Abstract
Description
The present invention relates to an improvement in a cone-beam X-ray CT apparatus for the head and neck region.
More specifically, the present invention relates to a cone-and-beam X-ray CT apparatus for head and neck that can improve a decrease in diagnostic ability due to artifacts peculiar to the X-ray CT apparatus by a simple method.
Conventionally, in a cone-and-beam X-ray CT apparatus for head and neck including dental use, an X-ray generator, for example, at each end of a swivel arm hanging from a swivel drive unit, with the subject's head in between, An image receiving device composed of an image intensifier with a 4-inch effective field of view and a camera system is arranged oppositely, and a cone beam X having a small diameter is formed on a portion of interest in the head and neck including the tooth jaw region of the subject while turning around a turning axis. An apparatus for obtaining a CT image by irradiating a line and performing image processing on data transmitted through a subject is known.
These devices include a local irradiation X-ray CT imaging device in which the pivot axis of the swivel arm does not move during imaging and the image is taken only by rotation, and the pivot axis of the swivel arm rotates as necessary during imaging. Panoramic X-ray CT imaging apparatuses of a type that can be moved in the horizontal direction perpendicular to the body axis from time to time and can capture a wider area are known. The X-ray exposure dose to the subject is small.
On the other hand, apart from these devices, a device capable of obtaining a wider reconstructed image area including the tooth and jaw region using an image receiving device comprising a 9-inch or 12-inch image intensifier and a camera system. On the other hand, it has been pointed out that the X-ray exposure dose to the subject per radiograph increases considerably in proportion to the area of the reconstructed image area.
Recently, an apparatus using a flat panel type (FP type) digital sensor as an image receiving apparatus is also known.
Since these devices are all cone-beam X-ray CT devices in the head and neck region including the tooth and jaw region, high X-ray absorption such as a metal prosthesis applied to the tooth and jaw region on the reconstructed image. It is known to be easily influenced by artifacts by the body, and in some cases it is known to cause a significant decrease in diagnostic ability, and some proposals for the purpose of improving it have also been made.
In recent years, cone-beam X-ray CT systems for head and neck including dental use are becoming more and more popular, and are smaller and lighter than ever, diversified functions, improved diagnostic ability, improved processing speed, and operation. There is a demand for improvement in performance.
The present invention has been made in view of the above-described problems. High X-ray absorption of a metal prosthesis or the like applied to a tooth and jaw region peculiar to a cone beam X-ray CT apparatus in a head and neck region including a tooth and jaw region. If it is determined that the reconstructed image is affected by the artifacts caused by the body, or if it is determined that the part of the reconstructed image is affected by the artifacts, the diagnosis is affected. In addition to the above-mentioned proposal regarding the region of interest, an object is to provide an apparatus that improves diagnostic ability by a simpler method that has less influence on the processing speed by using a normal tomographic method together.

According to the present invention, when it is determined that the MPR image reconstructed and displayed on the display device greatly hinders the diagnosis due to the artifact, the center position of the partial region of interest to be particularly interpreted in the axial image is visually checked. According to a fifth aspect of the present invention, in the first to fourth aspects of the invention, the display device is selected by specifying the region of interest and its center, and selecting a tomographic angle. A tomographic image that can be continuously or intermittently displayed on the display device is a tomographic image for the entire circumference (360 °) at a tomographic oscillation angle, that is, a number of tomographic images corresponding to 360 ° / tomographic oscillation angle. It is a construction algorithm.
According to a sixth aspect of the present invention, in the first to fifth aspects of the present invention, the display image mode switching means includes a tomographic image of a conventional method displayed at an arbitrary tomographic angle. It has frame advance angle setting means for setting or selecting an arbitrary angle for sequentially changing the display area by frame-advancing the entire area of interest without changing the angle. Yes.
According to a seventh aspect of the present invention, in the sixth aspect of the present invention, the tomographic image reconstruction algorithm is selected by specifying the region of interest and its center position and selecting the tomographic angle. All or part of the tomographic image for the entire circumference (360 °) at the specified swing angle is displayed on the display device, and the frame advance is performed without changing the tomographic swing angle for the displayed image. A tomographic image reconstruction algorithm capable of displaying the entire circumference of the region of interest or a part of the tomographic image on the display device by sequentially changing the display region in a frame-by-frame manner set by the angle setting means or at a selected angle. It is characterized by being.
These cone beam X-ray CT apparatuses described above are all intended for imaging of the head and neck region including the tooth jaw region, so that a high level of a metal prosthesis or the like applied to the tooth jaw region on the reconstructed image. It is known that it is easily affected by the X-ray absorber, and in some cases, causes artifacts that greatly reduce the diagnostic ability, whereas according to the present invention, it is provided on the screen of the display device. After switching to the display image mode for displaying the tomographic image of the conventional method by the display image mode switching means, the center position of the partial region of interest in the CT axial image reconstructed and displayed on the display device is displayed. By displaying with the cursor provided on the device, a normal tomographic image having an arbitrarily selected tomographic angle of the region of interest with the center position as the rotation center is displayed on the display device from the base X-ray projection data For example, the tomographic image reconstruction algorithm is arbitrarily selected from a plurality of tomographic swing angles such as 8 °, 15 °, 30 °, 45 °, 60 °, 72 °. As the tomographic image, tomographic images for the entire circumference (360 °) at the selected tomographic swing angle, that is, tomographic images of the number corresponding to 360 ° / tomographic swing angle are continuously or intermittently displayed on the display device. . Further, if necessary, the display area can be set by the frame advance angle setting means, or the display area can be sequentially changed at the selected angle in the form of frame advance so that the entire circumference of the region of interest or a partial tomographic image can be displayed on the display device. Artifacts as a CT image due to a high X-ray absorber such as a metal prosthesis are extremely reduced, and the area other than the center of the tomogram is blurred as a trapezoid image. In addition, the tomographic image from a direction with less obstruction shadow can be obtained by sequentially changing the display area in a frame advance manner at the angle selected by the frame advance angle selection means for the portion that has been blurred as a trapezoid image. Since the display is possible, the diagnostic ability of the specific part of the region of interest can be greatly improved by utilizing the above mode particularly when the influence of the artifact is large in the CT image with respect to the partial region of interest.
In addition, the metal part is extracted from the original image of the X-ray absorption coefficient distribution information obtained by filtering and back-projecting the base data, and the calculated projection image of the metal part is superimposed on the original X-ray projection image. The combined corrected X-ray projection image is again filtered and backprojected. Alternatively, the high X-ray absorber is identified on the measurement data. The measurement data of the high X-ray absorber region is converted. The measurement data of the high X-ray absorber region is smoothed. By executing reconstruction after transformation and smoothing, the present invention requires a complicated process such as obtaining an image of the X-ray absorption coefficient distribution information of the subject with little influence of artifacts. Effective use of conventional tomographic images that have been processed for tomographic image reconstruction using the tomographic image reconstruction algorithm programmed based on the conventional tomographic method from X-ray projection data (X-ray attenuation coefficient) obtained by radiography By doing so, it is possible to easily improve the deterioration of the diagnostic ability due to the influence of the artifact on the CT image.

In addition, this type of apparatus often has a plurality of reconstruction ranges with respect to a region to be imaged, such as large, medium, and small, although there are many cases. For example, when an FP type two-dimensional sensor is used, L (φ170 × H120), M (φ102 × H102), S (φ65 × H65), SS (φ50 × H50) reconstruction ranges can be alternatively selected. In the case of the apparatus of FIG. 1 of the present invention, for example, a reconstruction range of φ75 × H75 and φ50 × H50 can be selected alternatively.
FIG. 2 is a block diagram showing an overall embodiment of the present invention. In the figure, an
FIG. 3 is a flowchart of the main part of the present invention. FIG. 4 is a block diagram of the main part of the present invention. In the figure, the X-ray projection data (X-ray attenuation coefficient) from the entire circumference of the imaging target region is obtained by, for example, rotating 360 ° around the imaging target region of the subject M. The acquired CT image is reconstructed from the projection data, and the CT image displayed on the display device 11 of the X-ray CT image of the subject's imaging target area has no influence on the interpretation or diagnosis by the artifact. When the determination is made, the image diagnosis is performed as it is.
On the other hand, according to the tomographic image reconstruction algorithm of the present invention, as described above, the X-ray from the peripheral direction of the imaging target region is obtained by swiveling the periphery of the imaging target region of the subject M. Projection data (X-ray attenuation coefficient) is collected, CT image reconstruction calculation processing is performed from the projection data, and an X-ray CT image of the subject's imaging target region is read on the display device 11 as an artifact. Alternatively, when it is determined that the diagnosis is affected, the mode is switched to the tomographic image mode via the input device of the
On the other hand, if the tomographic angle set by the tomographic angle setting means 19 is 30 °, for example, and the frame advance angle setting means 20 sets the frame advance angle table to 10 °, for example, the tomographic image reconstruction calculation process As a result, the tomographic image displayed on the display device can be displayed in a frame feed pattern with 10 ° increments. Therefore, by effectively using the frame advance angle setting means 20, options for avoiding obstacles to interpretation or diagnosis due to CT image artifacts are further increased.
In the embodiment, the above-described tomographic angle setting means 19 is selected from preset tomographic angles such as 8 °, 15 °, 30 °, 45 °, 60 ° and 72 °. An arbitrary angle may be input, or, for example, a range of 6 ° to 76 ° may be selected stepwise in 2 ° steps. Similarly, the frame feed angle setting means 20 can select an angle from a preset angle in addition to inputting an arbitrary angle.
