に切換える。NAを連続可変の構成においは、さらに切換えのステップを細かくすることができる。スポットサイズの変倍と導光光学系の光路でのレーザ光のケラレを回避する範囲との関係は、制御部70が持つメモリに予め記憶しておけば良い。
